Of course, we discuss how she she found healing. Bringing home a baby doesn't always mean mom's eternal happiness. Not all mothers feel “the way they should” after the baby is born, and may feel lost if they don't know where to turn for resources. This endangers not only mom but also a newborn.In a time when 14000 children have been unalived in a completely manmade g-cide, this episode still has its place as one of the struggles mothers face. And for those of our momsisters delivering via C-section without anesthesia, delivering prematurely due to stress, shock, and manmade famine, PPD is a real threat with deep seated claws. Recently she told me about a new screening tool that she had created, and I wanted to share it here in case it's helpful. Here is some background from Sam followed by a link to the screening tool.Tell us a bit about you and your backgroundI'm a therapist in NY that specializes in trauma and Eating Disorders. I am passionate about providing trauma informed and weight neutral care as a result of my own experiences with weight stigma and disordered eating.   What made you decide to create this screening?Many people struggle with disordered eating, body image issues, disordered exercise and the impacts of weight stigma. If you rely solely on standardized diagnostic screening tools and the DSM criteria, you risk missing huge populations that could benefit from working on the above-mentioned issues.Can you share the screening tool development process with us?I wanted this screening to be as comprehensive as possible so anyone struggling in the following areas could be flagged as needing support: Attitude & Thoughts Towards Food, Unhealthy Food Behaviors, Unhealthy Exercise Patterns, Body Image Struggles & Common Medical Complications because of Disordered Eating. The screening questions in each category include DSM criteria, criteria for Orthorexia and reoccurring issues that I see in my own practice that aren't typically included in a diagnostic tool (e.g. Do you feel like you are waiting to be happy until you can “lose the weight?”).Once I was happy with the questions and format, I put a call out on IG for the assessment to be reviewed. I had 5 HAES-aligned providers, including folks in larger bodies, review the assessment and give me feedback.Were there difficulties and barriers that you faced in putting the tool together?The most difficult part was trying to keep this assessment concise, specifically in the body image and attitudes toward food sections. Since weight stigma and diet culture have catastrophic effects, it would have been easy to continue adding examples of how diet culture and weight stigma show up in folks' thoughts and behaviors.How do you hope that the tool will be used?First, it's a screening tool to help mental health professionals identify folks that need help with anything that falls under the disordered eating umbrella. I specifically hope this will circulate to professionals that don't specialize in Eating Disorders. Many disordered thoughts and behaviors go unflagged by those not trained in Eating Disorders because they are deemed ‘healthy' by diet culture.Second, I hope it can be a trailhead for professionals and their clients to explore how diet culture and weight stigma impact many areas of clients' lives.You can find the screening tool here! Highlights From Today's Episode “When my team and I are approaching patients, what we want to know is what matters to that particular patient, what gives them a sense of meaning, what gives them a sense of purpose in their life—and we want to approach them without judgement.” Timestamp (TS) 04:22 “If you have a patient who is experiencing distress, I often recommend that you don't say, ‘Would you like to see the chaplain?' or, ‘Would you like to see spiritual health?' I often recommend that you make the referral. We know how to interface with a patient. The reason I recommend that is because a lot of times, there's a stigma associated with receiving help when someone is in distress.” TS 13:51 “When a person has a new diagnosis, that can be so overwhelming. Often, once they have a plan in place with their provider, their distress goes way down. It might be helpful to call in spiritual health to provide a little bit of companionship to that patient. That can actually help the nurse to be more effective in their work so they're able to the medical needs that they're needing to get to while we can absorb some of the spiritual and emotional content that the patient might be carrying.” TS 28:06 “A very simple question is, ‘What matters to you?' It's so basic, but I think that can help a nurse get to, when they're having a conversation with a patient, what gives the patient a sense of meaning in their life. . . . And I think one thing that's really important is to know that spirituality and religion are very complex. There's a lot of diversity within any given group. It's important for nurses to know that when they're interfacing with a patient of a particular religious background not to assume that they maybe have all the same beliefs as other people within that particular religious group.” TS 30:07 “We consider staff to be part of our groups of people that we're responding to, and my team and I care deeply about our nurses that work in healthcare settings. . . . Nurses have complex lives, and they're dealing with all kinds of things within their own lives—challenges with family, appointment stressors, working very hard hours, having really hard patients, having heavy caseloads—all of those things can be extraordinarily overwhelming for healthcare staff and for nurses in particular.” TS 26:31

