He's been noted as an influential healthcare physician executive with numerous accolades. "This is the classic US vs. THEM. I was speaking in front of a large group of physicians, and I remember saying, 'I was always one of ‘US'. And I was always frustrated with ‘THEM'. Until one day I realized, I am now ‘THEM'." Join us for an inspiring conversation with Dr. Darryl Elmouchi, a cardiac electrophysiologist and President of Corewell Health West, one of the largest health systems in Michigan. Tune in as we take a deep dive into leadership in medicine, specifically why the best physicians have the potential of becoming the best leaders and what we can start doing today to unleash that potential. Pearls of Wisdom:   1. We should hone three skills: Kindness, discipline, and the ability to deconstruct complex problems into simpler instances. 2. We think of leadership as something that starts when your hair starts to gray. Leadership is not acting when you have a title. Leadership is simply about finding solutions to problems. 3. The moment we all start to take the time to understand the why behind what the THEM or US are doing, that "why" leads to the WE. 4. The ability to poke fun at ourselves or other things in the middle of our serious jobs really brings the team together and makes us human.

Pearls and Quotes Cardiac amyloidosis is no longer considered a rare disease, especially transthyretin amyloidosis in older male patients with HFpEF and aortic stenosis. Echocardiogram is the “gate keeper” of cardiac imaging and provides initial evidence of amyloid infiltration, while cardiac MRI can help refine the presence of an infiltrative cardiomyopathy versus other causes of increased wall thickness. The most clinically important types of amyloid heart disease are transthyretin (ATTR) and light chain (AL) amyloidosis. The workup to differentiate these disorders includes a gammopathy panel to screen for the presence of potentially amyloidogenic light chains (serum and urine electrophoresis WITH immunofixation and serum free light chains), and cardiac scintigraphy with Technetium-99m-labeled bone-seeking tracers (PYP, DPD, etc.) to identify cardiac aTTR infiltration if the gammopathy panel is unrevealing. There is still a role for endomyocardial biopsy in the diagnosis of cardiac amyloidosis! All patients in whom there is concern for cardiac amyloidosis and gammopathy panel indicates the presence of monoclonal light chains should have a biopsy to obtain a tissue diagnosis of likely AL amyloidosis. Alternatively, an endocardial biopsy may prove valuable in patients who have confusing phenotypic features between amyloid types, such as a patient with abnormal monoclonal protein and positive PYP imaging. Be suspicious of heart failure patients that do not tolerate typical medications that lower heart rate. In the restrictive cardiomyopathy of cardiac amyloidosis, patients are reliant on higher heart rates to compensate for the inability to augment stroke volume. Be suspicious of amyloidosis in patients with recurrent left atrial thrombi despite anticoagulation. Show notes CardioNerds Cardiac Amyloid, updated 1.20.21 1. What is cardiac amyloidosis and how common is it? Cardiac amyloidosis is adisorder caused by misfolding of proteins into insoluble forms which are deposited into extracellular spaces of the heart, commonly causing a stiff and thick heart with progressive diastolic dysfunction with restrictive hemodynamics and ensuing heart failure. The two most common types of amyloid protein that affect the heart are transthyretin (ATTR) and light chain (AL).

