I'm your host Robert Hackett, and today we're talking about congestion pricing — an area of mechanism design that's aimed at alleviating something everyone hates: traffic.Now you may have heard this term recently since New York adopted its own version of congestion pricing at the beginning of the year. This is the first program of its kind in the U.S. — and it's got supporters and detractors. We'll talk about that, and we're also going to talk about much more. In the first part of today's episode we'll trace the history of the economic ideas that got us here. In the middle, we'll dig deeper into the details of putting congestion pricing into practice, plus technological alternatives. And in the final part, we'll explore parallels to — and implications for — crypto networks.Our guests are Michael Ostrovsky, a Stanford Economics Professor who specializes in this area and who has done research on congestion pricing in New York. In Session 221, I had the opportunity to talk with Melissa Willa and Colin Davitian. These guys are a husband and wife team who founded the ABA provider, Kyo, formerly known as the Gateway Learning Group. Our conversation focused on two hot-button topics when it comes to providing services to individuals with Autism: Treatment Dosage and Value Based Care. With regard to the former, Melissa and Colin, along with their colleagues, conducted a retrospective analysis of treatment outcomes for learners in under their care. In doing so, they looked at the role that treatment dosage had on their overall progress, as measured by standardized assessments. You may already be wondering what they found, so don't worry, we get into that and lots more in this episode. We also try to place their findings in the context of existing studies that examined the relationship between treatment dosage and treatment outcomes. As I mention during the interview, there's lots of discussion of this topic that's happening, particularly in the various ABA social media groups, and as is true with so many other things, it's refreshing to elevate the discourse by looking at empirical analyses as opposed to anecdotal observations and hunches. If you're a clinical director or practice owner who's also interested in this topic, Melissa and Colin suggest strategies for mining the data you probably already have access to via client electronic health records. Towards the end of the podcast, we turned our attention to Value Based Care. Colin and Melissa define what that is, and contrast it with the more traditional fee-for-service approach for health care reimbursement. Not to spoil the punchline here, but these guys seemed pretty bullish on the potential for shifting Autism services to a Value Based Care model. Lastly, I want to thank Melissa and Colin for patiently responding to all of my “yeah, but…” questions. Even though I don't personally provide services in an insurance funded model, I am old enough to remember the days when accessing ABA services was next to impossible due to the lack of both funding and providers, so I'm fairly reluctant to cede ground as it relates to how many hours individuals can receive authorization for, and my line of questioning most likely revealed this. I've also seen cases where learners have received very intensive early intervention, and made so much progress that they entered kindergarten with little to no supports. As such, I think it's fair to say that I had some level of bias heading into this conversation. This week in the studio a man who in 1964 was born in South Miami to immigrant parents. His father Russian and his mother Cuban, he was raised in a strict household that taught him, if he wanted to be or do anything in life it was on him to make it happen. Fast forward to 1984 while working as a lifeguard he got to see his first Marijuana bust, and it was at this moment that the hook was set for my guest to become a law enforcement agent, working with Customs on the Miami boat crews, as an undercover detective posing as an international smuggler, leading an investigation into one of the biggest Marijuana and Cocaine trafficking case that the Northwest United States had seen. My guest worked on the Joint Terrorism Task Force and was even the acting Attaché in London, England. Today's episode features an interview with a man who operates a marketing business to help local companies do a better job in building awareness of their products and services. Brian Ostrovsky and his family started RVing about a year and a half ago. Originally from Sacramento, Calif., he and his wife raise five children ranging in ages from 4 to 13. They travel in a KZ Durango fifth wheel bunkhouse, which ensures everyone has enough space. Brian created a marketing workshop program with which he works with local chambers of commerce and main street businesses. His wife was a teacher, but now she coordinates the roadschooling activities of their children. He describes some curriculum their family uses to assist in educating the kids so they learn effectively in whatever subject level they are learning. When the family started RVing, Brian was traveling from community to community to equip businesses to become more effective in their marketing. He has a masters of business administration and a passion for helping small businesses, especially after seeing the internet was making it more difficult for them to compete effectively. Brian also developed software to help businesses with their marketing, which he distributes as well. It creates a robust web platform to help firms collect more reviews, send email newsletters and help leverage the power of a business owner's own network. Currently, 7,000 small businesses utilize his software. He authored a book called Marketing 3-4-5 which targets small business owners who may not be experts in marketing. The book describes three reasons any business must do marketing, four reasons customers opt to support a business, and five ways customers can find businesses. Brian outlines a series of easy steps owners can take to advance their companies with 15-minute activities. It gives them insight into simple things they can do reach more customers. For example, he developed templates and prompts which help people create interesting blog posts. One of Brian's biggest challenges is maintaining an internet connection, and he outlines some of the things he does to juggle service providers to maintain consistent service wherever