In this episode, we explore the intricate role of the vagus nerve as a central regulator within the microbiota-gut-brain (MGB) axis, examining its neuroanatomical structure, signaling mechanisms, and interactions with microbial metabolites and immune pathways. We discuss how vagal afferent fibers relay sensory input from the gut to the brain, including signals mediated by short-chain fatty acids (SCFAs) and gut-derived hormones, and how efferent fibers modulate gut motility, intestinal barrier integrity, and inflammation through the cholinergic anti-inflammatory pathway. Finally, we explore vagal dysfunction as well as associated conditions and symptoms, and we touch on just a few potential root causes. Topics: 1. Introduction Focus on the vagus nerve's role in the microbiota-gut-brain (MGB) axis. Bidirectional communication between the brain and microbiota. Overview of communication pathways: neural (e.g., vagus nerve), endocrine (e.g., HPA axis), immune (e.g., cytokines), and metabolic (e.g., SCFAs). 2. Overview of the Nervous System The CNS includes the brain and spinal cord - control centers for the body. The peripheral nervous system extends beyond the CNS The peripheral nervous system is divided into the somatic nervous system and the autonomic nervous system. 3. Autonomic Nervous System (ANS) and Subdivisions Sympathetic Nervous System (SNS) Parasympathetic Nervous System (PNS) Enteric Nervous System (ENS) 4. The Vagus Nerve and Role in the PNS Principal component of the parasympathetic nervous system. Governs "rest-and-digest" activities Contains both afferent (80%) and efferent (20%) fibers. 5. Vagus Nerve Anatomy Fibers originate at the base of the skull and extend into the gut wall. Fibers distributed throughout the mucosa, submucosa, and beyond. Interact indirectly with gut luminal contents via specialized gut cells, including EECs and immune cells. 6. Interaction with Intestinal Cells Enteroendocrine cells (EECs) release gut hormones in response to microbial metabolites. SCFAs, such as butyrate, activate free fatty acid receptors on EECs, stimulating vagal afferents. Immune cells within the gut wall modulate vagal signals during inflammatory responses. 7. Review of Functions Sensory input (afferent fibers): Detects gut-derived signals like microbial metabolites and mechanical stretch. Motor output (efferent fibers): Regulates gut motility, secretion, immune responses, and more. 8. Impact of a Diverse Microbiome on Vagal Activity Enhanced SCFA production boosts vagal activity. SCFAs improve gut barrier integrity, reduce systemic inflammation, and assist in regulating stress responses. 9. Examples: Intestinal Barrier Function Releases acetylcholine (ACh) to modulate inflammatory pathways. Helps enhance tight junction protein expression, preserving gut barrier integrity. Helps prevent the translocation of microbial endotoxins like LPS into systemic circulation. 10. Dysfunction of the Vagus Nerve Reduced vagal tone disrupts gut homeostasis. Conditions such as IBS, IBD, chronic fatigue syndrome, anxiety, depression, and POTS. Chronic stress, infections, and dysbiosis are common contributors. 11. Root Causes 12. Tying Back to the HPA Axis Low vagal tone is associated with increased HPA axis activity. Highlighting the interplay between the gut, brain, and stress response systems. 13. Conclusion Identifying potential root causes. Contributing lifestyle factors. In this episode, we dive into the interplay between the hypothalamic-pituitary-adrenal (HPA) axis and the microbiota-gut-brain (MGB) axis, highlighting their bidirectional communication through endocrine, immune, and neural pathways. We'll explore how gut-derived metabolites like short-chain fatty acids (SCFAs) and endotoxins like LPS influence HPA axis activity; conversely, we explore how dysregulated cortisol can impact gut barrier function, immune signaling, and more. We also discuss testing including stool analysis and DUTCH tests. Topics: 1. HPA Axis and MGB Axis Interaction The HPA axis and microbiota-gut-brain (MGB) axis are bidirectionally connected. Gut microbiota influences the HPA axis via metabolites and more 2. Overview of the HPA Axis and Cortisol Secretion Stress signals activate the hypothalamus to release CRH. Stimulates the anterior pituitary to produce ACTH. ACTH signals the adrenal cortex. The adrenal cortex releases cortisol, which binds to glucocorticoid receptors (GRs). 