You may think of allergies as causing sniffly noses and congestion in the spring or fall. But allergies can go far beyond that. As Dr. Kari Nadeau points out in this episode, allergies can affect us from head to toe, including eyes, nose, throat, lungs, sinuses, skin and gut. In the most dangerous instances, the whole body is threatened with an anaphylactic reaction. That's a medical emergency! One in three Americans will develop allergies at some point in our lives, so it's important to know what works to control them. We begin our discussion of your allergy survival guide with an explanation of what is happening during an allergic reaction. The immune system perceives some foreign compound, usually a protein, as dangerous even though normally it would not be. So it reacts by trying to flush the invader out by producing extra mucus. The turbinate sinuses can make one to two gallons of mucus a day, and naturally, it has to go somewhere. That's why you might be congested. Having all that mucus in the sinuses can also encourage bacterial growth, so if the allergic reaction persists, some people have to deal with sinus infections. Emergency Treatment In determining what works, you need to know the nature of the reaction. If you have two or more organs involved, if you are having trouble breathing or if you feel dizzy, you may be in the midst of an anaphylactic reaction. What works for that is an epinephrine injection and immediate medical attention. This is potentially life-threatening, so you will want to figure out what triggered the reaction so you can avoid it in the future. Once someone has suffered one anaphylactic reaction, they should keep epinephrine with them at all times in case of another episode. Epinephrine comes as a self-injector pen or a nasal spray (neffy). Can You Spot Drug Allergies? In the warnings that are rattled off as part of a TV ad for a pricey new drug, we often hear viewers cautioned not to take the medicine if they are allergic to it. That sounds like simple common sense, but it also has a Catch 22 quality. How do you know you are allergic to a medication unless you take it–and experience an allergic reaction for which you might need treatment. Most of these presumably are immune system-mediated reactions, in which the body produces IgE. That is how allergies to penicillin or sulfa drugs work. Some drugs cause a different type of reaction, not IgE-mediated but dangerous nonetheless. Lisinopril is the most commonly prescribed blood pressure medicine in this country. Like other ACE (ACE is short for angiotensin-converting enzyme) inhibitor medications, lisinopril can trigger angioedema. This swelling can affect the face, lips, tongue and throat, where it can compromise breathing. The most insidious aspect of this reaction is that it can occur after the person has been taking the drug without problems for weeks, months or even years. “Red man syndrome” or infusion reactions in people taking vancomycin can likewise occur without warning. The last type of drug reaction is not actually an allergy at all, although people occasionally use that terminology. It is better described as sensitivity. For example, a stomachache is a common reaction to the antibiotic erythromycin. Some people are disabled by this abdominal pain and try to limit their exposure to erythromycin thereafter. What Works and What Doesn't? Since the immune system is acting inappropriately to cause allergic reactions, treatment should involve immunotherapy. Eye drops can help eyes feel less itchy and irritated. Likewise, OTC nose drops or nasal sprays can often help the nose. The corticosteroid Flonase (fluticasone) and the antihistamine Astepro (azelastine) are good examples. During allergy season, some people find that a daily nasal wash (with a neti pot or NeilMed device) can help reduce the mucus and remove the allergens such as pollen causing the reaction. There are also oral antihistamines and inhalers for asthma. For decades now, allergists have offered their patients shots to help desensitize them to the allergen causing their trouble. Joe had these as a child and teenager and has been largely free of allergies since. Not everyone gets such lasting relief. Complications from Current Therapies Medications have side effects, and that is true of allergy medicines as with other drugs. Antihistamines, especially the older ones like Benadryl (diphenhydramine), are notorious for causing drowsiness. That's one reason it is often included in nighttime pain relievers as the “PM” in drugs like Advil PM. We worry about regular use of such antihistamines because it has been linked to a greater risk