In this episode Katie Oshita, RN, BSN, IBCLC and Mark Persaud, DDS discuss tongue tie both from a parent's and provider's perspective and how all of this led to Dr Persaud and his wife Amelia Persaud to create a multi-therapy office. The strategy here was to allow parents one location, one place to centralize care. More than that, this central design facilities provider communication and true learning from each other. The focus and passion behind his practice is to educate parents and walk them through the TOTs release process in a way that he felt was lacking when he went through these issues as a parent himself. Both of his children exhibited multiple signs and symptoms of TOTs as infants, and despite exhausting consultations with different medical professionals, he felt that there was a missing piece of the puzzle in improving their issues. This sent him and his wife down the rabbit hole of airway focused dentistry and therapy. Through several continuing education courses and a practice that focuses only on TOTs and therapy, Dr. Persaud has a unique exposure that allows him to look beyond the lens of just performing frenectomies, but also understanding holistically, the importance of pre and post therapy rehabilitation and wound management, which he feels truly makes all the difference in providing patients with consistent and predictable results. In this episode, Sachin and Dr. Eniko Loud talk about Dr. Loud's training, background, and interest in applying functional medicine practices to holistic dental care. Dr. Loud explains there is more to dental care than treating cavities and broken teeth. Sometimes teeth break when there is uneven pressure in the bite. Dr. Loud explains how this happens. She covers what kinds of problems mouth-breathing can cause, why there is so much dental crowding and a functional problem with braces. She decided to get certified in functional medicine. She started making an impact on her patient's health, with documents and dietary plans. She was excited about showing up for work. [6:30] Dr. Loud tells how diseases of the body can show up on the tongue seven to 10 years before they show up in the body. She says bad breath is one of the biggest indicators of disease. [8:11] Dr. Loud explains the patient's experience. The first appointment can take up to two hours. Based on functional medicine, there is a lot of listening and gathering of information. Before the appointment, Dr. Loud reviews records, diet, toxin exposure, and sleep habits. She connects all parts of the patient's history with their oral health. Patients are amazed. [9:38] Dr. Loud says this should be the standard of care in dentistry. Dr. Loud is a guide to her patients on their journey. She offers two ways, down the path of disease or the path of health. Most patients choose health. The patient is in the driver's seat and makes all the decisions. Patients receive a report card after their hygienist appointment showing markers of health. [12:15] The standard in Dr. Loud's practice is for patients to have zero bleeding. That is how the patients can measure their success and progress. [13:03] Bleeding gums might signal malnutrition. A lot of times, it indicates a lack of proper care. Mouth-breathing could be an issue or bacterial overgrowth even if they brush and floss properly. Toothbrush heads or mouth appliances may not be properly sterilized, spreading bacteria. Toothbrushes should be changed after two months. Dr. Loud likes electric toothbrushes. [15:15] Dr. Loud likes Oral B iO series brushes. Dr. Loud says it rotates and oscillates so it is very effective at removing plaque. [16:25] To tell if you have a tongue-tie, open your mouth wide and touch the roof of your mouth with your tongue. If you cannot, it is likely you have a tongue-tie. Dr. Loud explains some of the problems a tongue-tie can cause, and how the posture may be affected. Swallowing problems, sleep apnea, and snoring also may result from a tongue-tie. [19:33] Dr. Loud describes a 3-D scan that can show abnormalities from a tongue tie. An oral surgeon can perform a frenectomy to relieve a tongue-tie. [20:35] Dr. Loud recommends myofunctional therapy before and after a frenectomy to prepare the muscle for what it will have to do when the release is done. [22:15] Two different things impact teeth; bacteria and forces. Most patients don't know they have an overload of forces on their teeth. Dr. Loud explains what can happen when a crown is a few microns too high. This may change the joint position. If the forces are not distributed evenly, some teeth may be overloaded, crack, need a crown, have a root canal, and finally, extraction. [26:11] Joint position problems may cause more extractions than tooth decay. There are different schools of thought on bite forces and the joints. It would require the dentist to start asking better questions like, “Why is this happening to my patients?” Wanting to learn to fix it may be the biggest problem. [26:51] Dr. Loud works within a group called OBI Bioesthetic Dentistry and their focus is helping discover the root causes of dental diseases. Once they restore the natural anatomy of the teeth, the face immediately becomes rejuvenated. The focus is function. When you restore jaw joint function, muscles relax and don't clench. There is no more joint pain and muscle tension. [28:33] Orthodontics can aggravate the joint problem if the dentist doesn't know what a stable bite should be or doesn't have the knowledge to see the patterns of tooth wear. Dr. Loud explains the correct bite. [31:13] After Invisalign, if the bite is still not mechanically correct, there are options available to correct the joint position. Dr. Loud explains the