“ Anybody I've met since I got hurt, I feel like they don't know me and that they can't know me. For people that I've become really close to since I got hurt—it almost hurts me to know that this person won't actually ever know who I am or who I was. They're meeting an entirely different person … there's a before and after, and a massive wall that separates them.”   As a seasoned personal trainer and co-owner of Amplify Fitness, a gym in Boston, Mia Lazarewicz is used to helping athletes and adult fitness enthusiasts on the road back from injury. However, little could prepare the lifelong athlete for her own life-altering accident—breaking both legs in a gymnastics flip gone wrong in 2023.   In the immediate aftermath, Mia faced uncertainty and anxiety. As the days and weeks wore on, she struggled with shifts in her identity, not to mention the financial repercussions of medical bills and keeping a small business afloat.   But though there have been many dark moments, she found joy in striving, humor in her struggles, a deep sense of gratitude for her community, and fulfillment in unexpected endeavors—including writing a book, Becoming a Personal Trainer. It's Summer…and its getting HOT in here! Although the effect of heatwaves in pregnancy has been studied various times, the data remains limited because these studies are observational, with very heterogenous populations, so its hard to prove causation between heat exposure and adverse outcomes. Nonetheless, these associations are indeed concerning. YES…just to be clear, there are past studies that have observed associations between heatwaves and PTB, low birthweight, and in some studies…stillbirth. The most recent review on this was published just last year (June 2023) in the J Mother Child, and the title was “A Comprehensive Review on Hot Ambient Temperature and its Impacts on Adverse Pregnancy Outcomes”. This review of 23 articles did again find an association between heat stress and adverse pregnancy outcomes. PLUS, a new publication released July 3, 2024, now raises new concerns about ambient heat and in-utero childhood cancer programming! That article, from Lancet Planetary Health, is the focus of this episode. BUT WAIT there's more! We will also look at the relationship between ambient heat and PTB and oligohydramnios. The section is mostly data entry and includes some pretty routine, standard information. But understanding how this information is used in the application process is helpful for students in crafting it with care. Additionally, this section includes questions about a student's legal and institutional history - these questions are essential to get right for students to be fully considered and reviewed by schools. You find a bull's-eye rash around a tick bite. If you get to the doctor quickly, get a Lyme disease diagnosis, and take the standard antibiotics, everything will be fine, right?  Not for a sizable percentage of Lyme patients. And not for our protagonist in this episode, Siri. She's a seventeen-year-old Nordic ski racer from Vermont who's suffered repeated bouts of pediatric Lyme disease. Siri trains for cross-country ski racing at the Craftsbury Outdoor Center in Vermont, boosting her mood and stamina. But when she has deep fatigue, brain fog, and poor balance, training is often impossible. “There were many days,” says Siri, “that I just sat on the couch with my head between my legs.” Siri's second major onset of Lyme symptoms occurred at age 11. Perhaps due to previous infection and treatment, she does not test positive for Lyme then, so her doctors would not prescribe antibiotics. Yet her symptoms persisted, setting Siri and her mother, Kara, on a research and treatment odyssey. Our expert commentator for this episode, Dr. Steven Phillips, finds this is often the case for people with chronic Lyme and Lyme related symptoms. “[In] most cases of Lyme, the subjective features vastly outweigh the objective features. Subjective means what the patient feels, what the doctor can't see versus what the doctor can see,” says Dr. Phillips. He's a Yale-trained physician and author of the book, Chronic, about his personal struggle with Lyme and professional efforts to better understand the disease.  “What is very well recognized,” says Phillips, “is that a large percentage of patients develop chronic symptoms after acute Lyme disease.” In Siri's case, that meant trying a wide range of alternative therapies, some of which she'd rather forget.  Will Siri escape the physical and emotional depths of chronic Lyme? In this week's episode of the Spine & Nerve podcast, Dr. Brian Joves and Dr. Nicolas Karvelas expand on their recent discussion of pain chronification. The doctors discuss a fairly common scenario in which a patient with very chronic pain develops a relatively new pain condition. In this setting, the new onset pain process can be significantly amplified and difficult to treat. Previously, the doctors summarized processes including central and peripheral sensitization, which contribute significantly to the severity and refractory nature of chronic pain conditions. Listen as the doctors review chronification of pain in the context of a patient case example and discuss treatment approaches, particularly optimizing medication management. Chronic pain disease processes are very challenging to diagnose and treat, especially when multiple body parts are involved. However, there are a growing number of tools available to make a positive impact for our patients, and with a carefully crafted treatment plan, progress can be made. Allan and Darren begin their final episode of 2020 with the recent cabinet reshuffle, specifically Dan Tehan becoming Minister for Trade and Andrew Hastie becoming Assistant Minister for Defence. Tehan replaces Simon Birmingham, the new Finance Minister, and Allan explains what he most admires about ‘Birmo’, giving Tehan—himself a former diplomat—big shoes to fill. On the Defence side, we now have a Defence Minister, and an Assistant Defence Minister, who have both served in the Australian Defence Force—something unusual and notable. The discussion moves to the appointment of Will Hodgman, a former Premier of Tasmania, to be Australia’s next High Commissioner to Singapore. Allan wonders what specialised skills (if any) the government believes head of mission posts require, while Darren offers a very personal reflection on the wide range of abilities required to be an Ambassador, especially in a crisis situation. Next the conversation turns to the Richardson Review, chaired by friend of the podcast Dennis Richardson and which, at over 1300 pages in length, is a deep and comprehensive inquiry into the legislation governing Australia’s intelligence community. Allan explains why the report is so significant and lists some highlights. Liberal democracies across the world are grappling with the perennial question of “freedom versus security”, and the powers (and oversight) of intelligence agencies are central to these debates. Getting the balance right is important not just in and of itself, but for demonstrating that the liberal democratic model can manage uniquely 21st century challenges. Finally, Allan and Darren preview their “summer homework”. What is each looking to learn more about over the summer, and why? For Allan, the answer revolves around the degree of agency Australia has in the emerging international order, and for Darren the answer is—as always it seems—to understand more about China itself, and Beijing’s intentions. This week saw the release of the reports on the collision reports and Comprehensive Review of the incidents this summer between merchant ships in WESTPAC and the destroyers USS FITZGERALD and USS MCCAIN.The totally avoidable collisions resulted in the death of 17 Sailors and removal from our most important theater two of our most critical assets.Our guest for the full hour will be Bryan McGrath, CDR, USN (Ret.).Bryan grew up in Mount Laurel, New Jersey, and graduated from the University of Virginia in 1987. He was commissioned upon graduation in the United States Navy, and served as a Surface Warfare Officer until his retirement in 2008. At sea, he served primarily in cruisers and destroyers, rising to command of the Destroyer USS BULKELEY (DDG 84).During his command tour, he won the Surface Navy Association’s Admiral Elmo Zumwalt Award for Inspirational Leadership, and the BULKELEY was awarded the USS ARIZONA Memorial Trophy signifying the fleet’s most combat ready unit. Ashore, Bryan enjoyed four tours in Washington DC, including his final tour in which he acted as Team Leader and primary author of our nation’s 2007 maritime strategy entitled “A Cooperative Strategy for 21st Century Seapower.”Since retirement, Bryan has become active in presidential politics, serving first as the Navy Policy Team lead for the Romney Campaign in 2012, and then as the Navy and Marine Corps Policy lead for the Rubio Campaign in 2016.He is the Assistant Director of Hudson Institute’s Center for American Seapower, and he is the Managing Director of The FerryBridge Group LLC, a small defense consulting firm.
