The world of prehospital medicine is constantly evolving, driven by new research, technological advancements, and a shared commitment to improving patient care and provider well-being. As EMS professionals, staying informed about these developments goes beyond a professional obligation; it is an opportunity to improve our practice, champion our profession, and ultimately make a greater impact on saving lives. In this article, we will explore some of the latest research findings that are reshaping our field, from workplace culture to cutting-edge technology.   The Culture of Care: Supporting EMS Providers Our work is demanding, both physically and emotionally, and the culture within our agencies plays a critical role in our well-being. A recent systematic review in the International Journal of Environmental Research and Public Health revealed that many EMS providers avoid using organizational mental health services due to stigma and a perception that these programs lack genuine care. The study emphasizes the need for person-centered support and a cultural shift that normalizes seeking help as a sign of strength (Johnston et al., 2025). This cultural component also impacts retention. Another study in the same journal found that agencies with collaborative, team-oriented "clan" cultures had significantly lower turnover rates compared to those with rigid or chaotic structures. For leaders in EMS, fostering a supportive environment is not just about morale. It is a strategic imperative for retaining skilled clinicians (Kamholz et al., 2025).   Professional Recognition: Breaking Barriers Across the globe, paramedics are striving for recognition as integrated healthcare professionals. A qualitative study in BMC Health Services Research identified common barriers, including outdated legislation, inconsistent regulation, and insufficient funding. While the pandemic temporarily highlighted our capabilities, the momentum has waned. The study calls for targeted policy reforms and investments in education and leadership to solidify our role in the broader healthcare system (Feerick et al., 2025). Physical Demands and Injury Prevention The physical toll of our work is undeniable. A scoping review in Applied Ergonomics confirmed that musculoskeletal injuries, particularly to the back, are rampant in EMS. Tasks like handling stretchers and patient extractions are among the most strenuous. The review also highlighted fitness disparities, with male paramedics generally showing more strength but less flexibility than their female counterparts. These findings underscore the need for targeted injury prevention programs and realistic physical standards to keep us safe throughout our careers (Marsh et al., 2025).   Advancements in Cardiac Arrest Care When it comes to cardiac arrest, every second counts. A study in Resuscitation reinforced the value of bystander CPR, showing that dispatcher-assisted CPR significantly improves outcomes for untrained bystanders. For those with prior CPR training, acting independently yielded even better results. This highlights the importance of public CPR education alongside dispatcher support (Tagami et al., 2025). On the scene, our interventions matter immensely. Research in The Journal of Emergency Medicine found that for traumatic cardiac arrest patients, aggressive interventions like prehospital thoracostomy can be lifesaving (McWilliam et al., 2025). Meanwhile, a study in Critical Care Medicine revealed that extracorporeal CPR (ECPR) significantly improves outcomes for patients with refractory ventricular fibrillation, emphasizing the need for early transport to specialized centers.   The Role of Technology in EMS Technology is poised to revolutionize EMS, from dispatch to diagnosis. A study in The American Journal of Emergency Medicine demonstrated that large language models (LLMs) like ChatGPT could prioritize ambulance requests with remarkable accuracy, aligning with expert paramedic decisions over 76 percent of the time. This proof of concept suggests that AI could one day enhance resource allocation in dispatch centers (Shekhar et al., 2025). On the diagnostic front, machine learning is opening new possibilities. For example, a study in Bioengineering showed that analyzing photoplethysmography waveforms could estimate blood loss in trauma patients, offering a non-invasive way to guide resuscitation (Gonzalez et al., 2025). Similarly, research in Medical Engineering & Physics explored using multidimensional data to differentiate ischemic from hemorrhagic strokes in the field, potentially enabling more targeted prehospital care (Alshehri et al., 2025).   Addressing Disparities in Care Equity in EMS is a cornerstone of our profession, yet recent studies highlight troubling disparities. Research in JAMA Network Open found that ambulance offload times were significantly longer in communities with higher proportions of Black residents (Zhou et al., 2025). Another study in JAMA Surgery revealed that Black and Asian trauma patients were less likely to receive helicopter transport compared to White patients. These findings are a call to action for all of us to examine our systems and biases to ensure equitable care for every patient (Mpody et al., 2025).   Looking Ahead The research discussed here represents just a fraction of the advancements shaping EMS today. From improving workplace culture and injury prevention to leveraging AI and addressing systemic inequities, these findings have real-world implications for our protocols, training, and advocacy efforts. As EMS professionals, we have a responsibility to stay informed and apply these insights to our practice. Why should we listen to you when you talk about this topic?   Alexander McWilliam   Well, we said my name is Alex, which is true, but my full title is Doctor Alexander McWilliam. I've got a PhD in public speaking anxiety and performing under pressure. To my knowledge, the only person in the UK potentially globally with a PhD specialising in this field, and I've been coaching presentation skills confidence when presenting for almost  decades, so a wealth of experience. It makes me feel old, but. An expert in the field of anxiety and performing under pressure.   Darren A. Smith   Two decades we haven't even got any grey hair. What's that about? OK, so you're the only one with the PhD potentially in the world in this. So just tell me about that. How long did you study for to get this?   Alexander McWilliam   So my PhD took  1/ years in total, so it was three years. Multiple studies I looked at AI looked at all the interventions available for public speaking anxiety into reduction. That was one area of study. I developed a questionnaire to help identify specific public speaking concerns because there's so many in the world. But actually when we're coaching, we need to identify what it is specifically.   Darren A. Smith   Yeah.   Alexander McWilliam   And then a last study was using acting and improvisation to help reduce public speaking anxiety. So over the three years, there was all of these studies going on with multiple participants trying to figure out why they get anxious. How do we overcome that? And as there's so many interventions available, can something alternative, like acting in improv, which hasn't seen many mainstream sort of interventions, you see CBT, you see exposure therapy, things like that. But acting in improv, that was the unique route I wanted to go down and bring on my own expertise of being an actor and being an improviser.   Darren A. Smith   Yeah. Yep.   Alexander McWilliam   With that so 3 1/2 years of lots and lots of studying, lots of reviewing, lots of writing, and if anyone's doing it, if anyone's done it, they know that it is a mammoth of a task. You did research. You looked at research you practise it. Chronic pain can be incredibly debilitating, no matter the circumstances. But what do you do when every possible scan or investigation by medical professionals shows that there is no injury, no degeneration, absolutely nothing visible to warrant the often excruciating pain you're experiencing?  This was the experience of beloved 89.9 The Light breakfast host Kel McWilliam. The first sign of chronic back pain that would plague him for years was when he fell to the floor in agony, alone at home and unable to move. Eventually rushed to the emergency department on this occasion and others, he was admitted for days, needing strong pain medication to feel any relief.  After years of searching for answers two important health professionals steered Kel towards considering his mental health as the cause of his pain. This was start of Kel's difficult journey to face significant emotional challenges he'd closed himself off from. Dr. Robin McWilliam is a professor of special education at The University of Alabama . Dr. McWilliam is also the Founder and Director of the Evidence-based International Early Intervention Office (EIEIO). His research interests are in early intervention (birth to age 6), specifically models of service delivery, working with families, and child engagement. Dr. McWilliam developed the Routines-Based Model of Early Intervention (Birth-6) and am involved in its implementation in Australia, Canada, New Zealand, Paraguay, Poland, Portugal, Singapore, Spain, Taiwan, and the U.S. Dr. McWilliam developed the international community of practice called The RAM Group, to provide research and technical assistance on the Routines-Based Model. Dr. McWilliam teaches doctoral students, lectures, conducts workshops, and writes. He serve as the International Editor of the scholarly journal Infants & Young Children and serves on numerous editorial boards.

