CardioNerds (Dr. Dan Ambinder and Dr. Rick Ferraro) join Dr. Mansi Oberoi and Dr. Mohan Gudiwada from the University of Nebraska Medical Center discuss a case of constrictive pericarditis. Expert commentary is provided by Dr. Adam Burdorf, who serves as the Program Director for the Cardiovascular Medicine Fellowship at the University of Nebraska Medical Center. The case discussed involves a 76-year-old woman with a history of monoclonal gammopathy of undetermined significance, chronic obstructive pulmonary disease, type 2 diabetes mellitus, and squamous cell carcinoma was admitted to the hospital for worsening shortness of breath, swelling in lower extremities, hyponatremia, and urinary tract infection. CT chest to evaluate for pulmonary embolism showed incidental pericardial calcifications; the heart failure team was consulted for the management of her decompensated heart failure. Echo images were nondiagnostic. Subsequent invasive hemodynamic monitoring showed elevated right and left-sided filling pressures, diastolic equalization of LV and RV pressures, and positive RV square root sign with ventricular interdependence. Cardiac MRI showed septal flattening on deep inspiration and septal bounce, suggestive of interventricular dependence. After a heart team discussion and with shared-decision making the patient opted for medical management owing to her comorbidities and frailty. Today's show opens with a newly published medical research study from the UK on kids diagnosed with Myocarditis or Pericarditis. In a sample of 1.7 million youth and adolescents, not a single child, was was not vaccinated for Covid-19, came down with either of those heart conditions. In every instance of those who did have the heart problem, every child had been jabbed. This leads to another medical topic from Dr. Casey Means on the Joe Rogan Show. The data is clear. We in the United States are getting sicker and it's happening faster than most have realized or discussed. Using these two medical stories, I decided to talk about how we choose a doctor. Whether picking a pediatrician, because we want the best care for our kids, or a specialist because our lives may suddenly be at risk, we all want to find the best. But, what do we mean by that? What makes one doctor better than another? This discussion is directly analogous to the choice we are all facing in who should be the next President of the United States of America. While on the topic of the presidency, I think it's important to once again explain the Electoral College and why we much never let it be taken away. We do not pick a leader based on the popular vote of the nation. We pick a leader based on winning more of the states, which are represented by the Electoral College. Otherwise, we will find ourselves in a position where a handful of states will determine the presidency, while the vast majority will be ignored. White House Press Secretary Karine Jean-Pierre through a toddler style temper tantrum a couple days ago when questioned about the budget and the priorities of the Biden-Harris administration. She literally said questions are now “misinformation.” This is ludicrous and shows how close we really are to Orwell's 1984. We close with a story told by RFK, Jr. about Harris using the might of her office to arrest parents. It's a reminder that she already told us her “values” have not changed. She has demonstrated who she is by what she has done before. We have all the red flags we need to know her dictatorial tendencies are who she really is. In this episode, Dr. Danilo Malandrino discusses a pivotal study on acute autoimmune pericarditis, revealing critical differences in hospital outcomes between autoimmune and idiopathic cases based on a comprehensive Japanese registry. The findings underscore the need for careful evaluation of underlying autoimmune diseases in patients with pericarditis, highlighting distinct prognoses and the importance of tailored treatment strategies.
In this episode, Jian'an Wang examines a nationwide study on acute autoimmune pericarditis (AAP) in Japan, highlighting its prevalence, diagnostic challenges, and in-hospital outcomes compared to acute idiopathic pericarditis (AIP). The findings reveal that while mortality rates are similar, AAP is linked to a higher incidence of cardiac tamponade, underscoring the need for accurate diagnosis to optimize patient care.

Today on the show I speak to Prue about the wild ride that led her to freebirth. Starting at 14 Prue started dabbling in drugs and alcohol. By her 20's she realised it had become an addiction and she checked herself into rehab. She then ended up at the other end of the spectrum on extremely strong anti-depressants which she said made her a zombie and literally drool. She ended up weighing 100kg and knew something needed to change. She got off the pharmaceutical drugs and started body building but got down to a very unhealthy 35kg. When she finally let go of that and returned to a healthy weight and got her period back (which she had lost because she was so thin) she fell pregnant with her first. Her daughter was born in hospital and although straight forward for a hospital birth - it still wasn't the best experience and in the proceeding weeks she felt like something was off. At 6 weeks postpartum Prue was diagnosed with cancer and she tells us of how she felt like she was in a glass house looking at her daughter, not be able to be with her, bond with her or raise her in this hard time. Cut to covid - Prue felt forced to take the Pfizer vaccine and ended up with Pericarditis from it. This time Prue took the natural route and healed herself. This crazy story ultimately ended (or begun) with Prue choosing to Freebirth her next child - her little boy Koa. Although she had hired a midwife - she had always planned to not call her on the day and thats exactly what she did. Prue had a beautiful and simple birth at home despite a nuchal hand being present. I love Prue's fire and passion and her commitment to being vulnerable and open so others can learn from her story. Thank you Prue! More about Prue: 35 year old mum to a 5yr old girl born in hospital and a 8MO Boy freebirthed at home. Some obstacles in life to get here has been: Addiction, rehabilitation, eating disorders, cancer and vaccination reaction during covid. As a result of the vaccine reaction I endeavoured onto a successful self healing journey after ditching the recommendations of the cardiologist and GPs. I turned to holistic healing where I tried everything you can imagine from homeopathy, ice baths, saunas, yoga, anti inflammatory diet and the list goes on. Through discovering the power of the body and it's healing capabilities it was a no brained and complete certainty that freebirth was my dream when I fell pregnant with my second child.

