Dr. Dawn Harris Sherling is a board-certified internal medicine physician who is also board-certified in obesity medicine. She began studying journalism before switching to pre-med studies at the University of Florida where she graduated with honors and as a phi beta kappa. She earned her M.D. with honors from the Yale University School of Medicine and then completed her residency at Harvard's Brigham and Women's Hospital where she went on to serve as an attending physician and Instructor in Medicine at Harvard University. She moved back to her native Florida in 2008, and currently sees patients at a clinic for underserved populations and is an associate program director for the internal medicine residency at the Charles E. Schmidt College of Medicine at Florida Atlantic University. She is most recently the author of Eat Everything: How to Ditch Additives and Emulsifiers, Heal Your Body, and Reclaim the Joy of Food. Inspired to write this book after an additive-free trip to Italy seemingly cured her IBS, Dawn did a deep dive into the published literature on our microbiomes and how emulsifiers and other substances may be feeding the wrong bacteria in the wrong places, contributing to alarming rates of diet-related diseases. Carrageenan is a food additive extracted from red seaweed. It is used as a thickening agent, emulsifier, and stabilizer in a wide variety of processed foods, including dairy products, meat products, baked goods, and beverages. There is some concern that carrageenan may be harmful to human health. Studies in animals have shown that carrageenan can cause inflammation, digestive problems, and even colon cancer. However, the evidence for these effects in humans is less clear. One of the main concerns about carrageenan is that it can be degraded into a more harmful form called poligeenan. Poligeenan is a known carcinogen, but it is not clear how much poligeenan is present in food-grade carrageenan. Another concern is that carrageenan may interfere with the body's ability to absorb nutrients. Some studies have shown that carrageenan can reduce the absorption of iron, zinc, and calcium. Overall, the evidence for the safety of carrageenan is mixed. The US Food and Drug Administration (FDA) has approved carrageenan for use in food, but some scientists and health experts believe that it should be avoided. If you are concerned about the potential health risks of carrageenan, you can try to avoid foods that contain it. Carrageenan is often listed on food labels, but it may also be listed under other names, such as "seaweed extract" or "food additive." Here are some tips for avoiding carrageenan: Read food labels carefully. Choose whole foods over processed foods. Avoid foods that contain seaweed extract or food additives. Make your own food at home whenever possible. If you have any health concerns, be sure to talk to your doctor. They can help you decide whether or not you should avoid carrageenan.

In today's episode I interview Mira Dessy. Mira is known as The Ingredient Guru. She is the author of The Pantry Principle: how to read the label and understand what's really in your food. She is also a speaker, holistic nutritionist, and real food advocate. She emphasizes the importance of a healthy lifestyle through whole foods. After experiencing years of debilitating digestive issues and health challenges, Mira realized the healing strength of food after she learned to eat right, slowly but surely regaining her health. Mira was inspired to share the knowledge she gained on her journey to better health, and now her mission is to help people feel balanced, healthy, and to support them as they work to achieve their health goals. Eat Well, Be Well One of the things we tend to forget is that many of the health conditions that we experience, including the more serious ones like ulcerative colitis or arthritis or diabetes and the not so serious ones like body aches or bloating, are the result of years of exposure to non-food ingredients and not nourishing our bodies the way they need it. It's important to know that every body is unique, exceptional and one of a kind. There is no one-size-fits-all solution. And many times we end up having this low-level unidentified disruption in what is really going on. And the truth is that it's not just what you eat, it's what you absorb. So, if your gut is not in good shape, you are not going to be able to absorb the nutrients your body needs. If you eat foods with no nutritional value, your body is going to have to do something with it before it can get to the things that it actually needs. So, there is this whole paradigm of beginning to recognize that the gut is the key to what's going on with our health and well-being. Our food can be our medicine, but we have to recognize that we live in human bodies our bodies require us to digest the