Maternal Health Initiative (MHI) was founded out of Charity Entrepreneurship (AIM)'s 2022 Incubation Program and has since piloted two interventions integrating postpartum (post-birth) contraceptive counselling into routine care appointments in Ghana. We concluded this pilot work in December 2023. A stronger understanding of the context and impact of postpartum family planning work, on the back of our pilot results, has led us to conclude that our intervention is not among the most cost-effective interventions available. We've therefore decided to shut down and redirect our funding to other organisations. If you've got a belly fat issue you potentially also have a need to balance blood sugar. In this episode I answer a community member question. Community Member Question: “I am struggling with keeping my blood sugar balanced. I am wearing the CGM and it is showing my blood sugar going below 70 many times during the night and 2-3x during the day. Trying to figure this all out. Balanced meals don't seem to be doing much to help. Tonight I am having a protein shake with 40 grams of protein and 30 grams of fiber. Hoping this keeps things more steady overnight and I can get some restful sleep. Not asking for medical advice here, just moral support.” SHE IS JUST RECOVERING FROM AN ILLNESS. While there isn't necessarily wrong with a 70 or lower at night if you feel fine, crashing during the day definitely raises some questions. Clarified with the individual it was that it down (but not numbers given to what) 2-3 times during the day. How to Use a CGM to Balance Blood Sugar Observe first. Change second. Energy, food, activity, stress and sleep all matter. Did you simply record what is “norm” for you right now? What is your stable energy all day level? CGMs may need to be adjusted or recalibrated to you. Don't let arbitrary numbers dictate what's true for you. On the other hand if you feel the highs and crashes, then we want adjustments. Are you monitoring your stress levels and your sequence of food? Sequence can change the impact of a meal by 75%. Any meal eaten under stress or sleep deprived will spike blood sugar more than it would otherwise. Try comparing it to a mixed meal. Dine on real food and compare. You're mixing in a protein shake and this may not be working. But TESTING is key. A liquid meal like a smoothie at night may not support your stable levels as much as a solid meal that takes longer to digest. There is an appropriate time to do smoothies but there's also a time you're better off doing solid food that takes hours to break down and then acts like a “time-release” drip for more balance blood sugar. So you've tried X.. .how can you have the same food stuff in a different form or order (make ONE change and see if there is a difference. Document sleep, stress and exercise status: they are also impacting. Chances are poor sleep is impacting your responses. I would not suggest a shake at dinner if you want better sleep. Give your gut whole food. Experiment: In the protein shake you had: X protein X carb X fat. What happens if you adjust the protein, fiber, carb or fat? Not all at once, but changing one will change the ratio. How to Test to Balance Blood Sugar Levels What happens if you have a PLATE of whole food instead? Salmon Carb in legume soup or bean salad (to bump your fiber) Added fiber to reach same. Then try again … same meal in different order.. Carbs last.. What happens? Testing means you change ONE variable at a time. Keep in mind week 1 should be recording what is your usual habit. Week 2 and so on should be consciously testing ONE thing at a time. Other Factors in Monitoring Blood Sugar Balance with CGM Are you taking High doses of Vitamin C? This can create false reports of blood sugar if you're using some CGMs. Does this feel right? The monitor may not be functioning properly or require some calibration. Did you know we tend to default to having done something wrong! It's not always us! Throughout season 1, we have discussed how society, often times through the media, blames survivors of sexual violence. Frequently, media portrayals impact trial outcomes, a survivor's willingness to report, and more. In this episode, we speak to Dr. Danielle Slakoff, a professor and prominent researcher that studies the ways in which the media inaccurately portrays survivors of domestic violence and sexual harm. During the episode, we also talk about ‘the ideal victim'. According to her research and analysis of newspaper stories, women that are missing, that experience sexual harm or domestic abuse are portrayed differently based on race. The ideal victim has historically been white women and girls. They are frequently portrayed as being blameless and in need of protection. Black and Latina women, women that do not conform to this stereotype, are often blamed for the victimization. According to Dr. Slakoff's research, Latina and Black women and girls were portrayed much more negatively than the white women and girl victims. Specifically, they were portrayed as risk-taking at the time that the crime occurred and somehow responsible for the harm they experienced. This deep dive