From a very young age I was interested in today's birthday celebrant, the great Gundula Janowitz, at least partially because of a similarity between her first name and my last name! But even more than that, I was drawn to the ethereal purity of her voice, possessed of an immediately identifiable timbre unlike that of any other singer. From the 1960s through the 1980s, Janowitz was the reigning queen of the jugendlich dramatisch soprano repertoire, excelling in the roles of Mozart, Strauss and Wagner (the so-called wagneriennes blanches heroines), as well as being a Bach singer of the highest order and an exceptional Lieder singer. What is there not to love? Janowitz was also a frequent visitor to the recording studio, and to this day her recordings form a cornerstone of the Deutsche Grammophon catalog. On today's episode, however, I stick firmly to her live and radio recordings, some of them quite rare, of the repertoire for which she was so justly celebrated. We sample live recordings of her Arabella, Fiordiligi, Agathe, and Elisabeth in Tannhäuser, as well as a more surprising Elisabetta, the conflicted queen in Verdi's Don Carlo. Also highlighted is her exceptional and career-defining performance of the role of Leonore in Beethoven's Fidelio, in a rare live 1977 from the Orange Festival. Guest vocalists include Lucia Popp and Franco Corelli. Abstract: Objective: Temporal lobe epilepsy (TLE) is characterized by recurrent seizures generated in the limbic system, particularly in the hippocampus. In TLE, recurrent mossy fiber sprouting from dentate gyrus granule cells (DGCs) creates an aberrant epileptogenic network between DGCs which operates via ectopically expressed GluK2/GluK5-containing kainate receptors (KARs). TLE patients are often resistant to anti-seizure medications and suffer significant comorbidities; hence there is an urgent need for novel therapies. Previously we have shown that GluK2 knockout mice are protected from seizures. This study aims at providing evidence that downregulating KARs in the hippocampus using gene therapy reduces chronic epileptic discharges in TLE. Methods: We combined molecular biology and electrophysiology in rodent models of TLE and in hippocampal slices surgically resected from patients with drug-resistant TLE. Results: Here we confirmed the translational potential of KAR suppression using a non-selective KAR antagonist that markedly attenuated Interictal-like Epileptiform Discharges (IEDs) in TLE patient-derived hippocampal slices. An adeno-associated virus (AAV) serotype-9 vector expressing anti-grik2 miRNA was designed to specifically downregulate GluK2 expression. Direct delivery of AAV9-anti grik2 miRNA into the hippocampus of TLE mice led to a marked reduction in seizure activity. Transduction of TLE patient hippocampal slices reduced levels of GluK2 protein and, most importantly, significantly reduced IEDs. Interpretation: Our gene silencing strategy to knock down aberrant GluK2 expression demonstrates inhibition of chronic seizure in a mouse TLE model and IEDs in cultured slices derived from TLE patients. These results provide proof-of-concept for a gene therapy approach targeting GluK2 KARs for drug-resistant TLE patients. Copy rights belong to original authors. Abstract: Background: The mechanisms in the brain that explain the benefits of adherence to the Mediterranean diet (MeDiAd) for cognition are incompletely understood. Here, we investigated whether fractional anisotropy (FA) in hippocampus-relevant white-matter tracts mediates the association between baseline MeDiAd and verbal episodic memory over four years. Methods: Participants with baseline diffusion-weighted imaging data from the DELCODE cohort study were selected, including healthy older adults with and without subjective cognitive decline and patients with mild cognitive impairment (n = 376; age: 71.47 plus-or-minus sign 6.09 years; 48.7% female). Demographic, MeDiAd, and diffusion data were obtained at baseline. Verbal episodic memory was assessed at baseline and four yearly follow-ups. The association between baseline MeDiAd and verbal episodic memory's mean and rate of change over four years and the mediation of that association by baseline white-matter tracts were tested with latent growth curve modeling. Potential mediators were selected based on the association with hippocampal volume. Results: Baseline MeDiAd was associated with verbal episodic memory four years later (latent intercept; 95% confidence interval, CI [0.01, 0.32]) but not with its rate of change (latent slope) over this period. Only baseline Fornix FA, among four potential mediators (the cingulum ventral, corticospinal tract, and superior longitudinal fasciculus II), mediated this association (latent intercept; 95% CI [0.002, 0.09]). Conclusions: Higher Fornix FA explains the association between higher baseline MeDiAd and better memory four years later in pre-dementia stages. Abstract: Interleukin-6 (IL-6) has been long considered a key player in cancer-associated cachexia. It is believed that sustained elevation of IL-6 production during cancer progression causes brain dysfunctions, which ultimately result in cachexia. However, how peripheral IL-6 influences the brain remains poorly understood. Here we show that neurons in the area postrema (AP), a circumventricular structure in the hindbrain, mediate