process she uses to keep the musculoskeletal system in balance. If there is tooth wear, they add back the enamel that's missing with dental materials to restore the natural form of the tooth structure and stabilize the bite. [33:01] Dr. Loud discusses research that reveals why dental crowding is happening in children. It may be due to diet and mouth-breathing. Dr. Loud recommends working with a myofunctional therapist and a pediatrician to remove dairy and sugar from the diet to reduce inflammation, mouth-breathing, and apnea. [36:11] It wasn't until after Breath, by James Nestor came out that Sachin learned the connection between mouth-breathing and dental problems. Breath is a diagnostic tool. [38:13] About jaw size and wisdom teeth. Sachin's relatives in India chewed on sticks to make toothbrushes. They had perfect teeth, no extractions, no braces, without having access to dentists. Soft foods and processed foods have weakened American teeth. Myofunctional therapists prescribe biting appliances to strengthen the joints and the bite. [42:13] Dr. Loud explores functional medicine and has started a study of ayurvedic medicine. She is preparing to merge it into the functional medicine practice to address the emotional and spiritual parts of a patient, also. [43:41] Dr. Loud shares her thoughts on the Ayurvedic principle of oil pulling. She believes it works by the effects of the herbs and the surface tension of the oil trapping the bacteria. She recommends it for people who have dry mouths. She tells the types she uses. [45:12] Sachin thinks he should make an appointment with Dr. Loud on his next trip to Phoenix! She attended the School of Medicine and Dental Medicine in Romania, Oradea graduating with an MD and DDS degrees in 2000. In 2006 she obtained her doctorate from Case Western Reserve University and completed an AEGD residency, obtaining extensive experience in full-mouth rehabilitation and complex implant dentistry. She went on to complete a one-year Mastership in Implant Dentistry from American Dental Implant Association through Loma Linda University in 2009, then became the Dental Director for Bright Now Dental, overseeing the clinical development of 13 dental offices. In 2013, she completed a Laser Associate Fellowship certification from the World Clinical Laser Institute, and in 2017 she completed a one-year Mastership in Implant Dentistry from the Global Institute for Dental Education. Finally, in 2019 she became a certified functional medicine provider by the Institute of Functional Medicine, one of only 3 certified dentists globally at the time. She received advanced training for applying for functional medicine in clinical practice, bioenergetics, cardio-metabolic diseases, environmental health and detoxification, gastrointestinal diseases, and hormone and immune-related conditions. Following the completion of the IFM certification she attended the Kalish institute 12-month mentorship program for advanced lab interpretation, using lifestyle medicine and lab-based metabolic interpretations as part of a personalized medicine-based practice. She is a member of the American Dental Association, the American Academy for Oral Systemic Health, the Institute of Functional Medicine, and the International Academy of Oral Medicine and Toxicology. Dr. Melissa Seibert is the creator and host of the Dental Digest Podcast and CEO of Dental Digest LLC. Sergio E. Sanchez, DDSAssistant Professor, AEGD Program Director, Craniofacial Sciences, University of Connecticut, School of Dental MedicineDr. Sergio Sanchez was born and raised in Peru. He received his Dental Degree from Universidad Peruana Cayetano Heredia School of Dentistry in 2004. After dental school, Dr. Sanchez completed a Diplomaed certification in Dental Esthetics from the School of Peruvian Dentistry. In 2008, he joined the Advanced Education in General Dentistry (AEGD) residency program at the University of Connecticut (UConn) School of Dental Medicine, where he completed two years of residency with an additional AEGD Fellowship. Immediately following his residency, Dr. Sergio Sanchez became a clinical instructor for the AEGD program at UConn School of Dental Medicine. Part of his responsibilities was serving as a dental provider at Health Care Community Centers. In 2016, he was promoted to Assistant Professor and Program Director of the AEGD residency program.As Program Director of the AEGD residency program, Dr. Sanchez has many administrative responsibilities, including providing clinical supervision and didactic courses for over 30 residents per year. Part of his duties also includes managing the dental Emergency Services offered at UConn School of Dental Medicine and John Dempsey Hospital. Additionally, he is responsible for providing care to patients with acquired and developmental disabilities in the Operating Room at the John Dempsey Hospital. Dr. Jason Genta received his undergraduate degree in Economics from Saint Louis University, and is a 2011 graduate of Creighton University School of Dentistry in Omaha, Nebraska. Following dental school, he completed a 1 year AEGD at UCLA in Westwood California. In 2012, Dr. Genta moved to Chicago where he joined DecisionOne Dental Partners, who at the time, was a group practice with 4 locations. DecisionOne has now grown to 50+ dentists and 31 locations in Illinois and Wisconsin. Since 2016, he has served as Vice President of Clinical Affairs for the group, and plays an active role in Doctor Recruiting, Training, Mentorship, and New Practice Acquisitions and Mergers. Dr. Genta is passionate about the success of young doctors, and teaches a course titled “Life After Dental School” at multiple schools across the country.
