The government is moving to increase the use of saliva testing as it implements mandatory weekly surveillance testing of three thousand essential workers crossing through Auckland's border. But we been too slow to adopt the technology? Saliva based tests are in used widely in many parts of the world, and have a number of advantages over current nasal swabs- they're less invasive, quicker and cheaper. But it has only been in the last month that border and MIQ workers have been given the choice of a saliva test rather than a nasal swab. According to the Ministry of Health website, 815 border workers have opted for regular saliva testing, and the trial will be reviewed mid this month. New Zealander, Dr Anne Wyllie, is a Research Scientist in Epidemiology at the Yale School of Public Health, whose team pioneered the SalivaDirect test, which was given the green light in the US last year.

I don't think it'll add much to dealing with the community outbreak when we have the wastewater testing and the nasopharyngeal PCR. Regular saliva testing for staff at New Zealand's managed isolation and quarantine facilities could be underway in weeks, which aims to strengthen the country's border against Covid-19.Asia Pacific Health Group (APHG) was awarded a Ministry of Health contract in May to provide virus surveillance testing through saliva samples from staff at New Zealand's MIQ sites.Testing at the border has been a contentious issue, punctuated by revelations in April that an infected Grand Millennium security guard wasn't tested for six months, even though he was meant to be tested fortnightly.As at June 28, 161 MIQ workers were overdue for a test.Auckland's Grand Millennium MIQ facility. (Photo / File)It was estimated between 6000 and 20,000 saliva tests would be done per week.Early indications were MIQ workers would submit a sample every second day when the programme was rolled out in the coming weeks, following a prototype in Christchurch last week.Voluntary saliva testing had been offered at MIQ facilities since January but uptake had been poor.The 12-month contract was worth up to 60 million but was dependent on testing volumes. That meant tests could cost between $58 to $192 each.Labtests, which is one of five APHG facilities primed to process the saliva tests, gave the Herald exclusive access to its Auckland laboratory, which had processed 520,000 Covid tests since the pandemic began.Labtests general manager Chris Davey said there were no concerns about the efficacy of the test and was confident it was the best option."We don't believe that there are better tests out there."MIQ workers will drop off their tubes at collection booths based in their facilities. (Photo / Michael Craig)As explained by Labtests microbiology and molecular head of department Susan Smith and molecular section head Blair Shilton, the process will start by MIQ workers signing into an APHG phone app.This requires a person's National Health Index (NHI) number and date of birth, but only for the first sign-in.Through the app, the worker would scan the bar code on the tube, which their "sample" is going in, deposit the sample and scan the bar code again.The worker will then scan a QR code at a collection booth before dropping off their sample.Once at the lab, the sample is heated and further liquified in a water bath to make it easier to process. Then any viral RNA - evidence of the virus being present - is extracted.Reagents, substances that cause chemical reactions, are added to bind to any viral genetic material before it is amplified. Researchers are looking into how to expand Covid-19 detection - with a Kiwi at the forefront of developing more accessible technology. Air New Zealand employees undertaking regular surveillance testing are participating in a Institute of Environmental Science and Research study, that will test saliva for the virus.The study will include the SalivaDirect Covid-19 testing technology developed by Nathan Grubaugh and New Zealander Anne Wyllie at Yale University in the US.While the Ministry of Health say at this stage there is still no indication that saliva PCR testing could replace the mandatory nasopharyngeal testing, health experts say it's only a matter of time.The ACT Party has come out this morning calling for daily mandatory saliva tests for border and MIQ Staff.  So just how is saliva testing progressing?Dr Wyllie told Kerre McIvor that she wanted to make a test as effective as possible, and they worked off developing the same PCR technology as the nasal swabs but make it more affordable. "We started playing with this back in April. There's still such a dire need here in the US for testing, so anything we could do to increase access to testing." She says they validated their method with the National Basketball Association and their bubble.LISTEN ABOVE

Switching New Zealand's required method of Covid-19 testing could be on the horizon as the roll-out of voluntary saliva tests widens.Auckland Airport workers are now being offered saliva testing in a bid to add another layer of protection.It comes after the Government last month rolled out the additional testing method for border workers in quarantine facilities.While the Ministry of Health say at this stage there is still no indication that saliva PCR testing could replace the mandatory nasopharyngeal testing, health experts say it's only a matter of time.Otago University epidemiologist Professor Michael Baker said once everyone was convinced the performance of saliva testing was adequate it could certainly replace nasal swab testing."It means we could contemplate the idea of having daily tests as a way of increasing the speed of outbreak protection," Baker said.Baker said it was good New Zealand had multiple laboratories testing out these technologies as the benefits were huge."Saliva testing is a lot less invasive for people ... potentially switching to saliva testing, especially for workforce who are already feeling more vulnerable, could make the job that much easier."Mary-Liz Tuck, Auckland Airport general manager of corporate services, said it made asymptomatic testing simple and comfortable for airport and border workers, while providing the highest standard of protection for the community."We want to see as many protective layers as possible for our people and our community," Tuck said.The airport teamed up with New Zealand business Rako Science that established the Covid-19 surveillance testing in New Zealand using the Shield saliva test developed at the University of Illinois.A collection site in Auckland Airport's international terminal had now been set up to provide the saliva testing and was set to run for three months while scientists measured the effectiveness.Airport staff taking part in the saliva tests were doing so on a voluntary basis, along with the mandatory nasal-swab testing required by the Government's border policies.A Ministry of Health spokeswoman said PCR nasopharyngeal swabbing method was still considered the gold standard for Covid testing as it detects the virus the most effectively."This swab type will obtain the optimal specimen and is the preferred collection method for both symptomatic and asymptomatic testing due to its higher sensitivity in detecting the virus."She said voluntary saliva testing also recommenced this week at the Auckland quarantine facility and at the managed isolation and quarantine facility in Christchurch.Rako Science is using the RT-qPCR saliva-testing protocol developed by University of Illinois which has conducted 1.3 million on-campus tests.Rako Science's Chief Science officer, Dr Stephen Grice, said Rako's science committee had briefed the Ministry of Health in December last year that it had successfully validated and accredited the saliva test for New Zealand.He said they recently provided the ministry with additional data which confirmed the test was as accurate as tests using nasal swabs."Rako Science has the processing capacity for 10,000 tests per day and the sample collection system does not depend on medical professionals to administer the test."Saliva testing for Covid-19 had been accredited for use in New Zealand by International Accreditation New Zealand (IANZ).Separately, Air New Zealand staff were working with the Institute of Environmental Science and Research (ESR) to work out the effectiveness of saliva tests detecting Covid-19.The Rako Science saliva deployment had no connection to trials being run by ESR.Last month, Covid-19 Response Minister Chris Hipkins said Covid-19 the role and effectiveness of saliva testing was still evolving."The Ministry of Health will report back with its findings about the testing in early March," he said at the time.

