    On Tuesday's Mark Levin Show, in a Senate hearing, Secretary of State Marco Rubio testified on Iran negotiations, emphasizing condition-based sanctions relief and the critical need to address Iran's ballistic missile capabilities. Any sanctions relief for Iran would fund the regime's army, IRGC, and security forces rather than the suffering people. We need to arm the Iranian people to enable an uprising. This terrorist regime seeks to destroy America and Israel while the current efforts fail to apply maximum pressure. We should not negotiate with or cut deals with terrorists because it emboldens enemies. The isolationists and the Woke Reich give aid and comfort to our adversaries. There is a growing domestic Marxist-Islamist threat, funded by Qatar, China, and others, that is organizing, building enclaves, and infiltrating politics. In breaking news, there are reports of multiple explosions in Bahrain, hours after Iran claimed precise missile strikes on U.S. bases in Kuwait and the IRGC warned of further seismic responses to American actions. Iran appears emboldened rather than defeated by reading U.S. restraint as weakness. The IRGC needs to be pounded, and the Iranian people need to be armed.  Later, the core defect of Marxism is its fundamental misstatement and misapplication of human nature. Rather than being an economic philosophy aimed at equality or fairness, it uses economics and redistribution as camouflage to advance totalitarianism, seeking to control individuals, thought processes, and ultimately create a society of compliant robots. This ideology, like Islam, employs appeal to compassion and justice as propaganda while intending the opposite. It cannot work because human nature, created by God, cannot be intentionally destroyed and rejiggered by man. The practical goal is demographic transformation to build dependent, like-minded societies that can be dominated, eventually extending this control nationally.     Bennett Yellin and Peter Farrelly talk about meeting each other in school and immediately connecting over their shared sense of humor. Bennett talks about substance abuse in college, while Peter discusses being a very hard driver at work. You talk about getting very lucky working with Eddie Murphy and David Zucker, and about bringing Bobby Farrelly into the group when they were writing movies together. Peter talks about being extremely loyal, living in Ojai, and never feeling like Los Angeles was really his town. Bennett talks about growing up in Beverly Hills in an Orthodox Jewish family. Peter tells a story about using the wrong knives while staying at Bennett's house because meat is not supposed to touch milk. Peter says he doesn't think Rotten Tomatoes is fair, and he also doesn't think criticism is very helpful. Bennett recently wrote a horror movie, Día de Muertos. Peter is a good audience member and wants everyone to contribute. Bennett knew everything about movies, while Peter knew almost nothing about them. Peter also has a very happy crew. Bio:  -Peter John Farrelly (born December 17, 1956) is an American film director, screenwriter, producer, and novelist. Along with his brother Bobby, the Farrelly brothers are best known for directing and producing quirky and romantic comedy films such as Dumb and Dumber, Shallow Hal, Me, Myself and Irene, There's Something About Mary, and the 2007 remake of The Heartbreak Kid. Farrelly solo-directed and co-wrote the comedy-drama Green Book (2018), which won the Audience Award at the Toronto International Film Festival in 2018, the Golden Globe Award for Best Screenplay, and the Academy Awards for Best Picture and Best Original Screenplay. He has been married to Melinda Farrelly since December 31, 1996. They have two children. -Born and raised in Los Angeles, Bennett received his B.A. in Fiction from U.C.L.A.  Still not ready to enter the real world, he enrolled at UMass in Amherst for an M.F.A. in fiction.  It was there — on the first day of school — that he met and befriended Peter Farrelly.  On a lark, they tried writing a comedy together and this spec script ultimately got into the hands of Eddie Murphy and the Zucker Brothers, creators of Airplane and The Naked Gun.  Both Murphy and the Zuckers asked the duo to write movies for them, and their career was off and running. Yellin wrote exclusively with Peter for years until they asked his brother Bobby to join them.  The three went on to write a number of unproduced features together until they created Dumb and Dumber in 1994 and reunited in 2014 to co-write the official sequel chronicling the further idiotic adventures of Harry and Lloyd, Dumb and Dumber To. In 2007, the Farrelly Brothers branched out on their own and Yellin partnered with James Robert Johnson to create a professional writing duo that has endured for sixteen years.  Among the plethora of projects they've tackled during their career — some produced, others not — the two have co-written Let's Scare Jessica to Death for Paramount Pictures, the Fox situation comedy Unhitched, the direct-to-DVD thriller Joy Ride 2: Dead Ahead, Paramount Pictures' Hotel For Dogs 2, the Anchor Bay action-thriller In the Blood with Gina Carano, the 20th Century Fox family film, Santa's Little Helper, and the Warner Brothers re-boot of the Police Academy series, Police Academy: Takin' it to the Streets.  More recently, Yellin and Johnston have co-written a live action family stage show adaptation of the hugely popular Angry Birds IP, and their original supernatural thriller Dia de Muertos has recently completed filming and is set to be released in 2026. Intermediate-high risk PE is a dangerous and heterogeneous groupPatients with RV dysfunction, positive biomarkers, tachycardia, hypoxemia, and elevated lactate may have in-hospital mortality approaching 15%, rivaling STEMI. Notes Drafted by Dr. Jane Ehret. 1. What is the contemporary framework for risk stratification in acute pulmonary embolism? Modern PE risk stratification prioritizes hemodynamics and right ventricular (RV) function rather than clot burden. The 2019 ESC Guidelines classify PE into high risk, intermediate risk (low vs high), and low risk, based on: Hemodynamic status, RV dysfunction on imaging, and Cardiac biomarkers. This framework emphasizes early mortality risk but requires clinical context to guide escalation decisions. 