food, absorb the nutrients and metabolize those nutrients and so, if we are missing those steps and putting whatever in our mouths thinking that our bodies will handle it, we are likely to end up with some serious health issues. It's only when we come back and analyze what we are really eating and if our bodies are digesting these foods correctly (and also absorbing the nutrients), when we will see the benefits in our health. One of the things that is so important to keep in mind is that there are so many different systems and processes going on in our bodies but the best place to start is at the foundation. Regardless of what's going on with your body, if your gut is better, you're better. There may be other things like sleeping well or exercising or taking care of your mind but when we start at a foundational level, it provides the body with all of the basic building blocks it needs. After this is done, we can layer in the other things to find out what works best for your individual self. Where to Start The best place to start is in your own pantry. I encourage people to build the habit of reading the label in their foods and very best place to do this is in your pantry, go look at what's already in there. We as humans have our 20 favorite foods and we tend to shop on autopilot. And unfortunately, there are probably things in your pantry right now that you don't want in your body. When you start reading the labels you realize there are things like artificial colors, or artificial flavors in your foods and you definitely don't want to be putting these inside your body, if you want your health to be as good as it can be. I also encourage something called brand disloyalty. Don't buy the brand, buy what you need. This is because companies change, they merge or they get bought, and many times in these processes they don't change the brand, they just change what's in the package. Be loyal to yourself. 5 Things to Avoid First of all, avoid anything that has the word artificial on the label. If it is artificial, it's not real, it is not food and our bodies do not need it. This includes artificial sweeteners, artificial flavors, artificial colors, etc. Another thing to avoid is high fructose corn syrup. It is not good for our bodies, especially for our liver. There are substitutes that we also need to watch out for that are as bad or worse, like agave nectar with higher fructose by volume than high fructose corn syrup. You also want to avoid crystalline fructose because it is just dehydrated high fructose corn syrup. These tend to be in desserts and baked goods. Carrageenan is another thing you want to avoid. This is an emulsifier and thickening agent extracted from red edible seaweeds. The type of process followed to extract it results in a byproduct that is not good for our bodies. It has a negative impact on our gut health, it can cause ulcerative colitis like symptoms in the gut and in laboratory studies it was shown to cause renal bumps. Another sweetener to avoid is allulose. It is found in nature in very small quantities and now it has been replicated in laboratory settings to produce large quantities of it. It is really bad for the gut and makes you bloat. Unfortunately, it has found its way into many keto and paleo diets and protein bars also. Lastly, stay away from anything that is not a real word. If it is just letters like BHT, BHA, EDTA, TBHQ, etc. All those are abbreviations for really long chemical names and these chemicals are used to preserve foods so they can sit in shelves for longer. These are not foods for the body. It is better to stick to ingredients you recognize as foods. So next time you go to the supermarket just pick one category that you want to clean up, like non-dairy milks or canned soups or pickles or condiments. This way you don't have to spend 10 hours at the supermarket reading every single label. Once you have one category cleaned up you can move to the next one the next time you go grocery shopping. Not a One-Size-Fits-All You don't need to have a disease like IBS or Crohn's to be impacted by any of the non-food ingredients in many foods out there. It is true that if you have IBS or Crohn's the symptoms are going to be amplified and you may be in pain and trying to recover for weeks after being exposed to a certain non-food ingredient, but perfectly healthy people can also feel discomfort, bloating, constipation or diarrhea as a result. There is so much information available online that can be conflicting, overwhelming, or just plain wrong for your particular body. Part of the challenge we face is that for many of us it depends on how quickly our system responds. We could have slow motility and so the symptoms would not show for a while, or it could be also a matter of how much we are consuming of a specific ingredient. Also, some things affect