In 2017, Dr. Laage Gaupp had been a second-year diagnostic radiology resident when he traveled to Tanzania for an IR readiness assessment. He found that most of the infrastructure to support IR procedures were already in place; however, there was no formal training program. From there, he and other Road2IR co-founders launched East Africa's first IR training program, as a collaborative effort between Muhimbili University of Health and Allied Sciences (MUHAS), Yale Radiology, Emory Radiology, and many other partner institutions. Since then, graduates of the training program have gone on to become professors of IR in Tanzania as well as other countries. The early years of the program required a lot of flexibility and patience, due to the limited amount of resources. It was necessary to start with simple procedures like core needle biopsies, abscess drainages, and nephrostomy tubes. Additionally, Dr. Gichoya emphasizes that these ordinary procedures can make a drastic difference in a patient's life and even impact entire families. Being able to perform and teach a full spectrum of minimally invasive, life-saving procedures energizes her and other faculty members who donate their time and energy. Dr. Newsome has served as the program director for the MUHAS IR program, and she speaks about the challenges that arose during the COVID pandemic, in terms of healthcare policy in Tanzania, as well as restrictions for university faculty travel in the United States. Through the height of the pandemic, the training program persisted with virtual oral examinations, meetings, and lectures. The logistics of travel, equipment, and education are still major challenges today, and they are addressed by a dedicated team of individuals with common goals. Finally, we cover the concept of reverse innovation, aspects of healthcare in under-resourced settings that can inform the U.S. healthcare system. In einer Sonderfolge unseres Podcasts „Augsburg, meine Stadt“ erzählt sie unserem Kollegen Tom Trilges von einem geplatzten Georgien-Trip, einem Sommersemester ohne echtes Studentenleben und von Einkäufen für die Großeltern. Dr. Nora Gaupp vom Deutschen Jugendinstitut ordnet im Gespräch die Erlebnisse von Laura Freilinger in einen größeren Kontext ein. Bezogen auf das Familienleben relativiert Gaupp im Podcast die positiven Erfahrungen von Laura Freilinger, die ein Zusammenrücken im eigenen Elternhaus und eine Verbesserung des Verhältnisses zu den Großeltern beschreibt: Aus Sicht der Expertin sind solche Entwicklungen vor allem bei Familien zu beobachten, die vorher bereits intakt waren und bei denen die Möglichkeit bestand, in den Garten oder die Natur zu gehen. Bei ohnehin vorhandenen Spannungen habe Corona vor allem im Fall von räumlicher Enge Konflikte zwischen jungen Leuten und ihren Familien eher verschärft. Einig sind sich Gaupp und Freilinger darüber, dass sich niemand, trotz einiger positiver Aspekte, einen zweiten Lockdown wünsche. Die 19-Jährige schätzt nun wieder mehr die kleinen Dinge. „Ich freue mich, wenn ich mit Freunden mal wieder an den Weiher gehen kann. Das Sozialleben wieder zurück zu haben, ist ein gutes Gefühl“, sagt Freilinger im Podcast.

I call it the 777  Now last week i was a quite busy and actually had fully prepared show notes last friday and sat down on saturday to record.  While recording i got a called up to work at the Guns and Roses concert.  I thought i would finish it up after the concert but got home around 2am and had a full day of family events and work on sunday, by then the script was nearly outdated and what had been recorded need to be corrected to add and subtract a few day to the 7 day calendar. Lesson learned I need to simplify the show so i don’t get bogged down on too many details of the 777forecast and make my forecast more direct and compact. I was in Japan for a few weeks with my family while at Tokyo Disneyland nearly collapsed from abdominal pain, I thought it was just kidney stones but upon the insistence of my wife and father