In this episode, Dr Choong-Siew Yong and Associate Professor Jeffrey Looi discuss the benefits and drawbacks of telepsychiatry and digital mental health delivery for general psychiatric care and specifically for children and adolescents. The pre- and post-pandemic approaches are discussed, including the emergency response for younger people. The conversation also examines the benefits and risks of telepsychiatry, including equitable digital technology access and technical challenges.  It also covers the use of telepsychiatry in rural and remote settings, and to allow videoconferencing when patients and their families are in different locations.  The discussion also explores new business models of telepsychiatry and the associated benefits and risks. Dr Choong-Siew Yong is a child and adolescent psychiatrist in Sydney, NSW. He has long-standing interests in medical workforce and education, practitioner regulation, doctors' health and mental health policy. He has served on the executive committees of the Australian Medical Association, NSW Medical Board and Medical Council, and the NSW Doctors' Health Advisory Service. For the RANZCP, he has served as the Chair of the Committee for Education Evaluation, Monitoring and Reporting and Continuing Medical Education (now known as the Committee for Continuing Professional Development). He is the current Chair of the Member Wellbeing Subcommittee. A/Prof Jeffrey Looi, MBBS Syd, MD ANU, DMedSc Melb, FRANZCP, AFRACMA, is a clinical academic neuropsychiatrist, in private and public practice, and Head of the Academic Unit of Psychiatry and Addiction Medicine at the Australian National University School of Medicine and Psychology. Jeffrey leads the Australian, United States, Scandinavian-Spanish Imaging Exchange and the Consortium of Australian-Academic Psychiatrists for Independent Policy and Research Analysis. He has received multiple research and leadership awards including: a Fulbright Scholarship and Australian-Davos-Connection Future Summit Leadership Award and Fellowship of the AMA. Our guest today is Charles Edwards, CEO and Founder of One Telemed, a telehealth service for mental health patients in underserved communities for a majority of the Medicaid Managed Care Organizations in Louisiana.  One Telemed uses innovative HIPAA video/audio technology to connect individuals in need of licensed professionals, reducing the wait time for a new patient needing to see a Psychiatrist from 90 days to 3-5 days. For more information, please visit https://www.onetelemed.com/about A native of Lafayette, Charles has worked in the mental health field since his first job out of high school 28 years ago. His life journey has been defined by the call for service, a call he heard from God who reached out to him with a distinct voice. During the interview, Charles shares the story of the persistent voice that has led him to step up and serve others. His own life was changed by a traumatic brain injury he suffered the weekend before he left for De Vries Institute. He was playing basketball with buddies, and says, "That moment changed my life forever....I went up for a dunk, me and a guy got tangled there, and I landed from eight feet up onto my head. I had a blood clot on my brain and my spine. I didn't breathe for about 3 to 5 minutes." He regained consciousness but his brain started swelling and he was throwing up. The doctors didn't think he would make it due to the length of time without oxygen and the brain swelling. Charles recalls awakening from his coma in his hospital bed and hearing a voice calling him. "I hear my name, the same voice that called out to me before. I opened my eyes and saw a tunnel with a hand coming out. And I took it. I remember looking down at myself. Then I looked up and saw this ball of energy, this ball of light. It was God!" God asked him if he wanted to stay or go back and do His work. While Charles wanted to stay, God wanted him to go back and do the work he was meant to do. This near death experience introduced him to a higher force that has led him to serve others in love ever since. Charles is a persistent fellow who keeps going no matter the challenge before him. He has experienced miraculous events and circumstances that continually remind him that his life has purpose, as long as he follows God's calling. Before Charles founded One Telemed, he worked in a series of jobs, including as Community Liaison and a Regional Director of Community Relations for one of the largest mental health providers in the state of Louisiana. It was through that experience that he learned firsthand that people in underserved communities did not have access to mental health care. He came to realize that utilizing the traditional method of delivering services would not be an effective way to resolve the need for the growing mental health crisis. So, he heeded his higher power's call for him to serve. In August 2013, Charles pioneered the movement for Telepsychiatry services and started Advance Telehealth, a telemedicine technology company. In September 2017, he started a provider company and founded One Telemed with a goal in mind to improve patient outcomes by making mental health treatment accessible to all, no matter their