for dementia. A second-generation antihistamine such as Allegra (fexofenadine) is much less likely to make someone feel sleepy. However, Dr. Nadeau has seen patients on antihistamines suffer worse allergies if they stop suddenly. The People's Pharmacy has received hundreds of reports from people who experienced unbearable itching upon discontinuing Zyrtec (cetirizine) or Xyzal (levocetirizine). This can last for weeks. Doctors don't usually worry much about steroid nasal sprays like Flonase because they are topical. Presumably, nasal tissues pick up most of the dose. Just the same, using such a nose spray day after day for a long time could result in systemic steroid exposure that is not trivial. Stronger Medicine Dr. Nadeau is enthusiastic about the benefits of two potent prescription medicines. One is Xolair (omalizumab). It was originally developed to prevent asthma, but is now approved for chronic sinusitis, food allergies and chronic hives. Paradoxically, Xolair is one of those medicines that could cause a severe allergic reaction even on the first dose, so the FDA warns that the initial injection should be given in a healthcare setting prepared to treat anaphylaxis. This is uncommon, though, occurring in 0.1 to 0.2% of patients. The other medication Dr. Nadeau is prescribing for allergy patients who don't respond well to other treatments is Dupixent (dupilumab). The FDA has approved this medicine to treat a wide range of conditions, including eczema, asthma, chronic sinusitis, allergic reactions affecting the esophagus and chronic hives, among other things. Most insurance companies will not cover this pricey injection unless the patient has failed all other therapies. Fighting Air Pollution: What Works Air pollution makes allergy symptoms worse, so using an effective air filter inside the home is a good step. A HEPA (high-efficiency particulate-arresting) filter is ideal, especially as part of the air-handling system. If that's not possible, utilizing a MERV 13 in the part of the home where you spend the most time is a good second choice. Sonu One new option for treating allergies is acoustic resonance therapy with the SoundHealth Sonu headband. It uses vibration from sound to loosen mucus from the sinuses so that they can clear. The FDA has approved its use for children as well as adults. New research was just published demonstrating its helpfulness in treating children with nasal congestion (Oto-Open, April-June 2026). Dr. Nadeau has also been compensated for her role in conducting studies of this device (International Forum of Allergy & Rhinology, Dec. 2025). Dr. Nadeau is Professor of Medicine at Harvard Medical School and serves in the Division of Allergy and Inflammation at Beth Israel Deaconess Medical Center. She is an Adjunct Professor at Stanford Medical School. Dr. Nadeau is also the co-author of The End of Food Allergy, which provides strategies for treating and preventing food allergies in children. Welcome to The Art of Living Big. How has your adventure been going over the past week? I am suffering from the pollen, so you may kind of hear it in my voice. Pollen in Atlanta right now is unhinged. It’s always really bad. But right now it’s really unhinged. I am taking Zyrtec or Claritin, depending on the day. Flonase. I have a air purifier in my apartment. I’m wearing a mask, like an infected person walking around outside to keep the pollen out. I am Netty potting every time I go outside and come back in. I shower before bed and wash my hair and change my pillowcases, and I am still really suffering. It’s really bad. I got one of those memberships to get your car cleaned because the pollen was coming into my car. And so every other day, I am going to bring my car. It gives me, it gives me a project. You guys, it gets me outta the house, but I’m going to wash my car, go through the car wash because. Everything is just covered in yellow. It’s just insane. So anyway, I will keep this one short ’cause I know my voice is a little funky and , that may be really annoying. It may not have been annoying if I hadn’t said it, but now that I’ve given you permission to have it be annoying, you won’t be able to not hear it. But let’s, just move along with the show today. Okay. So in the spirit of keeping this one short today, I have a. A thought. Something I was thinking about this morning when I was getting ready for my day and I was thinking about this possible move to California. I was thinking about how. I realized I really need to be consistent with my working out and not from a, like I need to lose some weight perspective or I need my body to look different. It’s