Baptist Hospital (1982). At Wake Forest University he completed a residency in general pediatrics, served as chief resident, and completed a fellowship in ambulatory pediatrics. For over 38 years, he was an Adjunct Associate Professor of Pediatrics at the University of North Carolina-Chapel Hill helping to train a generation of medical students and pediatric residents as well as author several research papers. Scott Smolen established his roots in the business world by cutting lawns in his hometown of Bowie. When Scott was in middle school, he started a landscaping business with one employee—himself—and no plans for expansion. He was just looking to make a few bucks before going back to school. By the end of the summer, the business began to take root in the community, and Scott envisioned for the first time a future for himself as an entrepreneur. Over the next decade, the business grew strong throughout Bowie, Crofton, Gambrills, and Odenton. It had 25 employees and a long list of satisfied customers when Scott sold the company. The experience gave him a solid foundation for success as a businessman and allowed him to branch out to pursue other business opportunities and interests. Scott moved from Bowie to Piney Orchard and decided to pursue a new career in computer networking. Although he enjoys computers and networks, it lacked the personal element that Scott had come to enjoy as he built his landscaping business.   The following is an archived pedcast, first posted in April 2017. You can imagine in 37  years of practice, I have witnessed a lot of events, many terrifying and tragic for the children involved. I was reminded of one of those events recently when I was reading an article about ATVs or All Terrain Vehicles. Remember, I live in the south where ATVs and dirt bikes are popular in areas where there is a lot of open space. Many of my patients ride these vehicles with their very powerful engines. In my mind, powerful motors and children are a very bad mix.   The story that flashed back to me while reading an article about ATV injuries in children was actually a little humorous. Part of my routine during checkups is to go over some safety issues with all the school age children. You know, learn to swim, caution about diving, no flips on a trampoline, no playing with fire, explosives or guns as toys, and no motorized vehicles. A number of years ago, I was counseling this one family who had two daughters and lived in a very rural area. I had no idea that the Dad rode ATVs avidly but he did.  One time he brought his older daughter in for a checkup and I started into my litany of cautions.  The daughter interrupted me when I got to the powerful motor caution and she told me a story of a ride she had with her Dad on an ATV. It was an adult ATV and the daughter was riding on the back.  She said her Dad revved up the engine and the vehicle instantly  flipped resulting in the nose of the vehicle becoming the rear  and coming down upside down on them.  The young girl told me that DURING the flip, she screamed..."Dr. Smolen told me not to ride this thing!"  Fortunately, the ATV flipped in an area with a little depression in the terrain and the very heavy, moving ATV didn't come down on Dad and daughter with all of its force.  They were scrapped up badly but neither sustained a serious injury.  Both had helmets on thank goodness.  Dad did something right that day.   So back to the issue of powerful motors and children... a no brainer bad mix in my mind but I see families who can't wait to get their little boys on dirt bikes when they are 6 years old. You can imagine in the land of Nascar, speed and power can be an positive attribute for a toy, not a liability.  I know these parents don't want their children to get hurt.  No I think they are just anxious for their children to enjoy the thrill of speed and power that they enjoy... but at what risk?  According to the Academy of Pediatrics article I was referring to a minute ago, 3000 children under age 16 years have been killed on ATV's since 1982, 43% of them less that 12 years old. Probably tens of thousands more have been injured during this period of time. Here is a doc Smo pearl for you parents considering the purchase of an ATV or dirt bike for your kids, "A toy that can kill or cause lifelong injury, simply isn't a toy at all! But if you insist on allowing your children to ride these powerful vehicles, here are some tips from the AAP that might keep them safer: Make sure your child wears a motorcycle (not bicycle)-style helmet that fits snugly. Look for DOT or Snell ratings on the box. Never purchase a helmet that is too big for your child so he or she can “grow into it.” Suit up your child with padded, reflective clothing and protective eyewear. Do not allow children to ride on the street or between dusk and dawn. Allow only one rider on the ATV at a time. Always supervise children on ATVs. If you are buying an ATV, choose one with a seat belt, roll-bar, engine covers and a speed-limiting device. Well, thats this week's installment of portable practical pediatrics. Musical intro Can you spoil a child? The age old question.  Parents have debated this question so I thought it might be fun to take on this question today in light of what we know about child development. Defining Spoiling First let's define what we mean by spoiling. My definition is the following; “Creating childhood dependence and or needs that are unnecessary.”  Repeat!  Let's ponder that question a little further but looking at some of the evidence old and new. I'm not a psychologist but I have done some study of the subject, I have successfully raised two children, and spent my entire adult life with children. I think that gets me some credibility with regards to the subject at hand. Let's consider a newborn baby, totally dependent on his or her parents, right? Through the child's early experiences they hopefully learn to trust primary caretaker (mother) and form a strong attachment..what psychologists call a “secure attachment”.  Once they have developed this trust and feel secure with their caretakers, they are free to start exploring and learning.  Infancy and toddler life should be full of exploration, learning, movement, tastes, smells, language, problem solving.   As every child begins to inevitably bump up against limit they begin to demonstrate frustration and anger along with some defiance.  But eventually they become comfortable with the rules and develop their our own uniqueness- what Freud called our “ego formation”.   By this time the child has also developed reasoning skills, language, and is much more able to deal with frustration. Science Drive of Secure Attachment Without the security of a loving relationship with Mom or whomever, the rest of the child's psychological development is in jeopardy of being abnormal. Without that secure attachment, fear and anxiety begin to replace trust and peacefulness.  