Dr. Allington started working in his father’s dental office in Peterborough Ontario at the age of 14. After finishing his BSc and MSc at Queen’s University he did 1 year internship and Hoffmann La Roche in Basel Switzerland. In 2001 he entered McGill Dental School in Montreal Quebec graduating in 2005. Following a 1 year AEGD residency at the University of Connecticut, he returned to Montreal working in variety of practices in Ontario, Quebec and Vermont and teaching part time at McGill. His goal was to get as much experience as possible prior to moving to his ultimate destination Bermuda in 2008. He worked as an associate from 2008 until 2018 when he purchased the practice where he works full time. He has served as the President of the Bermuda Dental Association and is the current Chair of the Bermuda Dental Board. Dr. Allington is a Fellow of the Academy of General Dentistry, Fellow of the ICOI and has been an Invisalign gold plus provider. Recently, he has begun organizing dental continuing education bringing speakers to Bermuda to help educate his office and the rest of the Dental community.

Ashley is one of the nicest, most humble people I have had the pleasure of interviewing on the podcast. His channel has close to 60,000 subscribers and he has published well over 700 videos over the past several years, a massive contributor to our dental community! Dr. Pullen graduated from USC dental school in 1999. He then completed a 1 year AEGD in the Army Dental Corps in 2000. Dr. Pullen served 3 years in Landstuhl, Germany in the U.S. Army and then practiced as a general dentist for two years in Yorba Linda, CA. Dr. Pullen attended the Long Beach VA endodontic program and graduated in 2006. He opened a private practice limited to Endodontics in Brea, CA in 2007 and became board certified in endodontics in 2013. Dr. Pullen lives in Yorba Linda with his wonderful wife of 23 years and has two teenage girls and a 20 year old boy. His hobbies include speaking, surfing, Brazilian JiuJitsu, reading, and golf.
I highly recommend 1 more year of AEGD/GPR training after dental school. You guys learn the basics in dental school, but you don't know much more. You don't know what you don't know!

After working as an associate dentist for just over 2 years Dan recently purchased a practice in Charlotte, North Carolina. We talk about AEGD, AGD and CE courses and whether they are worth it. We also talk about insurance issues from dropping some types of insurance to going entirely fee for service. We discuss the fears around losing patients and possible problems if the economy changes. We also talk about buying a practice, hiring and building a team, turnover, and the anxiety it can cause. You can find our guests here: Will Jones Dentistry Accent Smile Center Show Notes: [01:56] Dr. Ford graduated in 2006. He did a residency in 2007. He filled in for a doctor in East Tennessee. He bought his existing practice in 2009. His practice is Accent Smile center in Dickson, TN. [02:54] He worked over the years to add additional services and create the practice according to his own philosophy. [03:09] He uses technology to help patient outcomes. He also does digital dentistry, laser dentistry, and implants. [03:47] Doctor Jones graduated in 2014. He focuses on surgery and sedation dentistry. He bought his dad's practice, and he is 100% owner, but his father is an associate 2 days a week. [04:24] They are currently struggling with when to add an associate and with hiring the right team members. They are also moving into a new office. [06:19] For Dr. Jones, taking a year long program allowed him to better implement what he earned. [09:04] The benefit for Dr. Ford was placing a lot of implants and the opportunity to hear professors from all around the United States. It also gave him the confidence to go out in private practice and tackle some of those bigger cases. [13:50] Sully thinks a good associateship is the gold standard of what you can do post practice. [16:05] Being reliant on a PPO makes it hard to be reimbursed for advanced dentistry. Maybe there is a middle ground to become less insurance dependent. [19:32] You can't just drop your insurance patients, you also have to have enough new patients to cover that gap. [20:36] The plan is to gradually drop one or two insurance providers first. Patients that value higher level service are what we want. [21:53] Sully believes that the insurance will get them in the door, but he still has an opportunity to do work beyond what the insurance covers. He doesn't want to lose his initial patients in the door. [24:01] Dropping some services, you can look at your adjustments and put that money towards marketing and be ahead of the game. [25:22] Dr. Ford is at the point where he's going to make some decisions about his insurance providers that he accepts. [28:37] Another option is continuing a discount with some patients even if you drop their insurance. [29:20] Dr. Jones shares his struggles with buying his father's practice. He also struggles with discovering what type of boss he wants to be. [31:16] He decided to find a way to be nice, yet let people know that it's his money on the line, and things need to be done a certain way. [32:00] They've hired three new people in the past two months, and it has been great. [33:46] In Dr. Jones's practice the staff turnover has led him to discover what type of boss he wants to be. [35:16] After