Her educational background has a dual appointment in exercise physiology and dietetics with a master's degree in metabolic physiology. In this episode of The Scope Forward Show, Praveen Suthrum interviews Dr. Haytham Gareer, Chief Medical Officer, Geneoscopy (RNA stool testing company). In November 2021, a month after Dr. Haytham Gareer joined Geneoscopy as CMO, the company raised $105 million in funding. The company is developing an RNA analysis platform to detect colorectal cancer (to be launched in 2023). A clinical study showed that when compared against the gold-standard colonoscopy, the platform demonstrated better sensitivity data in detecting advanced adenomas and colorectal cancer than other non-invasive tests. Dr. Gareer says Geneoscopy is going to increasing the number of colonoscopies being performed. However, it's obvious to me that digital biology is disrupting not just GI but many other specialties that are diagnosing cancer. I couldn't hold my curiosity and asked quite directly. Should gastroenterologists be worried about them? Should Cologuard be worried? Should Medtronic with its AI-based polyp detection system be worried? Because when there's a test that's going to detect adenomas with a fair degree of accuracy, why bother with diagnostic procedures? Dr. Gareer had a different and interesting view. As readers of Scope Forward, you already know my take. With more data, digital biology innovations such as RNA and DNA tests combined with machine learning would get extremely sophisticated. It'll even be able to predict things that we just can't with traditional approaches. Dr. Haytham completely agreed.

Check out today's questions:    Zoe: Hi there! Just listened your episode “how to cure constipation” as I am constipated right now. You mentioned that you would recommend an international cleanse. I didn't see the brand in the show notes. Could you please tell me which brand you recommend for that? Also I have already purchased the 21 day detox. Should I do a intensional cleanse first and then detox? Or is it okay to just start with the detox? I am getting my hormonal birth control out on the 22nd so I was planning on starting the detox on the day of removal or next day.   Anonymous: Dr. Cabral, by the time you answer this, I will have completed the CBO protocol and likely be continuing through the CBO Finisher. I am currently beginning month 3 of the CBO and have a need for more improvements in my constipation/trapped gas symptoms. I have been very diligent, and followed the protocol exactly as recommended. I tried the intestinal cleanse on week 6 and experienced the constipation and lack of motility in over two years, and I took the binder and 4 capsules 3x per day for 5 days with only one bowel movement. If this protocol does not provide any improvements, where do I go from here? What testing do you recommend? (I did the HMTA and Stool Test leading into the CBO). Thank you!!   Samantha: Hi Dr. Cabral, Thank you for all that you do! I would love your insight. I'm a 30-year-old woman from New York. I recently had surgery on my right side Bartholin gland. I had an allergic reaction to either the anesthesia or intravenous antibiotics they gave me during the procedure. I'm seeing an allergist in a few weeks to figure that out. I had dermatitis that spread all over my torso, chest, arms, and back; it was pretty severe. A dermatologist prescribed me a ten-day taper protocol of Prednisone (which was really intense!). At the time of writing this, I'm done with the Prednisone and the bumps from the allergic reaction are gone, but I'm still SO itchy and I'm breaking out in hives. The itching feels like it's coming from the inside out if that makes sense. Actually scratching the area doesn't provide relief. I've been diagnosed with mast cell activation syndrome and I'm listening to your previous podcasts about it now. I've also been taking allergy medication and eating a low histamine diet. This combination provides very slight relief. I feel hopeless and want to feel like myself again. Thank you for any guidance that you may have. I would love any advice on where to begin healing this blood disorder.   Cassie: Hi Dr Cabral. Thank you for all you do for everyone here. It's appreciate by so many of us! I have this “anxiety rash” is what my doctor labeled it as, but it happens with any heightened emotion, happy or sad and when I'm nervous. It starts on my neck and moves to my chest and when it's really bad it goes to my arms. Splotches on my face can appear too and my cheeks are always sort of red anyways. Sometimes a trigger is as simple as a customer at work complimenting me, and I feel it happening, which I don't really understand. Or being excited about something. Not all the time, but sometimes it can feel like a rush of adrenaline. It's come to the point where I feel uncomfortable in my own skin and I've hidden it for years by not wearing shirts or anything that goes below my collar bones or people always ask what's wrong. I'm getting married this summer and it makes me nervous I'm going to be a red mess because I know I'll feeling all the emotions that day. Doctors have just thrown anxiety meds at me, and that is not what I'm looking for. I am a very active person, exercise a lot and eat pretty healthy, I have noticed the healthier I eat the better it is but I can't figure out triggers with food. Any advice is so greatly appreciated! In this episode of Functional Medicine Research, I interview Tom Fabian, PhD in a detailed discussion about the GI-MAP stool test interpretation. We covered virtually every aspect of the