Because proteasome inhibitors are well known for causing thrombocytopenia and neutropenia. So making sure that the patients do meet eligibility for the treatment for that day, and do they have anemia or lower red blood cell counts. You want to make sure that, because of these therapies, that the patient has no symptoms or infections going into each therapy for that day.” TS 10:19 “[Bortezomib], interesting enough, it can cause hypotension, cardiac failure, and sometimes pulmonary edema. Switching that up a little bit, what makes it slightly different, carfilzomib … a lot of times we saw, even in the clinical trial, that there was a lot of hypertension or cardiomyopathies, or arrythmias that we saw with carfilzomib and different dosages that they have indicated from the FDA. So again, monitoring the hypertension … or heart failure.” TS 15:16 “We also want to keep in mind another adverse effect, and especially in myeloma—our patients come in the door already immunocompromised just by the disease state alone. But now I'm giving them therapies that can drop their neutrophil count, so neutropenia and thrombocytopenia, so they are at a higher risk of having serious infections, even including like pneumonia or having outbreaks of herpes zoster or shingles.” TS 16:50 “If the patient has shortness of breath or symptoms, hold the therapy. I think that's one of my biggest messages when it comes to cancer treatments and educating other healthcare providers, or even educating our patients and their caregivers or the care partners with them, is that we need to sometimes hold the therapy for safety.” TS 22:02 “I say keep a log, keep a book. Let me know when the symptoms happen. Are they happening the day of treatment? Are they happening two days later from the treatment? Are they happening a week later from the treatment? And being able to kind of guide which therapy is causing some of these adverse events or side effects alone. So, making them have calendars. When did you take the drug, when did you get your last infusion or your last [subcutaneous] injection? Always talk to your care team, whether it's in the academic center or next to your house in the community.” TS 26:17 “It's us learning how to listen to the patient going forward. We have tasks to do—we all have tasks to do in our lives—but we have to take a breath, be mindful who's in front of us, listen to them first, and then be able to talk to them and care for them upfront and see what the symptoms are. I think that's what we need to do. We have to take a breath in cancer.” TS 39:35

The name Mike Richter is well-known among hockey fans. Richter spent 15 years in the National Hockey League as a goalie for the New York Rangers, including in 1994 when he was a fixture in the net during the team's Stanley Cup winning season. Richter was also recognized as the most valuable player for the U.S.'s 1996 gold medal winning World Cup team, as well as a member of three U.S. Olympic teams, including in 2002 when the team won the silver medal. Richter was inducted into the U.S. Hockey Hall of Fame in 2008. But what is likely lesser known is that Richter is the current president of Brightcore Energy, a leading provider of integrated, end-to-end clean energy solutions to the commercial, institutional, and government markets. Furthermore, the emissions associated with these systems can be significant. “If you can address that, you're doing something important, and that's really where our focus has been, particularly the last few years,” he said. Geothermal Heating and Cooling Systems Traditional geothermal often requires significant open space for the geothermal borefield and can have material time implications in project development. Brightcore says its exclusive UrbanGeo solution combines proprietary geothermal drilling technology and techniques that increase the feasibility of geothermal heating and cooling applicability while reducing construction development timelines. “We typically go between 500 and 1,000 feet down,” Richter explained. “The ambient temperature of the ground about four feet down below our feet here in New York is 55 degrees [Fahrenheit] year-round.” The constant and stable underground temperature is the key to geothermal heating and cooling systems. Even when the air above ground is extremely hot or freezing cold, the earth's steady temperature provides a valuable heating or cooling resource. A geothermal system has pipes buried underground that fluid is circulated through, and a heat pump inside the building. In winter, the fluid in the pipes absorbs warmth from the earth and brings it inside. There, the heat pump “compresses” this heat, raising its temperature so it can warm the building air comfortably—even when it's icy cold outside. In summer, the system works in reverse. The heat pump pulls heat out of the building's air, sending it through the same underground pipes. Since the earth is cooler than the hot summer air, it acts like a giant heat sponge, soaking up unwanted heat from the building. This process cools the living space easily and efficiently, using a lot less energy than a regular air conditioner because the ground is always cooler than the hot outdoor air. So, whether it's heating or cooling, a geothermal system can keep buildings comfortable by moving heat between the building and the earth. “[It's] pretty straightforward and very, very efficient and effective, particularly—and this is key—at the extremes,” said Richter. “Air source heat pumps are excellent and they continue to get better,” he added.

