Dean's Chat hosts, Drs. Jensen and Richey, welcome Dr. Stephen Barrett to the podcast! Podiatrist and surgeon Stephen Barrett, DPM, MBA, ABPS, FAENS, brings decades of experience in foot and nerve care to US Neuropathy Centers in Marietta and Atlanta, Georgia. He is a highly-regarded expert in heel pain, pain management, peripheral nerve surgery, and diabetic peripheral neuropathy. He is proud to provide the best health care to his patients. Dr. Barrett earned his medical degree from Dr. William M. Scholl College of Podiatric Medicine at Rosalind Franklin University in North Chicago, Illinois and completed his residency at UTMB/Danforth Hospital in Houston. A pioneer in his field, Dr. Barrett was the first physician to invent and utilize minimally invasive endoscopic surgery to treat heel pain. He also performed the first diabetic peripheral nerve decompression in Spain, which was observed in Barcelona on closed-circuit television by 74 of Europe's top surgeons, neurologists, and endocrinologists. Dr. Barrett holds 5 patents; two for developing innovative endoscopic procedures — the endoscopic plantar fasciotomy (EPF) and endoscopic decompression of the intermetatarsal nerve (EDIN). He believes in preparing the next generation and has trained over 6,000 surgeons around the world to perform these procedures. The seminal textbook for orthopedic surgery, “Campbell's Operative Orthopedics,” also includes his procedures. Dr. Barrett is also the author of two textbooks, "Diagnostic Ultrasound of the Foot and Ankle", published in 2013, and “Practical Pain Management for the Lower Extremity Surgeon,” published in 2015. In addition to his textbooks, Dr. Barrett has published more than 50 articles in peer-reviewed medical journals, chapters included in other medical textbooks, and co-authored a textbook on the interpretation of neurosensory testing. He is also a contributing editor for the Journal of the American Podiatric Medical Association and on the editorial board for Podiatry Today and Practical Pain Management. Dr. Barrett is board-certified by the American Board of Foot and Ankle Surgery and the American Board of Podiatric Medicine. He formerly served as the president of the Association of Extremity Nerve Surgeons, where he was awarded the prestigious Jules Tinel MD Award in 2010. He now serves as the Chairman of the board of trustees for the Association of Extremity Nerve Surgeons. Caregiving or unpaid care work has often been taken for granted. To some extent, there is now a growing recognition of its value, as seen in initiatives like setting up EPF accounts for homemakers. However, with Malaysia becoming an ageing society, unpaid care work is increasingly carried out by older people for other older people, and by young people for their parents or grandparents. In the case of the latter, this group is referred to as the "sandwich generation.” The sandwich generation refers to young or middle-aged adults who care for both their aging parents and their own children. We recognise that women are often expected to shoulder the burden of caregiving and unpaid care work while also managing full-time jobs. This raises important questions: how do they find time to care for themselves, and what are the long-term impacts on their well-being? In this episode of GOSSIP, we invited Lee Min Hui, Senior Analyst in the Social Policy and National Integration Division at the Institute of Strategic & International Studies (ISIS).  While discussing the importance of valuing unpaid care work, Lee Min Hui shared a notable insight: “If the unpaid care work produced in Malaysian homes every day could be valued in GDP figures, it would create about RM379 billion in 2022 alone.” This leaves us with a new perspective on how profitable the industry of care work could be.  Want to learn more about unpaid care work? Early pregnancy failure (EPF), pregnancy loss in the first trimester, is a common pregnancy complication, with 15 to 20% of clinically recognized pregnancies ending in the first trimester. It is not uncommon for women with one or more cesarean sections, or myomectomies, to present with EPF. Options for treatment of early pregnancy failure include expectant management, surgical management (D&C), or medical management with medications such as misoprostol (miso). However, the limited information available regarding miso with EPF in those with prior uterine surgery may complicate the counseling of these women. What does the ACOG say about misoprostol use in the first trimester for these patients? Is misoprostol safe for EPF in women with 2 prior cesareans? In this episode, we will explore the data for miso use in these cases with a focus on the first trimester, but also review the data for miso use in the early second trimester in those with a scarred uterus.

