From January next year the price of medicines listed on the Pharmaceutical Benefits Scheme will be capped at $25 per script and that's regardless of who wins the upcoming federal election. The Labor Federal Government has made the pledge and the Coalition has agreed to match it. Federal Minister for Health Mark Butler told Peter Fegan on 4BC Breakfast, "We have one of the best medicine systems in the world." "It delivers Australians the best medicines available anywhere on the planet at affordable prices, but getting them through the system is taking too long." "We all agree it should be better," Mark Butler said. "I think there are patients out there who are waiting for medicines to be approved and who are either paying top dollar for them or even worse, might not be getting the therapies that they wanted and needed." Tea is a foundational form of herbal medicine. Partly because it's the epitome of how "plant medicine is people's medicine" because it's so accessible, inexpensive, and familiar. But also because it's a way we can cultivate real, lasting relationship with the plants.Especially when we work with bulk dried herbs, we see them, smell them, taste them, and can feel them. Our senses are engaged.It also requires time and patience. It's a form of slow medicine. It's a ritual and rhythm we can bring into our daily lives, and that's therapeutic unto itself. And it's also something a lot of people ask me about, since I've almost always got an herbal tea in my hand - no matter where I am. It could be a clothing swap, the grocery store or a party. I'll likely have tea. The question "what's in your tea, today?" is common. A adoção dos princípios da Slow Medicine na prática assistencial não é desejável apenas para profissionais da saúde já inseridos no mercado de trabalho, pelo contrário: o ideal é que os acadêmicos entrem em contato com a filosofia slow desde muito cedo em sua formação. No Brasil isso já é realidade em vários cursos médicos, nos quais os alunos de medicina tomaram a iniciativa de estruturar Ligas Acadêmicas de Slow Medicine que vêm influenciando positivamente seu aprendizado e até sua personalidade. O décimo princípio da Slow Medicine nos fala sobre o uso parcimonioso da tecnologia, um assunto que costuma causar muita confusão. Usar a tecnologia com parcimônia é bem diferente de não usar a tecnologia disponível. Ter parcimônia não significa ser mesquinho em relação aos recursos que a gente tem hoje, e também é bem diferente de negar a importância da tecnologia. Ter parcimônia é ter critério, é ter bom senso pra decidir quais as estratégias que são necessárias para aquela situação específica, pra aquele paciente específico. É sobre utilizar as tecnologias porque elas vão realmente trazer benefício, e não simplesmente porque elas estão disponíveis. Jennifer O'Brien helps people talk about caregiving and end of life. She encourages compassionate, real conversation through her book, The Hospice Doctor's Widow: A Journal, where she shares her story of caregiving through collages and writings.  After years of caring for people with serious illness as a physician, Jennifer's husband, Bob Lehmberg, was diagnosed with a stage IV, metastatic cancer. But caregiving for the man who had made a 40-year career of caregiving as a physician was not easy. I wondered, How do you be normal and allow the normal feelings that arise in any relationship when they are dying and you are going to live? She related a funny story or two in that regard and one particularly ugly spat early on gave her an aha--she would never allow herself to go there again because she was responsible for the memories she would create during this time. Preventable regrets is the flip side. Caring for Bob made her a better person, Jennifer said. "A lot of what caregiving is about, is that you're self aware, hopefully, but you're also managing your future survivorship."  One of the first things Jennifer said to me before our call was, at the end of life comes death. How true and how few of us want to face that never mind prepare for it. She stressed the importance of having the conversations now, before the illness, the accident, or the random act that finds you at the mercy of a hospital or ER. O sexto princípio da Slow Medicine se refere à qualidade de vida. De acordo com a Organização Mundial da Saúde, qualidade de vida é “a percepção do indivíduo de sua inserção na vida, no contexto da cultura e sistemas de valores nos quais ele vive e em relação aos seus objetivos, expectativas, padrões e preocupações”. Do ponto de vista da saúde, ela costuma vir atrelada à percepção que cada pessoa tem sobre seu nível de bem-estar físico, emocional, social e espiritual, o que nem sempre é simples de se definir e menos ainda de se atingir. Costumamos associar qualidade de vida em saúde a medidas quantitativas (mais exames, mais medicamentos, mais procedimentos), e não à qualidade das estratégias adotadas. Paradoxalmente, o resultado disso nem sempre (ou quase nunca) é uma boa qualidade de vida. A realização indiscriminada de exames e o uso excessivo de medicamentos "preventivos" pode, inclusive, ser mais deletéria do que as próprias doenças que se propõem a evitar, sem falar em seu possível impacto emocional negativo. No final do século passado, hospitais da Coreia do Sul passaram a oferecer ultrassom da tiróide nos exames de rotina. Menos de duas décadas depois, os diagnósticos de câncer na tiróide tinham aumentado 15 vezes. Esse tipo de tumor virou o mais comum do país, com cerca de 40 mil pessoas diagnosticadas. A maior parte delas se submeteu a tratamento. Muitas tiveram a tiróide retirada e passaram a ter de tomar hormônio para o resto da vida. Algumas tiveram sequelas como paralisia das cordas vocais. No fim, a surpresa: apesar da alta de diagnósticos e de tratamento, a mortalidade por câncer de tiróide continuou praticamente inalterada. E mais: em 1947 um estudo já mostrava um número supreendentemente alto de câncer de tiróide em autópsias de pessoas que tinham morrido por outro motivo. The mechanical model on which modern medicine is based has obviously brought technological wonders to the practice of medicine—and it should be celebrated for these extraordinary achievements.  But have we become so wedded to the machine metaphor that we ignore more fundamental aspects of human reality?  Can another way of conceiving of health and life be brought to bear on the practice of medicine positively, without discarding the achievements of the scientific age? Origin Story Mette’s story goes back to her parents, who did not want her to be an architect. Her father was an architect, and her parents taught her that architecture was a very hard business. She remembers the highs and lows and recessions. She found her way to architecture through a long path through urban planning and ending up at the GSD. When she graduated, she was diagnosed with MS. There she also met her husband and partner, Andrew. A week before her thesis review, she went blind in one eye and couldn’t hold the exactor to cut her model. Thinking it was stress, she pressed on. When she went to the doctor, she realized it was worse than she t thought. Mette and Andrew were starting their careers as architects with this horrible diagnosis and no idea how it was going to affect them. They were unsure of what to do because they believed in the power of architecture to make an impact on people’s lives, but they saw how much it sucked to be an architect and what little value society places on architecture. They were faced with a dilemma: how could they do good work, have a good life, and make a good living? Since then, the challenge has been to work to balance all three of those things. After a few years of working, they were lucky enough to start their own firm to figure out how to do things differently. How could they run their firm that was different from the way all the other architects were doing? How could they persevere to their triple threat: good work, good life, good money. Dr. Sweet is an associate clinical professor of medicine at the University of California, San Francisco.She practiced medicine for twenty years at Laguna Honda Hospital in San Francisco, where she began writing. In her book, God's Hotel: A Doctor,a Hospital, and a Pilgrimage to the Heart of Medicine, she lays out her evidence in stories of her patients and her hospital for some radically new ideas about medicine and healthcare in this country. In our attempts to get control of healthcare costs by privileging “efficiency,” she suggests, we've been headed down the wrong path. Medicine works best that is, arrives at the right diagnosis and the right treatment for the least amount of money when it is personal and face-to-face; when the doctor has enough time to do a good job, and pays attention not only to the patient but to what's around the patient. Dr. Sweet calls this approach Slow Medicine.
