Hospitals and health systems can play a major role in addressing our nation's healthcare disparities in our nation. In our latest episode – part of our hospitals and health equity series – we zero in on hospital rankings and how the metrics that are used can compel health systems to take much-needed action on health equity. “If hospitals were to focus more on health equity, they would be fulfilling both a moral responsibility as well as a legal responsibility,” says Tavia Binger, a health data analyst at U.S. News and World Report. “Nonprofit hospitals are actually required to spend portions of their revenue on community benefit activities – like providing care that is free or at a reduced cost for patients who can't afford to pay – in exchange for their tax exempt status.” In addition to the latest U.S. News Best Hospitals rankings, published on July 16, U.S. News has also introduced health equity measures and has released a list of hospitals excelling in health equity by “providing vulnerable populations with substantial access to high-quality care,” Binger says. Health Disparities podcast host Dr. Mary O'Connor speaks with Binger about how U.S. News and World Report is working to promote health equity. They also discuss how healthcare consumers can use hospital rankings to understand whether the hospitals in their communities are doing their part to provide care that is both high-quality and equitable. When aiming for healthy aging and reducing chronic disease risk over the lifecourse, exercise, specifically a combination of resistance training and cardiorespiratory fitness, has a fundamental role to play. In addition it can mitigate age-related declines and support independence in later years. A strong evidence base exists showing the benefits of increased strength and cardiorespiratory fitness on a range of important outcomes related to lifespan and healthspan. But some intriguing questions remain, such as: How strong is strong enough? At what point do we maximize the benefits of exercise? How “fit” do we need to be? Is simple participation enough or do we need a true training adaptation? How can this actually be implemented in practice so that it is doable? In this episode, Dr. Jordan Feigenbaum shares his insights on this topic and offers advice on how we can increase our probability of a long healthspan, through exercise interventions. In this episode, Dr. Blair Bigham and Dr. Mojola Omole speak with two co-authors of a new guideline published in CMAJ entitled "Preventive care recommendations to promote health equity".Dr. Nav Persaud and Dr. Aisha Lofters explain the importance of embedding health equity into healthcare and discuss specific ways public policy and individual primary care physicians can work together to promote equity in their practices. The conversation highlighted some key recommendations such as:Begin outreach at the age of 45 instead of at the age of 50 for colon cancer screening. This recommendation is based on the recognition that younger ages of diagnosis are being observed, particularly among people who are disadvantaged. The goal is to eliminate barriers for individuals ready to start screening and prioritize health equity during implementation.Offering publicly funded HPV self-testing for cervical cancer screening, especially for disadvantaged individuals who may face barriers to traditional screening methods.Implementing depression screening and offering online counseling programs or cognitive behavioral therapy for patients who screen positive.Providing resources for families experiencing poverty, such as a one-page sheet detailing available support.Prioritizing care for specific populations based on inequities in health outcomes, such as prioritizing cardiovascular disease screening in women due to discrimination and sexism within the healthcare system.Overall, Drs. Persaud and Lofters advocate for a shift towards recognizing that inequities in health outcomes exist and that primary care physicians need to embed equity into their healthcare practices.Join us as we explore medical solutions that address the urgent need to change healthcare.