The journey toward effective learning has been hijacked by negative emotions. With that paradigm in mind lets look back in time to a different era.  There was a time in the not too distant past, when many children were raised in orphanages. they got good basic care but they didn't get a lot of holding and close immediate attention to their emotional needs, and experts began to notice that as these children got older, they often displayed a great deal of difficulty making healthy adult relationships and tended to have psychological problems...something they called "hospitalism".   And it turns out that the most vulnerable time a child was susceptible to developing "hospitalism" was the first 3 months of life. Why, because - this is the time when a child develops the secure attachment. The first 3 months are critical to a child's personality development.  Without this basic trust, we can become lost. Trust is the basis of all human relationships that are meaningful. Without it, healthy relationships are impossible.  As I like to say, “If you can’t trust your mother, who can you trust?”   During the first 3 months of life infants are totally dependent for all needs…this is when their needs should be met without hesitation. If they need to be held…hold them If they need to be rocked…rock them If they need frequent feeding…feed them If they need to be swaddled..do it without hesitation   Remember our definition of spoiling..unnecessary dependence.  With very young infants, you will not create dependence because they are unable of anything but dependence!!   When Does Spoiling Begin? Sports are great, but your child can have too much of a good thing. Your child can learn a lot of valuable skills and life lessons on the court or field, but at some point, the physical and mental rigor of sports can be too much, leading to mental and physical fatigue and injury. In this episode of DocSmo.com, Dr Smolen reviews the AAP guidelines for limiting competitive sports and gives you some common sense guidelines of his own. Musical Intro:   Introduction Thanks for joining me today. This is Dr. Paul Smolen, your pedcast host. Sports season is here and unfortunately that means a lot of injuries for children. They are out there swimming, they're jumping, they are climbing and running. They're very very active so I thought today we would provide you with some information to help you navigate your child from the Little League to the Big leagues of life hopefully injury free.  Hold on your iPad. Here comes another important pedcast. As always by listening to the speakers you are agreed to my terms and conditions one of those conditions is that the speakers are not intended to be specific health advice or any particular child but rather general learning information for the parents.   The Overuse injury in children Now pediatricians are frequently presented with children who have an assortment aches and pain‘s and like most problems in medicine, the answer to those aches and pains usually lies in the history, that is the questions that pediatricians ask. I've learned after 37 years of practice that asking good questions and carefully listening to the answers will usually lead me to the why of most aches and pains. Your child is growing up in the era of sports camps, personal trainers, and almost constant competition. Many are trained like professional athletes. So today we're going to talk about the so-called overuse injuries that are so common in school in school aged children and adolescents. I often see young children who play on multiple teams competing daily with year-round competition in whatever pursuit they have chosen. Now, what are their parents thinking about? That kind of intensity would be hard for anyone to maintain especially a child! So let's explore a little today when it comes to children and athletic injuries.   What Kind of Injuries do Children Sustain? OK so let's get into the specifics of the patterns of injury that we see in children. I'm a simple kind of guy and I like to keep things very simple so I always try to find some simplicity whenever possible. In my world there are two types of musculoskeletal injuries; those that happen from when I called the big crash, “I fell off my bike and I know exactly the moment I hurt my arm”. Alternatively, those where no one can even remember when the injury happened. I call these the stealth injury type that probably came from lots of little injuries that were not memorable at the time but one day, Charlie‘s arm started to hurt. It's that simple. Big crash or stealth injury. That's what I'm listening for when people are telling me the story of their aches and pains. Now during this talk, we're going to exclude the big crash kind of injuries and we're only going to talk about the stealth injuries. The second type of injury; the little hurts that add up over time to something big. Now the child usually tells me what is causing the pain by telling me what their other activities are; soccer daily with travel on a travel team (ding, ding, ding), competition swimming with two practices a day (ding, ding, ding), or I sometimes hear tennis almost daily with coaching followed by tournaments each week or month (ding, ding, ding) or long-distance running adding up to about 40 miles a week (ding, ding, ding,). So, you're getting the idea that this is not exactly rocket science. Just listen! The location of the pain and the activity that the child is participating in, tells you what is injured. Remember, these are overused injuries, they micro injuries,

Introduction You can imagine in 34  years of practice, I have witnessed a lot of events, many terrifying and tragic for the children involved. I was reminded of one of those events recently when I was reading an article about ATVs or All Terrain Vehicles. Remember, I live in the south where ATVs and dirt bikes are popular in areas where there is a lot of open space. Many of my patients ride these vehicles with their very powerful engines. In my mind, powerful motors and children are a very bad mix. More on that in a few minutes. Music Introduction Motorized Vehicles are Not Toys The story that flashed back to me while reading an article about ATV injuries in children was actually a little humorous. Part of my routine during checkups is to go over some safety issues with all the school age children. You know, learn to swim, caution about diving, no flips on a trampoline, no playing with fire, explosives or guns as toys, and no motorized vehicles. A number of years ago, I was counseling this one family who had two daughters and lived in a very rural area. I had no idea that the Dad rode ATVs avidly but he did.  One time he brought his older daughter in for a checkup and I started into my litany of cautions.  The daughter interrupted me when I got to the powerful motor caution and she told me a story of a ride she had with her Dad on an ATV.
