More and more Canadians are unable to access public primary healthcare, according to a study published in the Canadian Medical Association Journal at the beginning of December. In fact, about 20% of Canadians have no family doctor at all, and many more have irregular access to clinicians. The CMAJ study compares the Canadian primary care system with New Zealand and eight countries in Europe including France, Germany, Italy and the UK. Dr Tara Kiran is the senior author of the study and a family physician and scientist at St. Michael's Hospital and the University of Toronto.

Dr. Teodor Grantcharov completed his surgical training at the University of Copenhagen, and a doctoral degree in Medical Sciences at the University of Aarhus in Denmark. Dr. Grantcharov is a Professor of Surgery at Stanford University and Associate Chief Quality Officer for Innovation and Safety at Stanford Healthcare. Prior to joining Stanford, Dr. Grantcharov was a Professor of Surgery at the University of Toronto and Keenan Chair in Surgery at St. Michael's Hospital in Toronto. He wasthe Founder of the International Centre for Surgical Safety – a multidisciplinary group of visionary scientists with expertise in design, human factors, computer- and data science, and healthcare research. He previously held Canada Research Chair in Simulation and Surgical Safety and was awarded the Queen Elizabeth II diamond jubilee medal for his contributions to clinical research and patient safety in Canada. Dr. Grantcharov was awarded the honorary fellowship of the Imperial College in London, the honorary fellowships of the Bulgarian, Danish and Brazilian surgical societies, the Spinoza Chair in Surgery from the University of Amsterdam and multiple national and international awards for his contributions to surgical education and surgical safety. Dr. Grantcharov's clinical interest is the area of minimally invasive surgery, while his academic focus is in the field of surgical innovation and patient safety. He has become internationally recognized as a leader in this area with his work on curriculum design, assessment of competence and impact of surgical performance on clinical outcomes. So how do you prepare and then navigate the storm once you're in the thick of it?In today's episode, we start with a discussion with Thom Mayer, MD exploring strategies to steel yourself for the inevitable chaos, underscoring the importance of maintaining a positive, agentic, proactive mindset. Vincent Santiago's research interests include eating behaviors, body image, and bariatric surgery. Vincent's doctoral dissertation explores the use of an intervention for food addiction (the concept that individuals can be addicted to foods, particularly highly processed foods). This intervention combines Adapted Motivational Interviewing, a person-centered counseling approach, and Cognitive Behavioural Therapy (CBT) skills for eating-related issues and will be tested in a randomized controlled trial. Vincent's Master's thesis explored psychosocial factors (i.e., adult attachment style, emotion regulation, and psychopathology) related to cigarette smoking following bariatric surgery. For his undergraduate thesis, he studied the influence of video messages on healthier eating among students who engaged in restrained and unrestrained eating. He previously worked in research positions at Toronto Western Hospital's Bariatric Surgery Program and the Centre for Addiction and Mental Health. Clinical Interests Vincent's clinical interests include the assessment and treatment of various mental health issues, including eating disorders, anxiety disorders (e.g., social anxiety disorder), and obsessive-compulsive disorder (OCD). Vincent is particularly interested in using approaches from CBT, Dialectical Behaviour Therapy, and Motivational Interviewing. Vincent has completed practicum placements at 1) the Psychology Training Clinic within the Department of Family and Community Medicine at St. Michael's Hospital; 2) the Intensive Residential and Day Treatment Program for OCD at Sunnybrook Health Sciences Centre; and 3) the Day Hospital Treatment, Eating Disorder Program at Toronto General Hospital. Vincent is also involved as a study therapist in clinical trials providing CBT related to post-bariatric surgery issues, as well as social anxiety disorder, substance use, and HIV sexual risk behavior. Professional and Community Involvement Vincent is committed to fostering the growth of psychology and those interested in the field. He is involved in mentorship programs with undergraduates, graduate students, and early career psychologists as part of the Ryerson Psychology Graduate Students' Association (PGSA), the Canadian Psychological Association (CPA), and the American Psychological Association (APA). He is also serving a 2-year term as a Student Representative in the Psychologists in Hospitals and Health Centres section of the CPA. Vincent co-organized the inaugural “Diversifying Psychology Recruitment Event” at Ryerson in 2020. The goal of this event was to provide more information about applying to graduate school for students from traditionally underrepresented groups, particularly students identifying as Black, Indigenous, and a Person of Colour. Dr. Rosanne Mills and I first met in med school in Calgary and then both did residency in Toronto. After that I went into emergency medicine while she pursued psychiatry but we have remained as close as ever. Rosanne and I have a conversation about the realities of anxiety and depression and how to help our mental health. We address