A review confirms better access to care should be a key priority. Health New Zealand's released a Clinical Quality and Safety Review using data from the past decade. It reveals the number of people waiting more than four months to see a specialist tripled between 2020 and 2023. Royal College of GPs Medical Director Luke Bradford told Mike Hosking fewer people are also choosing not to access care, especially since Covid. He's concerned there's been a drop off in immunisations and cancer screenings. However, there is a belief the quality of care is getting better despite taking longer to access it. It reveals numbers of people waiting more than four months to see a specialist tripled between 2020 and 2023. Chief clinical officer Richard Sullivan says Covid played a role and numbers are stabilising. But he says there's a long way to go. He also explained one target is 95 percent of patients waiting less than four months - but we're on about 60 percent, so improving that is a big focus. One topic that I don't think is talked about enough with expecting and new mothers (or people in general) is pelvic floor health. In my opinion, pelvic floor health should be considered as a form of self-care and a part of us taking care of our overall health. I can't even tell you the number of people I have met who weren't totally sure what their pelvic floor is, what it does, how it affects them, and that everyone has one. And when a woman becomes pregnant and gives birth to her baby, these are the times when she might experience some problems with her pelvic floor and wishes she had known more about her changing body and had support. Whether you had a vaginal birth or cesarean birth, your body has done some hard, amazing work. Our guest today, Liz Miracle, knows all about this since she has supported hundreds of women as a pelvic floor physical therapist. Today, she is sharing her tips on how to best take care of our bodies after childbirth, including our pelvic floors. Who is Liz Miracle? Liz Miracle, PT, MPT, WCS is the Head of Clinical Quality and Education at Origin, the leading provider of physical therapy for maternity, menopause, and sexual health, with deep expertise in the pelvic floor and full body. Liz is a Texas native and graduate of Texas Woman's University School of Physical Therapy. She began practicing pelvic floor physical therapy in 2006 and has since received her Clinical Specialization in Women's Health from the American Board of Physical Therapy Specialties. When it comes to carrying a child in the time of pandemic, what are the options for care that keep the parent and child as safe as possible?At this year's NCQA Health Innovation Summit, we heard some answers from Dr. Nancy Gin and Dr. Brenda Jackson, both of Kaiser Permanente. Together, the offered a presentation titled: Reducing perinatal risk, improving access, and reducing disparities through virtual perinatal care.To minimize risk during the pandemic, Kaiser Permanente used its telehealth capabilities to pivot many prenatal in-person interactions to virtual encounters. It created a perinatal remote monitoring pathway: a blended perinatal care model that allows patients to have some prenatal appointments conducted by video and monitors vitals for pregnant people and their babies. Dr. Nancy Gin is Executive Vice President of Quality and Chief Quality Officer for the Permanente Foundation, which oversees clinical quality for more than 23,000 physicians nationwide in 8 Kaiser Permanente regions caring for over 12.5 million members. She also serves as the medical director of quality and clinical analysis for the Southern California Permanente Medical Group (SCPMG), leading quality and patient safety for 4.7 million Kaiser Permanente members in Southern California.Brenda Jackson DNP – a Doctor of Nursing Practice – serves as Senior Director of Clinical Care Improvement for Clinical Quality in The Permanente Federation. Her work has emphasized inter-regional, multidisciplinary collaboration to implement strategic advancement in patient quality, safety, and care experience.

