BUFFALO, NY - June 11, 2025 – A new #research paper was #published in Volume 16 of Oncotarget on May 29, 2025, titled “Durable complete response in leptomeningeal disease of EGFR mutated non-small cell lung cancer to amivantamab, an EGFR-MET receptor bispecific antibody, after progressing on osimertinib.” A team led by first author Jinah Kim, from the University of Vermont Medical Center, and corresponding author Young Kwang Chae, from the Feinberg School of Medicine, reports a clinical case in which a patient with advanced non-small cell lung cancer (NSCLC) carrying rare EGFR mutations responded remarkably to amivantamab after other treatments had failed. The patient experienced a complete resolution of brain and spinal fluid metastases, suggesting that amivantamab may be a viable option for patients with uncommon genetic profiles and limited therapy options. Lung cancer remains one of the leading causes of cancer-related deaths worldwide. Patients with NSCLC who have rare mutations in the EGFR gene often face limited treatment options and poor outcomes, especially when the disease spreads to the brain or spinal fluid. This case involved a 67-year-old man diagnosed with NSCLC who had two rare EGFR mutations—G719A and A289V. After disease progression on osimertinib and other therapies, the patient began amivantamab monotherapy. Within six weeks, his lung tumor shrank by over 30 percent. By six months, imaging confirmed the disappearance of brain metastases and leptomeningeal disease, a serious condition affecting the membranes of the brain and spinal cord. Blood tests showed no detectable cancer-related mutations, and the patient, previously wheelchair-bound, regained the ability to walk and perform daily activities. This response has been sustained for more than 19 months. “Treatment produced a durable response over 19 months, including a 32.2% reduction in tumor size at six weeks, and complete resolution of brain metastases and LMD by six months.” Amivantamab is a bispecific antibody that targets EGFR and MET, two key drivers of tumor growth. While it is approved in combination regimens for common EGFR mutations, its effectiveness as a single agent in rare mutations or in treating brain metastases remains largely unproven. This case challenges the assumption that large antibody drugs cannot cross the blood-brain barrier and suggests that amivantamab may have potential in managing central nervous system involvement. Further research is needed to clarify how the drug achieves these effects and to explore its broader use in patients with rare EGFR mutations and limited treatment options. This case highlights three key findings: amivantamab may be effective against rare EGFR mutations, can be used as monotherapy, and may overcome the challenges of the blood-brain barrier. Although based on a single patient, the results provide encouraging evidence to support further investigation of amivantamab in treating difficult-to-manage forms of NSCLC. The creation of a landmark gene editing drug used to treat a baby with a rare genetic mutation which could help transform personalized medicine. Blood tests showed baby KJ had sky-high levels of ammonia, a toxic substance the body usually expels. The root cause was his genes - or more particularly a specific gene mutation. The race was on to try and treat him before his condition took a firm hold. His doctors came up with a radical solution - for the first time ever, they designed and applied a gene-editing drug in record time, specifically for him. China is reportedly developing high-speed torpedoes that will use artificial intelligence to overcome the use of decoys and other techniques in order to destroy our submarines. Should that happen, it would negate one of the few remaining advantages our navy enjoys over the Chinese Communist Party's considerably larger and increasingly well-armed one. It would also compound the betrayal JPMorgan's Jamie Dimon and Bank of America's Brian Moynihan inflicted by raising $4-plus billion for a Pentagon-designated “Chinese military company” called CATL. It's extending the lethality of Chinese non-nuclear attack submarines with advanced lithium ion batteries.  Those CEOs need to be held personally accountable for increasing the danger to American sailors with funds supplied by American investors. Peter Ordentlich, Chief Scientific Officer and Founder at Syndax Pharmaceuticals, a clinical oncology company, is focused on developing precision medicine approaches to treat cancer and chronic graft-versus-host disease. The menin protein plays a critical role in driving certain types of blood cancers, and the Synex therapy is a menin inhibitor that can disrupt this cancer-driving pathway. The platform is also developing an antibody addressing the unmet needs in cGVHD to reduce the disease-driving macrophages. Peter explains, "Syndax is a clinical oncology company, which means we're focused primarily on cancer, and we're primarily doing clinical development. We started the company really looking at resistance pathways to developing cancer, and we've been focused on two main areas. One in the indication space of leukemia, which are certain types of blood cancers, and that's with a program we have around the small molecule drug called revumenib. We are also focused in another area of diseases called chronic graft-versus-host disease. These are diseases that happen post-stem cell transplant, and there we're developing an antibody called axatilimab. And so those two programs are really what we've been focusing on for the last five or six years and have led to each of those drugs." "So menin is a very interesting protein, and basically what this is, it's called a scaffold protein. And you can imagine a scaffold, just something you build things upon. And so menin interacts with DNA through a variety of other factors, and you build on top of this a complex of other proteins that can turn genes on and off. And in the case of certain types of leukemias, the mutation that creates certain other cancer-causing proteins binds to menin. That's what drives certain genes that drive leukemia to always be turned on. And so what we've tried to do and others have tried to do is disrupt that interaction and essentially cause the scaffold to fall apart. In this special episode, Dr. Shaji Kumar from the Mayo Clinic speaks with Blood editor Dr. Laurie Sehn on a paper recently published in Blood, "Eliminating the Need for Sequential Confirmation of Response in Multiple Myeloma". The findings demonstrate eliminating the need for sequential confirmation of response in multiple myeloma. The study, involving 583 episodes of progression, found that simultaneous confirmation of disease progression using two different markers (e.g., serum protein electrophoresis and serum free light chain assay) was 98% accurate, compared to 82% for sequential confirmation. This suggests that simultaneous confirmation could improve clinical trial accuracy and reduce false censoring. The International Myeloma Working Group is set to revise its response criteria to incorporate these findings, potentially simplifying disease assessment and reducing the need for multiple blood draws.

Reflection Many do not realize the power of the Eucharist. And we as Catholics believe that when you go to the Eucharist, any venial sins as you have on your soul are forgiven. And what I see in this passage is a beautiful image of the wine, which is the wine of forgiveness, the consecrated wine. What is so beautiful about this passage is Jesus is doing one last thing before he claims it is all finished, and that is to drink the wine of forgiveness. It's a beautiful image of not only Jesus forgiving. Those who disappointed him, weren't there for him, condemned him, crucified him, but also it's a part of his way of saying that world of unforgiveness, of judgment, of condemnation is finished. And the Eucharist, brings in the world of forgiveness, which is the beginning of the kingdom that we now live in. Closing Prayer Father, we've been taught that the Eucharist is a reenactment of the death and resurrection of Jesus. But we don't really take it as literally as we should, because what it's saying is that every time we take the Eucharist, we are not only infused with the power of the Holy Spirit, but given the gift of being able to forgive. It empowers our forgiveness, which is the heart of everything that seems to me that we are missing in the world. A spirit of openness and forgiveness, acceptance, support. Connection. The post war neoliberals were divided over the European Economic Community, some viewing it as a protected enclave of the world economy that would hold back the global economic integration they hoped for. Others saw it as the beginnings of a borderless economic zone that would spread around the world, eventually subsuming all questions of politics and ideology to the logic of the market.