the question of what anxiety and depression actually look like and how to tell the type that needs treatment apart from times that are really just part of being a human being.I want to say, first of all, that though Dr. Rose and I are both physicians, we are not giving out personal health advice in this episode. Rosanne defines depression according to the DSM, the Diagnostic Statistical Manual, and we talk about how a significant depressive episode can cause dysfunction in our lives in some way. We also discuss how anxiety is normal and healthy until it starts dragging at daily life and prevents us from visting people, making phone calls, and the like. Dr. Rose offers a lot of insight into how depression and anxiety both manifest, when they are emergency issues, and explains different kinds of medication available. She also discusses medication side effects and modalites outside of medication that can help mitigate depression. Sylvi applies a weight-inclusive approach to helping individuals experiencing mental health related challenges and disordered eating behaviour.  Sylvi maintains dual practises in nursing and naturopathic medicine. After completing her Bachelor of Science in Nursing at Laurentian University in 2004, she practised acute care psychiatric nursing at St. Michael's Hospital in Toronto where she also completed a nursing research fellowship. She went on to graduate from the Canadian College of Naturopathic Medicine in 2009 after which she began her private naturopathic practice in Toronto. In addition to her naturopathic practise, Sylvi maintained her nursing practise by working on a mental health team for an Ontario health regulator where she was also part of a mental health response team for individuals in crisis. She has also completed additional training in Cognitive Behavioural Therapy for Eating Disorders. In speaking to the ways in which the healthcare crisis is both the result and rationale for erosion and greater privatization, JP Hornick says: “...you've got a kind of perfect storm that COVID maximized a crisis that was already existing. Decreased funding, unanticipated service costs, then you throw in on top of that supply-chain issues, particularly around PPE, but also, tools and resources. And then you've got an ongoing problem with recruitment and retention of staff. These are the new realities that hospitals have to tackle at the same time, trying to care for patients. And they're not allowed to carry deficits. So in the absence of additional funding, it means that the choices that hospitals regularly resort to are service cuts in order to try and balance budgets.But all this comes back to a very simple thing. It is a failure to adequately fund healthcare, both before and during the pandemic. And then the deterioration of the public system that results is used as an excuse in itself for increased privatization.” According to Dosani: “...often conversations like this can get tucked away in the category of health policy. This is about much more. This is about our way of life. This is about our way of being. This is really an attack on the common good that is so core and foundational to what it means to be Canadian…Public healthcare is a national treasure that makes us unique on the world stage. That is worth protecting.” About today's guest:  Prior to being elected president of OPSEU/SEFPO (Ontario Public Service Employees Union), JP Hornick was chief steward for more than a decade within the College Faculty Division, representing instructors at Ontario's 24 public colleges. She chaired the College Faculty bargaining teams in 2017 and 2021-22.  JP grew up in a family of public service workers – educators, correctional workers and many more. She is a labour educator and previously served as the coordinator of the School of Labour at George Brown College. She has served as chair, treasurer and director on several community boards and has been on the front lines of activism to advance issues of equity, women's rights, LGBTTIQQ2S rights, anti-racism and decolonization.   JP was elected OPSEU/SEFPO President in April 2022 on a platform that includes strengthening union democracy, building bargaining power, ensuring financial responsibility, fostering an inclusive union culture, and deepening connections to the labour movement and community.  As a palliative care physician and health justice activist, Dr. Naheed Dosani is dedicated to advancing equitable access to healthcare for people experiencing structural vulnerabilities like poverty and homelessness. He founded the Palliative Education and Care for the Homeless (PEACH) Program at the Inner City Health Associates, serves as the Health Equity Lead at Kensington Health in downtown Toronto and provides palliative care at St. Michael's Hospital at Unity Health Toronto. There is much political, professional, and cultural debate around the impact that cannabinoids, good or bad, have on health and wellbeing. This month's “RAPM Focus,” evaluates the relationship between cannabis use and sleep patterns among adult Americans. Editor-in-Chief Brian Sites, MD, interviews authors Calvin Diep, MD, and Karim Ladha, MD, on the paper “Recent cannabis use and nightly sleep duration in adults: a population analysis of the NHANES from 2005 to 2018,” first published in December 2021 (https://rapm.bmj.com/content/47/2/100). The study used observational data from the National Health and Nutrition Examination Survey (NHANES) and examined whether recent use of cannabinoids impacted on incidence of too little sleep or too much sleep. Dr. Diap