inlay i was taken to the emergency room and diagnosed with acute kidney infection, Pyelonephritis, caused by passing massive kidney stones that caused tearing in the ureter which is the tube between the kidney and the bladder. I was admitted to Mishima hospital for 7 days. It was pretty serious, my blood test revealed deadly levels of bacteria and high white blood cell count. The doctor braced me to prepare for the worse but hope for the best. I was on the verge of sepsis or septicemia which is a blood infection which could lead to septic shock which has a 50% mortality rate. Dr. Suzuki and Dr. Okubo and the whole staff at Mishima Hospital worked hard to heal me and by the grace of God i was cured in 5 days. But its a long road to recovery. It was pretty scary at first, being in a foreign land far from home. My Japanese is pretty limited but i am always trying to learn more and improve my japanese vocabulary. I speak survival japanese I can from greeting to and direction i talk about the weather, food and nod politely but i needed to communicate with the medical staff at the hospital if i wanted tell them how i felt and what i needed or just wanted to know what was going on inside my body. While in the hospital i used google translator app to converse with the doctors, nurses, pharmacist food service and staff. I am blessed to have a loving wife with caring in-laws. Both my mother and father inlaws came to my room several times to check in on me and bring me some much need snack and my favorite VitaC1000 dinks. My wife had to travel on with my boys, because the trip was for my kids and a chance for my wife to visit the grave sites of her recently deceased. Hospitals are no place for kids to hang out so off to Legoland and Kyoto they went. It’s safe to say that most of you may never spend a single day in a Hospital but let me tell you. The staff is so kind and caring the facilities are clean and well managed and the food, i ate everything they served me and enjoyed every bit of it. I tried new foods that prior to my stay in the hospital, might have otherwise passed up for the more popular japanese food such as my go-to sushi plates and udon noodles. But in the Mishima Hospital the dishes were more like japanese country cuisine bamboo shoots, nabe, wide varieties of fish, salads and side dishes. Needless to say after my first two days as i started to regain my health i ate very well. Fortunately i still have my mother and father in my life. My Dad sends my a prayer book from “Today” every two moths, i found it interesting that the first night in the hospital the lesson and prayer was titled “The Stone”. My mom is a retired nurse form the Ohio State University hospital and she is my personal medical assistant always there to help me understand what is wrong with me my wife or kids. She offered insight and loving comfort which was greatly needed at that time Laying in my hospital bed It game me plenty of time to think about my life, loved ones and the direction i would wish to proceed. I busted out some sophmorish artwork to keep up with my 4Buckeyes comic strip (learning that i can’t draw very well and i miss my computer to generate clean graphics and much better text … because my handwriting is worse than my 4 y.o. son) As Sinatra would sing in the standard “My Way”, “Regrets i have a few but then again too few to mention”. I decided that i would confess to this audience and anyone there after that i am not perfect, that i have said and done things that i wish i could take back or do again with a little more love in my heart. I resolved to be a better person every day and practice the golden rule of doing unto others as i would have them do unto me. I asked god for forgiveness, a chance to heal and the strength improve upon my life every day after. I was scared so reached out to my friend and family asking for their thoughts and prayers, something i never have done before but in the darkness of my hospital room thousands of miles from home, i cried in