O nouă ciocnire între Ungaria și partenerii europeni a avut loc în cadrul reuniunii de joi a miniștrilor de externe, desfășurată la Bruxelles. Întâlnirea trebuia să aibă loc la Budapesta, dar șeful diplomației europene, Josep Borrell, a convocat o altă ședință, în capitala UE. Se anunță o toamnă tensionată între Budapesta și restul capitalelor europene, în timp ce Ungaria va continua să dețină președinția rotativă a Consiliului UE, până la sfârșitul anului. La întâlnirea de joi dimineață a participat și ministrul de externe al Ucrainei, Dmytro Kuleba. Potrivit unui oficial familiarizat cu discuțiile și citat de Politico, oficialul ucrainean le-a cerut miniștrilor UE să permită atacarea instalațiilor militare din Rusia, spunând totodată că propunerea nu ar trebui considerată o escaladare.Apelul lui Kuleba a fost susținut în cadrul întâlnirii de Franța, Suedia, Letonia, Țările de Jos și Polonia, mai mulți diplomați remarcând că dreptul internațional nu interzice unei țări să intre pe teritoriul unui agresor pentru a se apăra.În tabăra „nu”, Slovacia, condusă de pro-rusul Robert Fico, a deplâns că întâlnirea nu a avut loc la Budapesta.În replică, ministrul maghiar de externe Péter Szijjártó a spus într-o postare pe Facebook, că „trebuie oprită furia Înaltului Reprezentant”. Oficialul ungar a mai afirmat că „nu vrem mai multe arme în Ucraina, nu vrem mai multe morți, nu vrem o escaladare a războiului”.Reuniunea este însă doar un preludiu pentru o toamnă tensionată între Budapesta și partenerii săi europeni. Iar semnele se observă.Ungaria a primit deja dovezi ale lipsei de solidaritate pe care ea însăși a manifestat-o, de mult timp, la adresa partenerilor europeni.Luna trecută, Comisia Europeană a respins o cerere din partea Ungariei și Slovaciei de a iniția o procedură de consultare cu Ucraina cu privire la sancțiunile petroliere impuse de Kiev producătorului rus Lukoil.Citeste siRoata se întoarce și Budapesta a ajuns acum la mâna Bruxelles-uluiDecizia ucrainenilor pune în dificultate Ungaria și Slovacia, care nu au făcut niciun fel de efort pentru a-și diversifica sursele de aprovizionare – dimpotrivă, și-au sporit dependența de ruși prin noi acorduri.Răspunsul sec al Bruxelles-ului a fost că, în acest moment, ”consultările nu par să fie justificate”.De asemenea, Bruxelles-ul nu a ridicat nicio obiecție cu privire la vetoul spaniol asupra unei oferte maghiare pentru achiziționarea companiei Talgo, un producător de trenuri de mare viteză. Madridul a invocat motive ținând de securitatea națională și considerentele strategice – un semnal cât se poate de deranjant pentru Budapesta.O întâlnire a miniștrilor de finanțe din zona euro, programată la Budapesta luna viitoare, ar putea fi anulată, evident, din cauza legăturilor tot mai evidente ale Ungariei cu Rusia. Summitul informal din noiembrie, care în mod tradițional se desfășoară în țara care deține președinția rotativă, este, de asemenea, improbabil, scrie Euractiv.Problema relaxării regimului de vize al Ungariei cu  Rusia și Belarus îi preocupă, de asemenea, pe europeni – mai ales statele care s-au confruntat recent cu acte de sabotaj pe teritoriul lor. It has been revealed that to date, a total of 3.4 million of the 13.1 million EPF members under the age of 55 have withdrawn RM8.9 billion from their flexible accounts since this option was made available in May. "The ability to play means we can indulge in a kind of illusion, not delusion, and make a distinction. It always amazes me that when the patient arrives, they like the routine of an analysis; nobody breaks that, it's an illusion; it is a piece of theater every time. We open the door to our patients and they lie on the couch, and yet there is something enormously gratifying as the patient works out their sense of  reality from that illusory field. I think it is exactly what the mother is able to bring to the infant - this capacity to play and this capacity to continue to evolve beyond the analysis as an internalization of that experience of being listened to and being with someone. The details of that is related to an intrapsychic surviving and non- surviving object in the analyst  who continues to think and feel and be with the patient in the consulting room.”   