income or location in Louisiana. One Telemed provides trauma and grief counseling over video conferencing technology, thus allowing individuals to talk with a licensed professional from anywhere in Louisiana in a private and safe environment for healing and growth. Other services offered include Brain Tap Therapy, Transition Care Management, and Clinical Oversight for Behavioral Health patients. Charles Edwards, owner of One TeleMed, received the Small Business of the Year Award by Junior Achievement of Acadiana in 2021. "I've always had a heart for service." In this special episode from JPMorgan Chase's seventh Annual Leadership Day, Anu Aiyengar, JPMorgan Chase's global co-head of mergers and acquisitions, sits down with the highest-ranking female CEO ever in the Fortune 500, CVS Health CEO Karen Lynch. Karen discusses how her commitment to mental and physical health in her own life has carried over to the work she's doing at CVS Health.   Personal commitment to mental and physical health Karen tells Anu that she had an early traumatic experience with healthcare when, at age 12, she lost her mother to suicide. She and her siblings were then raised by an aunt, who also died early—when Karen was still in her 20s. She says that her mother didn't know how or where to get the mental health she needed, and years later, sitting in her aunt's hospital room, Karen realized she didn't know the questions to ask or how to get the help she needed either.   “And both of those experiences sort of have fueled my passion around healthcare and really being able to make a difference so that people are educated about healthcare, that people have access to healthcare, that people understand their options that are available to them in healthcare,” she says. “So that's really the passion I get up with every single day, from a very young age.”   Karen notes that it wasn't just her passion that got her to where she is today: she's had help from many, including relatives, a high-school teacher, and mentors and sponsors throughout her career. From her aunt she learned the importance of being decisive and making decisions based on whatever information was available. “That was an important lesson because as leaders, as people kind of managing people, people are always, always looking at you and watching whether or not you're making those decisions,” she says.   Another key lesson Karen learned early was about the importance of taking care of your own mental and physical health. Today, she says, she does that through early morning workouts as well as end-of-the-day Duolingo lessons. “I think it's important for all of us to make sure we're taking care of our own selves because if you can't take care of yourself, you can't take care of others,” she notes.   Keeping the customer as the north star Leading CVS Health through the unprecedented challenge of the pandemic allowed Karen to put her leadership and priorities to the test. The last several years have seen huge changes in both mental and physical healthcare. “Before the pandemic we had 10,000 virtual visits for Telepsychiatry,” she says. “Last year we had 10 million. And so that just gives you a sense for the change and the ease that people have had with using virtual care.”   To thrive in the midst of all that change, Karen says she had to lean into focusing on her employees first, and then, most significantly, the customers and their evolving healthcare needs. “And we set sort of guideposts that we were focused on health and safety. We were focusing on our colleagues' safety, focusing on the importance of getting Americans vaccinated and then looking around the corner,” she recalls. “So we had to tactically make sure that operationally we could do all the things that we had to do, but at the same time, we had to set a sort of a north star because everything in the world was changing around us and consumers expectations in healthcare were changing dramatically.”   In the end, Karen oversaw the shift in CVS Health “from kind of a corner drug store to this broad national healthcare company.” Today the company is focused on being in the community—meeting people's growing interest in accessing care online. How does she do it all? “I think it's all about setting your own goal, setting your expectations, defining who you are and what you want to be and getting comfortable in your own skin,” she tells Anu. “And for women, sometimes that's hard. We just have to keep working at it. And I always say, there's always going to be those little voices in your head saying, You can't do this or questioning it. And you've just got to push beyond those voices and say, Yes, I can.”   Full transcript here

Today telehealth is used in a variety of settings such as school children for counseling and therapy, adolescence, teen agers who have busy schedules, college students may see their providers in between their classes. Many working adults have utilized telepsychiatry as well. Studies have shown that nursing home patients have found telepsychiatry beneficial. Prison and Jails where patients are incarcerated have also benefitted to telepsychiatry There is is a high level of satisfaction when using. Telepsychiatry and is comparable to those who go face-to-face.