actually ’cause I need my body to work a little bit different. I had a massage therapist come this morning, early. Early. She came at eight o’clock in the morning. I was barely awake. This is a woman that I’ve used for years and she just comes to my apartment. It’s, it sounds luxurious and it is that she comes to my apartment, but, , she’s really not any more expensive that if I went to a spa, , ’cause she just works for herself and came over. And I told her that I sit so much for work and I do try and sit stand throughout the day. I have one of those standing desks, but I sit so much that my back is just, it’s just tight. My lower back is tight, my shoulders are hunched ’cause I’m working away my keyboard, , looking down all the time and I’m like, I really need to be. Getting a lot more activity for my posterior chain, , for like all down my back. , I need to have my hamstrings engaged. The bottoms of my feet have been hurting, and I’m not walking, I’m sitting. But it’s because everything is just tight from being kind of like folded like a lawn chair, you know? I was thinking about that and about how I need to just move my body because it needs to be moved, not because the mood has hit me, or I suddenly have a goal for something. And I was thinking about all these choices that we have in our lives and how committed we are to those choices. And I remember years ago I used to ask coaching clients when we would come up with goals. For things is, this was before the navigate method, but I would say, are you hammer eggs? Are you hammer eggs? That was the question. And they would say, Hmm. It feels more like eggs. Eggs is like, you know, , an egg is, you know, the chicken doesn’t have to die to have to give you the egg, right? It’s, it’s the egg, ? But if you’re ham, you’re committed. You’ve committed so much to breakfast that you’ve given your life for breakfast. So is it eggs or is it ham? And I was asking myself like, oh, if I’ll think about it that way, am I hammer eggs with this moving my body? And I’m, I’ve moved into ham because it has to be ham. I was eggs before, so I moved, uh, you know, I worked out, I had my little workout thing I do once or twice a week when it, the mood struck. I was thinking about this move to the beach. Am I ham or eggs? I’m not sure. I’m, I’m leaning into ham territory of like, I’m going to make a brave choice. And so I started thinking about this with everything that we do, and if we put it in the perspective of what is it that I really want and do I really want anything? That’s just eggs. I wrote something on social media a week or two ago and so many people resonated with it. And what it was, this idea of being, having a life that is mid. I like, I’m in midlife, but I don’t want a life that’s mid, that’s how I explained it in the post mid is eggs. Mid is eggs. It’s not, it’s not ham. Now, I know this is kind of a silly little thing, but I wanted to share it with you this week so that you can just think through my choices. Think through your choices. Am I ham or eggs? Am I wishy-washy or am I committed? , Sometimes it makes sense to be eggs, and sometimes you choose eggs when it’s time to be ham. So. I think when you know the difference, that is how you live a big life. All right. I will see you next week with a much more lengthy episode when I am feeling like my brain is back in my body. If you have ever taken an antihistamine labeled "non-drowsy" and still ended up foggy and tired, you are not imagining it. Allergists have a lot to say about that label and about the foggy feeling that comes with allergy season in general. In part two of our live recordings from the AAAAI conference in Philadelphia, Kortney and Dr. Payel Gupta talk to allergists from across the US about what they actually think about antihistamines. What we cover in this episode about antihistamines: Non-drowsy does not mean the same thing for every antihistamine. Zyrtec, Claritin, and Allegra are all labeled non-drowsy, but some are more likely to make you sleepy than others. Why allergists have moved away from Benadryl. First-generation antihistamines cross into the brain more easily, causing drowsiness and a range of other side effects that are far less common with newer antihistamines. The foggy feeling might not be your medication. Congestion from untreated allergies disrupts your sleep, and poor sleep creates its own fog. Sometimes it is the disease, not the drug. Timing your antihistamine makes a real difference. Taking it before you head out gives it the best chance to work. And if it makes you drowsy, switching to a nighttime dose is a simple fix. Not all second-generation