Hypermobility is far more prevalent in dancers, circus performers and other aesthetic athletes. While some hypermobile artistic athletes have asymptomatic joint hypermobility, others have symptoms due to an underlying hypermobility or connective tissue disorder.How can a physical therapist or physician assess the bendy artist, beyond tools like the Beighton Score, and see what the artist might need by way of support?Aiko Callahan and Stephanie Greenspan, both DPTs who work a lot with dancers, circus artists and other bendy bodies, wondered this same thing. They worked with Annie Squires, DPT to publish “Management of Hypermobility in Aesthetic Performing Artists: A Review” and create a fabulous new tool, the Hypermobility Screening Tool.The two physical therapists talk through their process and how they assess new patients. It's not uncommon for clients to call us with a patient conflict and tell us the following: "I performed a perfect procedure and the patient is still livid. What did I do wrong?" The problem may not have been the procedure - it's possible the patient was not a good fit for the procedure in the first place. Patients suffering from these conditions generally need a referral to a provider who specializes in treating such individuals. How do you broach the subject? Preterm birth (PTB) is the leading cause of child death under 5, world-wide. Despite the long-lasting effects on both the child and their family and the existence of a variety of treatment options for expectant mothers at risk of preterm labour, no screening tools exist on the market for population-wide risk stratification. This project aims to develop a point-of-care (POC) predictive tool to identify women at risk of PTB at the 12th week of pregnancy by detecting cell-free nucleic acid biomarkers (so-called microRNAs or miRs) from the mother's blood. This will be done through engineering functionalised Peptide Nucleic Acid (PNA) probes for multiplexed microRNA sensing through oligonucleotide templated reactions and their incorporation into paper-based lateral flow assays (LFAs) for use at the POC. Good Morning, Colorado, and welcome to the Daily Sun-Up. It’s Monday January 4th, and we’re feeling lucky to start the day with you. Join us daily for an in-depth look at one of our top stories.  Today - Researchers at the University of Colorado and Yale have developed a smell test they estimate can be as effective for coronavirus surveillance as regular PCR tests. Before we begin, let’s go back in time with some Colorado history adapted from historian Derek R Everett’s book “Colorado Day by Day” Today, we take you back to January 4th, 1941 when a contract was signed authorizing the construction of one of the nation’s largest armament factories - the Denver Ordnance Plant. The works contributed to Colorado’s dramatic growth during World War II as people flocked to the state to help the country build up its defenses. And now, our feature story.  It may be cliché, in this time of heightened stress and frenzy, to remind you to stop and smell the roses. But seriously: Stop. And. Smell. The. Roses. You might find out you have COVID-19. Anosmia, which is the loss of your sense of smell, is a well-known symptom of a coronavirus infection. Studies have estimated its prevalence among people with COVID-19 at as much as 80%, which would make it as common of a symptom as cough or fever among symptomatic patients. But unlike those other two symptoms, it also occurs in people that are otherwise asymptomatic.  Now, two researchers at the University of Colorado and a Yale School of Medicine professor who grew up in Boulder think they’ve come up with a better way to conduct mass testing for coronavirus infections. Colorado Sun reporter John Ingold is here today to tell us more. When it comes to mass testing, frequency is just as important as accuracy. And PCR tests are cumbersome to administer and analyze. They’re also costly -- something that can add up quickly when organizations are trying to conduct testing on a large scale. Thus, a cheaper and simpler, although less accurate, test that can be conducted more frequently can be just as valuable.   Derek Toomre, a professor of cell biology at the Yale School of Medicine who was one of the authors on the study, says that [quote] “Your lack in accuracy is more than made up in frequency,”  Toomre said he’s talking with manufacturers, with the hope of producing hundreds of millions of cards that can be sold for 50 cents or less, assuming it receives federal approval. He said they also plan to donate large numbers of tests to nonprofits. And thank you all for listening. Finally, here are a few stories you should know about today: Two Pueblo County sheriff’s deputies were shot Friday afternoon while trying to confront a man who, from his home in Pueblo West, was making threats toward government buildings and law enforcement in Tennessee. The Paycheck Protection Program loans are coming back in the new federal COVID relief bill. Expect the new round to open this month.  