3. Cortisol Dysregulation Impact on Intestinal Health Intestinal lining anatomy: epithelial cells Tight junction proteins regulate nutrient trafficking and prevent pathogen entry. The mucosa contains epithelial cells, connective tissue (lamina propria), and a thin muscle layer. 4. Glucocorticoid Receptors (GRs) in the Gut GRs are intracellular receptors that modulate gene expression when activated. Cortisol binding causes GRs to translocate to the nucleus and bind DNA at GREs. GRs on epithelial cells. Modified tight junction protein expression. 5. Gut Microbiome's Role in HPA Axis Activity SCFAs, including acetate, propionate, and butyrate, produced by gut bacteria fermenting dietary fiber. SCFAs support gut integrity, reduce inflammation, and act as signaling molecules. A diverse and healthy microbiome can enhances HPA regulation via SCFAs. 6. SCFA Modulation of the HPA Axis SCFAs and cortisol. Research highlights SCFAs' ability to attenuate stress-induced cortisol increases. 7. Dysbiosis - Impact on the HPA Axis Dysbiosis reduces SCFA production, impairing gut barrier integrity and immune signaling. Dysbiosis, intestinal hyperpermeability and LPS. LPS activates inflammatory pathways. 8. Inflammation and Dysregulated HPA Activity Chronic inflammation and cortisol. Inflammatory signals from the gut exacerbate systemic and neural stress responses. 9. Symptoms of HPA Axis Dysfunction Chronic fatigue, disrupted sleep, mood disturbances... 10. Identifying Root Causes Dysbiosis, chonic infection / chronic inflammation... DUTCH Test Stool analysis Thanks for tuning in! During his tenure, Cleveland Clinic spun off 57 companies that raised more than $700 million in equity financing. There were none before his arrival. In this special episode of Healthcare is Hard, recorded live at the Digital Health Innovation Summit (DHIS) in Boston, Keith Figlioli led a keynote discussion with Chris about the current state of healthcare innovation. Some of the topics they discussed include:Embracing innovation from inside and outside of MGB. Chris described how his organization is tasked with supporting innovation that will help achieve strategic objectives, regardless of where it originates. With the realization that most health systems are large organizations not known for their nimbleness, he shared advice for innovators looking to partner with or sell to them about how buying decisions are made.Corporate venture capital in healthcare. With a steady growth in the number of health systems creating their own venture arms over the past several years, Chris shared best practices from being in the space for decades. For example, he talked about balancing the commitment to both strategic and financial goals, and how his organization catalogs the unmet needs of MGB's leading faculty. For entrepreneurs, he suggests getting to know the growing number of people in corporate venture roles, in addition to health system operators.Getting operators to embrace digital innovation. Fielding a question from the live audience, Chris discussed strategies for getting operators to adapt digital health innovation into established workflows, and how important it is to create a culture around innovation to achieve that goal. On one hand, he talked about how part of his organization's job is to get doctors to proactively and continuously think about new ways for how they might optimize their work. On the other hand, he talked about listening to unmet needs and addressing them directly as a way to ease adoption from the start. To hear Chris and Keith discuss these topics and more, listen to this episode of Healthcare is Hard: A Podcast for Insiders.