into the ways in which the media influences the way we think about who experiences harm and why is a critical piece of a broader conversation about race and the criminal justice system. Here’s what tends to happen to someone who’s had a fall: they’re AFRAID of having another fall...this fear causes them to avoid activities that challenge their balance...as they become less active, their balance and strength get worse...as this happens, they become more likely to have another fall...meanwhile, they’re avoiding activities they used to enjoy, and they start to experience things like depression and anxiety...their “quality of life” is getting steadily worse over time, all because of one fall… If this sounds like you, this information should be very helpful! The Case: Emily has 2 autoimmune diseases She decided to get tested for EBV (Epstein Barr Virus) which her doctor reluctantly did. Her doctor didn’t know how to interpret her results and dismissed her concerns. What Emily experienced is very common. Conventional doctors often are hesitant to test for EBV and don’t truly know how to read the results. In some cases, they don’t even order the right lab tests. Emily knew that there was more to the story than her doctor understood. She’d educated herself about EBV and knew that there could be a connection to her 2 autoimmune diseases. And, that her health could be on the line. The Investigation I was happy to help Emily better interpret her lab results. As we learned in the first case of suspected EBV, this can be a very complicated, confusing, and time-sensitive diagnosis. There are few people who understand it better than Dr. Kasia Kines, so I brought her back to discuss Emily’s case. Dr. Kasia, she is a Doctor of Clinical Nutrition and the CEO of EBV Educational Institute and Holistic Nutrition Naturally. She is also the author of the Amazon Bestseller: The Epstein-Barr Virus Solution. Four Markers to Test for EBV Epstein-Barr Virus (EBV) is complicated. In order to get a full picture of what’s going on with the virus (and determine if it’s latent or new), there are four markers that should be tested. These are tested through blood tests that many labs offer. The four tests are: EA IgG (Early Antigen) - this is the most important test and it’s often missed. If your doctor skips this one you won’t know your true EBV status. EBNA IgG (Epstein Barr Nuclear Antigen) - Indicates a history of infection in non-acute cases. This tells the story of the long term status of the virus. VCA IgG (Viral capsid antigen) - Indicates an acute infection and will remain perpetually present after initial infection. VCA IgM (Viral capsid antigen) - Is present in the early phase of an infection and usually disappears. For what the results mean, see the chart in this week's show notes here: Health Mysteries Solved Podcast - Episode 43 [chart courtesy of Dr. Kines’ website] The Often Missed EBV Test The EA IgG early antigen test is the most important in getting a big picture of what’s going on right now with the virus. Unfortunately, it is often skipped by doctors. When you have EBV, the virus stays in the body (and we may not be able to ever kill it off entirely). Dr. Kines says that 90 - 95% of the population carries the virus and for many, it doesn’t impact their day-to-day health. Despite this, someone may wake up and feel like they’ve been run over by a truck. Could it be the latent EBV reactivating? The early antigen test is key in determining this - and it must be tested early on (within the first few weeks of the ‘flare-up’ or reactivation). If this test is not done, you could have active EBV and not know it which can be dangerous. Symptoms of Active EBV A reactivation of EBV can present in a variety of ways. I could seem like the flu with severe fatigue and feeling like you’ve been ‘run over by a truck’. It can also present with cold-like symptoms (sore throat, swollen lymph nodes), or skin rashes, and even gut or digestive issues. Digestive issues could present as constipation, pain, or symptoms related to leaky gut. If you have an autoimmune disease like Hashimoto’s, it might present as a flare-up of the autoimmune disease. The real challenge with symptoms of EBV is that there are so many potential symptoms that could easily be something else. This is why EBV is often not considered or caught. How to Decrease Elevated EBV Markers When there is a flare up, the results on the lab can be very confusing - especially when the level is shown simply as above the maximum lab result. Dr. Kines has a protocol she recommends (and I use it with my patients too). This recommends a restricted diet and a lot of rest. It is all outlined in her book. It is also important to continue to monitor the markers. However, Dr. Kines points out that everyone’s levels are going to be different. Some will decrease significantly with lifestyle changes while others may take longer. However, how you feel is more important than what the labs say. If you feel good, and your labs are elevated, then consider that a win. She says that it’s easy for people to get demoralized but it’s not all about the numbers. Other Factors in EBV If