antihistamines are approved for pilots. Fexofenadine has the lowest penetration into the brain of any antihistamine tested, which is why the FAA cleared it as the only option pilots can take while flying. Studies have found that up to 40% of fatal accidents involve pilots with impairing substances in their system—ranging from allergy medications to sleep aids to alcohol. The problem? Many of these substances are legal and even commonplace, yet can significantly degrade judgment, memory, attention, and coordination. Dr. Trowbridge emphasizes that many pilots—and even their doctors—are unaware of FAA wait-time guidelines. He explains the FAA's “5x rule,” which states that a pilot must wait five times the recommended dosage interval before flying. So if a medication is taken every six hours, the pilot should wait 30 hours after the last dose. For 24-hour medications like Zyrtec, the wait time stretches to five full days. The discussion highlights the particular dangers of first-generation antihistamines like Benadryl (diphenhydramine), which are highly sedating and frequently found in sleep aids like Tylenol PM, NyQuil, and Unisom. These medications, even when taken the night before, can impair cognitive function well into the next day. Alarmingly, Benadryl is the most commonly detected OTC drug in fatal GA accidents. Dr. Trowbridge also warns about second-generation antihistamines like Zyrtec and Xyzal. While marketed as “non-drowsy,” these can still cause subtle sedation, especially in combination with alcohol or other medications. Alternatives like Allegra and Claritin are usually safer and FAA-approved—but only after personal ground-testing and AME consultation. Beyond antihistamines, they explore other drug categories. For pain relief, medications like aspirin, Tylenol, ibuprofen, and Aleve are generally safe, but anything with “PM” on the label likely contains sedating ingredients. Prescription painkillers like codeine are outright disqualifying. Dr. Trowbridge shares unconventional options too, like topical lidocaine, coconut oil, and even horse liniment—though with cautions about application and legality. Sleep aids are another minefield. Melatonin is the only one on the FAA's “go list,” and even it should be ground-tested first. Nasal decongestants such as Afrin and Sudafed can raise blood pressure and cause jitteriness, making natural remedies like saline rinses or cool vapor inhalation preferable. Cough medications also pose risks. Products with dextromethorphan (like DayQuil or Delsym) can sedate, as can multi-symptom formulas marked “PM” or “nighttime.” Gastrointestinal issues are more straightforward: most antacids like Tums and Maalox are safe, but anti-diarrheals like Imodium are not, due to sedation risks. UTIs are covered with non-sedating options like AZO and D-Mannose, but Dr. Trowbridge cautions pilots never to fly if symptomatic or on unfamiliar antibiotics. The conversation then turns to alcohol. The FAA's limit is 0.04%, but even lower levels can impair judgment, night vision, and reaction time—especially when combined with other medications or altitude-related hypoxia. Max cites an older FAA study showing that alcohol above 0.04% was found in 7% of fatal pilot crashes, with 3% involving both alcohol and drugs. Finally, Dr. Trowbridge emphasizes the importance of pilot self-awareness and due diligence. Most doctors are not trained in FAA regulations and may prescribe disqualifying medications unless reminded. He urges pilots to always research their medications, consult their AME, and even speak with pharmacists about interactions and cognitive side effects. Danielle and Elizabeth talk cats, fleas and worms. Yucky, but necessary. Danielle noticed fleas and worms on and in her outside cat, and the steps she took to rid her environment of them. She was pretty disgusted by those little cat tormentors, I have to say, Danielle keeps a very clean house so she was shocked when she saw signs of fleas and worms! Sidney: Hi doctor Cabral, I have a thick, deformed toenail and the skin around it often gets inflamed, and hurts a lot. I read that the only thing that helps is removing the nail altogether, but I would rather keep it. Is there any way to make it healthy again, or to stop it hurting as much? Thanks so much for you answer! Best, Sidney Kristin: I've been suffering from hives for a year. They started with what I thought was a spider bite on my scalp. A few weeks later I started getting itchy red hives all over my body. I have seen my general practitioner, dermatologist and allergist. I did a ton of bloodwork and they couldn't find any underlying issues or