Episode Description: Joel begins his conversation with Jan around Winnicott's conceptualization of aggression in development and in the analytic encounter. She noted that he had a very sophisticated developmental theory of aggression which culminated with the role that the destruction of the object plays in constituting reality. Jan explains that she has elaborated Winnicott's late theory of aggression with her notion of the ‘surviving object'. She distinguishes the 'surviving object' from the 'good object', especially as it stands apart from a moralizing position. She considers its internalization as an essential condition for healthy development. They discussed the role that insight continued to play for Winnicott after he emphasized the importance of the patient's experience in the analytic process. They also consider the ‘fear of woman' as a root of misogyny. After discussing the uniqueness of the analytic setting to facilitate play, fantasy, and “magic which is not psychosis,” Jan concludes by emphasizing the importance of in-person treatment in order to have an in vivo experience of the non-retaliatory analyst.   Linked Episode: Episode 144: Why Winnicott? En una era en la que la disforia sexual parece estar en aumento, particularmente entre las generaciones más jóvenes, surgen preguntas sobre el posicionamiento actual de la teoría pulsional de Freud y el estatus de las cuestiones sexuales en la formación psicoanalítica. ¿Estamos asistiendo a un cambio en el que la sexualidad pierde su lugar central dentro del pensamiento psicoanalítico? La atención se centra en la posible disminución de los temas sexuales en los informes de casos psicoanalíticos, como se ha observado en los últimos años, y sus implicaciones para la metodología psicoanalítica. El discurso se originó en la conferencia de supervisores y analistas en formación de 2021 de la Asociación Psicoanalítica Alemana (DPV) en Colonia, Alemania, que rápidamente se extendió a otros lugares, reflejando preocupaciones globales sobre el menor énfasis en la sexualidad en los informes de los candidatos para poder cualificarse. The place of sexuality in psychoanalytic treatment and training today: Can we observe a disappearance of sexuality in case reports and supervisions?      How does the evolving discourse on sexuality influence psychoanalytic practice and training? In an era where sexual dysphoria is seemingly on the rise, particularly among younger generations, questions arise about the current positioning of Freud's drive theory and the status of sexual issues in psychoanalytic education. Are we witnessing a shift where sexuality loses its central place within psychoanalytic thought? In this episode of the IPA Talks On Psychoanalysis podcast series, we explore these questions through a comprehensive discussion. The focus is on the potential diminution of sexual themes in psychoanalytic case reports, as observed in recent years, and its implications for psychoanalytic methodology. The discourse originated in the 2021 supervisors' and training analysts' conference of the German Psychoanalytical Association (DPV) in Cologne, Germany and quickly spread to other places, reflecting global concerns about the reduced emphasis on sexuality in candidates' reports for qualification. This exploration revisits some cornerstones of Freud's seminal theories but also examines the more modern theories and psychoanalytic techniques that emphasize transference-countertransference dynamics for the understanding of sexual fantasies and identities. The role of societal changes, shifts in sexual identity, and the impact of media on sexual behaviour and expression are critically analyzed. The paper featured in this episode has been previously presented at various events, including a SPP Webinar and the 53rd IPA Congress in Cartagena. Her work notably intersects psychoanalysis with culture and public discourse. She has carried out the “Psychoanalysis in Literature – Literature in Psychoanalysis” series at the Literaturhaus Frankfurt, fostering dialogue between contemporary writers and Freud's texts. De Clerck's clinical interests include theories on the dynamic interplay of narcissism and object love, of the unconscious and changes in sexuality, including homosexuality “neo-sexualities” and the  concepts of masochism. De Clerck's contributions extend to psychoanalytic portraits of literary figures like Albert Camus, Virginia