GI-MAP stool test including what the test results mean and how to use them in clinical practice. Dr. Fabian has tremendous knowledge of the gut microbiome and the intricacies of the GI-MAP stool test markers. This is a vital interivew to listen to if you're utilizing the GI-MAP stool test in your practice. Full transcript of the GI-MAP Stool Test Interpretation interview with Dr. Tom Fabian: Dr. Hedberg: Well, welcome, everyone to Functional Medicine Research. I'm Dr. Hedberg, and I'm looking forward today to my conversation with Dr. Thomas Fabian. He is a PhD., and he's a clinical laboratory consultant, translational science expert, functional nutrition practitioner, educator, and speaker. He is a former biomedical research scientist and deep expertise in the role of the human microbiome and health, chronic disease and aging. As a leading expert in translational applications of microbiome research and functional medicine and integrative health settings, Tom's primary focus is on providing educational resources and consulting services for practitioners and scientific advisory and consulting services for clinical testing laboratories. Dr. Fabian, welcome to the show. Dr. Fabian: Thanks so much, Nik. It's great to be here today, and I'm looking forward to the conversation. Dr. Hedberg: Excellent. So, we're gonna be talking about the diagnostic solutions, lab, GI-MAP test, and we're gonna cover interpretation, you know, what these markers mean. And so, for all the practitioners listening, they'll have a strong idea of how to approach this test and how to use these things clinically. So, why don't we start with...take it from the top in the pathogen section? And I wanted to ask you specifically about C-diff. There's toxin A and toxin B Clostridium difficile markers on this test. And what is your interpretation of this if it's positive and the patient is symptomatic, and then you treat them, and then they're no longer symptomatic, but the toxin still shows up on the stool test? Can you elaborate a little bit on that type of presentation? Dr. Fabian: Sure. No, I haven't personally seen that particular scenario but, in general, it's important to keep in mind a lot of people can be carriers of C-diff. So, the majority of the time that we see it detected positive, whether it's low levels or high levels, typically, patients don't have the classic symptoms. So, that suggests that they're probably just a carrier. And there's, sort of, kind of, a gray area in between where there still may be some effects of C-diff. Of course, that's one of the purposes of looking at the markers on GI-MAP like calprotectin, zonulin, etc. to see if there seems to be any evidence that may be have an impact, even if there aren't symptoms. So, we're also learning a lot more from research about factors that can control or influence the ability of various pathogens to thrive and also whether or not they can cause infection or if they have their, you know, typical pathogenic effects. So, that's essentially factors that influence virulence. So, one of the first things I want to mention is all the microbial markers on GI-MAP are assessed based on detection of DNA. So, when you're looking at DNA, you're looking at detection of the organisms or the genes but not necessarily whether the genes are being expressed. And that's definitely true for toxins. So, lots of research has been coming out in research years in terms of, again, as I mentioned, things that regulate toxins, and it's very specific. So, pathogens tend to only express those toxins under very specific conditions when conditions are favorable for them. So, for example, if you've detected C-diff and it really syncs up with what's going on with the patient, symptomatically,

But, if that balance is interrupted, or the microbiome is lacking, some of the bacteria can overgrow. At higher levels of growth, patients can experience symptoms. I am unsure if I should get a colonoscopy, ultrasound, or something else.Since the summer of 2018, I have been dealing with on and off again constipation, diarrhea, bloating and abdominal pain. It gets in the way of everyday normal activities, like teaching and being out somewhere (concert, gym, etc). I have gone to several doctors, including GI specialists, who in most part have ignored my issues. Recently, I have been having severe constipation and will not have a bowel movement for 3-4 days, and will only have one after I drink senna tea. Besides this issue, I am a healthy 29 year old female who works out four times a week, is within an optimal BMI range, sleeps 7-8 hours a night (some nights are more difficult than others to stay asleep), drinks almond milk, and eats organic/ grass-fed when I can. Darrel: Hey Dr. Cabral. Hope all is well with you, your family and those attached to Equilibrium. My question has to do with the recent pushback to those saying MSG is unhealthy. Seeing a lot of people saying that's not the case and it's gotten a bad rap due to racism, xenophobia etc. I'm still going to avoid it but it's an uphill task given my family and friends cook with it and when I attempt to convince them they cite articles like below or so-called experts like popular doctors appearing on cable network news refuting said ill effects. My naturopathic doctor recently did a very extensive stool test. We did it because I’ve been having pretty bad bloating and gas issues after eating pretty much any meal - for years.The results came back negative for the main suspects: yeast overgrowth, h-pylori, leaky gut, and so on. 066 Could You be Gluten Sensitive Even if you Tested Negative?