Contributor: Meghan Hurley MD Educational Pearls: Pericarditis is inflammation of the pericardial sac, which can arise from infectious or non-infectious etiologies Myocarditis is inflammation of the myocardium, which may accompany pericarditis Pericarditis clinical findings include: Diffuse concave ST elevation, classic for acute pericarditis with myocardial involvement. More common in younger male patients Elevated high-sensitivity troponin - higher levels may occur in young healthy patients Ultrasound may show pericardial effusions POCUS may be helpful in assessing left ventricular ejection fraction (LVEF) via E-point septal separation (EPSS) Elevation in EPSS correlates with decreased LVEF Treatments: Anti-inflammatories including NSAIDs and colchicine Monitor inflammation Repeat ultrasounds Risk factors in this patient's case: mRNA COVID vaccine - the risk of myocarditis from vaccination is significantly lower than that from COVID-19 infection Preceding infection References 1. Thank you for joining us for our 2nd Cabral HouseCall of the weekend! I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks… Natalie: I am interested to know Dr Cabral's thoughts on the safety of sweeteners like Stevia and Monk Fruit? Monk fruit especially is increasingly showing up in food and supplements and it's very difficult to find information online about it. Stacey: Hello, I was recently diagnosed with SIBO and I am wondering what protocol you recommend. I am also breastfeeding Thank you Stacey Hannah: My question is relating to an ongoing histamine intolerance. I've done the 21 day detox, CBO, 2 months CBO finisher, heavy metal & mold detoxes, two more 7 day liver detoxes, and the parasite protocol. Each protocol was helpful but the parasite protocol (I completed 8-wks) is where I saw the most help with my histamine intolerance. It is significantly better than before that protocol in particular, but I can only handle small amounts of high histamine or histamine liberating foods. I get muscle twitches, sour stomach after eating highly acidic fruits such as oranges/berries, my right foot feels weak/like I'm limping, my right ankle swells up especially with beer, and I get stuck burps/trapped air in between and during meals. I also have a persistent scalloped tongue. Any advice? Anonymous: Hello Dr. Cabral, I am an IHP Level 2 and RN and appreciate all the work you do and the course you made. I'm slowly chipping away on protocols as it seems I have symptoms of everything. That being said I have a history of recurrent incessant pericarditis resulting in a Pericardiectomy in 2021. My surgeon said this should have gotten rid of the pain - it has prevented me from being hospitalized again but I do still experience pain when stressed and sometimes around the heart when taking deep breaths. Additionally I struggle with a lot of jaw /neck pain. I'm looking for any advice or expertise around addressing pericarditis at the root cause as I know that these symptoms are probably still related to the pericarditis that was only addressed by surgery and not at the root. Rozita: Dear Dr.Cabral,I'll try to be short. Diagnosed w/ Hashimoto's 3 weeks ago.Tsh,Ft4,Ft3- normal,TPO -900.Also,thyroid nodules diagnosed in2004. I am taking Se,Zn,D3, Mg,Turmeric,VitB complex,Vit A,VitE, and a Multivitamin for seniors, that has Iodine 150mcg.Question: should I continue to take the Multivitamin w/ the included Iodine? Thank you very much for the answer. You are the Best! I live by Your videos and recommendations. Kind regards! Rozita 73 yr, Anonymous: Hi! Dr. Paul Alexander Liberty Hour – Why is the optimal health of females being sidelined this way? Females are at as much risk, if not more than males, due to the COVID mRNA technology gene injection vaccine. The science has matured and is telling us this. Be warned, women, do not let the medical doctors fool you anymore!
