Aug 7, 2024On this episode of the Bill Alexander Show, Bill talks with Geri Jewell. Geri is best known as Cousin Geri on the NBC sitcom, The Facts of Life. She was the first person with a disability to have a regular role on a prime-time series. She began her career doing stand-up comedy at the Comedy Store in 1978. In 1980 she performed at the 2nd Annual Media Access Awards, where she was introduced to Norman Lear by producer, Fern Field. After her groundbreaking role on Facts she has appeared on such shows as The Great Space Coaster, the Emmy award-winning movie Two of a Kind, Sesame Street, 21 Jump Street, The Young and the Restless, Strong Medicine, and the HBO hit series, Deadwood, which she also received Emmy consideration for. She also co-starred in the independent film "Night of the White Pants" and was featured on the FOX series, Alcatraz. When Geri is not working in television, she is a highly sought after motivational speaker and trainer in the areas of diversity, disability, and GLBT issues. Her training has taken her to such companies as Hewlett Packard, Master Foods, Johnson Wax, AT&T, Amgen, Mass Mutual, and Raytheon. She has also trained such government agencies as The C.I.A., The U.S. Treasury Department, Defense Intelligence Agency, and the U.S. Army. Everyone knows now the building certainty by Julie has been written, but I was like, No, the people need a full interview of just you. There's not an audio book currently, right?JulieNo, not an audio book.KevinSo for those of of those people in the audience who are more like my kids, prepping for school by listening to their books are supposed to read over the summer, I don't know how I feel about that, but some people might need some more like insight in an audio format in order to encourage them to do something that apparently no one does anymore, which is actually read a book.JulieI love it. Well, this is fun. I've never done it from this side. I mean, obviously I'm on the podcast pretty regularly, but never in this seat before where you're interviewing, I guess me. So this is.KevinFirst of all, we have to celebrate and I think we did this a couple times on social media, but people are buying your book. Yes. They read it and implement it. You know, fingers crossed. Hopefully you do good for your business and good for your career. But people are buying the book. It's already it's already fantastically successful, number one, in Amazon under real estate for new releases.KevinI think for long. So who knows? Yeah, I like the fact that you get equally nervous discussing such things as I do.JulieYeah, I know, I know. Well, it's a world like, I don't know. Some people know I used to write fiction, so I have fiction books out, but this is the still different. So this has been fun and it's been fun seeing people say, Oh, I bought multiple copies for my team. It's just it's been fun and the support has been great.KevinYeah. All right. Well, let the hot seat begin. The pleasantries are over.JulieOkay, I'm ready.KevinAnd my main question for you is there are nine, ten, ten chapters in the book. Are there a couple of chapters where you feel like not not the ones that are your favorite to write or you learn the most when you're researching or looking into something, but that you feel like people need. Like a lot of times a little more context.KevinWhen you work with 80 different homebuilders around the country, people are always like, How do you know what to talk about or insight? Because we just you can tell what people need. This was definitely something that people needed. But what are there particular chapters that you think people need more than more than others? When you look at what what folks are doing in today's landscape?JulieSure. One is that probably the shortest chapter. Well, except for the last one, just kind of worksheet. But chapter five is all about like, what do we prioritize? I think what we see with a lot of builders that we're like, Hey, how's content going in the room? Because it encompasses so much and then they just don't know which of those many things that are out there should I actually be working on.JulieSo it's about how to assess what you're currently doing, look at the needs of the company, look at what's out there in the future and how to actually figure out like, okay, what do we actually need to focus on right now and what do we need to start putting into place for the future? So that's kind of a short, quick chapter, but it's also helping them focus in on out of all these things we've talked about in the book, what what do I do first?KevinYeah, I think if you focus on those things that are most important, then you can start building momentum. And a lot of times people just get stuck. And like I talked to someone yesterday, well, we we partner with a company right now to help us create content. I talk to them for a half hour and then four or five weeks later they'll give me something in writing about what we talked about that she's just like, it just feels so slow and hard and it doesn't seem like this is the right, right way to be going about it.KevinSo I think we need to send them a copy of the book and I'll be helpful. But I agree. I think people don't. They're often disillusioned by response to content out of the gate. Mm hmm. I go I always go back to the podcast regularly. Now it gets 500 listeners per episode or so, sometimes a little more, sometimes a little less.KevinI think the