In this episode I'm sharing my recent lab test, 23 lab tests in menopause, to be exact. I'll share why so many, how often, what I'll learn from them. If you're at a loss for what to order or really, what you need and want a doctor to run this will help. Connecting the dots is not always easy! Though I ran almost all of these as a part of writing You Still Got It, Girl! Almost a decade ago, by ordering self-directed labs and doing the research to interpret them and take actions to either follow up with a physician or to consider dietary, exercise, and natural solutions that would improve results, I recommend you utilize a physician who's willing to and not threatened by your desire to learn more about the what, why and when for these tests. You know how you feel and what's right for you. Hopefully you're not settling for less than feeling great. Signs that you're not doing okay, in spite of your efforts include unexplained weight gain, inability to lose it, feeling lethargic/tired either on a regular basis or after exercise. Add to that insomnia, dizziness, brain fog, gut issues, and so many more. Having a basic understanding of the tests, that often are not ordered by Western trained docs, and why you want them, may help you be a detective and an advocate for your own health. Recent Lab Tests in Menopause Fasting Glucose Fasting levels above 125 are indicative of diabetes. Those 100-125 indicate prediabetes. Knowing this is helpful for monitoring your habits. A continuous blood glucose monitor can be a fantastic tool for you. https://www.flippingfifty.com/myglucose Fasting Insulin Metabolism issues or weight loss resistance are good reasons to check fasting insulin (beyond just fasting glucose). Insulin resistance can promote additional weight gain. A1C This number can indicate how well controlled sugar levels have been over the past 3 months and also can indicate a prediabetic or diabetic condition. Serum Ferritin Reliable indicator of body's iron stores. Low or very high ferritin may have a condition that results in fatigue. Vitamin D 25-Hydroxy This is important for energy and gaining muscle, particularly fast twitch muscle, as well as energy. Falling into “norms” is under optimal levels so this one can be important for women in midlife and beyond to know. Uric acid Optimal levels may support antioxidant activity but too high can be linked to oxidative stress. Vitamin A Concentrations of uric acid and vitamin A are closely associated. This test may be used to indicate a reason why uric acid levels are high. CRP A major marker of inflammation. Risk of disease increases significantly with high CRP. A Fasting Lipid Panel Looking at cholesterol, LDL, HDL and the total/HDL ratio can tell you what's happening in your body. It might be genetic, lifestyle or also occurring during menopause as the body generates more cholesterol to attempt to produce hormones. Free Testosterone This is the testosterone immediately available. Can support weight loss and improve memory and strength. Testosterone Elevated testosterone can lead to hair growth, acne, moodiness. But too low can lead to difficulty gaining muscle and bone density. DHEA-S Your precursor to the sex hormones and also an indicator of adrenal function. Variety of Lab Tests in Menopause Estradiol Check for optimal levels for some women, confirming they're in menopause and for others like me doing hormone replacement, that hormone levels are optimized. Estrone The estrogen product after menopause. Progesterone Can lower post menopause and make sleep and relaxing harder. Thyroid Hormone Tests TSH Released by your brain to signal your thyroid gland. It can be used to monitor dosing of your thyroid medication or as a screening test for the presence of thyroid disease. Free T4 The thyroid makes predominately T4. Free T4 is what your body uses to make the active thyroid hormone called free T3. These are advanced biomarker tests, not routinely done, but very helpful to determine if the thyroid can be affecting how you feel. Free T3 Symptoms (constipation, weight gain, tiredness, low libido) of low thyroid may be missed (passed off as normal or something else) unless the active thyroid hormone (free T3) is measured. High levels may indicate hyperthyroidism. Symptoms of hyperthyroidism include fatigue, anxiety, sudden weight loss, rapid heartbeat, irritability, more frequent bowel movements, difficulty sleeping, sweating and tremors. Free T3 is an advanced screening biomarker that helps screen routinely missed thyroid issues. TPA (Thyroid Peroxidase Antibodies) Thyroid disease has become very common and we can often pick up inflammation in the thyroid years before the thyroid fails. This is important because natural interventions may help reverse the inflammation of the thyroid gland. TA (Thyroglobulin Antibodies) This test can be positive in people with inflammation of the thyroid gland, and it can be seen years before complete thyroid failure – giving you time to take action! Reverse T 3 Reverse T3 is the storage form of the active thyroid hormone. Many conditions can cause you to store thyroid instead of use it as active free T3. If your reverse T3 is high then you may need to address the reasons why. Broader Selection of Lab Tests in Menopause **FSH (**Follicle-stimulating hormone) Levels