I welcome Dr. Sarah Pospos to the show to discuss her interesting career path in the world of psychiatry, specifically telepsychiatry. Dr. Sarah is a devoted B.rad listener, and in this show, you'll learn about her highly integrative approach to psychiatry—as you'll hear, I had to push and prod her to even talk about prescription medication! She is a strong supporter of incorporating exercise, nutrition, sleep, stress management, time management and breathing exercises and yoga in order to handle the very common, prevailing mental health struggles that have escalated in recent years, especially in the age of quarantine and the accordant lifestyle disruptions (such as anxiety, depression, PTSD, etc). The immediate successful interventions that can concur with something like a simple breathing exercise or a quick workout will be great takeaways from this show; in fact she cites research that proves a single workout can alleviate the acute symptoms of anxiety! You'll also hear about imposter syndrome: why it's become so prevalent and how to combat it without becoming overly arrogant, egoistical, and narcissistic, and learn a lot about how to achieve balance in both your brain and in your life. Dr. Sarah Pospos is a sports and perinatal psychiatrist and former student athlete. Her Masters in Psychopharmacology and Applied Psychology allow her to add even more insights on all things medication and human behavior—she has published quite a bit on work burnout, anxiety and depression at UCLA, UCSD, Harbor-UCLA and UCSB. Dr. Sarah is devoted to helping working professionals feel their best again, especially when dealing with anxiety, depression or other psychiatric challenges associated with work stress, intense workload, deadlines, no work-life balance and others. She incorporates exercise, nutrition, sleep, stress management, effective time management (especially as a full-time psychiatrist and new mom of 2 under 2), and other sustainable lifestyle changes.   TIMESTAMPS: Dr. Pospos is a psychiatrist who believes exercise, nutrition, and sleep and such are preferable to medication for handling life's struggles. [32:47] Exercise, art, and other activities can give you a sense of mastery, which, of course, helps with mental health. [35:13] Where do medications fit in? Are they overused, over prescribed? [37:34] There is such a continuum in diagnoses and labeling in this field. [43:29] There is a big effect on our mental health with navigating the internet and mobile devices. [50:09] Comparing yourself to others can lead to imposter syndrome. [52:16] It is important, especially for kids, to try to implement the mindset in the process rather than outcomes. Implementation of certain telehealth services would improve clinical quality nationwide by 20%, increase access to care by 20%, and reduce health care spending by 15% to 20%, a May-June 2022 report in Harvard Business Review of Kaiser Permanente and Intermountain Healthcare, two of telehealth's earliest adopters, show. Today on Where We Live, we hear how Yale New Haven Health and UConn Psychiatry are expanding their telehealth services well after hospitals and clinics have opened up in-person consultations to non-COVID patients. Fixing the behavioral health crisis is an absolute imperative in the movement to value-based care. The business model was conceptualized by Dr. Fareda Flores even before the coronavirus outbreak and has built-in security features for privacy. Patients and psychiatrists now have a platform to do telepsychiatry without even stepping into the confines of a clinic.

Today is mostly about Maddie and Leora's recent experience in the emergency room. AKA the whole episode is one big sh*t show. So, Maddie is sharing a positive thing instead of her “oh sh*t” moment. After posting about her health issues, several people who Maddie was not expecting to reach out actually did, and sent helpful recommendations and support. This is just one more example of the power of talking about mental health! This is why this podcast exists!   Big disclaimer for this one, guys. We are talking about our experiences getting help in a particular instance. Leora will also be providing some diagnostic criteria for panic attacks. It's especially important to remember during this episode that our podcast is not a substitute for professional support and/or advice.   The point of this episode isn't just to provide information about panic attacks to you all. It's also to discuss the experiences that we had at the ER, and how these motivated us even more to stand up to mental health stigma. Maddie mentioned directly to the staff at the hospital that sometimes when mental health is brought up, people are treated differently at the ER. Maddie was assured that this would not happen, but, unfortunately, that was not true.   Mental health is treated differently from physical health in the medical profession. The truth is, mental and physical health are interconnected. They cannot be separated. A panic attack is a perfect example- it is a mental health issue, but it causes physical changes. Your heartbeat quickens, you may feel sick or faint, your body temperature changes...this is no less real than any other physical symptom caused by an illness. But, because this is related to mental health, the ER does not view it as an emergency. This is why we do this podcast! Remember, if you have an experience like this, you are not alone. We are all part of a community that has each others' backs, and we are ready to start a movement to change the way the medical profession looks at mental health.   Tools Having had a panic attack can be a trigger for panic attacks (because of the fear of having one), so it's important to seek help if you've had a panic attack so you can prevent them in the future. If you're going to the ER while in mental health crisis, bring a trusted person with you if you can so that they can help you relay information. If you're the person helping, always ask permission and don't talk over the person who is in crisis. If you are told to leave the ER without any help or answers, ask to speak to a social worker. Telepsychiatry can be a way to talk to a psychiatrist more quickly than you can get an in-person appointment. You can ask a social worker to schedule one of these appointments for you if you are in the ER for a mental health issue. If you have a negative experience with the healthcare profession related to mental health, remember that you are not alone. Reach out to others, whether that be friends, family, a therapist, or an online support group.