first like six month, well, first two months. Anyway, that we did the podcast, there was like 30 to 50 people and you're like, Is all this worth it? All this prep and getting the technology in place and all the rest. So if you focus on what's most important and the reason why is most important, hopefully it gives you momentum to keep going.KevinSo I love that one. Is there another one that you like? Yeah.JulieI think just the beginning of the book that kind of lays out what is content, why do we need it and just kind of a foundation, because it is a word that people throw around. And so some people, when they think of content, they think of social media and some people think of content and they're just thinking renderings and photos on my site.JulieBut it's all of those things and more and I think I'm a very analytical person. I like a list. So in a way, a lot of the beginning of the book was just breaking things down into pieces where even if some of these things you just know from being in your role, it also is seeing it all in a structure of what topics are we going to cover, what mediums are we going to use, you know, photos, videos, whatever.JulieHow are we going to distribute it then? How are we going to analyze it? It just kind of lays it down in a framework that you can start wrapping your arms around because it's just large.KevinYeah, I think chapter two could have been its own like book in a way as well, which is just what topics should we cover? Which I thought you might say that one, but here's, here's my answer anyway. Why? Number two is extremely important and useful, but I wouldn't have picked it either, is the answer really is stop thinking that everything around you is boring or mundane and just relent to the fact that you need to talk about it all.KevinYou need to have content related to everything. So in chapter two starts on page 22, ends on page 46, and has all I mean, the table of contents anyway, alone. There's two and a half pages of things you can talk about. So that's, that's like excuse wiped away. There is no excuse of, well we don't have anything to talk about.KevinWe're just a fill in the blank. We're just a first time home builder and all these people say this to us. Well, I only build 20 luxury homes a year. Well, I only build on your lot. And that's like, stop saying only. It's all interesting to to a certain audience.JulieAnd the reason I included all of those two is because I think we get stuck in one thing, like, Oh, I've done a million walkthroughs of my house and I've done a million testimonials. It's like there's other you can take the same things and take a different angle at them. I think the real magic that comes in all of these topics is when you start combining more than one topic.JulieSo if you want to feature a floor plan, well how about also grab, you know, people who've lived with that floor plan and you're mixing the people, the homeowners and the floor plan. So a lot of it is just like kind of walling all those topics out on the table. So then you can start mixing and matching and instead of having to use your mental capacity to think of those topics, you can start using that creativity to then put together new and unique things to solve your problems and engage customers and all those good things.KevinAnd I think the fact that people go down that habitual path is a clue to actually the right answer with some with some nudging to get out of a rut is like I remember your former employer. There is this period of time where like every I don't know, I seem like every couple of days or once a week there was a Matterport walkthrough with an audio example and you went through a whole bunch of that, that library because you developed a process and a framework and the strategy and a why.KevinAnd once that momentum got going, it was relatively easy to keep it going. And I think that's what I really enjoy about the book, is it kind of unlocks that, Yeah, just do that. But on a on a different tangent from a different perspective or for a different purpose, again, like the same piece of content, the same advertising channel can be used for multiple reasons.KevinAnd I go to my generally least one of my least favorite forms of media in billboards of a billboard can be a complete brand message, or it can be a turn right here. Message the neighborhoods right around the corner. It's the same. It's the same platform, but used for a different purpose. And that changes everything about how you're thinking about it.KevinSame thing when it comes to content. Am I trying to educate someone? Am I trying to motivate someone? Am I trying to just not be boring? Because this is a complex subject and I need to break it into pieces that people want to absorb. I think it's just a fantastic, fantastic resource. Now, I can't remember I asked you this on the podcast or not, but it's worth asking again, when you wrote this, did you envision people going through it from beginning to end first and then using it as a resource to kind of get back around where you need it?KevinBecause it is I would say it's very snackable in that sense of like, again, you're like, I don't have time to read 130 pages.JulieYeah, I really have two ways I think about it being is, number one, somebody who's been in their role for a long time, they know what content is, but they just need that little bit of like inspiration and, you know, to get them on stock, like you said. And that way it would