She is the author of the book Cannabis and CBD for Health and Wellness and founded and operated seven bicoastal integrative cannabis medicine clinics in California, New York, and Connecticut.  Her training includes studying nutritional biochemistry at Cornell University, osteopathic medicine at Touro University, medical acupuncture at Harvard University, and research at Columbia University, Center for Complementary and Alternative Medicine in Women's Health. Through her work as a clinician, professor, and author, she has created medical protocols for the industry and internationally accepted methods for cannabinoid formulations, research, education, and training programs. What is the Entourage Effect? The entourage effect is a theoretic principle that suggests that compounds made in the cannabis plant's trichomes, cannabinoids, terpenes, flavonoids, and more work together to create a whole-body experience. Or Full-Spectrum with THC compared to a Broad Spectrum without THC but all the other goodies. “The cannabis experience is much more than just THC or CBD, as the cannabis plant is more than just a few cannabinoids. Think of it as different parts of the plant all working together in favor of your body,”. “No other plant is as efficient or multi-modal as cannabis.” How Does the Entourage Effect Work? Sometimes, analogies help explain things, so that's what we're going for here. Imagine your favorite movie or show soundtrack. Imagine all the instruments playing in that soundtrack. Percussion, string, wind, etc. Now, imagine the cannabinoids are the percussion, the terpenes are the string ensemble, and the flavonoids are the wind instruments. Sure, you could have a good song with just one piece of that symphony, but imagine the soundtrack just being percussion? It just isn't quite as good. That's the same idea with the entourage effect and the cannabis experience. Can you buy different parts of the plant on their own? Yes, you can! There are distillate and isolate products available, which are a pure form of a targeted cannabinoid created by removing everything except the desired compound (like THC pills). This will create a different experience than consuming full spectrum or broad spectrum extracts, which contain a variety of cannabinoids and terpenes. Getting to the Heart of Cannabis Health Risks Cell Press Cannabis has deleterious effects on cardiovascular physiology. Wei et al. (Cell 185, May 12, 2022) confirm that important inflammatory markers increase transiently after a single marijuana joint and prove a mechanistic link between THC induced vascular inflammation, endothelial dysfunction, cellular oxidative stress, and atherosclerosis using cell-based and mouse models of atherosclerosis. They provide a pharmacological model in which THC-mediated activation of CB1 receptor signaling pathways converge on MAP kinase, TNFa, and NF-kB outputs to create a proinflammatory and atherogenic environment in endothelial cells.   That genistein can antagonize the negative effects of THC with minimal central effects is exciting because genistein is a common component of soy and is already a widely consumed dietary product.   Electronic Cigarettes Versus Nicotine Patches for Smoking Cessation in Pregnancy: A Randomized Controlled Trial Nature Medicine Pregnant smokers were randomized to use either nicotine replacement therapy with patches (NRT, n=571) or e-cigarettes (n=569) for smoking cessation. For the primary outcome, validated prolonged quit rates at the end of pregnancy, the results were 4.4% for NRT and 6.8% for e-cigarettes (P=0.08). However, 25 participants in the NRT arm who reported abstinence also used e-cigarettes. When these participants were excluded from data analysis the quit rates were 3.6% for NRT and 6.8% for e-cigarettes (P=0.02). Low birthweight was less common in the e-cigarette arm, 9.6% versus 14.8% for NRT (P=0.01). The authors conclude that “e-cigarettes were markedly more effective than patches” and do not pose more risk.   Daily Cannabis Use, Cannabis Use Disorder, and Any Medical Cannabis Use Among US Adults: Associations Within Racial, Ethnic, and Sexual Minoritized Identities in a Changing Policy Context Preventive Medicine Reports Cannabis use has steadily increased in the United States, with daily and medical use associated with cannabis use disorder (CUD) and the negative consequences more frequent among marginalized groups. In this study, the authors use the National Survey on Drug Use and Health (NSDUH) to examine medical and daily use and CUD across the intersections of racial, ethnic, and sexual minorities. They found that sexual minorities were more likely to have medical and daily use and CUD than their heterosexual counterparts within each racial and ethnic group. However, when examining the intersection of race, ethnicity and sexual identity, there was more heterogeneity across these groups. In addition, they found that in states with medical cannabis laws (MCL) daily cannabis use was higher across all intersectional groups.     