Synopsis: Every first and third Wednesday of the month, The Straits Times helps you make sense of health matters that affect you. Organ donation usually takes place after brain death, which is when there is total and irreversible loss of brain function. A patient with brain death cannot breathe on his own, and cannot recover, but his organs may still function for some time as he is supported on a ventilator. This is when conversations on organ donations typically take place. In this episode, ST senior health correspondent Joyce Teo speaks with Associate Professor Tan Hui Ling, the Assistant Chairman of the Medical Board, Clinical Quality and Audit, and a senior consultant at the Anaesthesiology, Intensive Care and Pain Medicine department at Tan Tock Seng Hospital (TTSH) to find out more about the organ donation process and address some of the concerns that people may have about it. At TTSH, Prof Tan spearheaded a programme to train the multi-disciplinary ICU teams (doctors, nurses, medical social workers) on the management of brain death and organ donation.  She was the director of TTSH's neurological Intensive Care Unit and chair of the hospital's Brain Death and Organ Donation Taskforce from 2010 to 2018. Prof Tan was awarded the National Healthcare Group's Distinguished Achievement Award this year for her various contributions. In Singapore, under the Human Organ Transplant Act, all citizens and permanent residents will be considered as organ donors, unless they opt out of it. The act allows for the kidneys, heart, liver and corneas to be donated for transplantation in the event of death.  There is also t​he Medical Therapy, Education and Research Act​, which is a voluntary opt-in scheme​ that allows any individual ​aged 18 and above​ ​to donate ​his or her organs, tissues or even the whole body​ upon death,​ for transplantation, education and research. In 2017, the National Organ Transplant Unit introduced the donor coordinator role  to better support potential brain-dead donor families. This donation specialist will support the donor family and address their queries or concerns on the donation process. Cardiovascular events make up some of the most dangerous and time-sensitive conditions that get treated in the emergency room and hospital at large. For this reason, they make up a significant proportion of malpractice suits concerning cardiac care An LNC needs to be deeply familiar with all aspects of cardiovascular and treatment. She or he must know what results are unavoidable and which are preventable. Here to explain some of the most critical elements of cardiovascular care is Lailen Baua, an LNC with broad experience in many aspects of clinical care. Time is a critical factor. In some cases, delays are beyond a hospital team's control, such as those caused by distance from a well-equipped hospital or traffic delays. Lailan says that ideally it should be no more than two hours before a cardiovascular patient is receiving appropriate treatment, such as a balloon angioplasty. The first TOGA Podcast for 2022 is about the need for a national clinical quality registry in lung cancer. A/Professor Emily Stone, the Deputy Chair of TOGA, is a Respiratory Physician, Senior Staff Specialist and Head of Thoracic Medicine within the Department of Thoracic Medicine and Lung Transplantation at St Vincent's Hospital Sydney. She is a conjoint Senior Lecturer at the University of New South Wales and the Chair of the St Vincent's Hospital Lung Cancer Multidisciplinary Team. Emily is joined by Professor Rob Stirling, Respiratory Physician from the Alfred and Monash University who is the lead for the Victorian Lung Cancer Registry (VLCR) and also Professor Fraser Brims, Respiratory Physician from Sir Charles Gairdner Hospital and Curtin University Medical School and Institute of Medical Health.The high burden of disease and poor outcomes in lung cancer necessitate provision of optimal and timely care to ensure the best possible outcomes for people with lung cancer. A national clinical quality registry will reduce unwarranted variation in care by benchmarking performance and evaluate strategies designed to improve care, such as the expected lung cancer screening program. Registries are not just about collecting data.  Examples of varied care in lung cancer and a vision of what a clinical quality process should look like for lung cancer is explored in this podcast, together with an urgent call to action to demand funding for a national Clinical Quality Registry for lung cancer.

This conversation is with Dr David Richmond, who is currently a Non-Executive Director at Birmingham Women and Children's NHS Foundation Trust.David was Medical Director at Liverpool Women's Hospital for 25 years, and was involved with the Royal College of Obstetricians and Gynaecologists (RCOG) for 20 years. This culminated in becoming the Vice President (Clinical Quality) and then President of the College. Here, he was instrumental in delivering the first national outcome report for maternity which is now the National Maternal and Perinatal Audit programme. His clinical specialty was that of Urogynaecology. As Chair of the Audit Committee of the British Society of Urogynaecology (BSUG) he developed the national BSUG surgical database with others. The complications that can follow broken bones at an advanced age, most notably a broken hip, can literally be deadly. This reality, numerous programs, and reporting requirements have kept falls prevention front and center across the health care continuum. Now a new initiative wants to emphasize the physical and mental health benefits of mobility, and encourage greater mobility for older patients in conjunction with falls prevention, as part of a broadening vision of "age-friendly" care​. We explored what this looks like in practice, starting with the hospital setting, on the March 22 episode of WIHI: Mobility Matters for Age-Friendly Care.

So has joining up with something larger — to keep the pressure on, commit to stretch goals, and benefit from coaching and continuous learning. The Partnership for Patients, launched by the US Centers for Medicare & Medicaid Services (CMS) in April 2011, is attempting to be that “something larger” for some 3,700 hospitals that have signed on to reduce nine hospital-acquired conditions by 40% and hospital readmissions by 20% by December 2013. So, whether because of this association or because other sectors of health care tend to get more attention, the hard work that’s going on to ensure that all types of SNFs, and nursing homes, deliver high-quality and patient-centered care, has been somewhat obscured. We’d like to help change this by zeroing in on one aspect of the work. This WIHI looks at new developments with better coordination and communication between SNFs, local hospitals, and various community stakeholders, to reduce unnecessary transfers of patients to acute care settings. These avoidable admissions or readmissions can come from short-term-stay SNFs or long-term ones, from assisted living residences or rehab facilities. Unpacking what’s behind unnecessary transfers and what better, safer, actions might be taken has been the focus of IHI’s STAAR initiative and is captured in one of a series of STAAR How-to Guides: Improving Transitions from the Hospital to Skilled Nursing Facilities to Reduce Avoidable Rehospitalizations.   Our guide on the learning and momentum everyone can build upon from STAAR was IHI’s own Marie Schall, who’s also the first to point to the rich activity and resources available from Laurie Herndon and the widely recognized INTERACT program. Another huge resource is the American Health Care Association, which is mobilizing SNFs across the US to do their part to reduce readmissions within 30 days by 15% by 2015. David Gifford also spoke to these efforts. And, then there’s the amazing example of Kitsap County in the state of Washington, where Annette Crawford’s SNF has played a leading role building a new kind of coalition across the continuum of care to ensure that patients get the right care in the most appropriate setting.
