In this episode we discuss navigating non-invasive prenatal screening, also known as non-invasive prenatal testing, by reviewing two articles covering genetic counseling insights, informed consent challenges, and inclusive practices. You can find the Journal of Genetic Counseling webpage via onlinelibrary.wiley.com or via the National Society of Genetic Counselors website.    Segment 1 “Genetic counseling for fetal sex prediction by NIPT: Challenges and opportunities”   Chelsea Stevens is a clinical and research genetic counselor in the cardiovascular genetics clinic at Brigham and Women's Hospital. She has been working at BWH since 2022 and works primarily on projects aimed to understand genotype/phenotype relationships in cardiomyopathies. The publication we will be discussing today is the result of Chelsea's capstone project during her genetic counseling graduate education at the MGH Institute of Health Professions. She is from Connecticut and has been living in Boston for the past 6 years and loves working in a city with a strong genetic counseling network.   Courtney Studwell  is a laboratory genetic counselor in the Center for Advanced Molecular Diagnostics (CAMD) at Brigham and Women's Hospital. In the cytogenetics laboratory, Courtney serves as a liaison to clinical providers ordering genetic testing and helps to implement updated testing protocols that reflect patient needs and the evolving landscape of genetic testing technologies. In addition to working in the laboratory, Courtney also sees patients in the Center for Fetal Medicine and Reproductive Genetics at Brigham and Women's Hospital and is passionate about providing high-quality genetics care to patients at all stages of their reproductive process.   In this segment, we discuss: An overview of NIPS, its evolution and recommendations on utilizing it as a screening tool Insights into study participants' clinical practices and training regarding inclusive language for sex and gender Challenges and misconceptions faced by providers when offering NIPS Recommendations to address issues related to challenges and misconceptions   Segment 2 "What knowledge is required for an informed choice related to non-invasive prenatal screening?"   Jill Slamon is an Assistant Professor in Obstetrics and Gynecology in the Vanderbilt School of Medicine and the founding Assistant Program Director of the Vanderbilt University Master of Genetic Counseling Program. Jill has over 12 years of clinical experience in reproductive and prenatal genetic counseling with unique experiences in preconception counseling in the queer community. Her research interests focus on topics ranging from genetic counseling education and training, genetic counseling outcome measures, patient-reported outcome measures, and experiences of queer patients seeking preconception and prenatal care. Jill earned a Bachelor of Science from the Lyman Briggs College at Michigan State University. In this episode, Ayesha spoke with William Schaffner, MD, Professor of Preventive Medicine Health Policy and Professor in the Division of Infectious Diseases at Vanderbilt University; and Kelly Moore, MD, MPH, President and CEO at Immunize.org and Associate Professor of Health Policy at Vanderbilt University.Dr. Schaffner and Dr. Moore discussed the FDA's Vaccines and Related Biological Products Advisory Committee (VRBPAC) recommendation of a trivalent influenza vaccine strain selection, as opposed to a typical quadrivalent vaccine, with the 2024/2025 influenza season reflecting the removal of the B/Yamagata strain. A renowned infectious disease expert, having appeared on numerous media outlets including CNN particularly during the COVID-19 pandemic, Dr. Schaffner's primary interest has been the prevention of infectious diseases domestically and globally in both pediatric and adult populations. After residency training and a fellowship in infectious diseases at Vanderbilt University, Dr. Schaffner served in the US Public Health Service as an epidemic intelligence service officer with the Centers for Disease Control and Prevention (CDC) in Atlanta. After that tour of duty, Dr. Schaffner joined the faculty at Vanderbilt, establishing a long collaboration with the Tennessee Department of Health. Dr. Schaffner has been a member of numerous expert advisory committees that established national vaccine policy. Dr. Schaffner is also the current medical director and past president of the National Foundation for Infectious Diseases and has served on the executive board for the Infectious Diseases Society of America. Prominent infectious disease expert Dr. Moore served for 14 years as the director of the Tennessee Immunization Program and later founded her own consulting company, The Vaccine Advisor, to advise public health and industry before her current role as president and CEO of immunize.org, a leading nonprofit organization focused on national and global vaccine policy and immunization program implementation. Dr. Moore has served in a variety of immunization policy advisory roles with the World Health Organization (WHO) since 2016, including as chair of its Immunization Practices Advisory Committee. She practiced in primary care until 2018, when she specialized in weight management.  