Effect of AXS-05 (Dextromethorphan-Bupropion) in Major Depressive Disorder: A Randomized Double-Blind Controlled Trial AJP Psychiatry Altered glutamatergic neurotransmission is implicated in the pathogenesis of major depressive disorder. AXS-05 (dextromethorphan-bupropion) is an oral NMDA receptor antagonist and sigma-1 receptor agonist, which utilizes inhibition of CYP2D6 to increase its bioavailability. This phase 2 trial assessed the efficacy and safety of dextromethorphan-bupropion in the treatment of major depressive disorder. In patients with major depression, dextromethorphan-bupropion (AXS-05) significantly improved depressive symptoms compared with bupropion and was generally well tolerated.  The most common adverse events were dizziness, nausea, dry mouth, decreased appetite, and anxiety. Dextromethorphan-bupropion was not associated with psychotomimetic effects, weight gain, or sexual dysfunction.   Mental Health and Substance Use Among Homeless Adolescents in the US JAMA Network This study evaluated mental health and substance use outcomes among homeless and non-homeless adolescents in 2019. Alcohol, cigarette, marijuana, and binge drinking during the prior 30 days was assessed, along with lifetime use of cocaine, methamphetamine, heroin, ecstasy, and injection drugs or prescription opioid misuse. Results found current substance use ranging from cigarettes to alcohol were higher among homeless adolescents. Lifetime cocaine use was significantly higher among homeless adolescents, as were methamphetamine, heroin, ecstasy, and injection drug use. Homeless adolescents experience worse mental health outcomes, including depression and suicidality, and struggle with more SUDs than their counterparts.   The Importance of Federal Action Supporting Overdose-Prevention Centers NEJM In this prospective piece, the authors discuss the need for new approaches to harm reduction and substance use disorder treatment in the face of substantially increasing overdose deaths, particularly since the start of the COVID-19 pandemic. One such strategy they discuss is overdose-prevention centers, which operate in other countries and are associated with significant reductions in opioid-overdose morbidity and mortality. However, under Section 856 of the Controlled Substances Act, such facilities may be subject to federal legal sanctions. The authors recommend that the Biden administration declare they will not interfere with such public health interventions or declare that section 856 does not apply to legally sanctioned centers. Further, Congress should modify the Controlled Substance Act to exempt overdose-prevention centers.    Complex Persistent Benzodiazepine Dependence—When Benzodiazepine Deprescribing Goes Awry JAMA Psychiatry Benzodiazepines remain popular medications among patients due to rapid symptom relief and reinforcing effects. As clinicians and patients become more aware of potential risks, and clinical guidelines increasingly urge caution in prescribing, guidance for benzodiazepine deprescribing is needed. The authors propose a new clinical concept for patients experiencing significant psychological or functional decline during or after a benzodiazepine taper—complex persistent benzodiazepine dependence (CPBD). CPBD can be described as symptomatic or functional decompensation with or without the development of aberrant medication behaviors in the setting of benzodiazepine deprescribing–in the absence of a benzodiazepine use disorder. Further research is needed to validate this concept.    What is Success in Treatment for Opioid Use Disorder? Perspectives of Physicians and Patients in Primary Care Settings JSAT Opioid abstinence and treatment retention are typically used as measures of success of MOUD treatment. This study sought to identify other important treatment outcomes and patient-centered measures of success. Qualitative, structured interviews were conducted with physicians (n=14)  and patients (n=18) in 2 family medicine residency programs. The physicians (7 faculty and 7 residents) were experienced buprenorphine prescribers. Both patients and physicians identified 5 themes: staying sober, and improvement in physical health, mental health, relationships, and role functioning. Patients, but not physicians, identified 2 additional themes: tapering off buprenorphine, and decreased stigma and shame. The authors conclude that “clinicians and researchers need to consider a broader scope of success indicators.”