The Investigation Chances are you know someone with a gluten sensitivity despite reports that only 6-7% of the population suffer from it. That’s because testing is tricky and false negatives can make people think their health issues are caused by something else. In this episode, I want to break down what’s going on and help you determine if a gluten sensitivity might be at the root of your health mystery.    Understanding Gluten Intolerance Gluten is a protein found in wheat, rye, barley and oats. A person with gluten intolerance cannot digest the protein portion of these grains which triggers an immune response to what the body deems a foreign substance. This immune reaction causes inflammation in the digestive system and damages the lining tissue which, in turn, can cause symptoms like fatigue, bloating, diarrhea, constipation, gas, and cramping. The compromised lining can leave the person more susceptible to bacteria, parasites, and funguses.  Overtime, if a person who has a gluten intolerance continues to eat gluten, their small intestine may also become damaged which leads to malabsorption of nutrients. This can cause fatigue and weight gain. And, the person will be at an increased risk of insomnia, osteoporosis, hormone imbalances, inability to deal with stress, and mood swings. Over time, additional issues may arise like lactose intolerance, depression, leaky gut syndrome (leading to higher susceptibility to infections and other illnesses.) Gluten Intolerance is the Root Cause of Many Health Issues Symptoms of Gluten Intolerance There are many symptoms that could be the result of a gluten intolerance. How many from this list can you check off?    Weight gain Unexplained fatigue Difficulty relaxing, feel tense frequently Unexplained digestive problems Female hormone imbalances, (PMS, menopausal symptoms) Muscle or joint pain or stiffness of unknown cause  Migraine like headaches Food allergies/sensitivities Difficulty digesting dairy products Tendency to over consume alcohol Overly sensitive to physical and emotional pain, cry easily Cravings for sweets, bread, carbohydrates Tendency to overeat sweets, bread, carbohydrates Abdominal pain or cramping Abdominal bloating or distention Intestinal gas “Love” specific foods Eat when upset, eat to relax Constipation or diarrhea of no known cause Unexplained skin problems/rashes Difficulty gaining weight Anorexia Bulimia Rosacea Diabetes Osteoporosis/bone loss Iron deficiency/anemia hronic fatigue Candida Hypoglycemia If you checked off 4 or less it is very unlikely that you have a gluten sensitivity. If you scored between 5 and 8 there may be a reason to suspect it. If you scored 9 or higher there can be a good chance that you are gluten sensitive.   What is a Gluten-Free Diet? Gluten-free means avoiding all foods containing gluten, including wheat, rye, spelt, bulgar, semolina, couscous, kamut and some oats. Gluten can be hidden in processed foods and thickened products such as sauces and soups so read labels carefully. While this can be an adjustment at first, eating gluten free if you are sensitive will make a huge difference in your health.     What’s the Difference between Celiac and Gluten Sensitivity? Celiac disease is an extreme intolerance to gluten where eating gluten will actually damage the lining of your intestines and cause mal-absorption along with many other symptoms such as all types of GI issues, skin issues, headaches, arthritis, depression and the list goes on and on. A gluten sensitivity is when you do not have full-blown celiac and your intestines are not completely damaged but eating gluten makes you feel worse and can often cause very similar if not the same symptoms of those with celiac disease.   What are the Problems with Celiac Testing (and Why is it so Hard to Get a True Answer)? Testing for this is very tricky because the occurrence of false negatives is often likely. The only way to get a true celiac test is to go to the GI doctor and have a biopsy. Your intestines are lined with hair like structures called villi and in healthy intestines; they stand straight up like a shaggy rug. When a person has celiac disease, gluten causes damage to the intestines and the villi lose their integrity and become flattened. The biopsy looks at the state of the villi and only if they are flattened can there be a true celiac diagnosis. Once gluten is removed from their diet, the villi can be revived fairly quickly and thus the intestines will heal.  