be very good to just grab the pieces you need and flipping through it as you're, you know, you're stuck on something to possibly spark something.JulieThat's one reason I left the whole when somebody was helping me with the formatting, they put that whole huge table of contents in and at first I was going to remove it and then I was like, You know what? It's actually going to be really helpful for somebody because it almost works like an index. It's like an index that the first of the book, because if they are struggling with maybe some storytelling content and they're wanting to get ideas for that, they can go straight to that part to talk more about their people in their company and their culture and all of those things.JulieThen on the other hand, it's also going to be a good resource for somebody comes in new to your department from another industry, and you just need to get them up and running like you're our new social media person. But I've never been in homebuilding. I feel like it also will give those people just a good foundation to read it cover, to cover and just, you know, have a beginning place of this is this is the basics of content and where to start and where to start looking.JulieAnd it just gets them up and going faster. So I think it does more than one thing.KevinYeah, I would agree with that. I as you, as you were just talking, I was remembering the dummies books, the series.JulieYeah.KevinFor Dummies.JulieOh. And it's yellow.KevinAnd it's yellow. So I was like, you know what? This is almost like content marketing for dummies. Not not in, like, the condescending sense of the word, But there were certain topics where I would buy those books. For those of you who, like the other day, someone didn't know the song Ice Ice Baby by Vanilla Ice. So I am constantly reminded of how old I am.KevinThe dummies series was on every topic known to man, and it was kind of like in textbook format, but it tried to be entertaining as much as possible around every subject matter. But there were. There were some of those I would buy like Calculus for Dummies in college that was clearly just like, I need I need a supplemental.KevinLook at that topic. Go read it. Okay, I'm good. It was a reference piece for me. And then there were others where you just from cover to cover. You're like, I just need to start at the beginning and be led all the way through. And the same book could be used either way. So I think that's important. Okay.KevinWhy the name building certainty?JulieWell, I totally ripped that off from Kevin Oakley.KevinHe was giving you a given.JulieYeah, he. He promised he wouldn't sue me for that. So I think it was Summit's a couple of years ago.KevinYou're 29.JulieAnd part of your message was what is marketing? It's building certainty through content. So we were looking at content and marketing from that, from that viewpoint. So then when I start working on a book and started working on title, I just took that for you. And I think right now it is always what content should do. But right now, more than ever in the markets that we've been in is that's what we have to use it for.JulieYeah, it's the basics of like, what do we build, Where do we build it? What does it cost? That's, that's number one. You need to have that. Your website needs to have the basics. That's number one to content. If you don't.KevinThink it's worth, it's worth repeating slightly differently for everyone. Is pricing on your website is content that build certainty in one direction or another. And so even something as mundane as some companies have every price and then a909 for like even that there there needs to be thought put into it. I used to joke with people all the time, you know, when you just when every price change or adjustment this is in 2008 2000 and 2010 and we would publicly change pricing to match market versus promotions like you can't just every price change can't be ten grand or five grand or 15 because well, that clearly communicates to someone paying attention forever, subconsciously as someoneKevinis when, you know, sticks their finger up in the air. I think 10,000 less like why is it always around number round numbers are too convenient in something as complicated as what we do. It's just one. One small example of everything is content that can build certainty.JulieYeah, all the basic stuff. And then just building certainty with with the rest of it. The story building, you know, telling your story, talking about your brand. All of that in the end isn't just to talk about yourself because nobody cares. Nobody cares about your brand and your what they care about is that, you know, you're going to take care of them and that you're going to solve their problems.JulieSo it's looking at all your content from that different angle. Then it's just, you know, I'm going to slap something up there about us. It has to be about the home buyer.KevinAbsolutely. I think like the other day, my brother in law was in town from Sweden and he's he's watching all the cruise ship documentary things on the on Smithsonian Channel. And I saw ads for the first time in a while on television. And it was just every time there was commercial break extra, extra, extra, which if you don't know what extra is, it's just an office chair and it's from all accounts, like I love the chair that I have.KevinIt's fantastic. I have no desire for additional chair. But they talk about this chair in words and in depth that my