Rako Science is providing saliva tests for Auckland Airport and Air New Zealand staff, but epidemiologists want them rolled out more widely. The company says it is able to process 10,000 tests per day, and could deliver same-day results for swabs before 2pm. Dr Janet Pitman is the lead academic of the Rako Science advisory board.

We pre-recorded our conversation today over Zoom with Robbie Kidd, Shenandoah University professor and Chair of Biopharmaceutical Sciences to talk about Shenandoah University's Pooled Saliva Testing for Novel Coronavirus that will allow the school to expand its surveillance testing. Shenandoah University is conducting its own SARS-CoV-2 surveillance testing through the use of pooled saliva samples, making it only one of a handful of universities in the country to do so. The university has been doing surveillance testing for SARS-CoV-2, the virus that causes COVID-19, since the start of the academic year, beginning with the testing of incoming residential and commuter undergraduate students and continuing last month with select undergraduate populations.  Now, through Shenandoah University's Bernard J. Dunn School of Pharmacy, the university is expanding its SARS-CoV-2 surveillance testing program for asymptomatic individuals through pooled saliva samples, especially in high-contact populations such as student-athletes, residential students and conservatory students. Shenandoah joins the likes of Stanford and Syracuse universities in conducting pooled saliva testing. Robbie walked me through how the tests are conducted and what happens if there is a positive result on one of the pooled tests. This will ensure a higher degree of safety on SU's campus for students and faculty which I discussed with Mitch Moore in early August 2020. Saliva is a less-expensive, non-invasive, and safer sample collection method compared to nasopharyngeal swabs. Viral loads are also higher in saliva compared to nasal swabs. Shenandoah University performed its first round of surveillance testing using pooled saliva the week of Sept. 14. To date, Shenandoah has tested more than 800 students using this method, and more than 2,000 students total (saliva and nasopharyngeal combined). Surveillance testing is used to monitor for the incidence and prevalence of a community- or population-level occurrence of COVID-19, such as an outbreak. This involves randomly sampling asymptomatic individuals. The entire process is contactless, takes fewer than five minutes to complete, and is free of charge. Testing results can be returned in as few as six hours. Robbie estimated that 40-45% of those infected with the SARS-CoV-2 virus are asymptomatic, yet most are still infectious and can spread the virus to others. The goal(s) of the program are to identify the asymptomatic carriers, refer them for diagnostic testing, isolate them to prevent the spread of the virus, and to take care of these individuals while they convalesce. This will ultimately help keep Shenandoah University and the surrounding community safe during this pandemic. Shenandoah can currently test 320 students, faculty or staff each week, and expects to increase that number significantly in the weeks ahead.

Until this week, getting tested for COVID-19 in Minnesota has meant a bit of discomfort. Most tests statewide are conducted by a PPE-clad medical professional, swabbing a sample from deep in a patient’s nose or far in the back of their throat. It’s not terrible, but it’s also not comfortable. State health officials on Monday said they would be setting up a walk-up saliva testing facility soon at the Duluth Entertainment Convention Center. “Testing is free and you will not be required to have an ID or insurance. It is open to anyone who feels they need to be tested, whether or not you have symptoms, but especially if you have been exposed to someone who tested positive for COVID,” Dan Huff, an assistant state health commissioner, told reporters Monday. Conventional Hormone Replacement Therapy or “ HRT “ is best exemplified by the commercial drug Prempro.  Prempro is a combination drug that has both manmade estrogens and a progestin.  The estrogens are derived from the urine from pregnant mares and the progestin is medroxyprogesterone, a synthetic hormone not found in nature. Plus, doctors are still utilizing blood testing, which is incorrect, to determine a woman’s hormonal needs. Bioidentical Hormone Replacement Therapy or "BHRT" or natural hormone therapy, along with saliva testing, which is the only method of accurately determining what the balance and imbalance of any females endocrin system is. When the correct BHRT dosage is established with saliva collected over the course of one month and that dosage is taken as a sublingual lozenge, they are absorbed into the bloodstream as “free” hormones. They have not been packaged with a binding protein by the liver, because the liver did not have access to the hormones. Most of the “free” hormones are taken up by the red blood cell membranes, which are not analyzed by a common blood test. You might ask, “If there are hormones available that are natural to my body, why do doctors prescribe synthetic hormones?”  The explanation involves the mindset of physicians to traditional pharmaceutical medicine and the powerful pharmaceutical industry in the United States; as well as politics and money.