Some examples may include: Endometriosis Fibroids Sexual Assault Dyspareunia Transgender and LGBTQ sex barriers The internet is rife with anecdotal reports that cannabis use improves sexual experience. According to users, it can boost libido, arousal, and orgasm, among other benefits. But how much data are there to support these benefits? Unfortunately, research on how cannabis affects sexual function is limited, and the studies that exist are generally poor quality. “A lot of them are observational. There haven't been large prospective studies looking at the effects of cannabis or THC on sexual function or dysfunction,” says Saneal Rajanahally, MD, a private practice urologist in Atlanta who specializes in men's health, male reproductive medicine, and surgery. Still, the existing research gives reason to believe cannabis does in fact influence sexual function—both positively and negatively. Survey data consistently suggest that using cannabis has a beneficial impact on libido, sexual pleasure, and orgasm in both sexes. This week, Jill Ellsworth, founder and CEO of Willow Industries, talks about her transformation from running a successful cold-pressed juice company to creating startup Willow Industries, a manufacturer and service provider dedicated to cannabis decontamination.Ellsworth is a registered dietician with a Master's degree in food science nutrition. About seven years ago, she started a cold press juice company in Santa Barbara, California, with another location in Denver. She pivoted the company to a beverage distributor based in Denver, which she eventually sold. An entrepreneur, Ellsworth was looking for her next move. At the same time, cannabis had become legal in Colorado and she found that many of the food safety regulations that shaped her previous venture didn't exist in the nascent industry. While cultivators were required to test batches of flower for potential contaminants, they lacked the resources to remedy any potential problems. Ellsworth saw a need and decided the time was right to found Willow Industries and develop a system for cultivators faced with contamination. The move was also a consumer safety play. In food, the technology is called the kill step, and it's typically the final stage in the food manufacturing process — during which potentially deadly pathogens are removed from a product before it reaches a customer. She wanted to create a kill step for cannabis. Testing requirements have become more robust in the past five years, yet they remain very siloed and state-specific. The kill step remains optional, but Ellsworth says cultivators should expect it to become required with federal legalization. The team at Willow created a system that uses ozone to safely clean cannabis flower and trim without sacrificing quality, flavor or effect. The technology was designed specifically for cannabis. Ellsworth could have adopted technology from other industries and tried to make it work for cannabis, but she wanted to make a system that was appropriate and applicable for cannabis. The result is an organic, gentle solution that doesn't alter terpenes, cannabinoids or structure, and extends product shelf-life.Ellsworth sees her product as an insurance policy that protects companies against product recalls. Recalls can be a brand-killer, and Willow offers companies the ability to market “clean cannabis,” not unlike the USDA organic and Non-GMO verified labels found in the food and beverage industry.The WillowPure system is a plug-and-play machine that cultivators can rent or lease; the company also provides onsite processing as a service. Ellsworth prefers a leasing model to traditional sales. As they continue to innovate at Willow, leases make it easier for customers to upgrade to keep pace with the industry. The system processes 10 to 15 pounds per run, on average, but the amount of product and treatment time depends upon the type of product and amount of contamination. For companies using WillowPure as a kill step, treatment times range from one to four hours. Serious contamination cases can take up to eight hours to process. According to Ellsworth, the ROI turns from service to leasing at about 10,000 square feet of cultivation. At such volumes (and higher), a long-term lease agreement is ideal, but it depends on the market. For example, Oklahoma has many licensees with small batches and plant counts, so Willow's services business does well in the state. In Michigan, Massachusetts and Florida, it makes more sense for huge cultivations to bring a WillowPure system in-house.