brother in law was just like, You see this chair? This chair is like the best chair in the world. I go, I mean, it looks like it's cheap materials with a massage and heat function to me and it's chart and they're charging like triple what they should.KevinBut it just goes back to that. The care like that is a lot of times what people are thinking of what they want. Marketing to do is to increase absorption, increase the sales volume of a company. But there are a lot of people listening where your company is not really in that game. Your company's in the business of maximizing revenue per unit sold or margin.KevinIt's a higher end product. And so if you're talking about like if you're can you imagine buying something from what's like the cheapest furniture IKEA. Yeah like well yes an IKEA is unique in that they've really overindexed in the amount of content to help people understand what it is they're getting in a digital format. In particular, they're one of the first to have like air show this thing in your house.KevinBut we can still use IKEA if you are going to buy a sofa from IKEA or a sofa from Pottery Barn or I don't know, Henry Don Like if the amount of written content and imagery and the quality of such is on par, you would never buy it. You know, it's only the the overall and I think that's the other word that I think if I want to suggest book number two is in the series of content is something around the idea of curation because that's I just I think that more and more of that is that's what five chapter five is really about.KevinIt's like, why, why this and not that? And so it's it's both breadth and depth, but also curating it for an intended audience with that intended purpose.JulieYeah, I mean, I think the book is proof that there's plenty of plenty of things to talk about, plenty of ways to do it. It's then taking a step back and looking at what's most important for your home buyer and what's most important for the home builder, for whatever where you are in the in the market. So I think that's what I see with builders.JulieSometimes they're jumping out in front of things, trying to do content before just pausing it. Does that have to be a big, huge thing? Pausing, thinking through messaging, thinking through the final goal. Look at the what is the goal of this content and then working backwards through creating it.KevinAnd I think one of the final like master level things to uncover and understand that sometimes you're creating content as a marketing team that's not for you at all. Meaning it's not for the website, really. It and everything should be there. But I would say a good portion of the content I created in my career prior to coming over to do Convert was creating content specifically for the use of of others in a 1 to 1 interaction framework here.KevinHere is content that helps a salesperson overcome a specific objection as a proof source, as a packaged, curated solution to an objection that they need some help overcoming. It's not, it's not it's almost usually not the broadest possible audience that you can you can reach because it's hard to do that really, really well. And this is where I'm thinking about people like Tilson who have done such a good job, really going narrow and deep in their subject matter to create a really loyal and and devoted audience and what they do.JulieI mean, kind of on that, on that same realm, what I also see is people who create a great piece of content and then it goes out in one specific place for one specific moment. And it's like, why? Like I see this, this is great. Did you send it out in an email? Did you give it to your sales team?JulieIs it somewhere on your website? You know, it's just using those great things. You guys create in more places so more people get eyes on them.KevinYeah, the number of times we still hear Where is this on your website? Oh, it's not. It's just on our YouTube channel or just on on Instagram. And we're like, Huh, What? Yeah, And strange. Still still some challenges to overcome. Awesome. All right. And most of all, as I was writing it, really, the only goal was to add some value for it to be something that you guys could use and be homebuilder specific. I've read a ton of marketing books and content books. It's like, Well, that's cool, but doesn't really fit. So really, that's all it's for is just for, for you all to get some, get some good ideas out of it.KevinYeah. So we can't wait to see all the stuff that people make. Hmm. Yes. Neuroscientist Mona Sobhani made a profound and brave inner journey. It amounts to a transformation, an awakening. She used to be a hardcore physicalist. Around 2018, in the midst of a life crisis, she began questioning the tenets of conventional western science. They didn't hold when it came to explaining many nonphysical human experiences. So, she dove into the literature, did dozens of interviews and wrote a book about everything she learned and experienced on the way. ”I eventually became much more open minded”, she says. ”But I had an ego struggle. It's hard to let go of this box of beliefs. You just ignore things that don't match the beliefs. That's how the human mind is built. My mind was constantly being blown, with each interview I did.” Mona's ”Old me” would have dismissed someone's story about a spiritual experience as imagination or misinterpretation. Her ”New me” will listen with curiosity and compassion. Everybody experiences the world in a unique