allergies. Prednisone cleared it up, but came right back when stopped. Zyrtec (at first 4 a day, now just 1 a day) cleared up the body hives, but heat and tight clothing bring them back. The itchy bumps on my scalp have never gone away. I have tried apple cider vinegar, avocado oil, coconut oil. I changed bedding, shampoos, etc. I don't know what else to do. Do you have any advice? Eric: Hi Dr. Cabral. I'm a 64-year-old male that has struggled for many years with urinary tract problems (urinary tract stricture as a child that was surgically treated, recurring bouts of urinary tract irritation from various substances and stress, but never any indication of a UTI). Over the past several months I've been doing infrared sauna sessions 2X per week, which seems to make my urinary tract problems worse. I use a binder before each sauna session. Two questions: (1) Can a sauna session trigger a large jump in toxins being processed through the kidneys/urinary tract, including oxalate dumping (I'm high in oxalates on the OAT test)? I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks… Whitney: Hi Dr. Cabral, My question is regarding my zinc to copper ratios. On the HTMA test my zinc was wildly off the charts at 335, and copper was at 1.5. Zn/ Cu ratio is 223.33. I double tested to be sure I got a good sample and it's legit. I also then tested my blood serum levels and they were in normal range (zinc 87 / copper 103). Any idea what is going on and if/how I should take copper alone and avoid zinc ? I do have these white hairs coming in that are very course which is strange for my genetics (I'm 35 year old female, brunette). I'm also a little hypothyroid, cold hands and feet. The most obvious culprit to me is that I wear mineral (zinc based) sunscreen daily (I work outside). All the sunscreens I use claim to be non nano zinc but maybe they are still absorbing? Thanks SO much!!!! Joyce: Hi! I am waiting to receive my 21 day detox kit and my question is about the hours I should be "eating" on days 3-7. I get up usually around 8am so my first shake will be at 9am, lunch at 12:30pm, 2nd shake at 4pm and dinner at 7:30pm. The problem is that 7:30pm is too late for me to eat. Is it possible to modify these times so that I am not eating dinner at 7:30? Can I interchange the 4pm shake with the 7:30pm dinner? Please help before I get started on this program. I am looking forward to feeling better! Thank you, Joyce Kelsie: Hi Dr Cabral. Recently I have had some constipation with very large stool. It ends up causing bleeding and is very painful to pass. I've tried upping fiber and it doesn't help. Any tips? Jillian: My question is regarding my almost 2.5 yr old daughter. On and off since birth she has slept with her mouth open. This has gotten worse to where she sleeps with it open almost always for about 6 months now. She snores which interrupts her sleep and during the day seems to keep her mouth open while playing/running. Her voice is also a bit nasally. Despite this she does not get sick often but when she does get a cold the extreme mucus seems to last a month or more. Her doctor recommended she take Zyrtec nightly to see if it's an allergy issue and also recommended us for a sleep study. The Zyrtec does seem to help with the night snoring and coughing. My question is 1) are there long term affects of taking Zyrtec regularly? And 2) do you rec removing tonsils/adenoids? What do you rec for this? The doctors are suggesting that it is genetic and there is nothing that can be done to improve her condition. Episode 21 Danielle and Elizabeth getting caught up on all things cats and daily life.In case you hadn't noticed, Danielle and Elizabeth love cats! Danielle lives with 4 cats and Elizabeth with 3 (there are a couple of husbands in there too, who have learned to deal with it.) Elizabeth started with an update on Max, who she got as a kitten and who was plagued by itchiness, so much so that he scratched all of the fur off of his face, throat, and ears. Elizabeth took him to four different vets and many vet visits before finding a treatment that worked. She and Danielle started the podcast to help others in this situation. What finally worked for Max was Zyrtec plus anti-itch meds. Right now he's taking Zyrtec daily and being weaned off of a steroid while taking another medication for skin allergies. He's a little plump from the steroid but hopefully exercise will burn that off! He has turned into a bit of a holy terror, he's still very young and energetic.It was Danielle's birthday on May 4th, her cat Tony did a special hat trick for her!The discussion turned to indoor and outdoor cats, the risks and the