In this episode we will examine the various pathologies related to psychosis and neurosis in mental health that clinicians can see within practice. We also examine some of the key aspects of these pathologies so that clinicians can construct both meaningful and targeted treatment to this extremely difficult cohort of patients. To do this am am joined by Anna Basquil, a special mental health practitioner who joined the ambulance service in January 2020, seconded by her mental health trust to provide and help embed the 'dual assessment model' on patients. She works on the 'Mental Health Joint Response Unit' together with a Paramedic. Prior to this Anna was working in an 'early intervention service' with patients experiencing their first episode of psychosis. Anna worked in patients early years of diagnosis to help them both integrate and cope with their symptoms to help normalise and stabilise their lives. In the interview we discuss: Some of the fundamental differences between psychoses and neuroses. The demographics of each pathology Deep dive into Psychoses – definition, types, signs and symptoms. Examine schizophrenia as the most common type of psychosis. Brief look at prodromal phase, acute phase and recovery phase of psychotic episodes. Wat een belachelijk fijne, gezellige vibe heeft Shaina over zich heen. Als we samen in onze hoodies op de bank ploffen vind ik het ook heel moeilijk te bevatten dat ditzelfde meisje al vanaf jongs af aan stemmen hoort en schimmen ziet en behandeld wordt voor doodenge psychoses. Haar verhaal raakt me diep, het gebeurt niet vaak dat je zulke interessante mensen tegenkomt. Gelukkig gaat het goed met Shaina en heeft ze van haar zware periode een energiebron gemaakt waar ze nu anderen mee helpt. We hebben elkaar helemaal gevonden, het voelt alsof ik haar al jaren ken.

I will begin my discussion of schizophrenia with a statement from my book "Psycho'therapy' and the Stories We Live By: "One of the saddest expressions of a ruined identity are the words 'I am a schizophrenic.' To have one's total identity conflated with a fictitious disease that a person believes is incurable is tantamount to tragedy." (P.97) I will discuss the nature of the so-called symptoms of schizophrenia and then deconstruct the concept of the "psychoses" (defined as having lost touch with 'reality") of which schizophrenia is considered a prime example.

David Shannahoff-Khalsa is the Director of The Research Group for Mind-Body Dynamics at UCSD’s BioCircuits Institute, and a member of the UCSD Center for Integrative Medicine. Prior to coming to UCSD in 1994 he spent 23 years at the Salk Institute for Biological Studies in La Jolla, first working on the building blocks of the genetic code, and then pioneering novel studies in the neurosciences. He has discovered a novel step in the evolution of the nervous system that gives new insights to mind-body (psychophysiological) states and how to control these states. He has published widely in scientific journals and conducted three clinical trials using Kundalini Yoga meditation techniques for treating obsessive compulsive disorders and OC spectrum disorders, and pioneered the use of whole-head magnetoencephalography brain imaging to study these and other yogic meditation techniques. Background: The aim was to examine the agreement and differences between ICD-10 and DSM-IV in the classification of functional psychoses. Sampling and Methods: In a sample of 218 first-hospitalised patients, ICD-10 diagnoses were compared with DSM-IV diagnoses. Functional psychoses of both diagnostic systems were classified into the four diagnostic groups schizophrenia, transient/episodic psychoses, delusional disorders and affective disorders. Based on information from a 15-year follow-up, it was examined which course is associated with each diagnostic group. Results: Although in ICD-10 there was a higher frequency of schizophrenia and a lower one of affective disorders, a high agreement between ICD-10 and DSM-IV (kappa value of 0.82) was found. In both diagnostic systems, transient/episodic psychoses and affective disorders were mainly associated with a non-chronic course and schizophrenia was mainly associated with a chronic one. Nevertheless, several patients with transient/episodic psychoses showed a chronic course (ICD-10: 10%, DSM-IV: 15%) and more than one third of patients with schizophrenia a non-chronic one (ICD-10: 40%, DSM-IV: 33%). Conclusions: In the cross-sectional assessment, there is a high diagnostic agreement between ICD-10 and DSM-IV. With respect to the long-term course, the delimitation of transient/episodic psychoses from schizophrenia was neither completely achieved by ICD-10 nor by DSM-IV. Copyright (C) 2004 S. In order to examine the descriptive validity of ICD-10 schizophrenia, 1,476 consecutively admitted in-patients were included in the present study. ICD-10 schizophrenia (n = 951) was compared with other non-affective psychotic disorders {[}persistent delusional disorders (n = 51), acute and transient psychotic disorders (n = 116) and schizoaffective disorders (n = 354)] with respect to socio-demographic, symptomatological and other clinical parameters. Analyses revealed that it is possible to distinguish schizophrenia from other non-affective psychotic disorders according to ICD-10 criteria: schizophrenic patients were characterised by more pronounced negative symptoms and a lower global functioning. They were younger than patients with persistent delusional disorders and schizoaffective disorders but older than patients with acute and transient psychotic disorders. The results are in line with a high descriptive validity of ICD-10 schizophrenia and highlight the importance of negative symptoms for this diagnosis. Copyright (C) 2003 S. Karger AG, Basel.
