Larry Ostola speaks with Nick Etheridge and Phil Calvert about their book, Supervising a Peace that Never Was: Recollections of Canadian Diplomatic Personnel in Indochina, 1954–1973. Supervising a Peace that Never Was: Recollections of Canadian Diplomatic Personnel in Indochina, 1954–1973 is a collection of essays written by former Canadian diplomats who served on international commissions in Indochina. These individuals were initially deployed to oversee the implementation of the 1954 Geneva Accords (through the International Commission for Supervision and Control, or ICSC) and later, in 1973, to monitor compliance with the Paris Peace Agreement (under the International Commission of Control and Supervision, or ICCS). Over the course of two decades, young Canadians were posted to Vietnam, Cambodia, and Laos—regions in which Canada had previously played a minimal role. The chapters that follow reflect their personal memories and experiences during these diplomatic missions. Read the full book here: https://www.uvic.ca/research/centres/capi/assets/docs/books/supervising-a-peace-that-never-was.pdf Nick Etheridge joined External Affairs in 1967 and held postings in Australia, Vietnam, and Europe, including roles in the CSCE. He served as Canada's representative in Cambodia (1993), Chargé d'Affaires in the Baltic States (1993–95), and High Commissioner to Bangladesh (1996–99). He retired in 2002 as Director of the Defence Relations Division. Phil Calvert joined the Department of External Affairs in 1982 and served in Beijing during three postings. He held key roles in trade negotiations, including as Deputy Chief Negotiator for China's WTO accession, and was Ambassador to Thailand, Cambodia, and Laos from 2011 to 2016. He retired in 2016. In this episode, we explore some of the neural and cellular mechanisms underlying gastrointestinal motility, emphasizing the roles of the enteric nervous system (ENS) and interstitial cells of Cajal (ICCs) in coordinating smooth muscle contractions throughout the digestive tract. We detail disruptions in motility seen in dysautonomia, intestinal methanogen overgrowth (IMO), and small intestinal bacterial overgrowth (SIBO). And, we finish with a brief discussion on the impacts of low stomach acid. Topics: 1. Introduction to Gastrointestinal Motility - Coordinated contractions of smooth muscle that move food, liquid, and waste through the gastrointestinal (GI) tract. - Neural, hormonal, and microbial factors. - Intestinal methanogen overgrowth (IMO) and small intestinal bacterial overgrowth (SIBO). 2. Neural Regulation of Gut Motility - The enteric nervous system (ENS) embedded in the walls of the GI tract. - The ENS, central nervous system (CNS), and vagus nerve. - The myenteric plexus governs motor control and peristalsis, while the submucosal plexus regulates secretion, absorption, and blood flow. - Stress and motility 3. Cellular Regulation of Gut Motility - Interstitial cells of Cajal (ICCs), pacemaker cells located near smooth muscle cells. - ICCs generate slow-wave electrical activity. - Acetylcholine enhances smooth muscle contraction. 4. Dysmotility and Potential Contributing Factors - Impaired ICC function, smooth muscle dysfunction, nervous system dysfunction. - Dysautonomia, a dysfunction of the autonomic nervous system. - External stressors, hormonal imbalances, and more. 5. Intestinal Methanogen Overgrowth (IMO) - Characterized by an overgrowth of methane-producing archaea in the intestines. - Excess methane production affects smooth muscle activity and can slow intestinal transit. - Symptoms such as bloating, abdominal discomfort, constipation. - Breath testing. 6. Small Intestinal Bacterial Overgrowth (SIBO) - Excessive bacterial colonization in the small intestine, disrupting normal gut function. - Protective mechanisms including stomach acid and intestinal motility. - Low stomach acid (hypochlorhydria) reduces the acidic environment needed to inhibit bacterial colonization. - Bloating, diarrhea/constipation, and more. 7. Overlap Between IMO and SIBO - Excess hydrogen produced in SIBO can serve as a substrate for methane production in IMO. - Can exacerbate each other, creating a loop that worsens symptoms. 8. Addressing Dysmotility and Overgrowth - Determining the root cause of dysmotility. - Dysmotility can involve multiple regions of the GI tract. Thank you to our episode sponsors: 1. It's well-documented that international assignments for dual-career couples can be both exhilarating and complicated at the same time. Many organizations with international talents have not fully understood the complexities of dual international career couples in their workforce. This is what usually happens: One career takes precedence over the other Two partners equally sub-optimize their careers They individually pursue two careers at the expense of the family.   Often, as dual international career couples, we tend to focus on the challenges and issues that arise in creating meaningful and fulfilling lives and careers. This episode offers valuable solutions and explores the keys to success for dual-career couples living abroad. Guests Jannie Aasted Skov-Hansen and Dr. Paul Vanderbroeck, are the co-authors of the groundbreaking book, "The International Career Couples Handbook”, which explores real-life challenges of dual career couples and provides a new way to approach international mobility. It provides a good structure for the deep reflections and discussions you need to have as an international Career couple. Based on their lived experiences and extensive research this book offers great insights for both ICCs and organizations. Both are partners in an ICC ( International career couple) and parents. Jannie is also the Founder of Here We Are Global, a startup consultancy dedicated to supporting ICCs in their global careers. Paul is an Executive Coach and accomplished researcher in gender balance and leadership development.  