rewards for the cats.Elizabeth talked about how Max wakes her up, he's training her to be on his schedule using a very effective method...groan. Danielle talked about Tony getting unhappy and causes of that. Cats can be very particular!Elizabeth talked about how her cat Mercutio (Cushi) is sneezing and congested, and the vet not only prescribed meds but also recommended that she switch the cat litter to unscented litter, as cats' sense of smell is highly developed.Elizabeth talked about the radio show she co-hosts with her husband Richard called Passage to Profit that they record in the iHeart studios in Manhattan. Richard was ill so she had to go by herself this week and she got stranded in NY for hours because the trains quit running. Of course the station is right by Macy's, so she found good use for the time. There's a silver lining to every cloud.Danielle talked about NY some, then about cats in general, and how Jared Leto showed up in a cat costume at the Met Gala. Episode 20 with Lori Saitz, a 30 Day Traveling Sabbatical with her 19 year old cat Panther.This is the podcast all about cats! Danielle started by showing off her new glasses from SPEX Eyewear, where 100% of the proceeds go to cat rescue and animal rescue. She looked adorable, as always. In 2021, over 81 million in the US were diagnosed with seasonal allergies. Approximately 26% of adults and 19% of children. The global Nasal Antihistamines Market was projected to total US$ 1.9 billion in 2021 (prnewswire.com) In 2017, Claritin and Zyrtec sold almost $600 million in tablets. The question I'm asking is why? Environmental allergies develop when the body's immune system recognizes and overreacts to things like pollens, dust, animal dander or mold; things that typically don't cause a reaction in most people. It's spring when I recorded this - so many people were suffering from symptoms due to allergies. It's affecting quality of life. So, today we talk about some things you can do to get some quick relief from those symptoms. Some are simple new habits you can pick up to keep the allergens out of your home. Some are products that you may find helpful. BUT we are also going to think about long term solutions. I share about my son's journey with allergies, too. He used to have year-round allergy symptoms. But now he doesn't. After treating his body for Lyme disease, his allergies also went away. Whether or not allergies can be healed is debated. I've learned so much from you and share your podcast every chance I get! Long story short, I learned about natural health a little too late in life. After years of imbalance, infertility, dis-ease, and irreversible surgery, my husband and I are hoping to adopt an infant and I've recently learned that it is not our decision whether or not the child is fully vaccinated according to the standard vac schedule. It is the state's decision until the child is 6 months old when the paperwork has all gone through. So basically I will be forced to take the baby to every ‘well child' appointment and hand them over to a well meaning, but ignorant hospital employee for all of the neurotoxins mandated by the gov. After learning this I initially decided that I can't be a party to that. I've wrestled with this for months and decided that any poor child in the system would at least be better off with us where we can try to mitigate the damage immediately and give them a healthy full life to the best of our ability. So I'm wondering what is the best way to physically prepare the baby for each visit and how to detox from these unnecessary tox injections? Thanks again! Summer Summer: Hi Doc! My son is 7.5 years old and has suffered from allergy induced asthma for the last 6 years. We thought he was doing much better, then last October he was hospitalized for 3 days due to an asthma attack. Less than 3 months later he ended up with pneumonia and not to mention all the viruses he's had between. Of course the allergist wants him on Flovent 2 times a day, Albuterol, allergy shots & daily Zyrtec but this just doesn't feel right. We've done a food sensitivity and removed those items including dairy & gluten. We met with a holistic nutritionists that did a stool test & learned he has gut dysbosis & a presence of H . Ployri. He is currently on a supplement regimen to help those issues but still doesn't seem to be improving. We have an air doctor, water filter, only use natural cleaning products & hygiene products. We eat organic & only local grass fed meat. We have removed all the toxic burdens that we know of and are at a loss of how to help him heal. I appreciate any feedback for next steps. Thank you!