certain bodies in different ways than others. In the end, it's a matter of being conscious about what you are eating and how that makes you feel. Each body's response is different and it's our responsibility to keep track. As we previously said, there is no one-size-fits-all solution. The important thing to remember is that no matter what your story is, if you are conscious and you nourish your body, that's always going to be better than not doing it at all. Almond milk seems to be all the rage right now, but is this stuff really something we should be consuming or feeding to our children? Heck no! At present my skin in mostly clear, bloating after ever meal has continued ect. This week we are in the Fitlandia Kitchen with Lizz Hampton, CEO of GOODNUSS, a ground breaking milk-innovation company. Today, Lizz is going to show us step-by-step how to make our own nut milk. (Watch here). The commercial nut milk we buy in stores is typically watered down and pumped with fillers. Instead of getting the nutrients that are naturally derived, synthetic vitamins and preservatives are added by the nut milk manufacturers. While these additives help keep the price down, it is at the higher cost of a product with little to no nutritional value. The only way to guarantee we’re getting all the nutrients out of nut milk is to cold press it ourselves. “If we really want nutrients, we have to drink something that actually has nuts in it,” Lizz explains. “The average commercial nut milk product contains less than 2% nuts.” In stores, many nut milks have carrageenan, which has been linked to ulcerative colitis and connective tissue disorders. Carrageenan is derived from seaweed and has no taste, smell or nutritional value and is added to many nut milks, yogurts, juices, puddings and salad dressings. Many of the vitamins being added to these nut milks are fortified the same way as cow’s milk to mimic taste. Homemade nut milk is fresh and can be customized to our individual taste but it also helps us be completely sustainable. We can use the leftover pulp for baking, desserts or even a homemade face scrub. Here are some clever recipes on how to reuse your pulp: Three Creative Uses: Bread, Hummus, Chocolate "Fluff" Vegan Paleo Crackers Face Scrub Ready to milk your own nuts? Here are the ingredients needed for basic nut milk: Almonds Dates Goji berries Cinnamon Vanilla powder Himalayan sea salt The Basics of How to Milk Your Own Nuts Soak the nuts overnight. This starts the sprouting process and makes the enzymes more bioavailable so they’re easy to digest. This isn’t a necessity but helps break down the antinutrients from the nuts so we can get all the good stuff from them. Throw 1 cup of nuts into blender Add in 3-5 dates Combine equal amounts cinnamon and vanilla (about 1-2 tsp each) 2 tablespoons of goji berries to complement the dates. These berries are loaded with beta-carotene, a pigment that helps promote skin and eye health. Pinch of sea salt to taste Pour in 2-3 cups of water. Texture is customized based on how much water added. Blend 1 cup of almonds makes 24-32 ounces of nut milk. For skimmer milk add about 4 cups of water. Next, we’re going to take our nut milk bag, a nylon mesh bag and strain the milk over a bowl. Zusammenfassung Trotz beträchtlicher Fortschritte in der antibiotischen Behandlung bakterieller Erkrankungen blieb der Krankheitsverlauf und die Sterberate der bakteriellen Meningitis, insbesondere der Pneumokokkenmeningitis, innerhalb der letzten Jahre unverändert. Mit der Erkenntnis, daß das Ausmaß der intrakraniellen Entzündung positiv mit dem Verlauf der Erkrankung korreliert, gewann die Frage nach der Rolle der Leukozyten im Rahmen des Krankheitsgeschehens zunehmend an Bedeutung. Das Ziel der vorliegenden Arbeit war daher, die Bedeutung von Granulozyten, Monozyten und des Zusammenspiels dieser beiden Zellarten im Rahmen der pathophysiologischen Abläufe während der experimentellen Pneumokokkenmeningitis aufzudecken. Insbesondere wurden Veränderungen in den Parametern intrakranieller Druck, Liquorpleozytose und Blut-Hirnschrankenstörung in der Frühphase und im fortgeschrittenen Stadium der Meningitis untersucht. Hierfür kamen zwei Tiermodelle zur Anwendung: 1) Frühphase der Erkrankung: Hierbei wurde narkotisierten Ratten durch intrazisternale Injektion von Hitze-abgetöteten Pneumokokken (HKP) eine Meningitis induziert. Anschließend wurden über einen Zeitraum von sechs Stunden kontinuierlich Blutdruck, intrakranieller Druck und Temperatur überwacht. Eine Stunde vor Versuchsende erhielten die Tiere 1 ml Evans-blau zur Quantifizierung der Blut-Hirnschrankenstörung intravenös injiziert. Nach Ablauf des Beobachtungszeitraums wurden Liquorproben zur Bestimmung der Zellzahl und Evans-blau-Konzentration und Gehirnproben zur histologischen Aufarbeitung gewonnen. 