is a second-year resident in the Department of Anesthesiology and Pain Medicine at the University of Toronto. He is also enrolled in the clinician investigator program and will be starting his graduate studies at the Institute of Health Policy Management at the University of Toronto. His research interests lie in using clinician epidemiological methods to study patient-centered outcomes in the perioperative period, such as pain and disability, as well as opioid and cannabinoid use patterns at population levels. Dr. Ladha is a clinician scientist and staff anesthesiologist at St. Michael's Hospital at the University of Toronto. He received his medical degree from the Johns Hopkins University School of Medicine. He then completed his anesthesia training at the Massachusetts General Hospital and obtained a master's degree in clinical epidemiology from the Harvard School of Public Health. His research uses mixed methodologies to obtain a multi-faceted view of pain and recovery after surgery, including retrospective analyses of administrative databases and prospective observational studies in multi-center randomized controlled trials. *The purpose of this podcast is to educate and to inform. That's when we learned that our cholesterol rises with the onset of perimenopause. As we've said before, what you don't know can hurt you. Dr. Laura Chiavaroli is a Clinical Trialist & a postdoctoral fellow at the University of Toronto and St. Michael's Hospital. Laura has 15 years of experience in the field of nutrition & cardiometabolic health. Alongside her MSc & PhD mentor, Dr. David Jenkins, Laura became skilled at conducting randomized controlled trials & working with dietary patterns, including the Portfolio Diet which is a cholesterol-lowering plant-based diet. Laura also has extensive experience in nutritional epidemiology, specifically in the conduct of systematic reviews and meta-analyses (SRMAs), from her work with Dr. John Sievenpiper. In the past few years of her postdoctoral fellowship, Laura has expanded her experience in knowledge translation strategies, including patient- & physician-facing resource development and web-based mobile app development. Inadequate vitamin intake linked with insulin resistance University of Alabama, May 2 2022.    A greater risk of insulin resistance was revealed among women with acceptable calorie consumption whose folate and vitamin C intake were below the Dietary Reference Intake (DRI) values in a study reported  in Nutrition, Metabolism and Cardiovascular Diseases. Adequate dietary intakes of essential micronutrients are critical to prevent the development of insulin resistance and insulin resistance-related diseases. However, since excess calorie intake linked with obesity is also associated with those diseases, it is important to meet the Dietary Reference Intake (DRI) of those micronutrients without exceeding recommended daily calorie intakes. Among women whose calorie intake was categorized as acceptable and who met the Food and Nutrition Board's DRI of folate and vitamin C, the respective risks of insulin resistance were 59% and 66% lower than women who did not meet the DRIs. The study also showed that even with the consumption of over the recommended calorie intake, only 2% of women met the DRI of vitamin D and approximately only 30% met the DRIs of vitamins A and E, further emphasizing the fact that a significant proportion of women consume calorie-dense food rather than micronutrient-dense food.”   (NEXT)   Ingesting soy protein may ease severity of inflammatory bowel disease   Penn State University, April 26, 2022   A diet supplemented with soy protein may be an effective adjunct therapy for inflammatory bowel diseases, Penn State researchers reported after completing a study that included mice and cultured human colon cells. The findings are significant because inflammatory bowel diseases -- including ulcerative colitis and Crohn's disease -- are characterized by either continuous or periodic inflammation of the colon and represent a significant risk factor for colon cancer. The team found that soy-protein concentrate can exert antioxidant and cytoprotective effects in cultured human bowel cells and can moderate the severity of inflammation in mice that have an induced condition similar to ulcerative colitis. Researchers, substituted soy-protein concentrate into the diet of the mice and removed corresponding amounts of the other protein sources, equaling about 12 percent. They kept human equivalents in mind as they determined the amount. The dietary soy-protein concentrate at the 12-percent dose level ameliorated body-weight loss and swelling of the spleen in the mice with induced inflammatory bowel disease. Soy-protein concentrate mitigates markers of colonic inflammation and loss of gut barrier function in the mice with induced IBD.   (NEXT)   Study finds children with vegetarian diet have similar growth and nutrition compared to children who eat meat   St. Michael's Hospital (Toronto), May 2, 2022   A study of nearly 9,000 children found those who eat a vegetarian diet had similar measures of growth and nutrition compared to children who eat meat. The study, published in Pediatrics and led by researchers at St. Michael's Hospital of Unity Health Toronto, also found that children with a vegetarian diet had higher odds of underweight weight status, emphasizing the need for special care when planning the diets of vegetarian kids. The findings come as a shift to consuming a plant-based diet grows in Canada. In 2019, updates to Canada's Food Guide urged Canadians to embrace plant-based proteins, such as beans and tofu, instead of meat. Researchers found children who had a vegetarian diet had similar mean body mass index (BMI), height, iron, vitamin D, and cholesterol levels compared to those who consumed meat. The findings showed evidence that children with a vegetarian diet had almost two-fold higher odds of having underweight, which is defined as below the third percentile for BMI. There was no evidence of an association with overweight or obesity. Underweight is an indicator of undernutrition, and may be a sign that the quality of the child's diet is not meeting the child's nutritional needs to support normal growth. For children who eat a vegetarian diet, the researchers emphasized access to healthcare providers who can provide growth monitoring, education and guidance to support their growth and nutrition.   (NEXT)   Blackcurrant extract triggers same process as statin drugs   University of Connecticut , April 29, 2022   Polyphenol-rich blackcurrant extract may prevent metabolic dysfunctions induced by diets high in fat and cholesterol, according to research in mice.   The results, published in the British Journal of Nutrition, found the extract reduced the percentage of mice with diet-induced severe steatosis (fatty liver), hypercholesterolaemia and hyperglycaemia.  Total plasma cholesterol and glucose levels were significantly lower in the blackcurrant group compared to the control, yet plasma triglyceride (TAG) level were not significantly different.   The extract contained 25% anthocyanins and 40% polyphenols and the dose equated to a daily human consumption of about 540 mg of the extract and 135 mg anthocyanins.   The researchers said such an ingredient could help reduce disease risk since dyslipidaemia and hyperglycaemia were likely to contribute to metabolic diseases.   Videos:   .  Profound Blood Clots in Major Arteries: Dr. McCullough and Chris Gillespie ( 3 Clips on One page)   Over 17,000 Physicians and Scientists Agree: “There Is No Medical Emergency” – Dr. Robert Malone

Currently, he is one of the 2021-2022 Faculty Fellows at the Stanford Byers Center for Biodesign.  Andrew Petrosoniak MD is a trauma team leader and emergency physician at St. Michael's Hospital in Toronto, Canada. He's an assistant professor at the University of Toronto and he leads the translational simulation program at St. Michael's Hospital. His research focuses on the use of in situ simulation and design thinking to support human-centered care. He's also co-principal at Advanced Performance Healthcare Design, a firm that consults with hospitals, start ups and high stake industries using simulation to improve decision making and inform the design of systems, spaces and teams. In 2004, Patrick MacLeamy drew a set of curves based on a pretty self-evident observation: an architectural project becomes more difficult to change the more developed it becomes. For this earth-shattering revelation MacLeamy named the curve after himself (although the title should probably go to Boyd Paulson who drew the curve much earlier [see Noel's comment to this post]). You have probably seen the graph before. It gets trotted out in every slide deck promoting BIM / IPD / or early-stage environmental analysis.  The key point is that architects expel their effort at a time when design changes are relatively costly. MacLeamy and his disciples advocate shifting design effort forward in the project, front loading it, in order to reduce the cost of design changes.

One reason for this is physicians tend to delay attempts to become pregnant until “the time is right” for their career.  But, in the medical profession, it seems there is really no “good” time to have children. In this episode, Dr. Gary Sherman is host to Interventional cardiologist and Critical Care Specialist,  Dr. Sanjog Kalra. Dr. Kalra was born and raised in Canada, to parents of India origin. Following graduate studies at the University of Toronto in Clinical Pharmacology, Dr. Kalra completed his (MD) training at the University of Calgary. He returned to the University of Toronto in 2008 for Internal Medicine training before moving to Halifax for his Adult Cardiology Fellowship at Dalhousie University and finally, back to Toronto for Advanced Cardiac Catheterization and Intervention at St. Michael's Hospital. In 2015, Dr. Kalra moved to New York, to train under worldwide leaders in the field, as the first Complex, High-Risk (and Indicated) Patient (CHIP) fellow at Columbia University Medical Center. During his CHIP fellowship, Dr. Kalra gained expertise in complex and high-risk coronary interventions, chronic total occlusion percutaneous Cardiac Intervention, contemporary hemodynamic support implantation and management, and cardiac critical care.Dr. Kalra recently completed his tenure as the Director of Complex Coronary Therapeutics and the Associate Director, of the Interventional Cardiology Fellowship Program at Einstein Medical Center in Philadelphia, Pennsylvania. He has now returned to Canada to take up a position at the world-recognized Toronto General Hospital, where he is focused on helping to build a gold-standard complex Percutaneous Cardiac Intervention program. He is recognized nationally and internationally as an expert in the area of complex coronary revascularization, modern hemodynamic support therapies, and in the management of shock and critically ill heart patients. Dr. Kalra is married with 2 young children. He and his family remain closely connected to their roots in India and return frequently.