he may be. Changes in technology have made it possible for professionals like Brian to manage 7,000 customers from the road – and still have plenty of time to enjoy family activities. He tries to schedule business meetings and important phone calls in the middle of the day, from 11 a.m. to 3 p.m., when the children can be outside. That schedule also gives his family more time to explore a local area in the afternoon. The family has faced several challenges on the road, especially when the truck that hauls their home as well as the seven family members, decided to act up during the middle of a trip. But, thankfully, they were able to find an independent service center to fix them up and get them on their way. As a general rule, the family moves to a new location every week and tries to travel on weekends. To make travel more fun, they spend a lot of time in small towns where they seek out local ice cream shops on various main streets. In fact, Brian estimates his family has visited more than 250 small towns and main streets on their journey. Spending time in small towns also gives the family a completely different perspective of an area rather than zeroing in on bigger cities. Not only is it more challenging to tow an RV or even park a larger truck in big cities, but Brian finds that they enjoy a more authentic experience by spending time in smaller communities. Dr. Andrey Ostrovsky is the former Chief Medical Officer of the US Medicaid program. He is now the Managing Partner at Social Innovation Ventures where he invests in and advises companies and non-profits dedicated to eliminating disparities. He also advises federal and state regulators on how to incorporate human centered design into policy making. He previously operated a series of methadone clinics in Baltimore, Maryland. Prior to working on the front line of the opioid use disorder crisis, he served as the Chief Medical Officer for the Center for Medicaid and CHIP Services, the nation's largest health insurer, where he advocated to protect the program against several legislative efforts to significantly dismantle the program. He also led efforts to streamline Medicaid and make it more customer-centric. Before leading the Medicaid program, he co-founded the software company, Care at Hand, an evidence-based predictive analytics platform that used insights of non-medical staff to prevent aging people from being hospitalized. Care at Hand was acquired in 2016 by Mindoula Health. Before Care at Hand, Dr. Ostrovsky led teams at the World Health Organization, United States Senate, and San Francisco Health Department toward health system strengthening. Dr. Ostrovsky has served on several boards and committees dedicated to behavioral health, interoperability standards, quality measurement, and home and community based services including the National Academies of Medicine, National Quality Forum, Institute for Healthcare Improvement, and the Commonwealth Fund. Andrey holds a Medical Doctorate and undergraduate degrees in Chemistry and Psychology Magna cum Laude from Boston University and is a member of Phi Beta Kappa. Andrey completed his pediatrics residency training in the Boston Combined Residency Program at Boston Medical Center and Boston Children's Hospital where he was a clinical instructor at Harvard Medical School. Gudrun talks to Anna Geyer. Anna is Assistant professer at TU Delft in the Mathematical Physics group at the Delft Institute of Applied Mathematics. She is interested in the behaviour of solutions to equations which model shallow water waves. The day before (04.07.2019) Anna gave a talk at the Kick-off meeting for the second funding period of the CRC Wave phenomena at the mathematics faculty in Karlsruhe, where she discussed instability of peaked periodic waves. Therefore, Gudrun asks her about the different models for waves, the meaning of stability and instability, and the mathematical tools used in her field. For shallow water flows the solitary waves are especially fascinating and interesting. Traveling waves are solutions of the form representing waves of permanent shape f that propagate at constant speed c. These waves are called solitary waves if they are localized disturbances, that is, if the wave profile f decays at infinity. If the solitary waves retain their shape and speed after interacting with other waves of the same type, we say that the solitary waves are solitons. One can ask the question if a given model equation (sometimes depending on parameters in the equation or the size of the initial conditions) allows for solitary or periodic traveling waves, and secondly whether these waves are stable or unstable. Peaked periodic waves are an interesting phenomenon because at the wave crest (the peak) they are not smooth, a situation which might lead to wave breaking. For which equations are peaked waves solutions? And how stable are they? Anna answers these questions for the reduced Ostrovsky equation, which serves as model for weakly nonlinear surface and internal waves in a rotating ocean. The reduced Ostrovsky equation is a modification of the Korteweg-de Vries equation, for which the usual linear dispersive term with a third-order derivative is replaced by a linear nonlocal integral term, representing the effect of background rotation. Peaked periodic waves of this equation are known to exist since the late 1970's. Anna presented recent results in which she answers the long standing open question whether these solutions are stable. In particular, she proved linear instability of the peaked periodic waves using semi-group theory and energy estimates. Moreover, she showed that the peaked wave is unique and that the equation does not admit Hölder continuous solutions, which implies that the reduced Ostrovsky equation does not admit cusps. Finally, it turns out that the peaked wave is also spectrally unstable. This is joint work with Dmitry Pelinovsky. For the stability analysis it is really delicate how to choose the right spaces such that their norms measure the behaviour of the solution. The Camassa-Holm equation allows for solutions with peaks which are stable with respect to certain perturbations and unstable with respect to others, and can model breaking waves. Anna studied mathematics in Vienna. Adrian Constantin attracted her to the topic of partial differential equations applied to water waves. She worked with him during her PhD which she finished in 2013.