someone is following the protocol and not getting results (either in how they feel or their lab results), then there may be something else at play. Two of the most common culprits are heavy metals and molds. For more on heavy metals, listen to episode 024 and for more on mold listen to episode 006. Mold can really block any efforts to decrease EBV marker levels because it devastates the immune cells and system. In cases where mold or heavy metals are an issue on top of elevated EBV markers, the mold and heavy metals must be addressed first. Negative Reaction to the EBV Protocol Introducing a new protocol of supplements and dietary restrictions can trigger reactions. However, Dr. Hines recommends starting slow with the protocol. If there is a reaction, then you can stop the protocol and add in the different pieces one by one to determine if there is one thing that is causing an issue. She also recommends, as do I, that you always opt for top-quality supplements to minimize the chance that you’ll have a negative reaction. EBV Triggers to Avoid One surprising trigger the Dr. Kines mentioned comes from forest fires. This should be on your radar if you live near an area (like California) where there are a lot of forest fires. When fires burn, they give off a toxin called dioxin which has been shown to reactivate EBV. You can also be exposed to this toxin through cigarette smoke (even second-hand), campfires, fireworks, or the burning of debris. She also mentioned that wifi exposure should be closely monitored. This includes your own router but also the wifi used for smart meters that could be penetrating your walls. Additionally, she cautions against blue light exposure (commonly from TV, computer screens, and smartphones). Even your next vacation could expose you to EBV triggers (listen to the episode to find out how). All of these triggers can also reactivate the Epstein-barr virus. Mystery Solved Let’s take a look at Emily’s labs. Her VCA IgM was negative which made her doctor think the infection was not current, but she had an elevated early antigen. Her doctor completely missed that. While her symptoms weren’t dramatic, she knew something was wrong and the tests proved it. Because of her sensitive stomach, we opted for the protocol that included the amino acids and antioxidants Dr. Kasia talks about in this episode (Lysine, NAC, Selenium and Zinc). She tired Monolaurin but experienced die-off symptoms so we stuck with the original protocol. Happy Ending Emily’s digestive issues subsided in about 3 weeks and her energy started to slowly come back over the next 6 weeks. We retested the markers 12 weeks later and the EA was back to negative. We were also excited to see her VCA IgG went from 400 to 350 and that her autoimmune antibodies went down by 57 points too. She was thrilled with her progress and will continue with a maintenance protocol and self-care protocol. She’ll test the 4 EBV markers annually to ensure she stays on top of it. Eliminating Health Mysteries For Emily we were able to find the clue in her lab work that her doctor had missed. So, they will insert clauses that provide them with an option if the contractor does not perform the work or provide the services within the time frame specified. Also, if the contractor does not use the correct materials or refuses or fails to perform the work as required. No matter what you do not want to face a Termination for Default on a government contract. If you were hiring a contractor to perform work for yourself, you would want to protect your interests also. This is good business sense. Below I have listed the FAR references for termination of default for your reference. FAR References for Terminations for Default You will find the FAR reference for Termination for Default below: • 52.249.8 – Supply and Service, Fixed Price • 52.249.9 – Research and Development, Fixed Price • 52.249.10 – Fixed Price Construction Up to this point we have been providing you an overview of contract terminations. Now let's go more in depth. Starting with the termination for convenience. Termination for Default Earlier, we mentioned that a termination for default means that the government believes that the contractor may have not performed on the contract in accordance with the contract terms. Now, let's look at the steps involved if the government wishes to terminate a contract for default. Memorandum for Record The first step the government will take is to deliberate and decide the most appropriate action to take. The government will not take this action lightly. To do this the government will prepare and review a Memorandum for Record which addresses the factors contain in FAR 52.249-3. Once the factors have been determined and review the government will make a termination decision. What factors will the government review? Well, the government will review and evaluate the following factors: 1. The terms of the contract along with applicable laws and regulations; 2. The specific failure by the contractor on the contract and the excuses for the failure; 3. The availability of the supplies or services from other sources; 4. The urgency of the need for the supplies and/or services. To include the time period required to obtain them from other sources as compared with the time in which delivery could be obtained from the delinquent contractor. 