A slew of new workplace laws went into effect on Jan. 1, including one that benefits the lowest wage earners. Will it ever end? I also talked about how there are plans to remove Pap Tests as a primary screening tool for eradicating cervical cancer. That's a scary thought. The Pap Test has been an effective tool used to screen for cervical cancer over the last 50 years. It's baffling to me that is even a consideration. The rationale is that HPV tests are more sensitive and thus a more effective screening tool for cervical cancer care. While I am a believer in science and research, as someone who has screened GYN specimens for abnormal lesions myself, I can say with certainty that every woman is different and this one size fits all approach will be of detriment to women. Although Pap tests are designed specifically to detect cervical cancer, we are able to see cell changes associated with Herpes infections, hormonal changes, cancerous lesions of the endometrium, and many other things. To take away the Pap Tests means that these conditions might go undetected and untreated. Do yall know they have started doing uterus transplants? I don't know what to say about it. Facebook has debuted a new healthcare tool that provides users with personalized health resources and checkup reminders. The tool, called Preventive Health, is a healthcare feature currently only available in the United States. It accesses users' age and gender, then gives them information on what illnesses they are vulnerable to, as well as reminders for possible screenings or checkups. The new tool was created with preventive health in mind, particularly in avoiding the two leading causes of death in the United States—cancer and cardiovascular illnesses. To access the feature, users simply have to search Preventive Health on Facebook or click on an advertisement in their newsfeed. After doing so, users will be presented with various preventive health measures, such as getting a cholesterol test, relative to their age and gender. The tool also lists medical procedures and explains how frequently a person should undergo each. Users can tick off the procedures they have completed and schedule reminders for future tests or checkups using the tool. Preventive Health also features a map of health centers in the United States to let users know where they can access healthcare. Because of data sensitivity associated with the new tool, Dr. Freddy Abnousi [ab-NOO-see], Facebook's head of healthcare research, ensured that Facebook will not share any personal information to third-party organizations or insurance companies. Similarly, users' Preventive Health data will not be accessible to other Facebook users. Eventually, Facebook plans to expand Preventive Health outside the United States. For now, however, the tech company would like to see whether Facebook users will actually engage with the tool.

Since it's cancer prevention month, let's talk about this awful disease. Usually, people go to see a doctor because they're experiencing some symptom that they want to treat. According to Tammy Kohlschmidt, a certified clinical thermographer, that's entirely the wrong approach. As she explained to WellBe, thermography offers a way to catch dysfunction before it becomes symptomatic, helping people to prevent rather than treat disease — which is key to maintaining your health. Thermography, also known as digital infrared thermal imaging, is a screening method that uses camera images to measure your body temperature and blood flow, looking for patterns and variations that can screen for dysfunction and precursors to disease.  Because of the way the screening works, it's also able to detect abnormalities and dysfunction before other tests. Kohlschmidt explains that thermography can pick up cancer cells when they're about the size of a grain of rice, or around two years old, whereas a mammogram won't pick it up for eight years. We speak with Dr Kairi Kolves, who is the Principal Research Fellow and Course Convener at AISRAP. The Australian Institute for Suicide Research and Prevention (AISRAP) has been a research centre at Griffith University since 1996, delivering high quality research, clinical services, training & education in suicide prevention.. Since 2008 AISRAP has been a National Centre of Excellence in suicide prevention informing the Department of Health on policy, prevention and best-practice initiatives nationally. AISRAP is the only World Health Organization Collaborating Centre for Research and Training in Suicide Prevention in Australia, and one of only five centres in the world of this kind. AISRAP made significant contributions to the 2014 WHO Preventing suicide: A global imperative report and has collaborated with numerous stakeholders in suicide research and prevention both nationally and internationally over the past 2 decades. Its researchers are highly respected international experts, with over 200 peer-reviewed journal publications and presentations in the last five years. AISRAP manages the Queensland Suicide Register (QSR), one of the most comprehensive databases of its kind in in the Asia Pacific region. Since 1990 the QSR has informed critical suicide prevention