As I've mentioned in the podcast previously, my husband and I hold the same degree, and I have over 15 years of experience working with international organizations This handbook and Jannie's and Paul's knowledge provide powerful tools to navigate dual international careers with more ease. I highly recommend reading this handbook and so some of the powerful exercises they share.   Here are the top highlights you'll learn in this episode:   The difference between an international career couple and a dual-career couple  Gaps that International Organizations overlook when it comes to international career couples  (ICC's) Four unique challenges that ICC's face,  How to overcome them, and create lasting success    Juggling the packing, decisions, and career moves can make one feel uneasy. Abstract: The majority of human connectome studies in the literature based on functional magnetic resonance imaging (fMRI) data use either an anterior-to-posterior (AP) or a posterior-to-anterior (PA) phase encoding direction. However, whether and how phase encoding direction would affect test-retest reliability of functional connectome is unclear. Here, in a sample of healthy subjects with two sessions of fMRI scans separated by 12 weeks (two runs per session, one with AP, the other with PA), we tested the influence of phase encoding direction on global and nodal connectivity in the constructed brain networks. All data underwent the state-of-the-art Human Connectome Project (HCP) pipeline to correct for phase-encoding-related distortions before entering analysis. We found that at the global level, the PA scans showed significantly higher intraclass correlation coefficients (ICCs) for global connectivity compared with AP scans, which was particularly prominent when using the Seitzman-300 atlas (versus the CAB-NP-718 atlas). At the nodal level, regions most strongly affected by phase encoding direction were consistently mapped to the cingulate cortex and temporal lobe, with significantly higher ICCs during PA scans compared with AP scans, regardless of atlas. Further, we demonstrated that the observed reliability differences between phase encoding directions may relate to a similar effect on the reliability of temporal signal-to-noise ratio (tSNR) in the same regions (that PA scans were associated with higher reliability of tSNR than AP scans). Averaging the connectivity outcome from the AP and PA scans could slightly, but overall have limited value to boost the ICCs. These results were largely replicated in an independent, public dataset from the HCP-Early Psychosis (HCP-EP) study with a similar design but a much shorter scan session interval. Our findings suggest that phase encoding direction has significant effects on the reliability of connectomic estimates in fMRI studies. We urge that these effects need to be carefully considered in future neuroimaging designs, especially in longitudinal studies such as those related to neurodevelopment or clinical intervention. You can't possibly maintain a large network of contacts, so it's important to focus your efforts on the people who are most likely to be aligned with your goals and objectives. Find out how to identify your Inner Circle Contacts as Juliette shares how to efficiently prioritize your time and energy with these people in order to focus on the goals that you establish for yourself to achieve. Highlights03:14 In order to increase your effectiveness, you must create or tap into networks that are both strategic and productive. 05:13 You must learn to say no to low-value or no-value activities that do not align with your goals and you must learn to say yes to high priority action items that move you towards your objectives.10:27 Make sure you are prioritizing your ICCs, your Inner Circle Contacts, but first you have to identify them and cultivate those relationships. 12:31 Sometimes it's tough to know if you're going to get alignment. Somebody who could be new to your network could be very much aligned with you. So, what we're going to do with these groups is communicate. 13:45 Prioritize yourself. Prioritize your goals. Abstract: White matter hyperintensities of presumed vascular origin (WMH) are frequently found in MRIs of patients with various neurological and vascular disorders, but also in healthy elderly subjects. Although automated methods have been developed to replace the challenging task of manually segmenting the WMH, there is still no consensus on which validated algorithm(s) should be used. In this study, we validated and compared three freely available methods for WMH extraction: FreeSurfer, UBO Detector, and the Brain Intensity AbNormality Classification Algorithm, BIANCA (with the two thresholding options: global thresholding vs. LOCally Adaptive Threshold Estimation (LOCATE)) using a standardized protocol. We applied the algorithms to longitudinal MRI data (2D FLAIR, 3D FLAIR, T1w sMRI) of cognitively healthy older people (baseline N = 231, age range 64 - 87 years) with a relatively low WMH load. As a reference for the segmentation accuracy of the algorithms, completely manually segmented gold standards were used separately for each MR image modality. To validate the algorithms, we correlated the automatically extracted WMH volumes with the Fazekas scores, chronological age, and between the time points. In addition, we analyzed conspicuous percentage WMH volume increases and decreases in the longitudinal data between two measurement points to verify the segmentation reliability of the algorithms. All algorithms showed a moderate correlation with chronological age except BIANCA with the 2D FLAIR image input only showed a weak correlation. FreeSurfer fundamentally underestimated the WMH volume in comparison with the gold standard as well as with the other algorithms, and cannot be considered as an