way. It comes down to the first-person sentient experience, which is the hard problem of consciousness in science. ”In neuroscience, we don't have any way of measuring how it is to be you or me. You just have to take people at their word”, Mona says. ”Consciousness is the beginning, the middle and the end. What else is there? You can't really tell somebody that they didn't experience something, even though we do that all the time.” She soon realized that you have to ignore a lot of evidence to make the physicalist paradigm work. ”And that's not a very good model.” Mona Sobhani thinks there might be a paradigm shift underway in neuroscience. New papers present theories that say consciousness could be an energy field and that there is an interaction between the field and the brain. Some physicists today say things that intuitives have said for a long time and that are found in ancient texts. Mona's book, Proof of Spiritual Phenomena, is packed with references to scientists, philosophers, studies and books. It covers every conceivable spiritual field. She has herself acquired personal experience from many of them, like intuitive readings, meditation, breathwork, psychedelics, astrology and tarot. Psychedelics can broaden your consciousness vastly, she says. ”The boundaries between you and the rest of the world get blurred.” ”It's such a big problem that neuroscience only focuses on the everyday waking state.” It is difficult to find incentives for truly novel research in our current system, according to Mona. There is much bias and inertia. Scientists who apply for a grant must follow old research closely. ”You can only move just a little bit further. You must not shock the reviewers. True innovation is not rewarded.” The media is tainted with a similar bias. And when scientists communicate, it is often ”a disaster”, Mona says. ”They often say 'there is no evidence for that', but that is misleading. What it really means is that it hasn't been investigated. But the readers never know that.” Mona's website Mona's book

Her book Drug Dealer, MD: How Doctors Were Duped, Patients Got Hooked and Why It's So Hard to Stop[iii] (2016) is a good overview of what addiction is, and the dangers of prescription drugs. Her NEW book Dopamine Nation: Finding Balance in the Age of Indulgence[iv] that was just released last month, explores the exciting new scientific discoveries that explain why the relentless pursuit of pleasure leads to pain…and what to do about it. I'm Andrea Samadi, author, and educator from Toronto, Canada, now in Arizona, and like many of our listeners, have been fascinated with learning and understanding the science behind high performance strategies that we can use to improve our own productivity in our schools, our sports, and workplace environments. This week's interview with Dr. Anna Lembke on her NEW book Dopamine Nation is based on true stories of her patients falling prey to addiction and finding their way out again with stories that many of us might find to be shocking, but she explains that “they are just extreme versions of what we are all capable of.” (Dopamine Nation) When reading this book, or listening to this interview, I encourage you to think about your own life, your behaviors and what you might be running from since we are all running from something and like we have mentioned many times before on the podcast, awareness is the key to making any behavior change that can have a lasting impact on our productivity and results. My hope is that we can all take an honest look and find places where we might be leaking energy, to close those gaps, and redirect that energy towards our goals. We covered the topic of addiction at the start of this year with Aneesh Chaudhry (EPISODE 102)[v] on “Mental Health, Well-Being and Meditation: Overcoming Addiction Using Your Brain” and I first mentioned Dr. Lembke on episode #157[vi] “Overcoming Digital Addiction Using Neuroscience” after a discussion with our friends about technology use led me to Dr. Lembke. This episode was a popular one, with over 700 downloads in the first few days of release. Then when I posted that I was working on this episode, over Labor Day weekend, I had many messages from friends and colleagues who shared with me that they were very interested in this topic. I think this is something that we should all be aware of, since most of us also have not ever had any training on the topic of addiction, yet we all know someone who struggles in some way. We can also learn so much about ourselves with this information. Understanding how chemical, behavioral, and even digital addictions are formed/broken can help us all to navigate our lives, with a deeper level of awareness that can close up those gaps where we waste energy, to improve our productivity. Keep in mind, Dr. Lembke recommends her 30 day dopamine fast for people with less severe addictions and anyone who is struggling with a serious drug or alcohol addiction should seek further treatment from their medical provider. Back to the episode… Dr. Lembke's book, Dopamine Nation shows us what happens when we get too much of a good thing, but we can use this understanding to counteract the effects of this neurotransmitter in our brain, bringing us back to balance, and productivity. PART I The Pursuit of Pleasure In part 1 of the book, called The Pursuit of Pleasure, Dr. Lembke gives some examples of “how we are constantly trying to distract ourselves from the