policy and action plans of the Queensland State Government, funded by Queensland Health and more recently (since 2014) the Queensland Mental Health Commission. The QSR has recently been a pivotal resource of evidence based information assisting Primary Health Networks in planning and evaluating practical needs-based regional suicide prevention initiatives. In response to the absence of accurate and standardised recording systems for non-fatal suicidal behaviour, AISRAP designed and operates a unique database for monitoring of suicide attempts and self-harm presentations at hospital emergency departments (World Health Organization Suicide Trends in At-Risk Territories (START) Study). AISRAP has produced 16 volumes of Suicide Research: Selected Readings, a biannual critical literature review series that identifies and outlines recent advances and developments in international research in suicide prevention, collating all newly released publications that explicitly refer to fatal and/or non-fatal suicidal behaviours and related issues. AISRAP’s highly innovative Screening Tool for Assessing Risk of Suicide training in suicide risk assessment has recently been translated into 5 languages. The Institute’s Life Promotion Clinic is the only outpatient treatment/research clinic providing an alternative to hospitalised-based care for those with a history of suicidal behaviour. #ManyVoicesOneGoal #suicideprevention

Die aktuelle Studienlage weist darauf hin, dass die durchschnittliche Überlebensdauer von Parkinson-Patienten mit manifester Dysphagie bei ein bis zwei Jahren liegt und (Aspirations-) Pneumonien eine der häufigsten Todesursachen sind. Problem: Dysphagien werden in der Regel zu spät erkannt und eine entsprechende Therapie beginnt zumeist erst bei massiveren Schluckstörungen mit Gesundheitsschäden. Standardmäßige Schluckdiagnostiken zur Profilaxe werden bislang nicht durchgeführt und ausreichend valide Screening-Tools, wie etwa Patientenfragebögen zur Evaluierung von Schluckstörungen bei Parkinson-Patienten, fehlen in der klinischen Praxis. Beitrag: Diese Dissertation stellt den 26-Item-umfassenden Münchener Dysphagie Test – Parkinson’s Disease (MDT-PD) vor, ein Screeningverfahren in Form eines klinischen Patientenfragebogens zur Früherkennung von Schluckstörungen und ihrer Graduierung bei idiopathischem Parkinson-Syndrom (IPS) einschließlich einer bedienerfreundlichen Web- Applikation zur schnellen und örtlich flexiblen Auswertung (Betriebssystemunterstützung: Windows, Mac OS, iOS, Android u.a.). Daneben werden zwei Befundungsbögen zur standardisierten klinischen und videoendoskopischen Schluck-Diagnostik vorgestellt, welche klar definierte, ordinale Symptom-Rating-Skalen beinhalten. Methoden/ Validation: Der innerhalb drei Phasen und einem Pre-Test entwickelte Fragebogen wurde in einer Studie mit 82 IPS-Probanden unter Ausschluss von vordiagnostizierter Schluckstörung, Demenz oder chronischer Depression evaluiert (m=46, w=36; Ø Alter ± Standardabweichung: 70,9 J. ± 8,7 J.; Ø Erkrankungsdauer nach Erstdiagnose: 11,0 J. ± 6,3 J.; Ø H&Y: 3,3; Ø UPDRS III: 29,5 P. ± 13,3 P.). Als Vergleichsparameter kamen die neu konzipierten Symptomschweregradskalen innerhalb der standardisierten klinischen sowie videoendoskopischen Dysphagie-Diagnostik zum Einsatz. Die klinische Untersuchung bestand aus einem Ruhe-, Reflex- und Funktionsprüfungs-Part sowie einer Schluckproben-Testung; bei der instrumentellen Diagnostik wurde sich an das FEES®-Protokoll angelehnt, welches parkinsonspezifisch weiterentwickelt wurde: Neben der Erhebung funktioneller Parameter wurde sowohl die Gefahr für/ der Grad der laryngealen Penetration/ Aspiration innerhalb der Schlucktestung abgebildet als auch beginnende Dysphagie-Symptomatiken wie posteriores Bolus-Leaking und pharyngeale Residuen sowie Speichel-Akkumulation berücksichtigt und graduell unterschieden. In der klinischen sowie videoendoskopischen Diagnostik (Durchführungen im On-drug-state) wurde die Nahrungsaufnahme mit folgenden Konsistenzen, quantitativ deckungsgleich und in alltagsrelevanter Menge, geprüft: dünnflüssig (90 ml Wasser, blau eingefärbt), fest (1⁄2 Scheibe Mischbrot mit Rinde und Aufstrich, ≈8x7x1cm) und trocken/ bröselig (1 Keks, Ø 5cm) sowie die Einnahme von zwei Tabletten (teilbare ProLiveVita-Fit-Blocktablette, ≈19x8x7 mm; Placebo-Hepa-Lichtenstein, Ø 8mm). Ergebnisse: Der MDT-PD erfüllt die Testgütekriterien der klassischen Testtheorie. Durch die Receiver-Operating-Characteristics (ROC)-Analyse wurden zwei Cut-Offs für die Gruppengrößen nicht auffällig vs. auffällig (3,65) und nicht auffällig vs. aspirationsgefährdet (4,79) ermittelt. Die Diskriminierungsgüte des (nach den Regressions-Koeffizienten) gewichteten MDT-PD-Summenscores ergibt für die Dysphagie-Einteilungen a) unauffällig vs. auffällig eine Sensitivität (Sens) von 82% sowie eine Spezifität (Spez) von 71% (Kreuzvalidierung: Sens 82%/ Spez 62%/ Cut-off 3,62) und b) nicht auffällig vs. aspirationsgefährdet eine Sensitivität von 90% sowie eine Spezifität von 86% (Kreuzvalidierung: Sens 90%/ Spez 81%/ Cut-off 4,75). Für den Zusammenhang zwischen dem Kriterien-Summenscore und dem gewichteten MDT-PD-Summenscore konnte in den Studiendaten eine starke Korrelation mit einem Spearman-Rho Korrelationskoeffizienten von +0,699 (p