She became burnt out in her clinical practice due to the lack of ability to truly help her patients other than prescribing the next best pill. It was at that point that she started intervening at the root cause of disease and attacked obesity head on, using her own health journey as a catalyst. — As they were finalizing their manuscript, Ronni's daughter sent them a screengrab of an Instagram post from “jabbieapp” that read: “Never underestimate a cycle breaker. Not only did they experience years of generational trauma but they stood in the face of the trauma and fought to say, “This ends with me.” This is brave. This is powerful. This comes at a significant cost. Never underestimate a cycle breaker. Ronni and Jennie read it, and cried. A few weeks later, Ronni found a post on Instagram from Xavier Dagba and sent it to Jennie: “All this time you thought you were nothing but the black sheep of your lineage, whereas you were being initiated to be their liberator. Healing looks good on you. Keep going. Jennie cried, and something in her shifted. These two amazing sisters started their journey with a sincere desire to spare their children the pain they had endured—to break the intergenerational cycle of abuse and addiction so that they could live beautiful lives. They also hoped that they could reparent themselves and live beautiful lives of their own. And they did, if imperfectly. Though they are still learning and growing, Ronni and Jennie have each created something well beyond what we could have imagined at the beginning of this ride. This means they are more than survivors. They are, indeed, rockstars!!! Julie Bell Lindsay is CEO of the Center for Audit Quality, a nonprofit public policy organization representing U.S. public company auditors.Previously, Julie served as Managing Director and the Deputy Head of Global Regulatory Affairs at Citigroup, where she worked to formulate and execute regulatory policy strategy. Lindsay also managed Citigroup's $20 billion TARP repayment and $58 billion exchange offers in 2009. — Most individuals who have been victimized by their families do not become aware of their abuse until they have reached adulthood. We experience very distinct and expected dynamics when we grow up in a home that is characterized by violence, addiction, and abuse. Denial is the most basic of these. Fear is another key dynamic. Trying to unravel a lifetime of emotional pain, fear and dysfunction is a distressing slow task. Healing requires examining what has happened, and is happening right now at all levels —mentally, emotionally, and physically. This episode is about two sisters and survivors who found a way to come to light from a tragic cycle of inter-generational trauma and abuse. Research from the CDC shows that more than 60 percent of adults report having had a least one adverse childhood experience (ACE), and nearly 25 percent report three or more. The most prevalent are emotional abuse, parental separation or divorce, and household substance abuse. But other examples include physical abuse, domestic violence in the home, and emotional and physical neglect.  It may sound odd, but many people who were victimized by their families don't fully realize it until they are well into adulthood. Growing up in a home with addiction and abuse creates very distinct and predictable dynamics. Today's special guests, sisters and survivors Ronni Tichenor and Jennie Weaver found a way to emerge from a tragic cycle of inter-generational trauma and abuse. The past few years have not been easy for Nashville's WeGo bus network. But behind the scenes, the agency has been trying to make progress on improvements that riders have been requesting for many years. On today's show, we start by looking back over the bus system's recent history. In 2018, voters rejected a sprawling mass transit proposal. In 2019, budget cuts led WeGo to eliminate some routes. And then the coronavirus pandemic slashed ridership. We'll also talk with WeGo leaders and frequent bus riders about what's been going on since then. The agency has been using data and analysis to tweak its routes and schedules, and has had some success in running buses more frequently, as well as later into the night, and more often on weekends. Caitlin “Cat” Moon teaches in the Program on Law and Innovation (PoLI) at Vanderbilt Law School, where she also serves as the Director of Innovation Design and directs the PoLI Institute (innovatethelaw.com), Vanderbilt Law's innovation-focused executive education platform. Cat co-founded PoLI's Summit on Law and Innovation (SoLI), which brings together experts across legal, technology, and other disciplines in collaborative legal innovation projects. She currently teaches Legal Problem Solving, a course in human-centered design for law, as well as Law as a Business, Legal Operations, Leading in Law, and Data in Law Practice. Cat also contributes to the Medical Innovators Development Program and is on the faculty of Radiological Sciences at Vanderbilt School of Medicine, where she brings cross-disciplinary experience to innovation across medicine and the law. Cat's research focuses on how we might use the tools of innovation and human-centered design to improve and expand access to legal services and the law generally. She also studies the professional formation of lawyers in the 21st century from a holistic perspective. Cat is a co-creator of the Delta Model, a 21st-century framework for lawyer competency, and a co-founder, with Professor Alyson Carrel, of Design Your Delta, a playbook for holistic professional development grounded in human-centered design principles and methods.