2) Fortgeschrittenes Stadium der Erkrankung (Spätphase): In diesem Modell wurde die Meningitis mittels transkutaner Injektion von Streptococcus pneumoniae Serotyp 3 in die Cisterna magna ausgelöst. 24 Stunden nach Injektion wurden auch bei diesen Tieren die Leukozytenzahl und Evans-blau Konzentration im Liquor bestimmt sowie Gehirnproben zur weiteren Aufarbeitung gewonnen. Um die Beteiligung der Granulozyten an den pathophysiologischen Veränderungen während der Früh- bzw. Spätphase der bakteriellen Meningitis untersuchen zu können, wurden die Versuchstiere mit einem gegen polymorphkernige Leukozyten gerichteten Antikörper (Rabbit Anti-Rat-PMN-Antikörper) vorbehandelt, wodurch eine nahezu vollständige Depletion der Granulozyten erreicht wurde. Um ebenso die durch Monozyten bedingten Auswirkungen während der Frühphase der Pneumokokkenmeningitis feststellen zu können, wurde eine weitere Gruppe mit λ-Carrageenan vorbehandelt, einer Substanz, deren toxische Wirkung auf mononukleäre Zellen bekannt ist. In einer dritten Gruppe schließlich wurden beide Wirkstoffe in Kombination miteinander verabreicht. Für die Frühphase der Pneumokokkenmeningitis ergaben sich folgende Ergebnisse: 1) Die intrazisternale Gabe von Hitze-abgetöteten Pneumokokken führte im Verlauf von sechs Stunden bei den Ratten zu einem Anstieg des intrakraniellen Drucks, der Liquorleukozytenzahl und zur Störung der Blut-Hirnschrankenfunktion. 2) Die Depletion neutrophiler oder monozytärer Zellen bewirkte bei den Versuchstieren eine signifikante Reduktion der Liquorpleozytose und des intrakraniellen Druckanstieges. Gemessen an der Evans-blau-Extravasation wiesen diese Tiere auch eine geringere Funktionsstörung der Blut-Hirnschranke auf. 3) Bei den zweifach-depletierten Tieren waren diese Ergebnisse noch ausgeprägter. Sie zeigten bzgl. des intrakraniellen Druckanstieges, der Liquorpleozytose und Blut-Hirnschrankenfunktion keinen wesentlichen Unterschied zu unbehandelten Kontrolltieren. Für das fortgeschrittene Stadium der Meningitis zeigte sich folgendes: Nach Depletion granulozytärer Zellen ließ sich auch hier eine deutliche Reduktion des intrakraniellen Druckanstieges, der Liquorpleozytose und der Blut-Hirnschrankenstörung erreichen. Allerdings war diese Reduktion weitaus schwächer ausgeprägt als in den vorangegangenen Untersuchungen. Derzeit liegen noch keine Langzeituntersuchungen zur Wirkdauer des gegen polymorphkernige Leukozyten gerichteten Antikörpers vor. Daher ist nur zu vermuten, daß möglicherweise ein zunehmender Wirkverlust des Antikörpers während des Experiments für diese Diskrepanz verantwortlich sein könnte. Unterstützung findet diese Annahme durch den eindeutig höheren prozentualen Anteil neutrophiler Zellen in Differentialblutbildern von Langzeitversuchen verglichen mit denjenigen der Kurzzeitversuche. Da mit Carrageenan vorbehandelte Tiere zum Teil erhebliche Blutdrucksenkungen im Laufe des Experimentes aufwiesen, war es nicht möglich, diese Substanz in den Langzeitversuchen einzusetzen. Zusammenfassend konnte mit dieser Arbeit gezeigt werden, daß Granulozyten, aber auch Monozyten eine essentielle Rolle im Hinblick auf die Ursachen pathophysiologischer Veränderungen während der Früh- und vermutlich auch der späteren Phase der Pneumokokkenmeningitis spielen. Andere Methoden zur Depletion monozytärer Zellen sollten künftig angewendet werden, um die Auswirkungen einer Monozyten-Depletion auf die fortgeschrittene Phase der Pneumokokkenmeningitis genauer untersuchen zu können. Es kommen verschiedene Mechanismen in Betracht, wie Granulozyten und Monozyten zu diesen Veränderungen führen können: 1) Neutrophile sind als Produzenten gewebezerstörender Faktoren bekannt. Ihr Waffenarsenal umfaßt eine Vielzahl toxischer Metabolite, darunter freie Sauerstoffradikale, Stickstoffmonoxid und Enzyme wie Matrix-Metalloproteinasen. In vorangegangenen Studien wurde bereits die Relevanz dieser Mediatoren für die bakterielle Meningitis belegt (z.B. Pfister et al., 1990 a,b; Koedel et al., 1995; Paul et al., 1998). Ohne Mithilfe anderer Mitglieder des Immunsystems sind Neutrophile nicht fähig zwischen fremden und wirtseigenen Antigenen zu unterscheiden; ihre „Waffen“ richten sich in diesem Fall auch gegen den eigenen Wirt. Frühere Studien zeigten, daß im Liquorraum von einem Komplementmangel ausgegangen werden muß und somit hier der zellulären Abwehr die nötige Unterstützung fehlt, um das richtige Ziel der Zerstörung preiszugeben. 2) Monozyten/Makrophagen gelten als Hauptproduzenten von IL-1 und anderen Chemokinen, die als chemotaktisches Signal für Neutrophile dienen. Sie stellen damit unverzichtbare Komplizen und Vorläufer für granulozytäre Zellen dar, da sie wesentlich an deren Immigration in den Subarachnoidalraum beteiligt sind. Ferner könnten mononukleäre Zellen durch ihre Freisetzung von Glutamat direkt an den auftretenden Schäden beteiligt sein.
