The majority of Dr. Hammamieh's work aims to provide an understanding and develop countermeasures for diseases and injuries in support of service members. Currently, the MRSB lab is assessing the effect of spaceflight conditions onboard the International Space Station (ISS) on health and disease, and Dr. Hammamieh's team has multiple biological investigations sponsored by the ISS National Laboratory examining the effects of microgravity on the healing of bone and skin wounds. Wayne Jonas, MD, is a widely published investigator, practicing family physician, and professor of medicine at Georgetown University and at Uniformed Services University of Health Sciences. He is also a retired lieutenant colonel in the Medical Corps of the United States Army. Dr. Jonas was the director of the Office of Alternative Medicine at the National Institutes of Health from 1995 to 1999 and led the World Health Organization's Collaborative Center for Traditional Medicine. Prior to that, he served as the director of medical research fellowship at the Walter Reed Army Institute of Research. In recognition of Alcohol Awareness Month, we are joined by George Koob, PhD, director of the National Institute on Alcohol Abuse and Alcoholism (NIAAA). Dr. Koob talks about his background and career journey, along with major insights made through his research, including regarding alcohol withdrawal. He addresses stigma related to alcohol use (or abstinence from) and offers advice for the next generation of researchers and clinicians in this exciting and dynamic space, with so much potential for expanding knowledge. Dr. Koob shares resources for individuals, family members, physicians, or anyone looking for information related to alcohol use and alcohol use disorder, including Rethinking Drinking and the NIAAA Treatment Navigator. He emphasizes the importance of motivation (e.g., motivational interviewing) rather than confrontation and seeing your doctor, or encouraging your loved one to see their doctor, for a physical as a good first step in addressing any issues related to alcohol use. Dr. Koob finally explains his motivation for continuing his work in this field. In addition to serving as director of NIAAA, Dr. Koob is also a senior investigator at the Intramural Research Program of the National Institute on Drug Abuse where he directs the Neurobiology of Addiction Laboratory in the Integrative Neurosciences Research Branch. As an authority on alcohol use disorder, drug addiction, and stress, he has contributed to our understanding of the neurocircuitry associated with the acute reinforcing effects of alcohol and drugs and the neuroadaptations of the reward and stress circuits associated with the transition to dependence. He has published over 800 peer reviewed papers; mentored 13 PhD students, 85 post-doctoral fellows, and 11 K99's (Pathway to Independence Award postdoctoral researchers); and authored several books including the Neurobiology of Addiction. He received his PhD in behavioral physiology from Johns Hopkins University in 1972. He did post-doctoral studies at Walter Reed Army Institute of Research and the University of Cambridge (England). He subsequently held positions at The Salk Institute and the Scripps Research Institute. Dr. Koob is the recipient of many honors, including membership in the National Academy of Medicine (USA) and award of the Legion of Honor (France). She earned her Associates of Applied Science degree in Medical Laboratory Technology through the Community College of the Air Force as an enlisted airman and a Bachelor of Science degree in Clinical Laboratory Science on the Montgomery GI Bill as a civilian in 2007. MAJ Roth graduated from Creighton University in 2012 where she studied multidrug-resistant bacteria and received a PhD in Medical Microbiology and Immunology in the laboratory of Dr. Nancy Hanson.Following her commission into the U.S. Army in 2013, MAJ Roth served in multiple positions under the Bacterial Diseases Branch at the Walter Reed Army Institute of Research in Silver Spring, Maryland. In 2017, she was the first scientist accepted into the Army Medical Department's Program Management Acquisitions Internship Program at the US Army Medical Materiel Development Activity at Fort Detrick, Maryland. From 2019-2021, MAJ Roth was the Director of the Infectious Disease Program at the US Army Medical Research Directorate—Africa, based in Kisumu, Kenya, where she oversaw the execution of over $25M of research, development, and surveillance activities of infectious diseases of military relevance alongside Eastern African. In addition to her graduate degree, MAJ Roth is a certified Medical Laboratory Scientist (American