Eydie joined the Osher team in 2013 and is part of the Search Inside Yourself Leadership Initiative and an instructor in the Mindfulness-Based Stress Reduction (MBSR) program. She is also the primary investigator for a pilot study entitled "Mindfulness Centered Stress Reduction for Nursing Students." In addition to her work at the Osher Center, Eydie has been on the Vanderbilt School of Nursing faculty for over a decade and teaches in the specialty level Psychiatric and Mental Health Nurse Practitioner program. Eydie holds a Masters of Science and Nursing from Vanderbilt University and a Doctor of Jurisprudence from the University of Tennessee. Underwood Chair in Law and Director of the Criminal Justice Program at Vanderbilt Law School, and Affiliate Professor of Psychiatry at Vanderbilt School of Medicine, to discuss Professor Slobogin's recent monograph titled “A Primer on Risk Assessment: Instruments for Legal Decision-Makers.”   To read and download the primer, click here.  Chris Slobogin has authored more than 100 articles, books and chapters on topics relating to criminal law and procedure, mental health law and evidence. Named director of Vanderbilt Law School's Criminal Justice Program in 2009, Professor Slobogin is one of the five most cited criminal law and procedure law professors in the country over the past five years, according to the Leiter Report, and one of the top fifty most cited law professors overall from 2005-2015, according to Hein Online. Particularly influential has been his work on the Fourth Amendment and technology and his writing on mental disability and criminal law, appearing in books published by the University of Chicago, Harvard University and Oxford University presses and in journals such as the Chicago Law Review, Georgetown Law Journal, Northwestern Law Review, Pennsylvania Law Review, Stanford Law Review and Virginia Law Review. He is currently an Associate Reporter for the American Law Institute's Principles of Police Investigation Project.  In recognition for his work in mental health law, in 2016 Professor Slobogin received both the American Board of Forensic Psychology's Distinguished Contribution Award and the American Psychology-Law Society's Distinguished Contribution of Psychology and Law Award; only a total of five law professors have received either of these awards in their thirty-year history, and none has received both awards. Before joining Vanderbilt's law faculty, Professor Slobogin held the Stephen C. O'Connell chair at the University of Florida's Fredric G. Levin College of Law. Professor Slobogin holds a secondary appointment as a professor in the Vanderbilt School of Medicine's Department of Psychiatry.