pain and was overwhelmed with sorrow and guilt wanting to be a better husband, father, son and friend to all. With great delight i received the thoughts and prayers from those i asked. It built up my spirit and health. Let me tell you Humility and Prayer works. Remarkably i was healed faster than 7 days the doctor predicted, I was discharged after 5 days and got to re-join my family and travel on to Osaka. Still weak and sore and irritable i realized that changing my was would take time tobe the better person i desire to be. I thank God, my family, friends and the entire staff at Mishima Hospital. Dr. Suzuki, Dr. Okubo, Then nurses Hitomi, Nagisa, Yuka, Yutom, Kaho, Yuka, Azumi, Tomoko. The food service staff Kotomi and Mieko. The pharmacist Yuichi Naito I have a few check ups with my doctor and urologist coming up in the near future but i am on the road to recovery, feeling better. I have started new exercise routines to strengthen my body mind and spirit. While there are many causes for Kidney Stones, Hydration may be the best prevention flushing your system of Oxalate excessive vitamin D or protein. In the last 10 years i have been drinking lots of water but the fact is my stones have been forming all my life and working in the hot sun as a life guard and surf cameraman over the years may have caused dehydration. I think back to my youth and realize that i didn’t drink enough water, even though i was a swimmer surrounded by water i just didn’t drink enough water. My kids take water to school and refill their bottles while at school. I remember this days in school waiting my turn at the drinking fountain and those friendly taunts and teases while i took my turn drinking “Hey save some for the fish!”. My best advise is stay hydrated. Kraepelins Nosologie hat nach ihrer erstmaligen, teilweisen Präsentation in der 5. Auflage des Lehrbuches 1896 und der vollständigen Vorstellung in der 6. Auflage des Lehrbuches 1899 zu vielen verschiedenen Reaktionen von Kraepelins Zeitgenossen, Schüler und Nachfolger geführt. Kraepelin hatte ein System aus zwei grossen Gruppen erstellt, das die bisherigen – auch bewährten – Einteilungen radikal zerstörte. Die Kritik an dieser Dichotomie aus Dementia praecox und manisch-depressivem Irresein ist für viele Autoren der Hauptgesichtspunkt. Daneben findet sich Kritik am generellen, vom Weltbild des 19. Jahrhunderts geprägten Wissenschaftsverständnis Kraepelins, an seiner klinischen Methodik und der Vernachlässigung der psychologischen Komponente, der andere Schulen (BLEULER, FREUD, KRETSCHMER) wesentlich mehr Bedeutung zumessen. Aufgabe der vorliegenden Arbeit war es, die Beurteilung und Akzeptanz von Kraepelins Konzept der Geisteskrankheiten über den Zeitraum von 1900 bis 1960, also von der Vorstellung der vollständigen psychiatrischen Lehre Kraepelins 1897 bis über seinen 100. Geburtstag hinaus darzustellen. Daneben sollte die Besprechung von Kraepelins wissenschaftlicher Grundeinstellung über diesen Zeitraum analysiert werden. Die verschiedenen Ansichten zu Kraepelins Nosologie und seinem Wissenschaftsverständnis lassen sich anhand von Kraepelins Lebensdaten, historischen Ereignissen und der Weiterentwicklung der psychiatrischen Forschung in drei Phasen einteilen. In der ersten Phase nach Erscheinen der 6. Auflage des Lehrbuches 1899 bis etwa 1920 wurde Kraepelins Werk erbittert (THALBITZER) und polemisch (HOCHE) bekämpft und von vielen weiteren Autoren bewertet. Während jedoch die meisten anderen Autoren mit Einzelgesichtspunkten, die die Einteilung in Subgruppen in Kraepelins Lehre betreffen, nicht einverstanden sind, sind Kraepelins Hauptkritiker HOCHE und THALBITZER mit dem Gesamtaufbau von Kraepelins System, der wissenschaftlichen Grundlage und Logik nicht zufrieden Der Angriff THALBITZERS erfolgte gegen Kraepelins Sonderstellung der Involutionsmelancholie gegenüber den manisch-depressiven Erkrankungen. Auch die Unterdrückung der Zustände von einfacher Manie und einfacher Depression ohne Rezidiv und ihre ausschließliche