Society of Clinical Pathologists). This week we will discuss M-RNA vaccines. Our guest is Thomas VanCott, PhD. Thomas VanCott is currently the Chief Scientific Officer for Combined Therapeutics, a Boston based biotech company developing targeted mRNA therapies. Prior to this he served as the Chief Technology and Strategy Officer for Catalent Cell & Gene Therapy, a global CDMO manufacturing viral vectors for gene and cell therapies as wells as plasmid DNA & mRNA platforms based in Baltimore, MD. He was responsible for strategically enhancing CMC services to meet the market demand of increasingly complex gene and cell therapy products as well as leading the product development and internal R&D teams. Prior to this, he was the CEO for 10 years at a Maryland-based CMO/CRO (ABL) where he was responsible for the strategic international growth of the company. He has been involved in biologics product development for over 25 years. He has a PhD in physical chemistry and started his career as a Captain in the US Army stationed at Walter Reed Army Institute of Research (WRAIR) studying and developing HIV vaccines for international deployment from initial construction through preclinical development, GMP manufacturing and clinical development. Vaccines help prevent infection by preparing the body to fight foreign invaders (such as bacteria, viruses, or other pathogens). All vaccines introduce into the body a harmless piece of a particular bacteria or virus, triggering an immune response. Most vaccines contain a weakened or dead bacteria or virus. However, scientists have developed a new type of vaccine that uses a molecule called messenger RNA (mRNA) rather than part of an actual bacteria or virus. Messenger RNA is a type of RNA that is necessary for protein production. Once cells finish making a protein, they quickly break down the mRNA. mRNA from vaccines does not enter the nucleus and does not alter DNA. mRNA vaccines work by introducing a piece of mRNA that corresponds to a viral protein, usually a small piece of a protein found on the virus's outer membrane. (Individuals who get an mRNA vaccine are not exposed to the virus, nor can they become infected with the virus by the vaccine.) By using this mRNA, cells can produce the viral protein. As part of a normal immune response, the immune system recognizes that the protein is foreign and produces specialized proteins called antibodies. Antibodies help protect the body against infection by recognizing individual viruses or other pathogens, attaching to them, and marking the pathogens for destruction. Once produced, antibodies remain in the body, even after the body has rid itself of the pathogen, so that the immune system can quickly respond if exposed again. If a person is exposed to a virus after receiving mRNA vaccination for it, antibodies can quickly recognize it, attach to it, and mark it for destruction before it can cause serious illness. Like all vaccines in the United States, mRNA vaccines require authorization or approval from the Food and Drug Administration (FDA) before they can be used. Currently vaccines for COVID-19, the disease caused by the SARS-CoV-2 coronavirus, are the only authorized or approved mRNA vaccines. These vaccines use mRNA that directs cells to produce copies of a protein on the outside of the coronavirus known as the “spike protein”. Your article opens with, “In the last decade, suicide has become a leading cause of death for service members, claiming more lives than combat and transportation accidents.” This is a hard topic to ease into. What else do we need to know about service members and suicide for this article? MAJ Thomas H. Nassif Suicide is a difficult topic for a number of reasons. And many, if not all of us have been touched by it in some way, and the ripple effects across the unit and community when there's a suicide-related event. It's a really important topic, and there are many initiatives that the Army is engaged in that address risk factors and prevention. Our paper tackles a different part of the problem. you probably heard of prevention. What we're going to talk about today is postvention, in other words, what leaders do in the aftermath of a suicide-related event. First some terminology. So we define a suicide-related event as death by suicide, suicide attempt, or suicide intent. And by leader, we're referring to chaplains, behavior health providers, and unit leaders. And the leaders that we focused on in the article were chaplains, behavior health providers, and unit leaders. In September, a new version of the COVID-19 boosters became available to the public without any clinical testing. They are called bivalent vaccines and are being