5. The degree of essentiality of the contractor in the Government acquisition program and the effect of a termination for default upon the contractor’s capability as a supplier under other contracts; 6. The effect of a termination for default on the ability of the contractor to liquidate guaranteed loans, progress payments, or advance payments; and/or 7. And any other pertinent facts and circumstances. As you can imagine a termination for default has serious implications not only for the contractor but the government also. Next we will look at those implications. Impacts Both Government and Contractor As you can tell there is a ton of information that the contracting officer will have to gather and deliberate with their legal department. Basically, a termination for default hurts both parties. The government does not get the goods or services that it needs, and the contractor has a blemished record. As you can imagine a termination for default is overwhelming to the contractor. A termination for default results in suspension and debarment or criminal conviction. If you are a construction contractor your surety company will use your bond to find another contractor to perform the contract. That is if the government elects this route. All of this should not take lightly. If you are headed down this road it is imperative that you have a great legal team on your side. In addition, if your contract was terminated for default and you received a termination letter, you may still be entitled to payment for work that was properly performed in agreement with the contract before termination. Terminations - Other Factors Remember in the beginning of this blog we stated that the government may terminate a contract if it feels it is in its best interest to do so. As a result, the government has the right to termination all or only part of a contract to include: • Attempted fraud by contractor. • Contractor failure to meet quality requirements. • The Contractor failure to deliver the supplies or perform the services within the time frame specified in the contract. • Failure by the contractor to perform any other provisions contained in the contract. Will the government just start termination proceedings without my knowledge? No, the government will notify you. Up next, we will talk about the Delinquency Notices. Terminations - Delinquency Notices There are two delinquency notices available for use by the government. The first notice is called a “Cure Notice” and the second notice is called a “Show Cause Notice”. Basically, the FAR states that the government must notify the contractor that they are responsible for a condition that is threatening performance of the contract. See FAR 49.607 for more information on delinquency notices. Next, we will discuss the “Cure Notice”. Terminations - Cure Notice A cure notice is a written document sent to the contractor by the government, notifying the contractor that there is a contract issue that must be resolved. Cure Notices are issued when a contract is in jeopardy of being terminated prior to the delivery date. Before the government can use this notice, they must ensure that there is enough time left on the contract delivery schedule or any extension. Why? The government must provide the contractor with enough time to resolve the issues affecting the contract. This time period is generally 10 days. If the timing is not enough to permit a realistic “cure” period of 10 days or more, the “Cure Notice” cannot be issued. Terminations - Ten Days to Respond So, what happens if there is not 10 days or more available on the contract? The government has the option of issuing another delinquency notice called a “Show Cause Notice”. But first, let’s discuss what a contractor should include in their written response to the government regarding the cure notice. Contractor’s Response to Cure Notice It is imperative that you, the contractor, respond to the cure notice within the time frame specified on the letter. You will want to include the following information in your response to the government. • Discussion of alleged problem to be a condition affecting contract performance. • Any Corrective plan to cure the alleged condition. • Who will bear the cost of the corrective work? • Schedule of performance of corrective work • Impact of corrective work on completion of the contract. • Actions that contractor has taken and will take to reduce the effect of the condition upon completion. • Government’s role in causing the condition. • Any other relevant factors bearing on timely completion of contract. Terminations - Address All Allegations Remember that you only have 10 days to respond to the cure notice. It is important that you address all the allegations contained in the notice and present your side as best as possible. Now, let’s move on to the other delinquency notice, the “Show Cause Notice”. Terminations - Show Cause Notice As stated earlier a “Show Cause Notice” may be issued