2. Why is normotension insufficient to define “stability” in PE? Blood pressure is a late marker of circulatory failure in PE. Patients can maintain normal BP through Tachycardia, Increased sympathetic tone, and RV compensation. Many patients with preserved BP may already have shock physiology, including hypoxemia, elevated lactate, and RV failure — sometimes referred to as “normotensive shock.” 3. How should intermediate-risk PE be conceptualized clinically? Intermediate-risk PE is heterogeneous, ranging from patients who do well on anticoagulation to those who deteriorate rapidly. Intermediate-high risk PE is defined by RV dysfunction on imaging and positive cardiac biomarkers. Clinical features such as tachycardia, increasing oxygen requirement, and elevated lactate identify patients at highest risk within this group. 4. What are the strengths and limitations of commonly used PE risk scores? Legacy scores are useful for initial risk categorization but are static and limited in predicting short-term deterioration. Most scores were developed to predict mortality or complications at fixed time points rather than dynamic clinical trajectory. 5. What are the commonly used risk scores and clinical tools in PE, and what is each designed to predict? ESC Risk Stratification Algorithm: Identifies high-risk PE by hemodynamics. Uses PESI or sPESI in normotensive patients to distinguish low-risk from non–low-risk PE. Uses RV dysfunction and biomarkers to differentiate intermediate-low from intermediate-high risk. Forms the basis of many institutional PE pathways. PESI and sPESI: Validated to predict 30-day mortality. Widely used to identify low-risk patients appropriate for outpatient management. Heavily influenced by age and comorbidities. Bova Score: Predicts 30-day PE-related complications in normotensive patients. Composite PE Shock Score (CPES): Predicts normotensive shock in hemodynamically stable PE patients. Pulmonary Embolism Progression (PEP) Score: Predicts progression from intermediate-risk to high-risk PE within 72 hours of diagnosis. PE Short-term Clinical Outcomes Risk Estimation (PE-SCORE): Predicts clinical deterioration or death within 5 days of PE diagnosis. Hestia Criteria: Identifies low-risk PE patients safe for outpatient treatment. Wells' Criteria and Revised Geneva Score: Determine pretest probability for diagnostic triage. PERC Score: Rules out PE in very low-risk patients. 6. What is the role of biomarkers in PE risk stratification? Troponin and natriuretic peptides reflect RV myocardial injury and strain. Current guidelines treat biomarkers as binary (positive vs negative), despite risk being continuous. Biomarkers are most helpful for: Initial risk classification. They are less useful for: Short-interval monitoring and Detecting rapid clinical deterioration. 7. Why is lactate an important physiologic marker in PE? Lactate reflects global tissue hypoxia and impaired perfusion. Elevated lactate may identify patients with: Early circulatory failure and Increased risk of imminent hemodynamic collapse. Lactate is not currently included in ESC risk algorithms but may add important prognostic information in intermediate-risk patients. 8. How does trajectory influence decision-making in PE management? Risk stratification should be viewed as a dynamic process, not a one-time label. Worsening clinical trajectory may include: Rising heart rate, Increasing oxygen needs, Rising lactate, and Progressive RV dysfunction. Serial reassessment is essential for timely escalation of care. 9. What role do Pulmonary Embolism Response Teams (PERT) play in risk stratification? PERT facilitates: Multidisciplinary decision-making and Integration of imaging, biomarkers, and clinical physiology. PERT is most valuable for: Intermediate-risk and high-risk PE and Patients with complex comorbidities or uncertain trajectory. PERT enables a shift from category-based to physiology-driven PE care.     After being yanked out of class and arrested on their high school campus, 15 year old Isabelle Valdez and 14 year old Lois Lippert are thrown into the back of a cop car. For 30 minutes the girls are caught on the car's dash cam giggling and completely unbothered.   At one point, Isabelle looks at her best friend, “Oh you trying to look good for your mugshot?” Lois excitedly responds, “Oh shit, am I going to get a mugshot?...This is such a bonding experience, I love it.”   The girls are basically acting like they're going to be able to go home at the end of their “side quest.” After all, the police arrested them before they could even carry out their plan to murder a classmate, right?   Isabelle even comments that their plan was, “not a school shooting at least.” Lois agrees saying, “we'd be in prison for life.”   It's true, the girls weren't planning a school shooting.     Today’s Topics: Joshua Charles joins Terry for Friday with the Fathers 1) Gospel –Mark 11:11-26 – Jesus entered Jerusalem and went into the temple area. He looked around at everything and, since it was already late, went out to Bethany with the Twelve. The next day as they were leaving Bethany He was hungry. Seeing from a distance a fig tree in leaf, He went over to see if He could find anything on it. When He reached it he found nothing but leaves; it was not the time for figs. And He said to it in reply, “May no one ever eat of your fruit again!” And His disciples heard it. They came to Jerusalem, and on entering the temple area He began to drive out those selling and buying there. He overturned the tables of the money changers and the seats of those who were selling doves. He did not permit anyone to carry anything through the temple area. Then He taught them saying, “Is it not written: My house shall be called a house of prayer for all peoples? But you have made it a den of thieves.” The chief priests and the scribes came to hear of it and were seeking a way to put Him to death, yet they feared Him because the whole crowd was astonished at His teaching. When evening came, they went out of the city. Early in the morning, as they were walking along, they saw the fig tree withered to its roots. Peter remembered and said to Him, “Rabbi, look! The fig tree that you cursed has withered.” Jesus said to them in reply, “Have faith in God. Amen, I say to you, whoever says to this mountain, ‘Be lifted up and thrown into the sea,' and does not doubt in his heart but believes that what he says will happen, it shall be done for him. Therefore I tell you, all that you ask for in prayer, believe that you will receive it and it shall be yours.
