This week, commissioner Strohmaier gives an update on the Big Sky Passenger Rail Authority. The rail authority is a division of state government that encompasses 19 counties in Montana that are working to bring back passenger train service in the southern part of the state. 2023 is a major year for this effort: the U.S. Department of Transportation is studying which inactive rail lines could be restored. The North Coast Hiawatha stretched from Chicago to Seattle and passed through Missoula County until 1979, when the line was discontinued. Is more of a good thing always better? We know this isn't the case with exercise and eating, but does the same idea apply to meditation and mindfulness? Last week the Missoula Board of County Commissioners traveled to Washington D.C. to join nearly 2,000 elected and appointed county government officials from across the country at the National Association of Counties Legislative Conference. The conference focused on federal policy issues that impact counties and was an opportunity for the commissioners to engage in policy and educational sessions. They interacted with federal officials and participated in congressional briefings and meetings, and they even heard from President Biden, who once served as a county leader. Not all counties operate the same way, and each are governed by different state statutes. In this episode, Commissioners Slotnick and Strohmaier share the importance of going to the NACo conference and learning interesting insights from other county leaders. They share new perspectives and exciting ideas they heard about housing in mountain communities; including the value in building diverse neighborhoods and the concept of using deed restrictions in Missoula County to help keep the cost of housing from escalating. In dieser Folge sprechen wir ab der vierten Minute über Brendas Talk-Reihe zum Thema Sex Education. Zuvor war sie jahrelang Stilredakteurin bei der "Welt am Sonntag". Dort kam sie dann auch zu den Themen Sex und Beziehung. Heute schreibt sie, von Marseille aus, auch ein Buch zum Thema Sex Education und beantwortet 80 wichtige Frage in diesem Zusammenhang. In unserem Gespräch geht es um die Lust an Sex in Langzeitbeziehungen, Slow Sex, die Zukunft von Sex auch im Kontext von Robotics. Es geht natürlich auch um den Konsum von Pornografie und Sex in Zeiten von Corona. Ab Minute 23 sprechen wir auch darüber wie es ist ein Buch zu schreiben und wie man einen Verlag findet und wie es sich anfühlt, sich selbstständig zu machen, nachdem man jahrzehntelange angestellte Journalistin war. Später ab Minute 32 sprechen wir über Brendas Buch, das sie geschrieben hat, nachdem Ihr Ehemann verstorben war und sie Witwe wurde. Darin beschreibt sie, was sie dadurch und als Witwe über das Leben gelernt hat. Wir sprechen über Trauerschübe und die Angst vor dem Tod.

Zusammenfassung Die Schizophrenie ist eine der schwerwiegendsten psychiatrischen Erkrankungen mit weit reichenden psychischen und sozialen Auswirkungen auf die betroffenen Patienten und ihre Angehörigen. Die Lebenszeit-Prävalenz beträgt etwa 1%. Die Ergebnisse von Kopplungs- und Assoziationsuntersuchungen zeigen den starken Einfluss einer genetischen Komponente bei der Entstehung der Krankheit. Die bisherigen Forschungsergebnisse weisen darauf hin, dass es sich bei der Schizophrenie um eine genetisch komplexe Erkrankung handelt, deren Ätiologie sich durch das Zusammenwirken von Umweltfaktoren und genetischen Faktoren auszeichnet. Bislang ist es noch nicht gelungen, alle an der Pathogenese beteiligten Faktoren, vor allem aber die Mechanismen ihres Zusammenspiels zu entschlüsseln. Aufgrund von morphologischen und molekularbiologischen Untersuchungen sowie genetischen Kopplungs- und Assoziationsstudien fand sich eine Reihe von Suszibilitätsgenen für Schizophrenie, darunter auch das Gen DTNBP1. Es ist auf dem Chromosom 6 in der Region 22.3 lokalisiert und kodiert für das Protein Dysbindin-1. Im Gehirn nimmt dieses Protein Einfluss auf das Zytoskelett und die synaptische Plastizität und ist an der Signaltransduktion