Your baby is breech. Does that automatically mean you need a C-section? It's a question that can create anxiety, confusion—and plenty of conflicting advice. In this episode of Pregnancy Pearls, double board-certified OB/GYN and maternal-fetal medicine specialist Dr. Nicole Plenty breaks down what expecting parents need to know about breech babies and the complicated decision between a planned C-section and attempting a vaginal breech delivery. Dr. Plenty explains the different types of breech presentations, why most breech babies in the United States are delivered by C-section, and the very specific circumstances in which a vaginal breech delivery may be considered. She also discusses the potential risks to both mother and baby, why the experience of the delivering provider and hospital team matters so much, and how an external cephalic version (ECV) may offer another option by attempting to turn a breech baby head-down before delivery. Then, a listener who is 38 weeks pregnant with her third baby writes in with a real dilemma: after two uncomplicated vaginal births, her baby is now frank breech. One physician recommends a C-section while another says she may be a candidate for vaginal delivery. How do you make a decision when there may not be one perfect answer? We sit down with Dr. Ruthi Landau, the Virginia Apgar Professor of Anesthesiology and Director of Obstetric Anesthesiology at Columbia University, to get practical about what patients actually feel during cesarean delivery anesthesia and what we can do when those sensations become uncomfortable.We talk through why labeling sensations as pressure versus pain misses the point, and why the better clinical question is simple: “Is it uncomfortable?” From there, we unpack a safer communication loop that invites patients to speak up and gives them real choices for relief, whether that means dosing through an epidural, using IV medication, or escalating plans when neuraxial techniques are not working. For years on True Birth and in their OB/GYN practice, Dr. Abdelhak & Kristin Mallon have made an unpopular argument: when a baby is measuring big, the safest move is usually to induce, not to wait. The conventional literature did not back them up, and plenty of academics pushed back with the same line every time, that induction does not lower the risk of a cesarean or a shoulder dystocia. In this episode they walk through two pieces of high level evidence that land squarely on their side, and they explain why this was common sense all along. They start with the bigger cultural point. We do not have natural pregnancies anymore, so it is unrealistic to expect a uniformly natural birth. Electricity, processed food, far fewer daily steps, and later maternal age all push birth weights up, which means more babies are simply too big to deliver without help. Kristin brings the midwife lens to this, including a frank look at the Ina May Gaskin farm numbers and what they actually showed. From there they get into the data, share a real shoulder dystocia save from the floor, and detour into the things that genuinely scare them in delivery, like postpartum hemorrhage, plus Dr. Abdelic's running theory that growth problems are usually about the cord, not the placenta. The take home is blunt. If your provider tells you the baby is measuring large, the right question is not whether to induce. It is when. Induction of labour versus expectant management for large for date fetuses: a randomised controlled trial. Lancet. 2015;385(9987):2600 to 2605. A pragmatic trial across 19 centers in France, Switzerland, and Belgium. Women with singleton fetuses above the 95th percentile were randomized to induction between 37 0/7 and 38 6/7 weeks or to expectant management. Induction significantly reduced the composite of shoulder dystocia and associated morbidity, relative risk 0.32, 95% CI 0.15 to 0.71, p equals 0.004, without raising the cesarean rate and with more spontaneous vaginal deliveries. The 2026 meta analysis (the AJOG MFM study): Paladino, et al. Induction at 38 weeks for large for gestational age or macrosomic fetuses decreases the incidence of cesarean delivery: meta analysis of randomized controlled trials. Am J Obstet Gynecol MFM. 2026. Five randomized trials, 4,083 pregnant individuals, most induced at 38 0/7 to 38 4/7 weeks for an estimated weight above the 90th percentile. Induction lowered the cesarean rate and cut macrosomia (relative risk 0.53 for 4,000 grams or more, and 0.22 for 4,500 grams or more). Shoulder dystocia and fetal fracture both trended down but did not reach significance, which is expected for outcomes this rare. Roughly 25 inductions in this window prevent one cesarean. Key takeaways A big baby is the single biggest risk factor for shoulder dystocia, and a baby does not get smaller between 38 and 40 weeks. The evidence now shows that inducing for a large for date baby reduces shoulder dystocia and cesarean delivery and increases the odds of a vaginal birth. During childbirth, many things can evolve in different ways depending on many factors, including how the labor progresses, how the baby is doing, and how the mother's body responds throughout the process of delivery. In this episode we talk about what is happening during a vaginal delivery, if and when to consider having a C-section, when to use various approaches for assisted deliveries, why one way of giving birth versus another occurs, and so much more. I also talk about why, as an ObGyn, my preferred way of delivering babies is vaginally, when it is the safest option for both mother and baby. The way women give birth is changing, according to BBC analysis, which has discovered that one in four births in England are now emergency caesareans. The current figure is 26% - an increase of 8% in the past five years - and the rate of elective caesareans has also gone up. The Royal College of Obstetricians and Gynaecologists say the system is struggling to cope with this increased level of unplanned surgeries. To discuss what's behind the rise and the potential impact on women, presenter Kylie Pentelow is joined by BBC Health Correspondent Catherine Burns, President of the Royal College of Obstetricians and Gynaecologists, Dr Alison Wright, and Helen Cheyne who's a Professor of Maternal and Infant Health Research at the University of Stirling. Hannah Murray was at the height of her fame as an actor in 2016, having found fame as the much-loved Cassie in E4's teen drama Skins, she was now a regular in the HBO fantasy series Game of Thrones as Gilly. But behind the scenes Hannah was struggling with her mental health, and she found herself involved in an organisation which offered courses in magical healing. The following year, Hannah was diagnosed with bipolar disorder. She's since retired from acting and written a memoir about those turbulent times, called The Make-Believe – A Memoir of Magic and Madness. Women's rights charity FiLiA has launched a new national campaign to get Sexual Entertainment Venues, commonly referred to as strip clubs, off the streets. This is informed by new research examining how the venues are viewed and experienced by women living, working and moving nearby. They spoke to more than 700 women across Cardiff, Edinburgh and Manchester, with 55% of women opposed to the venues, and 8% expressing supportive views. Dr Laura Favaro, Director of Research at FiLiA, joins Kylie to discuss their report. The female CEO of dating app Hinge has said that Gen Z "need AI to start conversations on dating apps because they lack the confidence of older generations". Jackie Jantos told the BBC that daters in their 20s are spending less time with other people, compared to two decades ago, has affected their ability to meet and chat in person. Maddie shares how her twins were a complete surprise - a natural, spontaneous twin conception with no twins in the family, discovered at her 12-week ultrasound - and how reading Nat's book helped her find solid ground once the shock wore off. The ACOG recommends a multimodal approach to postoperative pain that includes nonsteroidal NSAIDs, acetaminophen, and opioids. Ketorolac is a favored NSAID for postop pain control. However, the optimal dose of ketorolac after cesarean delivery has not been determined.