present moment to be entertained” and “that we're all running from pain—we'll do almost anything to distract ourselves from ourselves” and that “we've lost the ability to tolerate even minor forms of discomfort.” When I thought about this part of the book, I couldn't agree more thinking of all the times I grab my phone to distract myself from something, anything difficult that comes my way, instead of staying in the present moment. Chapter three goes deeper into the science of brain chemistry, discussing two key features of the effects of dopamine: the brain's tendency to seek homeostasis, and the development of tolerance PART 2 Self Binding: Dr Lembke describes some encounters with her patients, and how to keep addictive behaviors under control. She covers Dopamine Fasting with an ACRONYM to help us learn how to use her 30-day Dopamine Fast to reset our brains. Dr. Lembke will explain her 30-day Dopamine Fasting Plan for people with less severe addictions, where she often sees people return to their “drug of choice” in a controlled way. 30 Day Dopamine Fast Data: what are you using, how much, how often? Objectives: what does it do for you? Problems: or downsides does it cause? Abstinence: stop using it for a month and see what happens Mindfulness: be prepared to feel worse before you feel better Insight: abstaining from our drug of choice gives us incredible insight that we cannot see without stopping. What did you learn? Next Steps: moving forward without the drug/behavior even when you miss it. Can you do that? Experiment: Go back out into the world, experiment and see what works and what doesn't. If there is something that you want to change in your life, try going without it for 30 days, and see what happens. Only you will know if this will work for you or not. Dr. Lembke noted that “even when moderation is achievable, many of her patients report it's too exhausting to continue, and they ultimately opt for abstinence in the long haul” (Dopamine Nation). PART 3 The Pursuit of Pain: Dr. Lembke explores the opposite side of the equation: seeking out things that are painful, in order for the brain to tend to increase feelings of pleasure immediately afterward in an attempt to regain homeostasis. Explains the “pain” side of addiction and the importance of finding balance, radical honesty and self-awareness because “people who lean too hard and too long on the pain side of the balance can also end up in a persistent dopamine deficit state.” (Dopamine Nation) After releasing EPISODE 157 that explained Dr. Lembke's work and her 30 day dopamine fast, I almost wanted to move on past this topic, as I say often, there are entire podcasts dedicated to addiction[vii], and they do a much better job than I ever could. But there is another reason I would rather skip it, and that's because it's a difficult topic. It's much easier to move on past it than talk about something I'm still trying to learn and understand myself, because we weren't taught this topic in school for us to know how it to handle it when it shows up in our life. I remember the extent of my education on this topic was in 9th grade, when our PE teacher said, “don't drink alcohol to cover up your problems.” I remember she appeared to be uncomfortable with the topic, but it's an important one. If you ask anyone, we all know someone who suffers with a chemical addiction (alcohol or drugs) and since this topic was never a part of our schooling, it's easy to criticize what we don't understand, let alone recognize it in our own behaviors. When I first encountered someone with an addiction, around 20 years ago, I couldn't understand why they couldn't just have one or two drinks and call it a night. Why did they have to keep going? What's going on in the addicted brain? This was years before we could type our questions into Google and get hundreds of articles to help us (like Dr. Lembke's work, or even Dr. Amen's work on the addicted brain), so I would go to our local library and find books that explained addiction to gain some understanding. I wish Dr. Lembke's first book was there, as it wasn't easy to navigate this topic. Not being the type to sweep anything under a rug, I found some ideas and solutions for this person to enter into a local rehab program[viii] to get further help, but this opened up a can of worms with a problem that was never discussed and made me really popular in that family, but this understanding gave me a new level of awareness that would help someone else years later. This awareness helped my husband with one of his best friends from high school who called one day to confide in him that he had a heroin addiction, and was entering a faith-based rehabilitation program, but wanted one of his friends to know what was really going on with him. His initial reaction would have been to say “what the heck is wrong with you? Heroin addiction? Are you an idiot? How did this happen?” but because of all that time I spent researching at the library, I explained to him how addictions happen, often beginning innocently (using pain killers after a surgery) or in his friend's case, using uppers to help him through his busy days). This explanation helped him to talk with his friend with more understanding and his friend did well in recovery, helping many others for a few years, until one day, it beat him, and he was gone. I know this is a complex topic, often resulting in death like we saw with my husband's high school