Dr. Robert Malone is one of the co-inventors of the mRNA technology behind COVID jabs and #1 Wall Street Journal bestselling author of the new book Lies My Government Told Me: And the Better Future Coming. He joins the show to discuss how he has built up tens of thousands of paying subscribers on his Substack newsletter (https://rwmalonemd.substack.com/) and millions of followers on alternative social media like WhatsApp, TikTok, Gettr, Gab, and Truth Social, despite being deplatformed and censored by mainstream news and social media because of his outspoken publishing of data and evidence regarding mRNA vaccines - the very invention he helped create. Malone is vaccinated himself, and nearly died from COVID before the vaccines were out in 2021. Despite the negative publicity, Dr. Malone still advises, consults, and is on the payroll of tens of healthcare, medical, and pharmaceutical companies worldwide, as well as Wall Street research firms. Business continues to improve for him to the point where he is not taking on any new contracts or clients. Dr. Malone tops off the discussion by advising work from homers on how to best take care of their hearts and overall health - particularly if you're vaccinated or previously infected with COVID - including getting cardiac troponin and Vitamin D levels tested. Exploring the relationship between all-cause and cardiac-related mortality following COVID-19 vaccination or infection in Florida residents - study conducted by the Surgeon General of Florida: Primary findings: “In the 28 days following vaccination, a statistically significant increase in cardiac-related deaths was detected for the entire study population (RI = 1.07, 95% CI = 1.03 - 1.12). Stratifying by age group revealed RIs were significantly higher for age groups 25 - 39 (RI = 2.16, 95% CI = 1.35 - 3.47) and 60 or older (RI = 1.05, 95% CI = 1.01 - 1.10).” https://floridahealthcovid19.gov/wp-content/uploads/2022/10/20221007-guidance-mrna-covid19-vaccines-analysis.pdf?utm_medium=email&utm_source=govdelivery 2. Clinical cardiovascular emergencies and the cellular basis of COVID-19 vaccination: Primary findings: “COVID-19 vaccines evoke rare but fatal thrombotic events, whereas messenger RNA55based vaccines appear to be associated with risks of pericarditis/myocarditis, with the latter being more predominant in young adults following the second dose.” https://www.ncbi.nlm.nih.gov/pmc/articles/PMC9444584/pdf/main.pdf 3. Cardiac complications following mRNA COVID-19 vaccines: A systematic review of case reports and case series: Primary findings: “Myocarditis/myopericarditis and pericarditis were the most common adverse events among the 243 reported cardiac complications, post mRNA COVID-19 vaccination. Males with a median age of 21 years had the highest frequency of myocarditis.” https://pubmed.ncbi.nlm.nih.gov/34921468/ 4. Postmarketing active surveillance of myocarditis and pericarditis following vaccination with COVID-19 mRNA vaccines in persons aged 12 to 39 years in Italy: Primary findings: “…increased risk of myocarditis/pericarditis was associated with the second dose of BNT162b2 and both doses of mRNA-1273. For young males receiving 2 doses of the same vaccine, data were compatible with between 4 and 7 excess events in 28 days per 100 000 vaccinees after second-dose BNT162b2, and between 9 and 28 per 100 000 vaccinees after second-dose mRNA-1273.” https://jamanetwork.com/journals/jamacardiology/fullarticle/2791253 8. Booster Vaccination with SARS-CoV-2 mRNA Vaccines and Myocarditis Risk in Adolescents and Young Adults: A Nordic Cohort Study of 8.9 Million Residents: Primary findings: “Our results suggest that a booster dose is associated with increased myocarditis risk in male adolescents and young male adults.” https://www.medrxiv.org/content/10.1101/2022.12.16.22283603v1 In addition to these studies, the FDA has cautioned that some COVID-19 vaccines carry the risk of myocarditis and pericarditis, especially in young men. https://www.fda.gov/news-events/press-announcements/coronavirus-covid-19-update-june-25-2021 Though not related to young men, on January 13, 2023, CDC cautioned that Pfizer/BioNTech's Bivalent booster vaccine might increase the risk of ischemic stroke in people over 65, a condition in which blood clots of other particles block the blood vessels to the brain. While the concern relates to those over 65, this new safety signal illustrates that we do not know the mid- or long-term health outcomes of the COVID-19 vaccines. https://ktxs.com/news/nation-world/cdc-flags-possible-safety-issue-with-updated-pfizer-covid-19-booster-ischemic-stroke-potential-health-risk-senior-fellow-cato-institute-dr-jeffrey-singer-omicron-variants-protection Although some have suggested that myocarditis and pericarditis are elevated after a case of COVID-19, a large study of about 600,000 adults concludes that is not the case. The study, “The Incidence of Myocarditis and Pericarditis in Post COVID-19 Unvaccinated Patients-A Large Population-Based Study” stated, “We did not observe an increased incidence of neither pericarditis nor myocarditis in adult patients recovering from COVID-19 infection.” https://pubmed.ncbi.nlm.nih.gov/35456309/