offered by Pfizer-BioNTech and Moderna. Bivalent means they contain not just one mRNA that codes for a protein from a viral strain, but two types. The first mRNA makes cells manufacture a toxic protein from the ancestral strain of the SARS-COV-2 virus, the original virus from Wuhan. The second mRNA forces cells to make a protein that comes from Omicron and its BA.4/5 subvariants. The logic here is to give people a well-rounded protection from existing variants as well as any upcoming mutations. Some immunologists believe that future mutations can still be closely related to the original Wuhan strain and that's why its mRNA is kept in the new booster. But others disagree. Dr. Jerome H. Kim, M.D., is the Director General of the International Vaccine Institute (IVI - https://www.ivi.int/), a nonprofit International Organization established in 1997 as an initiative of the United Nations Development Programme (UNDP), dedicated to the discovery, development and delivery of safe, effective and affordable vaccines for global public health. IVI is headquartered in Seoul and hosted by the Republic of Korea with 36 member countries and the WHO on its treaty. Dr. Kim served as the Principal Deputy and Chief of the Laboratory of Molecular Virology and Pathogenesis at Military HIV Research Program at the Walter Reed Army Institute of Research (WRAIR), in addition to being the Project Manager for the HIV Vaccines and Advanced Concepts Evaluation Project Management Offices, U.S. Army Medical Material Development Activity. Dr. Kim was also a Professor within the Division of Infectious Diseases of the Department of Medicine at the Uniformed Services University of the Health Sciences. From 2004 – 2009 he led the Army's Phase III HIV vaccine trial (RV144), the first demonstration that an HIV vaccine could protect against infection, as well as subsequent studies that identified laboratory correlates of protection and sequence changes in breakthrough HIV infections after vaccination. Dr. Kim graduated from the Yale University School of Medicine in 1984 and completed his training in Internal Medicine and fellowship in Infectious Diseases at Duke University Medical Center. In 2013 he received the John Maher Award for Research Excellence from the Department of Medicine, Uniformed Services University of the Health Sciences and throughout his career to date, Dr. Kim has authored over 200 publications.

The US Department of Health and Human Services extended the Covid public health emergency for another 90 days. This will allow many public health protections and financial aid programs to keep going at least another three months. And states have been promised 60 days' notice prior to any expiration of the emergency status in the future. This is the eighth extension.France approved a law 215-58 that will ban unvaccinated citizens from all restaurants, sports arenas and other venues, essentially making them persona non grata. And that's 9% of all French adults being reduced to second class status. Previously, the unvaccinated could get into establishments by showing a recent negative test, but no longer. There will also be increased penalties for fake passes.The White House plans to buy and give out 1 billion rapid tests. But did they check with manufacturers and distributors to see if that's even possible? They say it is, and the manufacturers say they can do it, but tests have been hard for the public to find. Research from Health Catalysts Group says the US has an estimated capacity of only 260 million tests this month, which is why critics say adding 1 billion more to demand will only crush the pipeline.Speaking of hard-to-find tests, what do you do if your job requires that you get tested on a regular basis in order to show up, do your job and get paid? But you can't find any? No one seems to have an answer for them, and the problem is hitting essential workers who can't work from home – often people of color – particularly hard. The result is lost wages even though the worker may not even have COVID.Has the Army found a super-vaccine? There's talk they've developed a universal COVID vaccine that could successfully combat all variants. They've been working on it at Walter Reed Army Institute of Research for a year. In December, the Army announced its pan-coronavirus vaccine had completed Phase 1 of human trials with positive results. As opposed to mRNA vaccines, this one does use part of the actual COVID virus, but a harmless part. It also has less restrictive storage and handling requirements than current vaccines. In the United States, cases were up 98% down from 159% Friday, deaths are up 57%, and hospitalizations are up 61% over 14 days. The 7-day