2020 was a vintage from hell for Mount Pleasant chief winemaker, Adrian Sparks. On January 7 he made the tough decision not to make any wine from that year's crop, due to widespread smoke taint. The next day, he found out parent company McWilliam's was going into administration. Going on three years later, Adrian joins us on the Drinks Adventures podcast with nothing but good news from Mount Pleasant. You heard from the Calabria family last season why they didn't want to take on Mount Pleasant as part of their acquisition of McWilliam's Wines. The companies went separate ways after 80 years, and the Hunter Valley winery now has a new lease on life under the ownership of investment group Medich Family Office. The Mediches have gone all in to revitalise the brand, funding a renovation for the cellar door, new viticulture equipment and an optical berry sorter that most winemakers can only dream of. Even the labels have had a revamp, with founder Maurice O'Shea's family crest back on the bottle.  I started by asking Adrian to fill in the gaps since I last reported on Mount Pleasant in my 2020 podcast documentary on The Rise and Fall of McWilliam's Wines.

Thanks to Calabria's investment, McWilliam's has relaunched its Hanwood Estate range of tawny fortifieds with refreshed branding that takes some cues from whisky, with age statements front and centre. McWilliam's senior winemaker Russell Cody and assistant winemaker Mel McWilliam are ‘custodians' of the fortified barrel collection at the Hanwood Winery, including the 30-Year Old material that is considered Very Rare. In this special episode, produced in partnership with McWilliams, Russell and Mel talk about the resurgence of these historic wines, and the evolution at Hanwood to making Tawny fortified in a slightly drier style.  You'll learn how tawny is best consumed, and why you shouldn't call it ‘port' – which Russell himself is still wrestling with. In this episode, you'll meet the winery's new owner, Calabria Family Wines, which was established in the Riverina in 1945 by Italian immigrants Francesco and Elisabeth Calabria, and is today led by second generation family member, Bill Calabria AM. Bill is still on the winemaking team today, and he joins us this episode along with his sons Andrew Calabria, the company's sales and marketing manager, and Michael Calabria, general manager. The year 2022 was pivotal for Calabria, as the company acquired the McWilliam's Wines brand and Hanwood Winery along with Deakin Estate and La La Land Wines, cementing it among the largest family-owned wine groups in Australia. Vintners & Co. Calabria also appointed sixth generation family member, Scott McWilliam, as global brand ambassador for the McWilliam's brand. And first up this episode, I asked Scott what had happened since that supposed 2020 sale of McWilliam's, which seemed like a done deal.

There are a few reasons, one of which is that the ASHA Code of Ethics includes several components connecting goals and objectives to effective services. And you can't have goals and objective without our dear friend, data.Data is a common thread that weaves itself throughout the crafting, implementation, and analysis of whether or not somebody met their goals and objectives. You need data about what a person can currently do in order to write goals that are achievable and meaningful, you need to collect data to assess the efficacy of your treatment, and data helps you determine whether or not those goals were met.There are a few guidelines to help us in this goal and objective writing process. Enter the SMART goal.