friend, or we see with celebrities who have been unable to break the cycle, and the pandemic has magnified this issue for those who were stuck in their homes for all of this time, but with the understanding of our brain in mind, my hope is that this topic no longer is swept under the rug, but talked about openly to find solutions with our brain in mind. Let's meet Dr. Anna Lembke and explore her new book, Dopamine Nation, together to gain a deeper understanding for those who struggle with serious addiction, to those with less severe, and see if her 30 day Dopamine Fast could be a solution to tighten up the gaps and improve our productivity. Welcome Dr. Lembke, thank you so much for agreeing to speak with me on the podcast today. I've got to tell you that before I hit send on your email to invite you on the show as a guest, I thought twice, a bit nervous about you actually replying and saying yes because I knew I needed to talk about a topic that I have avoided going deeper into, but at this point, It was obvious that I couldn't avoid it any longer, so thank you for agreeing to speak with me so quickly, allowing me to be more authentic and open. Dr. Lembke, before we get to the questions I have on your most recent book, Dopamine Nation, I wanted to ask a question that ties into where I first saw you, in the movie The Social Dilemma (which scared the living daylights out of me) where you talk about how “social media is a drug—that directly affects the release of dopamine and the reward pathway”[ix] and you talk about how with all of your knowledge and experience, you are still worried about your own kids and their time spent using these apps. I know your kids are a bit older now since that film was released, but what did you tell your kids DAILY about how our brains respond to certain apps on our cell phones? NOTE: This question sums up everything I want to ask you in this interview, and that at the end, we can come back to your answer here, and I know it will sum everything up perfectly. I launched this podcast helping educators and those in the workplace to understand how to apply the most current neuroscience research into the classroom and workplace because it's so important, and many of us need this information, but it wasn't taught to us in school. Either was the topic of addiction, and this is why I thought it was so important to reach out to you, because your first book on this topic, Drug Dealer MD: How Doctors Were Duped, Patients Got Hooked, and Why It's So Hard to Stop (2016)[x] explains what is addiction and who is at risk, Dopamine Nation goes beyond chemical addiction (drugs and alcohol) to understanding the Social Dilemma you spoke about in the Netflix movie and beyond that with how our brains respond to anything we do to escape “even minor forms of discomfort.” Q1: So diving into your book, Dopamine Nation, I thought I'd seen it all, but I'm sure there's a lot you see in your practice that shows to what extent we distract ourselves from whatever it is that's painful in our present moment. You give some good examples that drill down this point, and I thought about how often I use my phone to distract me from difficult times in life (From serious life challenges to minor things). I know we can all think of what we do to escape from life, but can you explain why not being In the present moment and dealing with life's challenges as they come up (whether we are using our phones as an escape/drugs/alcohol, romance novels, binge watching Netflix, whatever it is we do) only make our challenges worse? Q2: I think I've got an understanding of what happens to our brain when we are in a dopamine deficit. Would it be accurate to say this is what happens when we cut something out that we liked, and experience withdrawals? 2B) What happens to our brain when we overindulge? I had never heard of the idea that you mention in the article one of my friends put on the windshield of my car[xi] about how too much pleasure (with our phones, or video games or whatever it is) can tips us towards feeling pain. I'm not sure I have ever felt this, or I'm not aware of it. What is the pleasure /pain balance and how do we know we have had too much of a good thing? Q3: But you say there's good news, and that our brains can reset if we do what you call a dopamine fast (30 days away from whatever we were doing) and our brains can go back to balance or baseline. With the young man who was playing video games, he went back to doing what he enjoyed by modifying his behavior and making sure he kept his work and gaming separate. You talk about after the 30 days, that you experiment and see what works and what doesn't. I know an alcoholic can't after a month of abstinence go back to “controlled” drinking (as much as they would like to). How does the dopamine fast work and is there something we should watch out for to make sure our brains don't get flooded with dopamine again? Dopamine is not the only neurotransmitter involved in reward processing, but most neuroscientists agree it is among the most important. Dopamine may play a bigger role in the motivation to get a reward than the pleasure of the reward itself. Wanting more than liking. The more dopamine a drug releases in the brain's reward pathway (a brain circuit that links the ventral tegmental area, the nucleus accumbens, and the prefrontal cortex), and the faster it releases dopamine, the more addictive the drug. Q4: When I saw your rewards and