In 2008, Australia and New Zealand ratified the United Nations Convention on the Rights of Persons with Disabilities (CRPD). The CRPD challenges traditional mental health legislation by emphasising the respect for inherent dignity, legal capacity, liberty and physical and mental integrity of persons with disabilities on an equal basis with others. The application of CRPD in mental health care requires a realistic and balanced interpretation of the CRPD as well as education and training of clinical staff in human rights framework.Prof Neeraj Gill and Dr Ava Carter will outline the contemporary human rights framework. They examine civil and political rights, including involuntary psychiatric treatment and coercive interventions, along with economic, social and cultural rights. The discussion will also cover the application of the UN CRPD in mental health care in Australia as well as social determinants to health, social justice and public mental health.Prof Neeraj Gill is a psychiatrist at Gold Coast Health. He is a Professor of Adult Psychiatry at Griffith University and Professor of Public Mental Health at the University of Canberra. He is the Regional Vice-President, Oceania of the World Federation for Mental Health and the Secretary of the World Psychiatric Association Section on Psychiatry, Law and Ethics. He served as an elected Director on the Board of the Royal Australian and New Zealand College of Psychiatrists (RANZCP) from 2020-2023.Neeraj was awarded his Doctor of Public Health degree from the University of New South Wales (UNSW) based on his doctoral thesis on 'human rights of people with mental disabilities'. He has published many papers on human rights, mental health law, and Aboriginal and Torres Strait Islander social and emotional wellbeing.Dr Ava Carter is a dental surgeon and medical doctor, currently specialising in consultation liaison psychiatry and developing her skills in medical administration and management. She is passionate about collaborating to improve outcomes and drive system change in organisations in the healthcare sector, and is driven to elevate the voices of others, particularly in healthcare education and lived experience. Clinically, Ava has worked in public health services, in allied health, and in the private health sector, and currently holds an operational role as Senior Registrar at Canberra Health Services. Scott Fisher, MD is a former college basketball team captain, Joseph Scott Fisher graduated with a BA in Economics from The University of Chicago in 2004 and worked for a year in options trading at the Chicago Board of Options Exchange before matriculating at and subsequently graduating from The Feinberg School of Medicine at Northwestern University in 2009. His General Psychiatry Residency training was also done at Northwestern University before he completed his Child and Adolescent Psychiatry Fellowship at the University of Miami/Jackson Memorial Hospital. He has developed expertise in the treatment of depression, anxiety, bipolar, and attention deficit disorders in his private practice located in South Florida.   In 2018, he completed the Certificate in Psychedelic Therapy and Research at the California Institute of Integral Studies, which included training in MDMA, psilocybin, LSD, and ketamine assisted treatments. He takes an interest in research into psilocybin-facilitated and MDMA-facilitated psychotherapy as novel tools for treating depressive, anxiety, trauma-related, and eating disorders. He is Board Certified in Adult Psychiatry by the American Board of Psychiatry and Neurology.   Dr. Fisher is an integral member of Segal Trials Center for Psychedelic and Cannabis Research, Dr. Fisher serves as a Lead Facilitator for centers' Psychedelic studies. Dr. Fisher is one of only few licensed psychiatrists in the country with advanced certification in Psychedelic-assisted psychotherapy from California Institute of Integral Studies, the only accredited institution of higher learning in this field. Shari started as a CNA in 1978, then became an LPN and graduated from RN school in 1983. Her background includes inpatient Med-Surg, Ortho, Post-Partum and Nursery, Chemical Dependency and Adult Psychiatry and Endoscopy. For the last 20 years, Shari has worked at Hospice of the Valley, in Phoenix, Arizona, as an RNCM in Home Hospice. Currently, Shari is the President of the Phoenix Chapter of the Hospice and Palliative Nurses Association. She is passionate about Death Café and has benefitted tremendously from it. About Deb: Deb Bershad is a nurse with diverse experience. She has worked most nursing positions available in the acute care hospital setting. She spent 17 years in the acute care setting, until burnout took its toll. She left the field and found her most rewarding and challenging work to date. For the next ten years, she worked as a teacher, facilitator, and Professional Development Coach. Personal tragedy prompted a 3-day retreat in which she re-clarified her goals and passion in life. She teaches and mentors faculty, both in and outside of psychiatry, students, and trainees. Dr. Gold is a graduate of the University of Pennsylvania, where she earned a B.A. and M.S. in Anthropology, in addition to being a Benjamin Franklin Scholar and Phi Beta Kappa. Today, two psychiatrists, Dr. John Palmieri and Dr. Eric Rafla-Yuan, join us to answer these questions and talk about the future of 988. John Palmieri, MD, MHA, is a Senior Medical Advisor at SAMHSA and currently serving as the Acting Director for the 988 and Behavioral Health Crisis Coordinating Office. Prior to his arrival at SAMHSA, Dr. Palmieri was the Division Chief for Behavioral Healthcare at the Arlington County, Virginia, Department of Human Services. Dr. Palmieri is a licensed physician in the Commonwealth of Virginia and is Board Certified in Adult Psychiatry. He graduated from Brown University Medical School and completed his Adult Psychiatry Residency at Massachusetts General Hospital. Eric Rafla-Yuan, MD is the 2021 American Psychiatric Association (APA) Jeanne Spurlock Congressional Fellow, and a voluntary assistant clinical professor in the Department of Psychiatry at the University of California San Diego, where he founded and led the psychiatry residency diversity committee. Remote psychiatry is one part of the diverse practice of regional, rural, and remote psychiatry that is unique to countries such as Australia and New Zealand.  