Patients who already started a gluten free diet prior to their biopsy can often get a false negative because of this. To get a proper diagnosis, the patient should eat gluten (equivalent to at least 1 slice of wheat bread) every day for 30 consecutive days before the biopsy There is also a blood test that can identify the antibodies the body produces against gluten. Some GI doctors use this as a celiac test. The problem here is that the gluten antibodies are produced in significant numbers only if the person has a good functioning immune. With a weak immune system, antibody production will be low and lead to a false negative result. Furthermore, there are several types of antibodies that can be tested such and some of the main ones include IgE, IgG and IgA. IgE shows an immediate response and this often comes out negative unless someone has an anaphylactic shock type reaction to the food which is far less common. IgG and IgA are the delayed antibodies and are more likely to be found through testing because gluten-containing foods take several hours to several days to show symptoms and inflame the immune system.    Testing for Gluten Sensitivity When a celiac test is negative but someone feels worse after consuming gluten (this can happen from 1-72 hours post consumption) they most likely have a sensitivity to gluten. The reason the other tests are negative is because gluten reactions vary in strength. The easiest way to describe this is if you think of a spectrum from 0 to 100 with 0 being no sensitivity and 100 being extremely sensitive. If someone is all the way at 100 it would equate to celiac disease and/or extreme intolerance and often result in a positive biopsy but anything below that would not show up on those tests. So the problem is what if you are at 95? or 85? These cases would show up negative for celiac but as you can see from the example, 95 is still very high on the spectrum and the person would have a pretty severe sensitivity to gluten with all the same symptoms. Testing for sensitivities is also tricky because people can react to gluten through various allergenic pathways and being able to find a possible reactive pathway is the way to see a true result. The major proven pathways include antibody pathways and inflammatory cell mediated pathways. Antibodies can also be tested in blood, but only the IgA and IgG should be used (not IgE) as those are the delayed ones. IgA and IgG antibodies can also be tested in stool and saliva. There are different opinions on what is best (some say gluten is in contact with stool more than blood or saliva thus stool may be more accurate) but most likely testing in all 3, (blood, saliva and stool) is probably better. A positive result in any one of these means there is a sensitivity.    A Negative Gluten Sensitivity Test Does Not Rule Out Gluten Sensitivity It is possible that all 3 antibodies tested in all 3 body fluids can turn out negative but a reaction can still be occurring. This is possible because the body can react to gluten on a cellular level without necessarily producing antibodies, or if the antibody response is weakened due to immune weakness and other diseases. Another way to look at gluten sensitivity is through the inflammatory pathway of the white blood cells. The ALCAT technology has made their name in this area and they test by collecting blood, separating out the while blood cells and then introducing gluten to the white blood cells to look for information. If the while blood cell changes shape, the result is positive. The severity of the change in the white blood cell indicates the severity of the sensitivity. If there is no change to the white blood cell, there is no sensitivity. Persistent Gluten-Sensitivity Symptom Despite Negative Tests Another big piece of the gluten puzzle is a gluten intolerance due to the slight genetic change of the seeds of wheat. In Dr. William Davis's book Wheat Belly, he explains how a geneticist (about 50 years ago) was able to slightly modify the wheat seed to allow the wheat plant to grow faster and be harvested more often. This was lauded as a great help in the fight against world hunger but the genetic modification in the seed created a plant that now has a higher gluten content. Plants can naturally mutate from strain to strain over time but this process takes hundreds of years and this was done overnight. The human DNA is not familiar with this new strain of wheat and thus there are many people who are intolerant to this new wheat strain. Dr. Davis uses a wonderful analogy in his book to describe it. The new wheat is 99.9% genetically the same as the old wheat which doesn’t seem like it would be that harmful, but apes are also 99.9% genetically the same as humans and yet we are quite different. Intolerance to this genetically modified strain of wheat is not a blanket intolerance to gluten which is why eating it causes symptoms but a gluten sensitivity is not detectable in tests.   