Auflösung in der rezidivierenden manisch-depressiven Psychose ist ein wichtiger Diskussionspunkt (RÉGIS 1907). HOCHEs Polemik richtete sich gegen Kraepelins Krankheitsformen im allgemeinen, die er für nicht zielführend in der Diagnostik hält. Lediglich SOUTZO (1906) sah Kraepelins Werk als „Quintessenz“ der modernen Psychiatrie an. Unterstützung für Kraepelins Werk kam von DENY (1907) und DREYFUS (1909) in der Frage der manisch-depressiven Psychosen und von WALKER (1907) in der Frage der Dementia praecox. JASPERS (1913) sieht in der Erstellung von Krankheitsformen eine nicht erreichbare Aufgabe. Die Auseinandersetzung mit dem der Nosologie zugrunde liegenden Wissenschaftsverständnis Kraepelins ist vor 1920 spärlich (HOCHE 1906 und JASPERS 1913). Während HOCHE den gesamten wissenschaftlichen Ansatz Kraepelins mit Polemik überzieht, ohne Verbesserungsvorschläge machen zu können, geht JASPERS von einem vielschichtigeren psychopathologischen Modell aus, als das strenge morphologisch und klassifikatorisch geprägte Verständnis Kraepelins. Nach 1920 scheint Kraepelins Werk anerkannt. Nur noch ein Artikel mit harscher Verurteilung der fehlenden Dynamik der Kraepelinschen Dichotomie erscheint (MEYER 1922), es überwiegen leichte Meinungsverschiedenheiten (KRETSCHMER 1919, GAUPP 1926) über methodische Fragen. Schon 1926 stellt GAUPP Kraepelins Werk – trotz der bemerkten Mängel – als historische Leistung dar und beschreibt die alle anderen Lehren verdrängende Wirkung von Kraepelins Nosologie auf die Diagnostik der modernen Psychiatrie. Die zweite Phase beginnt mit Kraepelins Tod im Jahre 1926 und endet 1928. Werk und Persönlichkeit des Forschers Kraepelin werden in den Nachrufen mit Ruhm und Lob überhäuft, die Leistungen Kraepelins werden bereits als epochal eingeschätzt (BIRNBAUM 1928). Deutliche Ablehnung zeigen nur wenige Autoren (HENNEBERG 1926, CLAUDE 1927, HALBERSTADT 1927) Die schärfste Kritik kommt ein weiteres Mal von MEYER (1927), der eine Reihe sachlicher Differenzen beleuchtet und an Kraepelins Forschungsmethode die Vernachlässigung des konstitutionellen Hintergrundes bedauert. Zu Beginn der dritten Phase, 1929, erscheinen zwei Artikel von GRUHLE und MAYER-GROSS mit Meinungsverschiedenheiten in einzelnen Punkten. Beide bedauern die fehlende Einbeziehung der Psychologie in Kraepelins Werk. Die mit MEYER (1922) begonnene Diskussion über das statisch-anatomische Wissenschaftsverständnis Kraepelins ist hier auf dem Höhepunkt. Dieser systematische Fehler in Kraepelins Nosologie wird jedoch mit dem wissenschaftlichen Verständnis des 19. Jahrhunderts und den damaligen Ansichten über den psychisch kranken Menschen entschuldigt, denen sich Kraepelin nicht entziehen konnte. Das Festjahr 1956 findet Kraepelins Werk in eine historische Perspektive gestellt, die Kritik – sofern überhaupt vorhanden – fällt milde aus. Kraepelins Werk gilt für die Autoren GRUHLE, SCHNEIDER, WYRSCH (alle 1956), BRACELAND (1957) und KAHN (1959) als Grundlage moderner Psychiatrie, Kraepelins Werk gilt als unentbehrlich für die moderne Diagnostik. Ein – im Hinblick auf die in den 60er Jahren beginnende Debatte der „Neo-Kraepelinianer“ – früher Denkanstoß zur Überarbeitung des Systems wegen der zu scharfen und klinisch nicht haltbaren Trennung zwischen manisch-depressiv und schizophren kommt von CONRAD (1959). Kraepelins Nosologie hat sich innerhalb von zwei Dekaden gegen die vorhandenen Lehrmeinungen durchgesetzt und gilt ubiquitär seit etwa 1920, wurde zwar abgewandelt durch den „Neo-Kraepelinianismus“ und moderne Forschungsergebnisse, ist aber als konkurrenzloses Fundament der Psychiatrie für die Diagnostik in ICD-10 und DSM-IV unverzichtbar. Die Anreicherung von Kraepelins System durch die moderne Forschung zeigte zwar den Mangel des Systems an Variabilität, aber erst durch diese Anreicherung wurde erkannt, daß Kraepelins Nosologie kein abgeschlossenes Kapitel einer wissenschaftlichen Meinung ist, sondern die wandlungsfähige, weiterhin gültige Grundlage einer klinischen Einteilung.