of this hormone indicate phases of hormones including puberty, reproduction and menopause. Levels rise significantly after menopause. ****This test can be helpful for someone who isn't sure what's happening or wants to confirm levels are dropping. **SHBG **(**Sex Hormone Binding Globulin) SHBG is used to keep excessive free estrogen out of the blood stream, looking at these levels can help identify if hormone control health habits can improve your overall health. Sex hormone binding globulin is a protein mainly produced by the liver, and studies show that diets high in simple sugars cause the liver to produce less SHBG. Low levels of SHBG are seen in obesity, prediabetes, insulin resistance and type 2 diabetes. Other Tests Total Cortisol Early morning cortisol screen for adrenal gland function. 4 Sample Saliva Test Measures throughout the day to get a more accurate picture of your stress hormone pattern. ACTH Stimulation Test Administered by a physician to test response Omega 3 and 3/6 Index For muscle and joint aches and pains, and brain health, knowing your omega 3 and 6 index can help you improve overall health and decrease inflammation. Food Sensitivity Testing Depending on which level of comprehensive test you do, you can test for 90 common food sensitivities (or more) and learn what is low, moderate, and highly sensitive for you. Avoiding moderate and higher food sensitivities for 3 months while also taking extra care of your gut can help reduce symptoms of gas, bloating, or other digestive issues stemming from leaky gut situation. Adding probiotics, glutamine, oregano oil and foods known to support gut bacteria can all help improve the gut before attempting to reintroduce. GI-Map For gut issues that aren't resolved with dietary changes, scores will tell you your gut bacteria levels and which may be out of balance so you can choose very specific probiotics to help rebalance, as you avoid foods that show moderate and high on your sensitivity results. Today we're going to talk about how to protect your kidneys. The kidneys can take a beating and can even regenerate. They are composed of roughly one million tiny filters, which filter your blood. But as you age, you start losing these filters—so it becomes even more important to start eating better and decrease exposure to toxins. The two worst things for your kidneys are sugar and oxalates. It's crucial to stop consuming sugar and high-carb foods and to start limiting your exposure to high-oxalate foods. However, consuming cheese or dairy with foods that are high in oxalates, but are otherwise healthy, can help counter kidney damage from oxalates. People with high uric acid levels have an increased risk of dementia and Alzheimer's. There's a relationship between these diseases and uric acid. Uric acid directly leads to insulin resistance. [19:30] Your Body Is Retaining Uric Acid As A Survival Mechanism We tend to retain uric acid. Our uric acid levels can be higher as a survival mechanism, thanks to our primate ancestors. We don't hibernate for six months. Therefore, we don't need to be storing this fat for the winter that never comes. 88% of American adults are metabolically unhealthy. Getting Covid is associated with a 40 to 60% increased risk of type two diabetes. Individuals with higher uric acid levels have a higher risk of dying from Covid. [25:10] Your Body Can't Handle More Than Five Grams of Fructose At A Time Your body can handle some fructose, about five grams at a time. When your body is presented with high levels of fructose, it overwhelms what the body can do. When fructose is delivered in the context of fiber, it slows the absorption. So, it's better to eat an apple than drink apple juice. Vitamin C aids in uric acid excretion. The most common way to overdo fructose consumption is by drinking fruit juice. A 12-ounce glass of orange juice might have 30 grams of sugar. There's no way the body will be able to deal with that appropriately. [35:30] Tart Cherries As A Remedy For Gout and Lowering Uric Acid Levels Tart cherries have been researched for decades. Originally, tart cherries have been looked upon as a remedy for gout. It has been noted that people who ate cherries would have fewer gout flare-ups. Tart cherries are pretty darn effective in lowering uric acid levels. [36:40] The Health Benefits of Drinking Coffee Drinking coffee is associated with a dramatically lowered risk of Alzheimer's. Coffee is also associated with lower uric acid levels. Plus, coffee reduces inflammation, upregulates antioxidant function, and improves detoxification in the body. Also, coffee helps us grow new brain cells, no matter our age. [41:45] The Relationship Between Animal-Based Protein and The Role of Uric Acid Fiber is so essential for human health. Most of Dr. Perlmutter's food is plant-based. Regarding our uric acid story, certain meats or animal products are worse than others. Organ meats, sardines, anchovies, and scallops can raise your uric acid levels. The most significant issues for uric acid are fructose, purines, and alcohol. [48:20] Should I Follow A Ketogenic Lifestyle If I Have Gout Attacks? The most significant player in gout is fructose. When you research how to fix gout, there will be no talk about removing sugar or fructose from your diet. The human requirement