The Role of Genetics in Gluten Sensitivity and Gluten Intolerance Genetics are often ignored in the gluten intolerance discussion. Genes are something that you are born with and are carried down from generation to generation. There are a set of genes recognized as celiac genes and they are called HLA DQ2 and DQ8. We now know that the DQ2 gene has 2 pieces each with 2 alleles for a total of 4 and the DQ8 has 1 allele for a grand total of 5 different alleles. We need to see all of them. Studies show that if you have a specific number of these gene segments, your body is not designed to process certain gluten proteins and therefore you may have celiac disease already or will be likely to become gluten intolerant/sensitive. Gluten sensitivity is not a disease like celiac disease but can be looked at as a state of genetics. If a person is not designed to process gluten properly based on their genetics but eat it anyways, the end result is a constant attack on the immune system which can subsequently lead to autoimmune disease. But, if we knew a person’s propensity was to be gluten sensitive, we can stop the disease before it happens.   Testing for the Gluten Genes Most labs only do 2 of the gene pieces, but it is essential to have a complete profile of all of them. I have been searching for labs and found a panel that does all the pieces. They are a subsidiary of Labcorp so there is a good chance the test may even be covered by insurance which is a huge plus!   Best Test to Determine Gluten Intolerance The only way to know for sure if gluten is an issue for you is to look at all the pathways through various tests and if any are positive then you have an issue with gluten. Please don’t just take one negative result as an absolute - especially if you do not feel well when you eat gluten or if you have an autoimmune disease. After a lot of research, I believe that looking at genetics holds the biggest key in the gluten puzzle. All the other tests measure only a fraction of how a person’s immune system can react to gluten but genetics can actually tell us what the immune system is going to do in the future. You cannot control what genes you are born with, but you can identify them and change your diet and lifestyle to accommodate them to both heal a health issue as well as prevent a potential future health issue.     Eliminating Health Mysteries Could gluten be the root cause of your health mystery or that of someone in your life?  If you find that you don’t feel well when eating wheat at restaurants, this may be your issue. Mia: I have been struggling with bad acne on my neck jawline and chin. I’m taking estroblock 2 times per day I’m pretty healthy. Also I am 27 years old and I am so tired 10 days before mensturation! So very tired, brain fog and acne before my period. Do you ever suggest progesterone cream before period?  Mark: Hi Dr Cabral. Why don't you use the GI-MAP GI Microbial Assay Plus Stool Test? I've heard that it is the best parasite checking stool test on the market because it use DNA analysis. Would love your thoughts. Nina: Hi Dr Cabral - LOVE LOVE LOVE your show, i was recently introduced through an Ayurvedic podcast I listen to where you were a speaker. What i love is how you mash up ayurvedic, TCM, and other functional medicine areas to address health. As it should be! I have found so much wisdom in Ayurveda to address a bout of insomnia 2 years back where dr's were pushing sleep meds and I fortunately found holistic health, and of course discovered nothing is 'just' insomnia, etc etc. Have come a long way and incorporated different healthy practices into a blended approach that fits my lifestyle. Am curious your take on something... Ayurveda (at least what I hear and read) seems to frown upon smoothies - lowers agni, hard on digestion etc. What's your take? I see you promote smoothies and now that I am feeling healthier I would like to try them out (will pay attention to digestion etc) but i see them as a great opp to get so much nutrient dense food in one meal! What’s your stance on gluten, even if it’s organic non gmo? Same with org non gmo grains? Non gluten org Oats? I'm a vata pitta so even though i strive for lower carb/sugar, i have some carb at least with lunch and often also with dinner. Bless you! Nina Jessica: Hello wonderful Doctor Cabral. My mom is about to turn 60, and has been battling sinus infections since I can remember, every winter and spring in particular. A lot of the time she takes antibioitics to help, and they do. (though I keep telling her bad in the long run) She has seasonal allergies and teaches elementary students in a room that is not well ventilated. Lately she has been sick for weeks, out of school with an upper respiratory infection, with wheezing, a heavy chest, and cough when breathing. She is taking inhalers, Albuterol (I think a steroid) (so bad right?!) which also seem to temporarily help. Can you explain why these inhalers mask the issue. What can she do to help this naturally and stop having so many sinus and chest infections? She is ready to get control of her health and get to the bottom of this vs masking the issue w drugs. Unlike conventional tests, the stool test kit does not require dietary restrictions or bowel preparations and is simply designed so that patients can collect their own samples at home and mail them for laboratory analysis.