It's Hump Day! Sam and Emma discuss some of the biggest stories of the week, including (of course) Trump's arraignment in Miami yesterday. Then, they're joined by Dr. Titi Afolabi, resident physician in internal medicine at Mass General Brigham (MGB), to discuss the MGB resident physicians recently winning their National Labor Relations Board (NLRB) election. First, Sam and Emma run through today's biggest headlines, including updates on Fed policy, the GOP's devious dealings on the debt ceiling, the NLRB's expansion of the definition of “employee,” and a deal between UPS and their teamsters. Next, Sam and Emma parse through coverage of Donald Trump's arraignment on the right, including Fox News' dubbing of Biden as a “Wannabe Dictator,” and the classic “everyone who's against Trump is a pedophile/commie/CRT-fan” arguments from myriad right-wingers, before looking ahead to the Democratic primaries in 2024 and what role RFK will play amidst his shift towards free-market politics. Dr. Titi Afolabi then dives right into the impact of the COVID-19 pandemic on exacerbating the already extreme nature of Mass Gen's residents' copious work and low pay, further undercutting the care they could provide their patients. After walking through the particular economic context of this unionization drive, Dr. Afolabi, Sam, and Emma tackle the various cohorts involved at MGB and why the residents are the first to organize, before wrapping up the conversation by analyzing future organizing strategy after the recent NLRB decision, and where they fit into greater Residency organizing. And in the Fun Half: Sam and Emma tackle Cornel West leaving the People's Party for the Green Party, and the response from PP, also touching on Dave Rubin's meeting with Balázs Orbán (no familial relation, yes political relation), and watching Nicholas Carlson, Global EIC of Insider, engage in grassroots union busting. Chris from Albany discusses the importance of Emma's recent trans-people-in-sports take, Guy from Charlotte dives into the absurdity of certain “civil war” attitudes, and Ryan from Central Florida parses through the state of his state's politics. John from VA shares a personal story, Charlie Kirk takes on woke condiments, and Sam discusses the evolution of RFK Jr. and his role in the upcoming presidential election, plus, your calls and IMs! In training to become a physician, medical residency can be a grueling period. Now, medical residents across the country have begun fighting to unionize their ranks. In Boston, residents at Massachusetts General Brigham — a major medical system — recently garnered enough votes to file for a union election. In her first episode as host of the "First Opinion Podcast," editor Torie Bosch speaks to two MGB residents, Minali Nigam and David Bernstein, with differing opinions on the best next step forward for their cohort.

He then had 20 years running Private Military Security businesses; founding both Sandline International and Aegis Defence Services. He has published ‘A Dangerous Enterprise: Secret War at Sea'. Join me as I catch up with Simon Hall from the UK, this episode is both geeky and eclectic. Simon was a college friend who I lost contact with over 20 years ago. The magic of social media and in particular Facebook meant I would get the odd update on his car life. There was a lot of uncover in your conversations, classic cars, an aspiration to own a Jaguar (Jagwire) before he was 30 and doing some strange things with a car usual reserved for burying the dead. One film from Simon's childhood led him on a journey to import the hero car from the film. He then had to restore the car which nearly killed him, literally. If you're interested in MGBs, Jaguars, Pontiac and Hybrid BMWs then this episode has all of the above! So, grab a seat, get comfortable, and immerse yourself in this captivating conversation. It's a must-listen for any member of our car tribe! My thanks go out to Simon for the conversation!Song: Jerry Reed - East Bound and DownYouTube reference video: The Cannonball Run Countach: Supercar Legend | Full Documentary - YouTubeClick for all the links: For enquiries: DannyPMedia@outlook.comSupport the showClick for all the links: For enquiries: DannyPMedia@outlook.com