for sugar in our diets is zero grams per day. We subsidize the growth of corn from which high fructose corn syrup comes from. We are supporting the diseases that are sucking up our healthcare resources. [01:00] About Dr. Johnson and The Inspiration Behind His Research Johnson started studying high blood pressure because it's such a problem in our country. High blood pressure has something to do with the kidneys and salt. A substance called uric acid seems to be important in what causes high blood pressure. As he studied uric acid, he started to realize it was more than just high blood pressure and gout. Uric acid is actually involved in obesity, metabolic syndrome, and diabetes. Eventually, he found that fructose led to an increase in uric acid. [04:20] What Is The Connection Between High Uric Acid and Inflammation? Our body is made up of protein, carbohydrates, fat, bone, etc. Uric acid is considered a waste product. When we eat meats, drink beer, and eat sugar, these foods can generate uric acid. Uric acid is what is excreted from the body. If uric acid is built up, it can lead to inflammation and gout. 9 million people in the United States suffer from gout. A lot of people have low-grade inflammation in their blood. This inflammation can accelerate diseases like atherosclerosis and heart disease. [09:10] Why Is Your Body Producing Excess Uric Acid? If your uric acid is very high, it increases your risk for mortality. Plus, uric acid is being linked to cancer. Uric acid is involved in the process to make fat. When you eat enough fructose, it triggers uric acid. The body switches from making a lot of ATP to storing the energy in fat. Uric acid is actually important in survival. Humans are very sensitive to fructose. Fructose is in table sugar and high fructose table sugar. [24:20] Why The Keto Lifestyle Is So Powerful Our bodies can make fructose. Potatoes and rice do not contain fructose, but they contain what we call high glycemic carbs, it triggers this response in your body to make fructose. You will start making fructose in response to these carbs. That's why carbs are driving obesity dramatically. The keto diet is powerful because it blocks these two major food groups: fructose and high glycemic carbs. [29:30] The Benefits You Get From Exercise When you exercise, you do things that help the mitochondria regenerate. If the mitochondria regenerate, they can go back to normal. When the function of the mitochondria improves, your energy improves. You can diet all you want, but if your mitochondria are bad, you will also go back to your original weight. When you're young and gain weight, you can lose it much easier. [38:55] One of the Many Benefits of A Ketogenic Lifestyle When you eat sugar, you have to absorb the sugar. The ability to absorb sugar varies from person to person. If you're eating sugar every day, you're going to absorb the sugar very fast. When you put an animal on a high sugar diet, it will learn how to absorb it fast and absorb it very efficiently. If you don't eat sugar, those transport systems to transport the sugar into the body get turned off. [43:35] The Importance of Oxygen For Creating Energy When you make ATP in the mitochondria, you use oxygen. One of the main reasons we need oxygen is for our mitochondria to make the energy we need. If we don't have enough oxygen, we can't make energy very well. Our body likes to use glucose as fuel. Insulin resistance reduces the amount of glucose that gets into our muscles, liver, and fat. High uric acid levels are associated with an increased risk of death from all-cause mortality, cardiovascular death, and stroke. Additionally, it can increase the risk of erectile dysfunction and lead to the formation of uric acid crystals in the blood vessels and joints. Fortunately, there are ways to reduce uric acid levels and improve health. The first step is to identify and avoid hidden sources of fructose. Are we eating too much or are we eating poorly? Fatty liver, a preventable diseaseIf the liver receives too much fructose it cannot handle its excess but how much is too much?. The liver turns sugar into fat and alcohol into fat. 45% of Americans have fatty liver disease. Eating cereal, juices, at breakfast injures the liver, as it takes the excess and converts it into fat.The Latino population has a higher risk of consuming sugar, being more vulnerable, becoming insulin resistant and developing Diabetes 2.We will share with Dr. Lustig graduated from MIT in 1976, and received his M.D. from Cornell University Medical College in 1980. The food industry sells juice as fruit but they are not the same. Fruit juice is harmful because they are processed products, which lose their nutritional value.Sugar is not dangerous because of its calories or because it is fattening. It is not a food. When consumed in excess, it becomes an addictive toxin. Become your own health expert:To be aware of what we should do to take responsibility for our health it is important to take into account the following tests:- Abdominal circumference: it should be less than 101 in men and 88 in women. - ALT test, liver function test, the maximum limit should be 25 but it has gone from 25 to 40, since almost everyone has a fatty liver. - Uric acid test: indirect measure of protein and sugar