Dr. Isfahan Chambers-Harris earned her B.S. in Biology from the City University of New York- Medgar Evers College and Ph.D. in Bio-Medical Sciences from  Morehouse School of Medicine, Cardiovascular Research Institute. On today's episode we discuss: — Climate: The Immunization Action Coalition and the Vanderbilt School of Medicine discuss the progress of developing a COVID-19 vaccine, detailing the 11 vaccines in phase 1/2 clinical trials and the inclusion of multiple countries in the development of a Target Product Profile, a tool for quantifying critical vaccine characteristics. The authors emphasize the need for well-established infrastructure and logistical planning in order to distribute a potential COVID-19 vaccine safely and effectively to the world in a timely manner. — Epidemiology: Researchers in the United States and United Kingdom used Twitter data to predict human mobility in and out of China and estimate global spread of COVID-19 and found a high correlation between country-level Twitter user visits and reported COVID-19 cases. These findings highlight the utility of geolocated platforms for public health authorities to develop response protocols and assess impending risks of COVID-19. — Transmission and Prevention: Based on literature of prostaglandins in SARS-CoV-1 and MERS-CoV, a group from the University of Edinburgh cite the potential roles of multiple prostaglandins (including PGD2, PGE2, PGI2) in the pathogenesis of COVID-19 and consider the possible benefits of NSAID usage in COVID-19 patients. — Management: A study found that among 59 patients with symptoms consistent with COVID-19, fibrinogen levels were markedly higher in patients who ultimately tested positive on RT-PCR compared to those who tested negative. Additionally, the neutrophil-to-lymphocyte ratio was increased in critically ill COVID-19 patients. These results suggest that fibrinogen may have value as a diagnostic marker in patients with suspected COVID-19 and that neutrophil to lymphocyte ratio may be useful as a prognostic marker indicative of COVID-19 disease severity. — Adjusting Practice: A cross sectional study of 26 heart failure patients found a 16.2% decrease in daily step count on wearable accelerometers during the first 3 weeks of quarantine compared to regular activity suggesting that widespread COVID-19 quarantine mandates were detrimental to physical activity habits in these patients. These findings highlight the importance of physical activity during the pandemic in reducing deterioration in cardiovascular health. — R&D Diagnosis and Treatment: Investigators in China measured SARS-CoV-2-specific antibodies among moderate and severe COVID-19 patients as well as non-COVID-19 patients and found that IgA and IgG were higher among severe patients compared to moderate patients whereas no difference was observed between the groups in IgM. Based on their findings, the authors suggest that IgA-IgG serological diagnosis of COVID-19 may be more effective than the traditional detection of IgM-IgG combined antibodies. --- Support this podcast: https://anchor.fm/covid19lst/support

The Cumberland River experiences a high concentration of litter after rainfall events. This litter eventually degrades into microplastics which can negatively impact wildlife and human health. Most of this litter flows into the Cumberland River through smaller streams. The Cumberland River Compact worked with Vanderbilt School of Engineering students to design a device that can be placed in smaller streams in Nashville to capture surface litter before it ever reaches the Cumberland. In this interview, we hear from the students about the creation of the device, lessons learned, and next steps for implementation. Can changes in your psychology create physical symptoms? Is there such a thing as "non-disease"? In this utterly compelling episode, a true medical detective - Dr. Clifton Meador - explains how he came to understand that some symptoms and diseases emerge from the "life narrative" of the patient. If a doctor listens hard enough, the patient will often volunteer the reasons for his or her health struggles. He asserts that all patients should consider their diet, chemical exposures and stress levels, and partner with their doctor in a "medical detective" team in order to uncover the root causes of symptoms and illness. Dr Meador says: "I came to fully understand that the mind and the body were one – not separated, not disconnected. What affected one also affected the other. Sitting above all the molecules, tissues, organs, and mind of the human body was an integrated person. This person was connected to a family and perhaps to a spouse, and the family was connected to some social structure and society at large. All this social structure impinged on the person, and the person impinged on the social structure. There was a continuum all the way from society to the person to the organs and even down to the molecules. There were no separated pockets or islands. There certainly was no mind separated from a body. " ---------- Dr. Clifton K. Meador Biography Dr Clifton K Meador has been practising and teaching medicine for over 60 years. He has written fifteen books and published articles in esteemed medical journals such as NEJM. Fascinated by patients with symptoms of hidden origin and by patients who carried diagnoses of non-existent diseases, he recorded his experiences in his books Symptoms of Unknown Origin, Puzzling Symptoms and Fascionomas – fascinating medical mysteries. A Graduate of Vanderbilt University School of Medicine in 1955, Dr. Meador trained at Columbia Presbyterian Hospital in New York, and Vanderbilt Medical School where he completed a National Institutes for Health fellowship in Endocrinology. After practicing medicine, he joined the faculty of medicine at UAB, where he was Professor of Medicine and then Dean of the University of Alabama School of Medicine at UAB from 1968 – 1973. He returned to Vanderbilt and St. Thomas Hospital, serving as chief of medicine and chief medical officer of Saint Thomas from 1973 to 1998. He then served as the first Executive Director of the Meharry-Vanderbilt Alliance from 1999 to 2012. He is professor of Medicine Emeritus at both Vanderbilt School of Medicine and Meharry Medical College. Dr. Meador lives in Nashville, TN. His wife Ann Cowden, is a well-known portrait artist. He is father of seven children, seven grandchildren and two great grandchildren.