The KGB was the main security agency for the Soviet Union. It emerged as a direct successor of preceding agencies such as the Cheka, GPU, OGPU, NKGB, NKVD and MGB, and was nominally attached to the Council of Ministers. It was the chief government agency of carrying out internal security, foreign intelligence, counterintelligence, and secret-police functions. Did it morph into the current FSB, or is that a different beast? Did any of the culture, traditional and methods of the KGB pass to its descendant agency and operatives? What can we know for sure of this brutal and secretive organ of power, and what relation does it have to Putin and his kleptocratic regime? Dr Yuri Felshtinsky is a prominent author, historian, and journalist, and expert on Russia and the former Soviet Union. He has appeared in print, TV, and radio interviews worldwide, and is widely known as co-author of the book “Blowing Up Russia” with Alexander Litvinenko, a former Lieutenant Colonel in the FSB who was poisoned with radioactive polonium in London in 2006. His latest book – “Blowing up Ukraine: The Return of Russian Terror and The Threat of World War III”, was researched before the invasion of Ukraine and is the first comprehensive investigation into the lethal methods Russia has used since 1999 to take over Ukraine – culminating in the full-blown unprovoked war in 2022 and mounting atrocities. ---------- BOOKS: Blowing up Ukraine: The Return of Russian Terror and The Threat of World War III (2022) Blowing up Russia: The Book that Got Litvinenko Murdered (2019) The Putin Corporation: The Story of Russia's Secret Takeover (2012) Lenin And His Comrades: The Bolsheviks Take Over Russia 1917–1924 (2010) ----------
For our 1st Home Base Staff Pick, Our very own Team Leader of Veteran Outreach Marine Veteran Ryan Casavant gives some love to fellow Marine Veteran and … Dancehall and Reggae artist Orville Burrell, AKA Shaggy. Ryan talks about his road to the Marines, his deployment experience, and what he does at Home Base to support his peers and families. And as we look at those making history, this month in Black History Month, you cannot find any other GRAMMY-winning Marines out there… Shaggy has released 'Com Fly Wid Mi which has 11 Frank Sinatra songs all done in his own Reggae style, produced by collaborator Sting. Cheers to these two Marines who have served and continue to serve in many ways. Some news:-As we know, it's been 1 year as of Feb 24th since the War in Ukraine. It is hard to believe, and not only do we think of those Ukrainian troops when it comes to their needs body-mind and soul, we think of the trauma that the civilians have been enduring. There are many people around MGB who have been helping since the get-go like BWH Navy Veteran Dr. Eric Goralnick and MGH's Dr. Jarone Lee and many others - and now there seems to be even more need when it comes to mental health, not surprisingly. -In more celebratory news, Home Base was very happy to officially announce on Feb 22nd Last week, a first of its kind major federal funding of $5 million as we welcomed a visit from Senators Elizabeth Warren (D-Mass.) and Edward J. Markey (D-Mass.) and Representatives Stephen Lynch (MA-08) and Ayanna Pressley (MA-07) to our national center of excellence. These funds will help support Home Base's signature clinical programs serving families of the fallen - and active-duty special operations forces experiencing ongoing issues from traumatic brain injury – TBI.Senator Warren who is a member of the Senate Armed Services Committee, spoke some familiar words, not unlike what our friend and colleague Green Beret Jillian Kitching said that always resonates with me…About the simple equation, that their service = our service to help them to take care of them, when they return.Warren said, “I think of it this way - A lot of what we do is about military weapons and all the technology. But The other half is about the people.”- “If we are going to have a strong military, then we have to treat our people better.”Thank you Shaggy, and Ryan for your service, and your ongoing service to your fellow comrades and families. Special thanks to Tracy Bufferd for all your efforts and support in connecting Home Base with Dancehall, Reggae and all the genuine support of service from Shaggy and team. And… If you want more Shaggy at Home Base, check out the LIVE conversation at the Veterans Day Dinner we released February 2020. __If you are your loved one is experiencing any emotional, mental health struggles, you are not alone and please contact Home Base at (617) 724-5202, or visit www.homebase.org.Theme music for Home Base Nation: "Rolling the tree" by The Butler FrogsFollow Home Base on Twitter, Facebook, Instagram, LinkedInThe Home Base Nation Team is Steve Monaco, Army Veteran Kelly Field, Phebe Garver, Justin Scheinert, Chuck Clough, with COO Michael Allard, Brigadier General Jack Hammond, and Peter Smyth.Producer and Host: Dr. Ron HirschbergAssistant Producer, Editor and corresponded (Home Base Staff Picks): Chuck CloughChairman, Home Base Media Lab: Peter SmythTo Donate to Home Base where every dollar goes to the care of veterans and military families that is cost to them, go to: www.homebase.org/donate.Home Base Nation is the official podcast of Home Base Program for Veterans and Military Families, a partnership of the Massachusetts General Hospital and the Red Sox