intake. - Triglycerides in blood: They are a manifestation of the sugar in the organism. They indicate what the liver is doing in the body. It is a way of testing insulin resistance. - Fasting insulin: Insulin levels is not related to obesity, but to metabolic health, which is much more important. It is the most important test to doEating real food is what the population should be induced to eat.Two conditions that real food must have: protect the liver and feed the gut. This means having a diet low in sugar and high in fiber. The gut must be a barrier. When it becomes full of toxins, it becomes permeable, food is not processed well and passes into the bloodstream. After reading many studies and papers on Uric acid and gout it is clear most so called research, or at least their pub med versions are mostly a market strategies of pharmaceutical companies. This is no different. This doesn't mean such publications are 'wrong' rather they are omitting large bodies of research that would complicate their message. By omitting vital data simplifies their story, their explanation with the objective to have you agree with their conclusion.
When I was at medical school the subject of Uric acid levels was typically paired with gout, high purine levels in the diet and one of the most ubiquitous drugs, allopurinol, which is used to reduce uric acid levels by inhibiting xanthine oxidase, preventing crystals that can lodge in our joints causing pain.But there is a lot more to do with uric acid than just gout as you'll hear on today's podcast. In fact there's so much more, I'm surprised I haven't come across more on this subject in the past.Dr. Perlmutter is a Board-Certified Neurologist and six-time New York Times bestselling author. He serves on the Board of Directors and is a Fellow of the American College of Nutrition and as a member of the Editorial Board for the Journal of Alzheimer's Disease. People with high uric acid levels have an increased risk of dementia and Alzheimer's. There's a relationship between these diseases and uric acid. Uric acid directly leads to insulin resistance. [19:30] Your Body Is Retaining Uric Acid As A Survival Mechanism We tend to retain uric acid. Our uric acid levels can be higher as a survival mechanism, thanks to our primate ancestors. We don't hibernate for six months. Therefore, we don't need to be storing this fat for the winter that never comes. 88% of American adults are metabolically unhealthy. Getting Covid is associated with a 40 to 60% increased risk of type two diabetes. Individuals with higher uric acid levels have a higher risk of dying from Covid. [25:10] Your Body Can't Handle More Than Five Grams of Fructose At A Time Your body can handle some fructose, about five grams at a time. When your body is presented with high levels of fructose, it overwhelms what the body can do. When fructose is delivered in the context of fiber, it slows the absorption. So, it's better to eat an apple than drink apple juice. Vitamin C aids in uric acid excretion. The most common way to overdo fructose consumption is by drinking fruit juice. A 12-ounce glass of orange juice might have 30 grams of sugar. There's no way the body will be able to deal with that appropriately. [35:30] Tart Cherries As A Remedy For Gout and Lowering Uric Acid Levels Tart cherries have been researched for decades. Originally, tart cherries have been looked upon as a remedy for gout. It has been noted that people who ate cherries would have fewer gout flare-ups. Tart cherries are pretty darn effective in lowering uric acid levels. [36:40] The Health Benefits of Drinking Coffee Drinking coffee is associated with a dramatically lowered risk of Alzheimer's. Coffee is also associated with lower uric acid levels. Plus, coffee reduces inflammation, upregulates antioxidant function, and improves detoxification in the body. Also, coffee helps us grow new brain cells, no matter our age. [41:45] The Relationship Between Animal-Based Protein and The Role of Uric Acid Fiber is so essential for human health. Most of Dr. Perlmutter's food is plant-based. Regarding our uric acid story, certain meats or animal products are worse than others. Organ meats, sardines, anchovies, and scallops can raise your uric acid levels. The most significant issues for uric acid are fructose, purines, and alcohol. [48:20] Should I Follow A Ketogenic Lifestyle If I Have Gout Attacks? The most significant player in gout is fructose. When you research how to fix gout, there will be no talk about removing sugar or fructose from your diet. The human requirement for sugar in our diets is zero grams per day. We subsidize the growth of corn from which high fructose corn syrup comes from. We are supporting the diseases that are sucking up our healthcare resources. Diseases are significant money makers for the healthcare industry. It's time we start thinking about prevention. [52:25] What Role Does Mental-Emotional Stress Play With Uric Acid Levels? When we are experiencing stress, our ability to make rational, forward-thinking decisions is compromised. When your decision-making is compromised, you won't be making good lifestyle choices. Therefore, under stress, you will eat more foods that have sugar. Eventually, you will raise uric acid levels and feed the flame of poor metabolic health.