Canon Gideon Byamugisha of Uganda will share his personal story of living with HIV and about his work and leadership against stigma, shame, denial and discrimination, inaction and mis-action. Further, he will discuss his ministry and theology to uphold the human dignity and rights of people living with and most vulnerable to HIV and AIDS. Then, Dr. Jenny Dyer will will reflect on the critical role of faith leaders to shape policy and funding for Global AIDS since the beginning of the Bush Administration. She will look at the sustained support over the past three Administrations and the success of legislation, in part, thanks to their advocacy.In 1992, Gideon Byamugisha of Uganda became the first religious leader in Africa to publicly announce that he was HIV positive. Before long, others came forward and Byamugisha founded the African Network of Religious Leaders Living with and Personally Affected by HIV and AIDS (ANERELA+), now an international organization with 10,000 members in 48 countries. Byamugisha holds a B.A in Education (Honors) from Makerere University, Kampala, a Bachelor of Divinity (First Class) from ATIEA Nairobi, and a Masters in Contextual & Applied Theology (University of Birmingham, United Kingdom). For his pioneering leadership, St Paul's Cathedral, Namirembe Diocese (Kampala, Uganda 2001), and Holy Cross Cathedral, (Lusaka Diocese, Zambia, 2003) conferred upon him the ecclesiastical title of Canon in the Anglican Church. Canon Gideon, born in 1959, is the eldest of 14 children. In 1991, he lost his wife, Kellen to AIDS. Their daughter, Patience, is now 29. In 1995, Canon Gideon married Pamela (herself HIV positive and widowed by HIV/AIDS at a very young age). Thanks to advances in the prevention on Mother-to-Child Transmission, Pamela and Gideon have two HIV-negative children, Love, 15, and Hope, 13. Jenny Dyer is the Founder of The 2030 Collaborative. As such, she directs the Faith-Based Coalition for Global Nutrition with support from the Eleanor Crook Foundation.Dyer teaches Global Health Politics and Policy as a Lecturer in the Department of Health Policy at Vanderbilt School of Medicine, and she has taught Religion and Global Health as a Lecturer at Vanderbilt School of Divinity.Dyer formerly served as the Executive Director for Senator Bill Frist, MD’s Hope Through Healing Hands from 2008-2018, and prior to that, she was the National Faith Outreach Director for the DATA Foundation and The ONE Campaign, Bono’s organization, from 2003-2008. In these roles, she has worked with religious leaders, authors, artists, and other faith-based leaders to promote awareness and advocacy for global health and development issues.Dyer has written several academic articles and opinion pieces on the intersection of religion and global health. She has been published in TIME, Forbes.com, Huffington Post, Roll Call, Christianity Today’s Her.meneutics, Patheos, Relevant Magazine, Christian Post, Dallas Morning News, Washington Post, and The Tennessean, to name a few.She is a contributor of Why Save Africa: Answers from around the World (2011) and a co-compiler of The Mother & Child Project: Raising Our Voices for Health and Hope (2015) and The aWAKE Project: Uniting Against the African AIDS Crisis (2002). Her forthcoming book as a co-editor, The End of Hunger: Renewed Hope for Feeding the World, will release in October 2019 with InterVarsity Press.Dyer holds a B.A. in Religion from Samford University (1999), a Master of Theological Studies from Vanderbilt Divinity School (2001), and a Ph.D. in History and Critical Theories of Religion from Vanderbilt University (2007).

After completing his fellowships in 2009, he joined the faculty of Vanderbilt School of Medicine in the Department of Pathology, Immunology and Microbiology and was concurrently a Health Research Scientist for the U.S Department of Veterans Affairs. Dr. Vigerust currently maintains an Adjunct Assistant Professor position at Vanderbilt University Medical Center in the Department of Neurological Surgery and is Clinical Assistant Professor at the University of Maryland Eastern Shore School of Pharmacy.