accurate substitute for manual segmentation, as it also scored the lowest value in the DSC compared to the other algorithms. However, its WMH volumes correlated strongly with the Fazekas scores and showed no conspicuous WMH volume increases and decreases between measurement points in the longitudinal data. BIANCA performed well with respect to the accuracy metrics - especially the DSC, H95, and DER. However, the correlations of the WMH volumes with the Fazekas scores compared to the other algorithms were weaker. Further, we identified a significant amount of outlier WMH volumes in the within-person change trajectories with BIANCA. The WMH volumes extracted by UBO Detector achieved the best result in terms of cost-benefit ratio in our study. Although there is room for optimization with respect to segmentation accuracy (especially for the metrics DSC, H95 and DER), it achieved the highest correlations with the Fazekas scores and the highest ICCs. Its performance was high for both input modalities, although it relies on a built-in single-modality training dataset, and it showed reliable WMH volume estimations across measurement points. Link to bioRxiv paper: http://biorxiv.org/cgi/content/short/2020.06.12.149203v1?rss=1 Authors: Buser, N. J., Madan, C. R., Hanson, J. Abstract: On-going, large-scale neuroimaging initiatives have produced many MRI datasets with hundreds, even thousands, of individual participants and scans. These databases can aid in uncovering neurobiological causes and correlates of poor mental health, disease pathology, and many other important factors. While volumetric quantification of brain structures can be completed by expert hand-tracing, automated segmentations are becoming the only truly tractable approach for particularly large datasets. Here, we assessed the spatial and numerical reliability for newly-deployed automated segmentation of hippocampal subfields and amygdala nuclei in FreeSurfer. In a sample of participants with repeated structural imaging scans (N=118), we found numerical reliability (as assessed by intraclass correlations) to be generally high, with 92% of the subregions having ICCs above 0.90 and the remainder still above 0.75. Spatial reliability was lower with only 11% of regions having Dice coefficients above 0.90, but 70% with Dice coefficients above 0.75. Of particular concern, three regions, the hippocampal fissure, the anterior amygdaloid area, and the paralaminar nucleus, had only moderate spatial reliability (0.50-0.75). We also examined correlations between spatial reliability and person-level factors (e.g., age, inter-scan interval, and difference in image quality). For these factors, interscan interval and image quality were related to variations in spatial reliability. Examined collectively, our work suggests strong numerical and spatial reliability for the majority of hippocampal and amygdala subdivisions; however, caution should be exercised for a few regions with more variable reliability.

«International Criminal Court, is in fact an International Caucasian Court for the persecution and humiliation of people of colour», slo han fast. Denne typen kritikk har blitt veldig vanlig blant noen av de mest toneangivende lederne på det afrikanske kontinentet. ICCs rolle sto i sentrum for begivenhetene da president Yahya Jammeh i desember først aksepterte valgtapet i Gambia, for så å snu etter at valgvinneren erklærte at Gambia ville beholde sitt ICC-medlemskap. Jammeh reiste senere til ikke-ICC-medlemmet Ekvatorial-Guinea. Hvordan preger ICC politikken i afrikanske land?

Utility of electronic patient records in primary care for stroke secondary prevention trials

Medizin - Open Access LMU - Teil 18/22

Background: This study aimed to inform the design of a pragmatic trial of stroke prevention in primary care by evaluating data recorded in electronic patient records (EPRs) as potential outcome measures. The study also evaluated achievement of recommended standards of care; variation between family practices; and changes in risk factor values from before to after stroke. Methods: Data from the UK General Practice Research Database (GPRD) were analysed for 22,730 participants with an index first stroke between 2003 and 2006 from 414 family practices. For each subject, the EPR was evaluated for the 12 months before and after stroke. Measures relevant to stroke secondary prevention were analysed including blood pressure (BP), cholesterol, smoking, alcohol use, body mass index (BMI), atrial fibrillation, utilisation of antihypertensive, antiplatelet and cholesterol lowering drugs. Intraclass correlation coefficients (ICC) were estimated by family practice. Random effects models were fitted to evaluate changes in risk factor values over time. Results: In the 12 months following stroke, BP was recorded for 90%, cholesterol for 70% and body mass index (BMI) for 47%. ICCs by family practice ranged from 0.02 for BP and BMI to 0.05 for LDL and HDL cholesterol. For subjects with records available both before and after stroke, the mean reductions from before to after stroke were: mean systolic BP, 6.02 mm Hg; diastolic BP, 2.78 mm Hg; total cholesterol, 0.60 mmol/l; BMI, 0.34 Kg/m(2). There was an absolute reduction in smokers of 5% and heavy drinkers of 4%. The proportion of stroke patients within the recommended guidelines varied from less than a third (29%) for systolic BP, just over half for BMI (54%), and over 90% (92%) on alcohol consumption. Conclusions: Electronic patient records have potential for evaluation of outcomes in pragmatic trials of stroke secondary prevention. Stroke prevention interventions in primary care remain suboptimal but important reductions in vascular risk factor values were observed following stroke. Better recording of lifestyle factors in the GPRD has the potential to expand the scope of the GPRD for health care research and practice.