von Neuronen über NMDA- und GABA-Rezeptoren und damit am Glutamat- und Dopaminstoffwechsel beteiligt (Benson et al. 2001; Harrison und Weinberger, 2005). Morphologische Untersuchungen zeigen, dass die Konzentration von Dysbindin-1 in wichtigen Bereichen des Gehirns bei schizophrenen Patienten vermindert ist (Talbot et al. 2004; Tang et al. 2009). Eine Reihe von Assoziationsstudien, teils auch familienbasiert, haben in unabhängigen Kollektiven mit verschiedenstem ethnischen Hintergrund signifikante Assoziationssignale an unterschiedlichen Varianten im DTNBP1-Gen gefunden (Straub et al. 2002b; van den Oord et al. 2003; Schwab et al. 2003; Morris et al. 2003; van den Bogaert et al. 2003;, Tang et al. 2003; Kirov et al. 2004; Williams et al. 2004; Numakawa et al. 2004; Funke et al. 2004; de Luca et al. 2005; Duan et al. 2007; Riley et al. 2009 und Voisey et al. 2010). Dabei handelte es sich teilweise um Einzelbasen-Polymorphismen oder Haplotypen im DTNBP1-Gen, die sich als signifikant mit Schizophrenie assoziiert gezeigt haben. Folgestudien konnten diese Ergebnisse oftmals aber nicht bestätigen (Turunen et al. 2006, Datta et al. 2007, Joo et al. 2007, Holliday et al. 2006, Peters et al. 2008, Sanders et al. 2008; Strohmaier et al. 2010). In der vorliegenden Arbeit wurden die sechs Einzelbasenpolymorphismen, rs3213207, rs2619538, rs1011313, rs1047631, rs2056943 und rs2619522 untersucht. Bei ihnen hatten sich in Studien zuvor teilweise deutliche Assoziationsbefunde gezeigt. Die Marker wurden hier erneut an einem unabhängigen kaukasischen Kollektiv von 503 Patienten und 1290 gesunden Kontrollpersonen untersucht. Zustätzlich wurde die Patientengruppe in vier verschiedene Kategorien unterteilt, je nachdem ob Angehörige mit psychischen Störungen, mit Schizophrenie oder Angehörige 1.Grades mit Schizophrenie in der Familienanamnese vorlagen, sowie als vierte Gruppe das Patientenkollektiv insgesamt. Dabei zeigten sich in den unterschiedlichen Patientengruppen signifikante Assoziationshinweise und Trends zur Assoziation bei den Markern rs3213207 und rs2619538, der Marker rs1011313 zeigte lediglich Trends zur Assoziation mit Schizophrenie. Die höchste Signifikanz erreichte mit einem p-Wert von 0,034 der Marker rs3213207 in der Gruppe der Patienten mit schizophrenen Angehörigen. Hier trat der heterozygote Genotyp A/G signifikant häufiger bei Patienten auf als die homozygoten Genotypen A/A und G/G. Ebenfalls Signifikanzniveau erreichte der Marker rs2619538. Dabei zeigte sich in der Gruppe der Patienten mit schizophrenen Angehörigen ersten Grades ein signifikant niedrigerer Anteil homozygoter Träger des selteneren Allels Adenin bei einem p-Wert von 0,044. Die Marker rs1047631, rs2056943 und rs2619522 zeigten in dieser Arbeit keine Hinweise auf Assoziation mit Schizophrenie. Aufgrund der Ergebnisse dieser Arbeit und der anderer Forschungsgruppen besteht weiterhin Grund zur Annahme, dass es sich bei DTNBP1 um ein Suszeptibilitäts-Gen für Schizophrenie handelt. Zwar konnten auch in dieser Arbeit die positiven Assoziationsergebnisse von Vorläuferstudien nicht in vollem Umfang repliziert werden, jedoch stützen die Ergebnisse die Annahme, dass Variationen im DTNBP1-Gen an der Pathogenese der Schizophrenie beteiligt sind. Dabei weisen die Ergebnisse in dieser Arbeit durchaus darauf hin, dass es einen Zusammenhang gibt zwischen positiver Familienanamnese in Bezug auf Schizophrenie und genetischen Variationen im DTNBP1-Gen. Gründe für die Inkonsistenz in den Ergebnissen der bisherigen Untersuchungen könnten die abweichenden Stichprobengrößen sowie die unterschiedliche ethnische Herkunft der untersuchten Kollektive sein. Auch die Einschlusskriterien für die Patienten- und Kontrollgruppen wichen voneinander ab. Vermutlich aber ist eine der Hauptursachen auch die genetische und phänotypische Heterogenität der Erkrankung an Schizophrenie. Weitere Studien mit höheren Fallzahlen werden nötig sein, um die genetischen Risikomarker auf dem DTNBP1-Gen und ihr mögliches Zusammenspiel mit Markern auf anderen Genen genauer zu detektieren.