Our guest in episode 52 almost needs no introduction. Dr. Barry Franklin is one the most prolific exercise physiologists of our time. He is a past president of the American College of Sports Medicine, and has his bio below attests, he is one of the most acclaimed professionals in our field. This conversation is not about exercise physiology, however. The depth and breadth of Dr. Franklin's knowledge extend far beyond exercise testing and prescription. This is a conversation about how to maximize your personal and professional development to lead the most fulfilling and impactful career possible as a fitness professional. In his early 30's, Dr. Franklin intuitively embraced Peter Drucker's recommendation that “everyone should find at least one interest outside his/her primary area, and make that secondary pursuit into more than just a hobby.” For him, that secondary pursuit, the study of highly successful people in all walks of life, became a passion. To learn more, he read countless books on the topic and carefully studied the behaviors of highly successful people using the methodologies of a research scientist (his academic training). He simultaneously conducted a sobering experiment on himself. When he started applying these behavioral skills and strategies to his own life, he began attracting good luck and related professional opportunities, but in an exponential manner. He then took what he had learned and developed a college course titled “GPS for Success,” which he taught at Central Michigan University (2012−2017). Since the mid-1990s, he's participated in an escalating number of media (e.g., YouTube TV) and speaking invitations on this topic, and has given success-related commencement addresses at major universities. This hobby, turned passion, turned college course, culminated in his latest book, GPS for Success. This conversation will unpack in great detail the tangible framework Dr. Franklin presents in his book for becoming a super-achiever in any professional endeavor. Make no mistake about it, these are time-tested and well-studied lessons of how to become a super-achiever from someone who has walked the walked, becoming a super-achiever in his own right, in our field. Get your notebook ready, many actionable insights to come in this conversation. Show Notes Page: https://www.wellnessparadoxpod.com/podcast/episode/52Our Guest: Dr. Barry Franklin, PhDBarry A. Franklin, PhD, is Director of Preventive Cardiology and Cardiac Rehabilitation at Beaumont Health, Royal Oak, Michigan. He holds faculty appointments as Professor of Physiology at Wayne State University School of Medicine and Professor of Internal Medicine, Oakland University William Beaumont School of Medicine. Pursuing his interest in combining exercise physiology with cardiology, Barry and his associates have studied the physiologic and clinical responses to numerous occupational and leisure-time activities in people with and without heart disease. Other research interests include the prevention of heart disease; cardiovascular risk reduction; obesity and metabolism; exercise testing and prescription; and lifestyle medicine.Dr. Franklin has written or edited nearly 700 publications, including 547 papers, 100 book chapters, and 26 books. In April 2020, Beaumont Health, Michigan's largest healthcare system, made the decision to terminate its established anesthesia group in favor of Texas-based NorthStar Anesthesia, a private equity firm that promised the health system lower staffing costs.  According to reports, more than a dozen physicians and surgeons resigned from the hospital system due to concerns over NorthStar's reputation.NorthStar's contract began on January 1 of this year, and on January 25, Eric Starkman, a reporter at Deadline Detroit, reported the death of a patient due to an anesthesia-related complication during one of the most routine medical procedures: a colonoscopy. Did the replacement of Beaumont's anesthesia team by a private equity firm contribute to the death of this patient?  Not only is Beaumont not talking, but according to reports, staff members are being threatened and intimidated into silence. To help us we will discuss how private equity takeovers in medicine may be harming patients, and what you can do to protect yourself, we are joined by Karen Sibert MD, Clinical Professor in the Dept of Anesthesiology and Perioperative Medicine at UCLA, and a past president of the California Society of Anesthesiologists.Dr. Sibert's advice to all patients undergoing anesthesia: "Ask, 'who is my anesthesiologist?' And if the answer is, 'there isn't one,' they should leave."  Further, "if there isn't a physician overseeing your anesthesia care, I can't reassure you that's going to be a safe situation. I think that every patient has a right to know who is taking care of them. There should be complete transparency, and I think physician-led care is the safest care."Get the book!  https://www.amazon.com/Patients-Risk-Practitioner-Physician-Healthcare/dp/1627343164/Dr. Karen Sibert's blogs:  https://apennedpoint.com/how-could-a-patient-die-from-anesthesia-for-a-colonoscopy/ https://www.kevinmd.com/blog/post-author/karen-s-sibertOne of our personal favorites, which we cite in our book: https://www.kevinmd.com/blog/2011/11/unsupervised-anesthesia-care-nurse-anesthetist-threat-patient-safety.html

Think through reactive messages within that context.Highlight how these partnerships will facilitate continued care in a market, which otherwise might lose a valuable healthcare resource.Share positive economic benefits the transaction will bring for local communities.Communications that support the vision, rationale and benefits of a deal will also need to be relevant to the regulatory bodies whose approval may be required.Ensure stabilization where there is only one healthcare agency within the community. This goodwill can be valuable as health systems seek stakeholder and community support for their transactions.Public perception and support of healthcare providers have been extremely positive during the pandemic to date, as evidenced by homemade banners, balcony tributes, and praise on social media.Health systems and their staffs have borne personal risk and financial pain by focusing on patients and public health at the expense of all else. But that goodwill can also be quickly forgotten. As health systems race to the altar to beat out competitors for M&A targets and other strategic relationships, it is critical that they are thoughtful in structuring their deals and justifying the activity. For example, acquisitions and partnerships involving lucrative executive compensation or severance packages will be viewed negatively if undertaken by a system that instituted large compensation reductions across the system or even furloughed or laid off employees during the pandemic.Internal communication has never been more important. We always counsel clients to focus on internal audiences first when announcing an acquisition. But this is even more critical right now since staff have been working tirelessly to treat patients with COVID-19 as well as recover financially from the last few months.Any effective communication plan will need to clearly articulate the benefits to staff and allay any fears of layoffs.

DTE Energy says it expects to sign a deal with the city to swap a decommissioned power plant on the east riverfront so it can be used to store Jeeps built at a new factory being pursued by parent Fiat Chrysler. [Freep] Why is this sad, you ask? This is the important line. FCA said it needs DTE’s Conners Creek land for storage of vehicles for the Jeep plant to be built at Mack and St. Jean. - Detroit Free Press n my opinion, this somehow feels quintessentially Detroit, a repeat of what we’ve always done: surrender to the automobile. Weren’t we supposed to be trying to reclaim our riverfront? And wasn’t DTE Energy crowing about redeveloping that parcel? (Why yes. Yes, they were.) How do triggers impact us in our lives, in our performance, and in our relationships? This week, we're exploring these very fundamental questions. The fact of the matter is, we ALL have triggers. It's simply an artifact of the way our minds work. What's most important is that we know what our individual triggers are and how we can be intentional about responding when we encounter them. This week we're hearing from Jodie Eckleberry Hunt. Jodie has a Ph.D. in Counseling Psychology from Indiana State University and is board-certified in Health Psychology. She has been the Director of Health Psychology and Executive Health and Wellness at Beaumont Health since 2016.I truly believe you'll gain a much more nuanced understanding of what a trigger is, how to identify your own, and what to do to be more responsive, rather than reactive, in the face of them.Additional Resources:How to Liberate Your Mind7 Principles for Making Marriage Work (John Gottman)EOS LifeJodie's WebsiteAdvertising Inquiries: https://redcircle.com/brandsPrivacy & Opt-Out: https://redcircle.com/privacy
