In this episode, Ayesha spoke with Amit Etkin, Founder, President and CEO of Alto Neuroscience, a company developing precision treatments for neuropsychiatric conditions. Alto Neuroscience is focused on designing candidates that target core brain processes in patient populations not adequately treated by currently available medications. Alto is currently pursuing candidates in major depressive disorder (MDD), bipolar depression and schizophrenia. Mental health disorders are a leading cause of disability globally. In the US alone, 1 in 5 people are living with a mental health disorder, with an estimated $280 billion spent on mental health services in the country in 2020. Despite significant need, the current standard of care consists of treatments that are ineffective for many patients. Dr. Amit Etkin founded Alto in 2019 and is the president, CEO and serves as chair of the company's board of directors. Prior to founding Alto, Dr. Etkin was a tenured professor of psychiatry and behavioral sciences at Stanford University, where he held multiple leading roles. His research contributions during his tenure at Stanford led to Dr. Etkin being awarded the National Institutes of Health (NIH) Director's Pioneer Award, the most competitive and prestigious NIH grant, and the first in history to be awarded in clinical psychiatry. Bu bölümde ise kılavuzun 3. bölümünü (KOAH'ın Önlenmesi ve Yönetimi) ana hatlarıyla özetlemeye çalışacağız. GİRİŞ KOAH yönetimindeki genel amaç; semptomları, alevlenmeyi ve riskleri azaltmaktır. Stabil KOAH tedavisindeki amaçlar içinde; semptomları tedavi etmek, egzersiz toleransını arttırmak, sağlık durumunu geliştirmek, hastalık gidişatı önlemek, alevlenmeleri önlemek ve tedavi etmek ve mortaliteyi azaltmak yer alır. KOAH hastalarında tedaviyi yönlendirmek amacıyla havayolu obstrüksiyonunun ciddiyeti, semptomlar, alevlenme geçmişi, risk faktörlerine maruz kalma ve komorbiditeler değerlendirilmelidir. Bu bölüm KOAH'lı hastaların yönetiminden ve önleme stratejilerinden bahsedeceğiz. Sigarayı bırakma, KOAH'ın doğal seyrini etkileme konusunda en büyük kapasiteye sahiptir. Günlük semptomları iyileştirir ve alevlenme sıklığını azaltır. Klinisyenler mümkünse potansiyel tahriş edici maddelere sürekli maruz kalmaktan kaçınmaları konusunda hastalara tavsiyede bulunmalıdır (Kanıt D). Aşılanma İnfluenza aşısı, KOAH'lılarda ciddi hastalanmayı (hastane yatışı gerektiren durumlar gibi) ve ölümü azaltır. Sadece birkaç çalışma KOAH alevlenmelerinde influenza aşısını değerlendirmiş ve kişi başına düşen toplam alevlenme sayısında anlamlı azalma olduğunu göstermiştir. İnfluenza aşısı KOAH'lı hastalara önerilmektedir (Kanıt B). Pnömokok aşıları, pnömokok konjuge aşısı ( PCV20 veya PCV15) ve pnömokok polisakkarit aşısı (PPSV23) 65 yaş üzeri bireyler için onaylanmıştır. Ayrıca 19-64 yaş arasında kronik akciğer hastalığı olan, organ nakilli, sigara içen bireyler için de önerilmektedir. Aşılanma konusunda bir doz PCV15 sonrasında PPSV23 ya da sadece bir doz PCV20 önerilmektedir (Kanıt B). Pnömokok aşısı toplum kökenli pnömoni riskini ve KOAH alevlenme sıklığını azaltmaktadır (Kanıt B). Kronik kalp/akciğer hastalığı, immün yetmezlikli hastalar, bakımevi hastaları, ciddi RSV enfeksiyonu geçirme riskine sahiptir. CDC'ye göre her yıl RSV kaynaklı 60.000-160.000 hastane yatışı olmakta ve 6.000-10.000 ölüm görülmektedir. Bu yüzden RSV aşısı 60 yaş üstü kişilerde veya kronik kalp/akciğer hastalığı olan kişilerde önerilmektedir (Kanıt A). Ergenlik çağında Tdap aşısı olmayan KOAH'lılar için Tdap aşısını (Kanıt B) ve 50 yaşın üzerindeki KOAH'lı kişiler için zonaya karşı koruma için Zoster aşısını (Kanıt B) önermektedir. COVİD-19 aşısı tüm KOAH'llı hastalar için önerilmektedir (Kanıt B). Stabil KOAH'ta Farmakolojik Tedavi Semptomların ve alevlenme sıklığının ciddiyetine göre tedavi planı belirlenir. Tedavi semptomlara göre arttırılabilir veya azaltılabilir. Başlangıçtaki farmakoterapi hastanın hangi GOLD grubunda olduğuyla başlar:

Her ne kadar deprem bölgesi olsak, konu ile ilgili acı tecrübelerimiz olsa da; son deprem bize bir defa daha afet yönetimi ile ilgili sınıfta kaldığımızı gösterdi. İlk 72 saat eğitimlerini bangır bangır veren kurumlar maalesef olay yerine ertesi gün ulaşabildi. Kar yağışı ve kazalar da eklenince kara yolu ile ulaşım neredeyse imkansız oldu. Hava alanlarında pistlerin hasar görmesi de cabası… Afet yönetimini gerçekten biliyor muyuz? Koordinasyon, güvenlik çemberi, lojistik, depolama.. Afet planları sadece kâğıt üzerinde mi? Arama kurtarma, sağlık ve lojistik halen bir araya getirilemedi. Sağlık ekipleri olay yerine gittiğinde enkaza girecek gerekli ekipmanı ve eğitimi yok, bu depremde gördüğümüz gibi bir çok enkaz başında arama kurtarma ekibi varken sağlıkçı yok birçoğunda ise tam tersi durum. Hızlı bir şekilde tüm Türkiye'de yardım amaçlı malzemeler toplandı, ancak lojistik planlama zayıf olduğu ve kriz merkezi kurulumu geç gerçekleştiği için ihtiyaç olan malzemeler ya yollarda ya da ihtiyaç olan yere ulaşmamış durumda kaldı. Tırlar meydanlara malzemeleri boşaltı döndü, güvenlik endişesi ile çoğu olay yerine varmadan yükünü bıraktı. Haberleşme bambaşka bir problem oldu. 112 komuta merkezleri de hasar aldığı için kullanılamaz hale geldi. Çağrılar başka illere yönlendirildi. Koordinasyon sağlanmadığı için mobil komuta merkezi kurulmuş olsa dahi bir çok ekip ile iletişim kurulamadı. Afet ve Yönetimi Halk dilinde “kıran” olarak da tabir edilen afet, insanlar için fiziksel, ekonomik ve sosyal kayıplara neden olan, normal yaşamı ve insan faaliyetlerini durdurarak veya kesintiye uğratarak, toplulukları olumsuz etkileyen doğal, teknolojik veya insan kökenli olaylar olarak tanımlanmaktadır. İnsanların yaşadıkları çevrede meydana gelen doğal olaylardan haberdar olmaları, bunları nedenlerine kadar ayrıntısı ile tanımaları ve bu olayların tekrarı durumunda bunlardan hiç etkilenmeme veya en az oranda etkilenmelerine olanak tanıyan çalışmaların tümüne ise “Afet Yönetimi” denilmektedir .1 Afet Yönetimi Aşamalar1 Günümüz afet yönetimi kavramında; kayıp ve zararların azaltılması, hazırlık, tahmin ve erken uyarı, afetleri anlamak gibi afet öncesi korumaya yönelik çalışmalar “Risk Yönetimi”; etki analizi, müdahale, iyileştirme, yeniden yapılanma gibi afet sonrası çalışmalar ise “Kriz Yönetimi” olarak kabul edilmektedir.2 3 Etkin bir afet yönetimi; afet öncesi, afet sırası ve afet sonrası ihtiyaç duyulan tüm çalışmaları kapsamaktadır.4,5 Afet Yönetiminin Aşamaları En basit şekilde sınıflandıracak olursak, afet yönetimi; afet öncesi, afet esnası ve afet sonrası olarak 3 kısımda ele alınabilir. yukarıda gördüğümüz şemadan da anlayacağımız gibi hangi sınıflama ya da aşamayı kullanırsak kullanalım, afet söz konusu olduğu zaman tüm basamaklar iç içe ve koordine bir şekilde olmalı, birbirini takip etmeli. Ancak bu şekilde etkili bir afet yönetiminden bahsedebiliriz. Önleme ve Zarar Azaltma Zarar azaltma afet yönetiminin en önemli ve maalesef en göz ardı edilen bileşenidir. Abstract: The striatum is the primary input nucleus of the basal ganglia, widely studied for its complex roles in health and disease. Functional magnetic resonance imaging (fMRI) studies are essential for discerning striatal function, however the relationship between neuronal and hemodynamic activity, critical for interpreting fMRI signals, has not been rigorously examined in striatum. We find that optogenetic stimulation of striatal neurons or afferents evokes negative striatal fMRI responses in rats that can occur despite broad increases in local neuronal activity. Intra-striatal pharmacological manipulations suggest that opioidergic, but not dopaminergic transmission contributes to negative striatal fMRI signals (the latter instead associated with positive signals). Striatal neuronal activity peaks are also associated with negative hemodynamic signals in behaving rats. Negative fMRI responses are observed in human striatum under conditions of anticipated neuronal