Foundation. This week I travel to Market Garden Brewery in Cleveland, Ohio. My guests were Sam McNulty and Patrick Daniels. Sam is the owner of the Market Garden Brewery and Pat is the director of brewing at MGB. We discussed the inception of the Market Garden Brewery located in the Ohio City area of Cleveland, Ohio. We also talked extensively about water chemistry, brewing processes, brewery expansion, and what breweries need to do to set themselves apart from a rapidly growing craft beer market, not just in Ohio, but across the country. The beers we sampled during the interview were a Czech Pilsner, Hefeweizen, IPA, and freshly canned 3-hour old Hazy IPA. All of which were delicious. This week on episode 466, we reach over the Atlantic to London. Mistress Glam Bitch, talks about playing with strangers, from a top point of view. John is a life long car guy. All of his favorite toys had wheels. Growing up on the north side of Chicago, two of his favorite early memories includes drives down Lake Shore Dr. in an MGA with the top down, (which really started his love of British Sports Cars) and when the orange pedal car was brought downstairs for a sidewalk drive. That was officially his first car. His second was a fairly ratty, but still loved red 1960 TR3. There were a variety of cars in the early days, foreign and domestic. A 1969 Road Runner, a 1973 Grand Am with a four speed, and a 1972 MGB were among a few early cars. Later came a long string of practical and sporty BMW 3 and 5 Series cars. Somewhere in the late 80 and early nineties was a 944 Turbo. In 1996 John bought a 1963 Austin Healey BJ7 3000. He met his long-time friend Mark Baker when Sport and Specialty did some much-needed maintenance on the car. Re-bitten by the British Car bug, and deciding it was “now or never” John found a 1960 Bugeye race car and brought it back to racing duty. Next, he joined North Suburban Sports Car Club and went to his first driving school in 1998, and received his Novice Competition License. He has held a competition and instructors license since 1998. He also holds a Chief and Operating Steward's license from Midwestern Council and serves as the Vintage / Historic Racing Chair and Vintage Motorsport Council delegate. He campaigns his lovely yellow 1969 Alfa Romeo GTV and BMW 2002 with MCSCC and the Vintage Sports Car Drivers Association (VSCDA) in general vintage races and the B Sedan Trans Am series. He also runs his track and street ready1983 Caterham Super Seven when the occasion arises. After a long and successful career in sales and marketing and general management, John knew it was time to do something he truly loved. After the passing of his friend Mark, the business that he knew so well through his friendship with Mark was in danger of shutting down. The business had a talented and dedicated crew and good business basics in place, so after helping the family operate the business in 2012, he began discussing some purchase options to keep the business alive and growing. At the end of 2012, he purchased the business and hasn't looked back. He couldn't be happier, he works with a talented crew, gets to go racing when he can, and has successfully turned his passion into his livelihood! What could be better?
MGB 5:10 Dustin Tarr Interview pt2 You may have noticed that the MGB Wrestling podcast has been off the air for a while now. Mason has been doing lacrosse and wrestling and hasn't had time to go to indie events and we haven't really attended many either anyways. The price of gas/petrol has been prohibiting some of the longer trips we used to take and Jackson and I have mainly been attending local baseball games instead and regarding for the JGB Sports Podcast. This might not be the last episode but they will be few and far between going forward. I would like to thank everyone who did support the podcast in the last 5 seasons it meant a lot to us. For episode 7 of Good Cop Bad Cop Baseball Podcast Dustin agreed to appear alongside his brother Chris, so before we recorded that we talked some wrestling to address some things that were touched on in the original interview from MGB Season 3 Episode 53. The original interview can be found at the 26:30 point of this episode if you wish to listen to that too. There we talked about his training, his career in wrestling, his matches down in N.E.W. where we saw him and all the other random questions we like to ask too. With a lot going on in the MGB household these last few weeks we have been unable to attend the Indie events we planned on reviewing so with 205 Live ending a few weeks ago this seems the ideal point to release on Visionaries Global Media for the first time Jackson's first weekly podcast. We started in January 2019 when he was still 4. Season 2 we will release at a later date when he was 5 years old.