Remote psychiatry brings a number of unique challenges and benefits for clinicians and is not for the faint hearted. Working in resource poor areas, across large geographical areas means that psychiatrists must be adept in providing the whole range of psychiatric skills and expertise to entire communities, differentiating itself from metropolitan practice most familiar with psychiatrists in Australia and New Zealand. Associate Professor Mat Coleman leads today's discussion from Geraldton in Western Australia, where he calls in from his mobile phone.Joining Mat are Doctors Neil Jeyasingam from Broken Hill, Siva Bala from Cairns and Sela Mulholland from Broome. He inherited the program from her in 1994 and for the next six years further developed and enhanced the program. Recent studies confirm this but show the growing rate among Black youth, which now surpasses that of their white counterparts. Despite making up 15% of the population, Black youth make up 37% of youth suicides. The highest demographic is young Black males. The assumption would be that depression significantly contributes to the increase in suicides. However, new studies indicate that much of the increase correlates to ADHD and untreated learning disabilities. While recent trends show the growing number of suicides in Black youth and adolescents, even more alarming is the rate of suicide among very young Black children. In this episode, Dr. Napoleon Higgins shares insights about how ADHD may be contributing to the increased number of suicides in Black youth and identifies why this is happening now. He also gives recommendations for educators and healthcare providers about cultural sensitivity and offers expert advice for parents of children with ADHD on diagnosing, treatment, and suicide prevention.   Dr. Napoleon Higgins is a child, adolescent, and adult psychiatrist in Houston, Texas.  He is the owner of Bay Pointe Behavioral Health Services and South East Houston Research Group. Dr. Higgins received his MD from Meharry Medical College in Nashville, Tennessee. He completed his residency in Adult Psychiatry and his fellowship in Child and Adolescent Psychiatry at the University of Texas Medical Branch Galveston. He is the President of the Black Psychiatrists of Greater Houston, Past President of the Caucus of Black Psychiatrists of the American Psychiatric Association, and Past President of the Black Psychiatrists of America, Inc. Season five of our podcast continues with another presentation from our 2020 annual conference: ‘Engaged Phenomenology' Online. This episode features a presentation from Mary Coaten, Durham University.   ABSTRACT: My paper explores doctoral research on the therapeutic mechanisms of Dance Movement Psychotherapy (DMP) in an in-patient setting for acute adult psychiatry through the qualitative dynamics of movement and the symbolic and metaphoric processes present. Previous research has focussed on the efficacy of DMP in relation to psychosis spectrum disorders, but little on the mechanisms, especially the role of the moving body within phenomenological approaches. I drew on the phenomenological tradition through Heidegger and Jung, utilising similarities between the two to develop ideas about body movement, space and time. For Brooke (1988), Jung and Heidegger understood the body as the incarnation of psychological life and not as the meaning-less body of anatomy; they saw psyche and dasein as spatial, viewing distance and closeness as lived realities, and not merely in absolute time which they both argued is a limited abstraction from lived reality. “Jung's method is primarily hermeneutic-phenomenological; the psyche is not “mind” or an inner realm more or less linked to the body, but is the embodied life world, and Jung's descriptions of it - of its autonomy, spatiality and bodiliness, for instance – achieve ontological clarity when it is articulated as Dasein." (Brooke, 1988:ii). The results demonstrated an altered sense of space and time and a specific imbalance in engaging with the future and the past. The study revealed gender differences in the use of space and sense of self. Both men and women's movement lacked structure, a lack compensated for through my movements. Participants expressed their sense of self differently by gender. The men engaged more with one another as a group and women focussed more on the individual bodily self. Symbolic and metaphoric communications indicated a relationship between an altered sense of space and time, and the movement dynamics present acted synchronistically with the symbols and metaphors.   BIO: I am a dance movement psychotherapist (DMP) with a special interest in psychosis and have recently completed my doctoral thesis at Durham University which explored the therapeutic mechanisms at play in the acute psychotic episode. For the past 15 years, I have delivered DMP groups within the acute inpatient mental health setting. My work is informed by a Jungian and phenomenological framework which highlights an embodied approach to psychopathology. Dr. Victor Ramos is employed at Kaiser Permanente Dept. of Adult Psychiatry in the San Francisco Bay Area. He's worked extensively in medical settings, providing brief services to patients in need of a better understanding of how to best manage the impact of their chronic health problems; in addition to screening for common mental health disorders. Dr Ramos completed his pre-doctoral internship and post-doctoral fellowship with Contra Costa County Health Services, where he specialized in Pain Psychology. Dr. Ramos completed his doctoral degree in Clinical Psychology from the Wright Institute in Berkeley, CA; a Master of Arts degree in Counseling from the Clara University and a BA in Religious Studies from the University of California Santa Barbara. He has advanced training in various therapeutic orientations (ACT and CBT) and uses an eclectic-interpersonal approach to meet the unique needs of each patient. I am honored to have the opportunity to speak with a senior Medical Scientist, David Ostrow, MD, PhD, LFAPA and advocate for therapeutic uses of cannabinoids and other plant based derived medications in prevention and treatment of acute and chronic conditions. Dr. Ostrow also holds licenses and is certified in Adult Psychiatry, HIV medicine, Addiction Medicine and Cannabinoid Medicine. Throughout Dr. Ostrow’s career, he has sought to shine light on the role of social and societal factors that limit healthcare access and increase co-morbidities among poor, minority and other marginalized groups, a driving factor that has gained widespread attention during the current Covid19 epidemic. Since the late ‘90s, Dr. Ostrow has been an advocate for replacing failed national drug policies in the US with more effective, compassionate and harm-reduction approaches, including regulation and legalization of adult cannabis use, integration of Cannabinoids as Medicine into the healthcare training curriculum, and expungement of criminal records and rehabilitation of persons incarcerated for non-violent cannabis possession.