activity increases. Our results prompt consideration of local cellular and neurochemical environments along with neuronal activity in fMRI signal interpretation. Copy rights belong to original authors. C Abstract: Background: Current clinical assessments of Posttraumatic stress disorder (PTSD) rely solely on subjective symptoms and experiences reported by the patient, rather than objective biomarkers of the illness. Recent advances in data-driven computational approaches have been helpful in devising tools to objectively diagnose psychiatric disorders. Here we aimed to classify individuals with PTSD versus controls using heterogeneous brain datasets from the ENIGMA-PGC PTSD Working group. Methods: We analyzed brain MRI data from 3,527 structural-MRI; 2,502 resting state-fMRI; and 1,953 diffusion-MRI. First, we identified the brain features that best distinguish individuals with PTSD from controls (TEHC and HC) using traditional machine learning methods. Second, we assessed the utility of the denoising variational autoencoder (DVAE) and evaluated its classification performance. Third, we assessed the generalizability and reproducibility of both models using leave-one-site-out cross-validation procedure for each modality. Results: We found lower performance in classifying PTSD vs. controls with data from over 20 sites (60% test AUC for s-MRI, 59% for rs-fMRI and 56% for d-MRI), as compared to other studies run on single-site data. The performance increased when classifying PTSD from HC without trauma history across all three modalities (75% AUC). The classification performance remained intact when applying the DVAE framework, which reduced the number of features. Finally, we found that the DVAE framework achieved better generalization to unseen datasets compared with the traditional machine learning frameworks, albeit performance was slightly above chance. Conclusion: Our findings highlight the promise offered by machine learning methods for the diagnosis of patients with PTSD. The utility of brain biomarkers across three MRI modalities and the contribution of DVAE models for improving generalizability offers new insights into neural mechanisms involved in PTSD. Dr. Amit Etkin is pioneering a revolution in the way mental health medications are designed, developed, prescribed, and brought to market. Precision psychiatry is the future of mental health treatment and Dr. Etkin is leading the way with his drug development platform, Alto Neuroscience. In this episode, Dr. Etkin explains why psychiatric drugs have failed, what he is doing about it and why it will work. Prior to Alto Neuroscience, Dr. Etkin was a tenured professor in the Department of Psychiatry and Behavioral Sciences at Stanford University and a member of the Wu Tsai Neurosciences Institute. Dr. Etkin has received multiple awards, most notably the NIH Director's Pioneer Award in 2017. He is trained as both a psychiatrist and neuroscientist. The overarching aim of his work in this field for the last 10 years is to understand the neural basis of emotional disorders and their treatment, and to leverage this knowledge to better understand how the brain works and to develop novel treatment interventions. 27.04.2020- Çağhan Kızıl: “Dünyada neredeyse her yerde salgın devam ediyor. Bazı ülkelerde küçülmeye gitmiş olsa bile sokağa çıkma yasakları devam ediyor. Ne zaman normale döneceğimizin yanıtı, normalin ne olduğunu tanımlamakla başlıyor ve bu da süreci nasıl işleteceğimiz ile ilgili. Hala artan bir salgında ölüm oranlarının bu şekilde artması, bir süre daha devam edeceğini gösteriyor olabilir. Sadece İstanbul'da değil, başka şehirlerde de ortalamanın üzerinde ölüm oranı var ve bunların bilimsel açıklaması yapılmadı. Artması gereken testlere rağmen test sayıları azalıyor. Etkin test sayısını azalttığınızda vaka sayısı da düşecektir. Salgının azalması kavramı, bir kişinin kaç kişiye enfeksiyon yaydığı ile ilgili bir endeks. Türkiye'de salgın azalmıyor.