about six months, Dr. Ford started doing an upgrade on his practice. Not all team members have the same vision. [36:21] Once you get clear on what type of boss you want to be and what your vision is, there are some things that aren't negotiable. [37:52] There are challenges when employees have been doing things a certain way, and you want to upgrade to digital from paper. [39:13] Become clear on what your goals are and then share that vision and that passion. When people aren't on board, that's where the hard conversations come in. [40:16] Being a good leader is something you constantly have to work at. It's also important to surround yourself with good leaders and good mentors. [42:05] Your long-term success comes down to your leadership skills. [43:34] It's important to define roles. [45:44] The more direction you give, the better the team response. [49:51] It's human nature not to listen to the new guy. [52:00] It's realistic to come into a practice and maintain the same team, but you will probably need to add people as you grow. There could be generational issues, and it's always good to take the time to communicate and build relationships. Bill is a 4th year dental student at A.T. Still University’s Missouri School of Dentistry and Oral Health in St. Louis, Missouri and will graduate in May. Bill is a native of the great state of Idaho and graduated with a degree in Exercise Science and Business Management from Brigham Young University. He and his wife, Morgan, are the proud parents of 2 year-old boy/girl twins and are expecting their third child in June. Bill is a recipient of the Army’s Health Professions Scholarship and, following graduation, will attend Basic Officer Leadership Course in San Antonio for 6 weeks before completing a 1-year AEGD at Joint Base Lewis McChord in Tacoma, Washington. Nicholas K. Roy, DMD, FAGD graduated from University of Connecticut School of Dental Medicine in 2007 and participated in an AEGD at the VCU school of dentistry the following year. Dr. William Nudera earned a DDS from the University of Illinois at Chicago, College of Dentistry in 1999. Upon graduation, he served in the United States Air Force Dental Corps where he completed a one year AEGD residency program. During his time of service, Dr Nudera received a National Defense Service Medal for his involvement in “Operation Enduring Freedom” and was awarded the title of Air Force Dental Office of the Year in 2002. Following his service, Dr. Nudera returned to Chicago where he continued to practice general dentistry while pursuing an education in Endodontics at the University of Illinois at Chicago. Dr. Nudera completed his Endodontic training in 2005 earning his Endodontic specialty certificate as well as a Masters Degree in Oral Sciences. Dr. Nudera is a Diplomate of the American Board of Endodontics. He has several national and international publications including multiple scientific research contributions to the Journal of Endodontics and a chapter in the endodontics textbook 3D Imaging in Endodontics. Dr. Nudera maintains a full-time private practice limited to Endodontics at Specialized Endodontic Solutions in the Chicago suburb of Bloomingdale, Illinois. In addition to his full-time practice, Dr. Nudera speaks nationally and internationally on a variety of Endodontic topics. Dr Nudera currently holds a faculty position in the Department of Endodontics at the University of Illinois at Chicago, College of Dentistry. Dr. Nudera is a member of the A.B.E. Anthony became interested in dentistry when he was 9 years old and had his one and only filling done. His dentist was one of his mom’s best friends; so, he was always felt very welcome at the practice. Dr. Carbonella shadowed him in high school for 100 hrs and became completely hooked. He went to UConn undergrad with every intention of getting accepted into UConn School of Dental Medicine. The summer before dental school started, he was offered a USAF scholarship for school. After graduating, Dr. Carbonella went onto active duty, did AEGD residency in California, got lots of good training in every specialty, then was assigned to McGuire Air Force base in New Jersey. He’s been at McGuire for the past 3.5 years and has been moonlighting for the past year for private practice experience and to save some extra cash. In the past has lectured to younger "airmen" on the benefits of retirement planning and being smart with money. It's a passion of his. Next spring he is leaving for San Antonio Texas where he will start the TORP (Tri Service Ortho Residency Program, Army, navy, and Air Force). Other than that, he is the oldest child of 4 boys, who all went to UConn. He is engaged to his lovely fiancé Angela who is a jersey girl through and through, Columbia University graduate, and a child psychologist. I grew up in Indiana, moved to SLC as senior in high school. Geoff was born and raised in Vancouver Canada, but ended up going overseas to Sydney Australia where he did his dental degree. He sought to return to North American soil by completing a GPR and an AEGD, then working at a DSO for 3 years. During that time, he married an Australian and has returned to Sydney to start a family. Geoff has been in private practice in Australia now for 4 years. Geoff has done some training in Orthodontics under world renowned Derek Mahony, some basic implant training with Zimmer, and his restorative influence is from Frank Spear.