the function of IL-6 in cancer-associated cachexia in mice. We found that circulating IL-6 can rapidly enter the AP and activate AP neurons. Peripheral tumor, known to increase circulating IL-6, leads to elevated IL-6 and neuronal hyperactivity in the AP, and causes potentiated excitatory synaptic transmission onto AP neurons. Remarkably, neutralization of IL-6 in the brain of tumor-bearing mice with an IL-6 antibody prevents cachexia, reduces the hyperactivity in an AP network, and markedly prolongs lifespan. Furthermore, suppression of Il6ra, the gene encoding IL-6 receptor, specifically in AP neurons with CRISPR/dCas9 interference achieves similar effects. Silencing of Gfral-expressing AP neurons also ameliorates the cancer-associated cachectic phenotypes and AP network hyperactivity. Our study identifies a central mechanism underlying the function of peripheral IL-6, which may serve as a target for treating cancer-associated cachexia. Copy rights belong to original authors. I'm thrilled to be here. Jenny Wheeler: Your latest book, The Liz Taylor Ring, has got a wonderful title for starters, but it's your seventh novel and it's received a great series of accolades. It's been welcomed by many publications people would listen to. It was one of Town and Country's best books for February, the Katie Couric people thought it was one of the best books of 2022 and Pop Sugar's most anticipated reads of 2022. Did that give you a bit of a lift to start? Brenda Janowitz: My goodness yes. It was so incredibly exciting. Certainly by book seven, you think you'd be used to it and everything would be old hat, but it feels like with every book you're starting fresh and you're starting over, so it's always a miracle when anyone likes your book. It feels like, okay, it's not terrible. We've got something here. Jenny Wheeler: I know you've had a lot of success in the past, but would you consider that this was almost a breakthrough book for you? Is it that good? Brenda Janowitz: I do think each book is reaching a little higher for me. Brenda Janowitz is Pop Sugar's book correspondent and the author of six novels. Her latest, The Grace Kelly Dress, tells the story of an heirloom wedding gown and three generations of women whose lives are influenced and changed by the dress. Brenda and I could have talked all day!! Brenda and I discussed — among other things —  her Pinterest board for the cover design, her wedding obsession, how waiting in line at CVS informs her character development, not being so hard on herself, her son who was born at 31 weeks, and her husband’s shirt. Her bio: Brenda’s work has appeared in the “Modern Love” column of The New York Times, The Washington Post, Redbook, The New York Post, Publisher's Weekly, and many other publications. She is a graduate of Cornell University and Hofstra Law School, and worked as a lawyer and federal clerk before becoming a writer. Be sure to listen in on this one!!

On today's episode I talk to writer Jill Eisenstadt. Jill is the author of the novels From Rockaway, Kiss Out and Swell and with her sister, director Debra Eisenstadt, the co- writer and co-producer of the feature film, The Limbo Room. In the 1980s, she was part of the 'Literary Brat Pack' whose members included Bret Easton Ellis, Jay McInerney, and Tama Janowitz. Other work of Jill's has appeared in such publications as The New York Times, New York Magazine, Vogue, Elle, The Boston Review, as well as many others. Jill is also the recipient of a Columbia University Writing Fellowship, a National Foundation for Advancement in the Arts Teacher Award and a National Endowment for the Arts Fiction Grant. Nach 12-monatiger Einnahme einer hochaktiven antiretroviralen Therapie (HAART) kommt es in ca. 45% der Fälle zum Auftreten des HAART-assoziierten Lipodystrophiesyndroms (LDS) (23). Das LDS beinhaltet u.a. Dyslipoproteinämien mit Gesamtcholesterinwerten von oftmals >300 mg/dl. Am häufigsten werden sie unter einer HAART beobachtet, die Proteaseinhibitoren (PI) beinhalten. Die Hypercholesterinämie gilt als etablierter Risikofaktor für die Entstehung einer Atherosklerose. Gegenstand der aktuellen wissenschaftlichen Diskussion ist die Frage, ob auch die HAART-induzierte Hypercholesterinämie atherogen wirkt. Mehrere Fallberichte und prospektive Studien über kardiovaskuläre Ereignisse bei HIV-infizierten Patienten unter HAART sind bereits veröffentlicht. Ein direkter Nachweis atherosklerotischer Schädigungen der Koronarien erfolgte bisher jedoch nicht. In der vorliegenden Arbeit wurde untersucht, ob eine HAART-induzierte Hypercholesterinämie zu morphologischen und funktionellen Veränderungen an den Herzkranzgefäßen führt. 1.1. Patienten und Methoden Patienten: Gruppe A umfasste 20 HIV-infizierte Patienten mit PI-haltiger HAART und HAART-induzierter Hypercholesterinämie >300mg/dl seit mindestens 24 Monaten. 17/20 Patienten wurden auf morphologische und 12/20 Patienten wurden initial und 5/20 nach 12 Monaten noch einmal auf funktionelle Veränderungen der Koronarien untersucht. Die Kontrollgruppe B umfasste 8 HIV-infizierte Patienten ohne HAART mit Gesamtcholesterinwerten 0,05), wohl aber nach Adenosinbelastung signifikant niedriger als bei Gruppe B (66±17 versus 35±11, p0,05). Nach Adenosinbelastung stieg die Myokarddurchblutung aber signifikant weniger stark an als in Gruppe B (1,58±0,44 versus 2,93±0,68, p