average of new cases has been trending up since December 14. The five states that had the most daily deaths per 100,000 are Indiana, New Mexico, Michigan, Maryland, and Pennsylvania. There are now over 23 million active cases in the United States, at 23,031,325.The five areas with the greatest increase in hospitalizations per capita: Puerto Rico 157%. The U.S. Virgin Islands 153%. Louisiana 142%. Alabama 140%. And Florida 131%. The top 10 areas with the highest number of recent cases per capita according to The New York Times: West Feliciana, LA. Pitkin, CO. Teton, WY. Greensville, VA. Covington, MS. Tom Green, TX. Uvalde, TX. Kodiak Island Borough, AK. Rolette, ND. Koob, Ph.D., is an internationally-recognized expert on alcohol and stress, and the neurobiology of alcohol and drug addiction. He is the Director of the National Institute on Alcohol Abuse and Alcoholism (NIAAA), where he provides leadership in the national effort to reduce the public health burden associated with alcohol misuse. As NIAAA Director, Dr. Koob oversees a broad portfolio of alcohol research ranging from basic science to epidemiology, diagnostics, prevention, and treatment. Dr. Koob earned his doctorate in Behavioral Physiology from Johns Hopkins University in 1972. Prior to taking the helm at NIAAA, he served as Professor and Chair of the Scripps' Committee on the Neurobiology of Addictive Disorders and Director of the Alcohol Research Center at the Scripps Research Institute. Early in his career, Dr. Koob conducted research in the Department of Neurophysiology at the Walter Reed Army Institute of Research and in the Arthur Vining Davis Center for Behavioral Neurobiology at the Salk Institute for Biological Studies. He was a post-doctoral fellow in the Department of Experimental Psychology and the MRC Neuropharmacology Unit at the University of Cambridge. Dr. Koob began his career investigating the neurobiology of emotion, particularly how the brain processes reward and stress. He subsequently applied basic research on emotions, including on the anatomical and neurochemical underpinnings of emotional function, to alcohol and drug addiction, significantly broadening knowledge of the adaptations within reward and stress neurocircuits that lead to addiction. This work has advanced our understanding of the physiological effects of alcohol and other substance use and why some people transition from use to misuse to addiction, while others do not. Dr. Koob has authored more than 650 peer-reviewed scientific papers and is a co-author of The Neurobiology of Addiction, a comprehensive textbook reviewing the most critical neurobiology of addiction research conducted over the past 50 years. Concussion Blood Test & Treatments for CTE, TBI and Brain ImpairmentsDr. Michael Lewis - Reports on a blood test for a concussion and what to do about CTE or TBI. The https://www.banyanbio.com/ (Brain Trauma Indicator Test at Banyan Biomarkers) measures two biomarkers: proteins known as UCH-L1 and GFAP, that are released upon injury to the brain and pass through the blood-brain barrier. Elevated levels of these proteins can be detected within 15 or 20 minutes of injury. The test can be taken within 12 hours of injury, and results can be obtained within three or four hours. Patients are currently diagnosed with concussion based on a combination of symptoms as well as imaging. However, CT scans don't always detect concussion. "Over 90% of CT scans (for concussion) are negative. And you get 200 times the radiation of a chest X-ray. It's expensive; it's not terrific," said Hank Nordhoff, chairman, and CEO of Banyan Biomarkers, maker of the new test. It can help determine whether a patient further needs a CT scan, based on a physician's concerns. It's not concussions that cause CTE. It's repeated hits, a study finds. "Today's action supports the FDA's Initiative to Reduce Unnecessary Radiation Exposure from Medical Imaging -- an effort to ensure that each patient is getting the right imaging exam, at the right time, with the right radiation dose," FDA Commissioner Dr. Scott Gottlieb said in a statement Wednesday. Concussion Diagnostic Costs ReducedThe test could also help reduce costs significantly. Whereas a CT scan can cost $800 to $1,500, Nordhoff predicts that the new test would cost closer to $150. The test would be available to hospitals, Nordhoff said, and he hopes a handheld sideline device could be commercially available in the near future. The Concussion - Subconcussion StudyThe FDA approved the test as part of its Breakthrough Devices program after evaluating a clinical study of 1,900 blood samples from people thought to have a concussion or mild traumatic brain injury.