when the contract delivery schedule is not sufficient to permit a realistic cure period of 10 days or more. This notice will advise the contractor that the government is considering terminating the contract for default. Also, the government has not made a final decision on this matter. The government needs to determine if the contractor failed to perform was from causes beyond its control and without fault or negligence on the contractor’s part. If the contractor fails to present any facts within the 10-day period, the government may consider this an admission that none exist. Do not under any circumstance fail to submit your response in writing to the contracting officer. The government will consider this is an admission of fault. Besides the government may not have all the facts or may have left out important facts in your favor. What should a contractor include in their written response to a “Show Cause Notice”? That is what we will discuss next. Contractor’s Response to Show Cause Notice It is suggested that a contractor address the following when responding to a “Show Cause Notice”: • Discuss your contract performance over the term of the contract. • Make sure you address your control over the work performed on the contract. • State your reasons why you are not at fault or negligent regarding the alleged deficiency. • Present the percentage of cost completion on the contract. • Make sure to address the fact that the project completion will be delayed because of this wrongful termination. • Include the delay to project completion caused by this wrongful termination • Address a possible conversion of Default Termination to a Convenience Termination. Things to Consider When writing your response to either a cure notice or show cause notice it is important to remove the personal feelings from the letter and state the facts as you see them. Now is not the time to let your emotions get the better of you. Make sure that you have someone review the letter being as objective as possible. Remember a Termination for Default has serious consequences not only for you but the government also. A contractor that received a Termination for Default must disclose this fact when bidding any new work. This will impact your past performance rating as well. In addition, the government may hold the contractor responsible for costs if the default is converted to a Convenience Termination, because the contractor may pursue recovery of allowable costs associated with the termination. Is there an Excusable Behavior? If you can show that your failure to perform the contract was excusable, your contract cannot be terminated for default. In order to be excusable, the failure must be beyond your control and not your fault or neglect. Examples of Excusable Behavior The following are examples of excusable failures: • Acts of God • Acts of public enemy • Actos of government • Fires • Floods • Epidemics • Quarantine restrictions • Strikes • Freight embargoes • Unusually severe weather. What Happens if the Default is Excusable? If after the termination you are found not to be at default or the default is excusable, the termination will be treated as one for the convenience of the government. As a result, the government will remove the bad image that the termination for default gave you and you will recoup some of your money back as well. This is great news for you and your business. However, what happens if the buying agency or contracting officer does not agree with you? Or worse, you believe their decision is not correct? Well, the contracting regulations provide contractors with several remedies, all the way from filing a simple protest or dispute to taking the government to court. Summary While no contractor wants to face a termination for default there are things that you can do to protect yourself. Are you a craftsperson or an entrepreneur? This is a personal lifestyle choice - but be clear on which you have chosen. As a craftsperson, you can be perfectly content creating handcrafted products in low volumes and selling them to a niche audience. If this describes you, you can ignore the rest of this post. You just need to cover your material and living expenses in order to continue doing what you love. However, if you are an entrepreneur with a vision of building a scalable venture, you need to understand and adhere to established business principles, such as standard markup pricing. Even if you’re small now, you need to plan for big. If you’re just starting out, you may not realize how much margin you will need to build into your retail price in order to build a viable, long term business. While selling solely through a website is a great first step, most brands need to explore other channels to accelerate their growth. If you get your pricing and margins wrong, you’ll be in for a rude awakening when you start approaching retailers (and you’ll probably end up with a greater appreciation for why so much stuff get’s manufactured overseas!) Combining standard markup pricing with market research. If you’re basing your price strictly on a multiple of your costs -also known as Standard markup pricing you’re starting