Teala Davies alleges that Jeffrey Epstein sexually abused her beginning when she was 17 years old, after luring her in under the guise of offering support and mentorship. She claims Epstein flew her around the world on his private jet and brought her to his properties in New York, New Mexico, Florida, Paris, and the U.S. Virgin Islands, where the abuse took place repeatedly. Davies says the sexual abuse was not only frequent but psychologically damaging, leaving her with lasting trauma and a sense of dependence that made it difficult to escape.Davies also alleges that Epstein transported her internationally as part of his trafficking network, presenting her as part of his entourage while continuing the abuse behind closed doors. She says the relationship was marked by coercion rather than consent, and that she experienced ongoing trauma as a result. Her legal complaint outlines long-term emotional damage, citing flashbacks, dissociation, and a persistent fear of retaliation. Heidi Allen was working her shift at the D&W Convenience Store in New Haven, New York on Easter Sunday 1994 when she seemingly vanished into thin air. In an 8-13 minute window, Heidi disappeared from the store with no signs of a struggle. Episode No. 715 features artist Kandis Williams. The Walker Art Center, Minneapolis is presenting "Kandis Williams: A Surface," the first survey of Williams' career. The exhibition spotlights how Williams has used collage as a tool of Black feminist resistance, to dismantle entrenched histories and power structures, and to rebuild dominant narratives. The exhibition, which was curated by Taylor Jasper with Laurel Rand-Lewis, is on view through August 24. The exhibition catalogue was published by the Walker. Amazon and Bookshop offer it for around $45. Williams is also included in "Performance on Paper" at the Hammer Museum, University of California, Los Angeles. It features prints and drawings created at the intersection of music and dance by about twenty artists active from the 1960s to the present. It was curated by Naoko Takahatake with Jennie Waldow, and is on view through August 10. Williams' previous museum solo exhibition was at the Institute for Contemporary Art, Virginia Commonwealth University. They have been included in group exhibitions at the Museum of Modern Art, New York, the Frye Art Museum, Seattle, at the Studio Museum in Harlem, and in the Hammer Museum's Made in LA biennial. From the minute Donald Trump came down the golden escalator, the Republican establishment has hated his guts. In the 2016 primary, the brash New York billionaire was treated by mainstream GOP leaders and commentators as a joke responsible for degrading the political process. As the real estate tycoon gained momentum, establishment figures had two choices: Declare yourself "never Trump" or pretend to have seen the light. Many fled to organizations like the Bulwark or the Lincoln Project, but others stayed around waiting for their moment to recapture the Republican Party from the MAGA populists. What is death anxiety?  We spend the first 15 minutes of the podcast addressing this question.  And maybe this was unfair to our guests, the fabulous dynamic duo of palliative psychiatrists Dani Chammas and Keri Brenner (listen to their prior podcasts on therapeutic presence and the angry patient).  After all, we invited them on to our podcast to discuss death anxiety, then Eric and I immediately questioned if death anxiety was the best term for what we want to discuss! Several key points stood out to me from this podcast, your key points may differ: The “anxiety” in “death anxiety” is not a pathological phenomenon or a DSM diagnosis;  it references an existential concern that is fundamental to the human experience .  To me,” awareness of mortality” might be a better term, but in fairness, the idea of “death anxiety” was coined well before the formal establishment of “anxiety disorders.” The ways in which death anxiety manifests in our patient's choices and behaviors varies tremendously, and our responses as clinicians must be individualized. There is no “one size fits all” approach. In one example Dani discusses, a pain level of 1.5/10 might be overwhelming, because for a patient  in remission from cancer any pain might signal return of cancer.  Some manifestations of death anxiety can be debilitating, others lead to tremendous personal growth, connection to others, and a drive toward finding meaning in their illness experience.   Death anxiety impacts us as clinicians, not only through countertransference, that word that I still can't define (sorry Dani and Keri!), but also through our own unexamined fears about death. As clinicians who regularly care for people who are dying, we might find ourselves becoming “used to” death. Is this a sign that we are inured to the banality of death, and less able to empathize with the death anxiety experienced by our patients or their families? Or could it reflect our acceptance of the finitude of life, prompting us to live in the present moment? Perhaps it is something else entirely. The key is that looking inwards to understanding our own unique relationship with mortality can deepen our ability to authentically accompany the experiences of our patients.