Woolf, painters like Maria Lassnig and notably Lucian Freud. She chairs the EPF's  “Psychoanalysis and Literature group and serves as a Consultant on the IPA Culture Committee. "Instead of the analyst being in a position where they know something about the patient, they are with the patient. As Winnicott says in his late work, if you are a philosopher in your armchair, you have to come out of your armchair and be on the floor with the child playing. I don't think that one should act that out with an adult patient- however  it is that approach to actually being with the patient, listening to the patient's words, listening to their state of mind without preconceived ideas. That's almost impossible, but  Winnicott says that psychoanalysis is an objective study, an objective way of looking at things without preconceived ideas, without preconceived notions. It links with what you said about ‘normative' - if we go into the consulting room feeling that our patients need to be as we are or need to fit in some kind of norm, then I don't think this is psychoanalytic. I think it is against the whole aim of psychoanalysis.”   Episode Description: Jan begins her conversation with Joel by sharing her background in theater and the steps she took to train as an analyst. She describes what drew her to Winnicott and how she sees him as broadening, not replacing, Freudian thinking. She distinguishes her understanding of Winnicott from others who believe that, by speaking of the importance of the environment, he minimized constitutional factors and the unconscious. She interprets what he meant by the environment in terms of the ‘psyche-body' and the mother's unconscious. Jan discusses a paradox in Winnicott in that he offers a positive theory of health while also being uniquely non-judgmental and non-pathologizing. She concludes with a controversial observation that a five times weekly in person training analysis is essential to achieve a deep regression that will familiarize analysts with the primitive parts of their personalities so they will be able to accept and deal with those parts of their patients' personalities.   Our Interviewer and Guest: Joel Whitebook, PhD is a philosopher and psychoanalyst.  He is on the Faculty of the Columbia University Center for Psychoanalytic Training and Research and was the founding Director of the University's Psychoanalytic Studies Program.  In addition to many articles on psychoanalysis, philosophy, and critical theory, Dr. Whitebook is also the author of Perversion and Utopia (MIT) and Freud: An Intellectual Biography (Cambridge).   Jan Abram, PhD is a training and supervising analyst of the British Psychoanalytical Society and in private practice in London. She is Visiting Professor of the Psychoanalysis Unit, University College London, and is currently Vice President of the European Psychoanalytic Federation for the Annual Conferences. She is President-Elect for the EPF to start her term in March 2024. She is a Visiting Lecturer and supervisor at the Tavistock Clinic, in London. In 2016, she was a Visiting Professor for the University of Kyoto, Japan, where she resided for a writing sabbatical. "When you're with a patient you take all that you know in your head, all the theory, and you throw it away. You have to listen to the patient and then maybe afterward something becomes clear - you use that ‘in-between' as a way maybe in the next session. But if you were sitting there and thinking: ‘Now the patient is in the paranoid/schizoid position…' that would be disastrous. You have to listen with your guts, your emotions, your intellect, and your body, in order to understand what is going on in a particular moment, in a particular session. Then later on you might be able to make sense of it through theory and through supervision."    Episode Description: We begin with considering the cultural and linguistic contributions to intrapsychic processes and the analytic encounter. Jeanne shares with us her life story involving her 'temporary' visit to California, which became a 37-year stay that included her becoming a psychoanalyst. We discuss the meaning to her and to her analysands of her being German and how she worked with that clinically. She moved to Vienna and began teaching and practicing analysis there, enabling her to compare the two psychoanalytic cultures and methods of practice. We also take up the importance of the German language as the vehicle through which Freud discovered the unconscious.