dopamine chart that show how much dopamine is released with chocolate vs sex vs drugs, and you say that learning “also increases dopamine firing in the brain.” Where would learning or other healthy habits fit on your rewards/dopamine release chart? How can we be sure we are not being “indulgent” with healthier habits like learning/exercise? In your article[xii], the young man who played video games was able to go back to playing video games with a modified schedule. Then I read about how the brain changes with high dopamine rewards. (Experience dependent plasticity). Does this mean that high reward behaviors you can't limit, and you can never go back to them? Don't we eventually experience tolerance with all behaviors, and over time would find them boring anyway? (Your example reading your novels they were never as exciting as the first read, or when we rewatch a Netflix series we loved, it's never as good as the first time). Where does tolerance fit into the equation? Experience Dependent Plasticity The brain encodes long-term memories of reward and their associated cues by changing the shape and size of dopamine-producing neurons. For example, the dendrites, the branches off the neuron, become longer and more numerous in response to high-dopamine rewards. This process is called experience-dependent plasticity. These brain changes can last a lifetime and persist long after the drug is no longer available PART II Self-Binding chapter four: Dopamine Fasting chapter five: Space, Time, and Meaning chapter six: A Broken Balance? Q5: Can you explain your ACRONYM for DOPAMINE and what happens to our brain when we take a month off of using our drug of choice? Dr Huberman[xiii] said it really well in his recent interview with you, the first 10 days suck. Why does this dopamine deficit feel so bad? “A week would be good, but in my experience, a month is usually the minimum amount of time it takes to reset the brain's reward pathway. If you don't feel better after four weeks of abstaining, that's also useful data. That means the cannabis isn't driving this, and we need to think about what else is. So what do you think? Do you think you would be able and willing to stop cannabis for a month?” Younger people recalibrate faster than older people, their brains being more plastic. Furthermore, physical withdrawal varies drug to drug. It can be minor for some drugs like video games but potentially life-threatening for others, like alcohol and benzodiazepines. Mindfulness practices are especially important in the early days of abstinence. Many of us use high-dopamine substances and behaviors to distract ourselves from our own thoughts. When we first stop using dopamine to escape, those painful thoughts, emotions, and sensations come crashing down on us. Q5B) Why does tolerance occur? Dr. Lembke, I could spend the next week asking you more questions, but know I've got to wrap up this interview. Q6: To close out our questions, I wanted to give something for our listeners to be able to apply on this topic. I know that you openly talk about something you stopped doing in the book that you enjoyed, and I was on the tail end of letting go of a habit that I loved when someone put the article on my car about your 30 day dopamine fast, showing me how important it was to understanding this at the brain level. Going back to the first question I asked you, “what do you tell your kids daily about dopamine/the pleasure/pain balance and dopamine deficit and the risk of addiction” what should we all know dopamine, and breaking free of its hold over us? She is currently Professor and Medical Director of Addiction Medicine, Stanford University School of Medicine. She is also Program Director of the Stanford Addiction Medicine Fellowship, and Chief of the Stanford Addiction Medicine Dual Diagnosis Clinic. She is a diplomate of the American Board of Psychiatry and Neurology, and a diplomate of the American Board of Addiction Medicine. Dr. Lembke was one of the first in the medical community to sound the alarm regarding opioid overprescribing and the opioid epidemic. In 2016, she published her best-selling book on the prescription drug epidemic, "Drug Dealer, MD – How Doctors Were Duped, Patients Got Hooked, and Why It's So Hard to Stop" (Johns Hopkins University Press, 2016). Her book was highlighted in the New York Times as one of the top five books to read to understand the opioid epidemic (Zuger, 2018). "Drug Dealer, MD" combines case studies with public policy, cultural anthropology, and neuroscience, to explore the complex relationship between doctors and patients around prescribing controlled drugs. It has had an impact on policy makers and legislators across the nation. Dr. Lembke has testified before Congress and consulted with governors and senators from Kentucky to Missouri to Nevada. She was a featured guest on Fresh Air with Terry Gross, MSNBC with Chris Hayes, and numerous other media broadcasts. Using her public platform and her faculty position at Stanford University School of Medicine, Dr. Lembke has developed multiple teaching programs on addiction and safe prescribing, as well as opioid tapering. She has held multiple leadership and mentorship positions and received the Stanford's Chairman's Award for Clinical Innovation, and the Stanford Departmental Award for Outstanding Teaching. Dr. Lembke continues to educate policymakers and the public about causes of and solutions for the problem of addiction.