Advocate, Educate, Inspire Episode 50: Special Episode 50th MiCannaCast has hit our 50th episode and we are celebrating it by talking about it all.  We discuss our favorite episodes, guests, strains, concentrates and evening dispensaries.  Listen to Cannadave and Groovee talk about everything from their first episode to number 49. This was huge for MiCannaCast as you can imagine because we started out by doing one month or every other month. This year we have been really consistent with content with a new episode out every wednesday afternoon at 4:20 Eastern Time.  We discuss why it's such a huge moment for the co-host that we hit 50 episodes when we did. MiCannaCast Guests We go through all 50 episodes and talk about each one. We've had the Pot Brothers at Law on famous for "The Script" an "Shut the fuck up." We were able to get Marc from Pot Brothers at Law to Recite the Slam Poem!  We had StickRadio on two different podcasts 420 episodes and our Cannabis Culture Music and Movies episode! Stick is big in the Detroit market and having him on helped us learn tips from his radio days.  We also had a good buddy of ours on Drew from Waxxin and talking about online cannabis tips. The Second episode was on getting licenses in the Michigan Market. Other Guests we Had on that we enjoyed learning from were John Generic Fertilizers. He taught us ways on growing and possibly ditching bottle nutrients and using his generic line.  Great information on lighting grows mediums as well. We had Josh K. from Cali on he is a Cannabis Brand Builder huge in marketing and how to grow your brand in the booming industry. North Coast Services was on an episode and taught us that there are businesses working in the cannabis industry who can help you out just like ad agencies due for corp companies. They're like a Cannabis Cultivation Service who can help trim, defoliate, consult and much more. Ashley came on and discussed what it's like working in the industry and delivery service. We had a fan interview us which was an awesome experience Chris B. interview. One of the more common symptoms for people with Parkinson’s is trouble sleeping. Some of us can’t get to sleep, some can’t stay asleep, while others are awaken by tremors, painful dystonia cramping or realistic nightmares.  I can’t stay asleep.  My sleeping journal is more of an awakening journal. One day it reads “up at 3a,” the next “up at 1a,” followed by “up at 3a, up at 3:30a, and up at 4:20a.”  I’ve tried Melatonin, Magnesium Glycinate, light therapy, aromatherapy, silence, music, meditation, reading, more levodopa, later bedtimes, earlier bedtimes and the list goes on. Sleep is important for everyone. When you don’t get enough sleep you are less productive, less attentive, less present, less creative, less active, less everything it seems except tired. WebMD suggests it also puts you at “greater risk for depression, heart attack, stroke, high blood pressure, diabetes and death.” I know you’re not supposed to believe everything you read on the internet, but it’s WebMD. Right? I’m also told by researchers that sleep is the time your brain needs to redistribute chemicals into all of its nooks and crannies and for your mind and body to assimilate after each day’s journey. (Both points seem especially relevant when you have a degenerative brain disease which is triggered by the lack of production of the brain chemical dopamine.) One of my doctors suggested if I don’t get regular REM sleep, I am increasing my risk of a mental meltdown. Needless to say, sleep is an issue that I must address.  My neurologist suggested Mirtazapine, an antidepressant that is also apparently a good sedative. It also causes an increased appetite and weight gain. Neither of which I need right now. I suggested an alternative. As Canada legalizes Cannabis, I asked, “What about CBD Oil?” For transparency, I’m not a pot smoker. I have before, I inhaled, and I liked it well enough. But, I never smoked weed regularly and have not recently.  My neurologist didn’t discourage me and suggested it wouldn’t hurt to try, but it is trial and error. I did some homework. In discussions with users and by reading about CBD Oil, there are a few things I learned.  Cannabis is hit and miss. Some in the Parkinson’s community swear by it and others who’ve tried it never found it to be effective. I’ve heard from several sources it’s a 50/50 proposition. There is no prescription. How much to take, when to take it and how to take it are up to you to discover and debate. It’s fairly intimidating. Some days I feel like I couldn’t make ramen without the instructions on the package. I brought this up to Barinder Rasode, the founder of the National Institute for Cannabis Health and Education. She agreed with me, “I share your frustration about no knowing how much or what type of Cannabis to take.” She had to do trial and error when figuring out her own regiment of CBD oil for her sleeping issues. Rasode is bullish on cannabis as it relates to treating symptoms of Parkinson’s disease, “It reduces both the tremors and some researchers think it actually saves the neurons from further damage caused by Parkinson’s.” She admits those findings are anecdotal at this point, but is hopeful for more scientific trials in the wake of legalization.   The truth is there has been little research on Cannabis and Parkinson’s. So, not much is actually known. However, one trusted medical professional shared with me that while it is very likely CBD oil will make me drowsy, it is less likely to support and promote a regular sleep cycle.  What the heck. I’ll give it a try. Let the adventures into the world of Cannabis begin. Sweet dreams? I hope. Atlanta, GA- Integrated Worldwide Innovations (IWI) will hold the first Cannabis Health and Awareness Seminar in Atlanta, GA on June 24th-25th at the Ramada Suites located at 450 Capitol Ave. The purpose of this seminar is to introduce career and financial investment opportunities, educate individuals on the newly passed laws in their state and the healing properties of this plant. Prior to launching Willow Industries in 2015, Jill founded Vibrant Earth Juices (VEJ), a Santa Barbara-based cold-pressed organic juice company. Named Female Entrepreneur of the Year in Emerging Business in 2013, Jill became passionate about pasteurization techniques, employing High Pressure Processing (HPP) to prevent microbial growth and increase the shelf-life and freshness for her juice line.