at the wrong end of the equation. I strongly suggest starting with a target retail price and working backward to see if your product will be viable. You will still need to ensure you are able to generate a sustainable profit margin - thus the need for standard markup pricing - but you’ll be imposing an additional level of discipline by aligning your numbers with the reality of the marketplace. Here is a step by step guide to using the Indie Brand Builder Margin Calculator to determine standard markup pricing: STEP ONE: Research your industry and competitors to determine high and low retail price thresholds You will need to determine how the quality, features and benefits of your product measure up against both direct and indirect competition. By direct competition, I mean any other product that has similar features and is used to solve the same problem or address the same desire of the consumer. By indirect competition, I mean products that aren’t in your category but provide a similar outcome or function for the consumer (ie - a book vs. an e-reader). Make a quick spreadsheet listing each competitor’s name, product features and retail price. STEP TWO: Pick your target product price After doing your research, you should have a reasonable idea of the general price range you expect consumers will be willing to pay - also known as the MSRP (manufacturer's suggested retail price). You will now need to choose a target price that you think represents an optimal balance between generating revenues and generating demand. At this point, the price is simply a hypothesis as you will still need to factor in manufacturing costs Keep a laser Focus on your gross margins. Anything less than 50% - 60% and you may be headed down a slippery slope STEP THREE Plug the numbers into the Pricing and Margins Spreadsheet Calculator Input your target retail price in on the far right to match your figure Next, plug in your target wholesale price at half of your retail price. You will now see your “retailer” margin at 50%. Adjust the freight and duty percentage as required. By default, it is set to 20% - which is a conservatively high ratio (SoYoung's freight and duty is typically around 17% coming from China) . Before making any final decisions, you should validate this by talking to shipping and customs brokers. You can also use this estimator tool. Start playing with your manufacturing cost numbers to see what they should be to get your gross margin to 60% This will allow you to target an acceptable manufacturing cost STEP FOURNegotiate with your factory or suppliers. Now that you have a target cost, you can take this to your factory or use it as an internal target for producing your goods. If you are manufacturing internally, note that this number is just meant to capture your Cost of Goods: that is the cost to produce one unit of product. While it will include labor cost related to manufacturing, it should not include overhead costs associated with running your business such as administration, marketing or selling your products. STEP FIVENegotiate with retailers and distributors. Your target wholesale price is just that: a target. If you are negotiating volume transactions, retailers will typically ask for a discount. When they do, you can simply plug this into your spreadsheet to see how it affects your margins. If you’ve calculated for 60% margins, this will give you some room to maneuver and still make a reasonable profit. Distributors will expect at least a 20%-30% off your wholesale price - though sometimes they will be able to give you an order that can be taken directly to the factory (if applicable) and shipped straight to them, which reduces your risk and cost of carrying the products. Why you need to target a 60% profit margin when pricing products: You will likely have other selling costs such as marketing, paying commissions to sales reps, shipping merchandise and providing samples. Larger retailers will ask for a discount off your wholesale price You need to cover all of your business overhead, including your own salary Your business needs to generate a profit in addition to your salary in order to be viable over the long term. You may have to sell a portion of your inventory at a discount or write it off completely You will need to generate enough cash flow to continue financing the business. Other Factors to Consider Economies of scaleIn the short term, you may be willing to settle for slightly lower than standard markup pricing margins as you build your volume up to a point where you can realize efficiencies in your manufacturing costs. Just be sure these cost projections are based on solid agreements with your suppliers, rather than wishful thinking. Business ModelWhat I’ve proposed here is based on a traditional wholesaling model - but the world is changing quickly. If you are committed to an online-only approach, integrating with your own retail location or going the multi-level marketing route, then these rules may not apply. Other entrepreneurs have been very successful blazing their own trail - but having more margin always allows you more flexibility if things don’t go as planned.
