Myth #1: GLP-1s Cause Dangerous Muscle Loss The claim: “GLP-1s cause massive muscle loss.” Truth: This is an overstatement. Some loss of lean mass is normal with any kind of weight loss — whether it's through diet, medication, or surgery. What studies show is that with medications like semaglutide (Wegovy) and tirzepatide (Zepbound), about 20–25% of the total weight lost comes from lean mass, and the rest is fat — which is exactly what we're targeting in obesity treatment. That 20–25% figure isn't unique to these meds; it's actually pretty typical in weight loss without focused resistance training or optimized protein intake. You may also hear “You'll lose all your muscle and become frail on GLP-1s.” Truth: You won't “lose all your muscle.” In fact, muscle loss is preventable by maintaining adequate protein intake, resistance training, and managing weight loss pace. Furthermore, many patients gain strength and mobility as excess weight comes off. And lastly, my favorite myth is “You can't preserve muscle on GLP-1s.” Truth: That's completely false — muscle loss isn't inevitable on GLP-1s if you take the right approach. You can absolutely preserve muscle by making a few intentional choices: aim for enough protein each day (a good goal is around 0.8 grams per pound of body weight), include some strength or resistance training a couple times a week, and avoid losing weight too quickly. These simple steps go a long way in protecting your lean mass while still getting all the benefits of weight loss. One study on semaglutide showed that people lost an average of about 15% of their body weight, and only around 3–4% of that was lean mass. So if someone drops 30 pounds, maybe 6 to 8 of those pounds might be lean mass—not ideal, but definitely not disastrous either, and very manageable with the right lifestyle habits. The truth is, while some lean mass loss is expected with any type of weight loss, research shows that most of the weight lost on GLP-1s is actually fat, not muscle. For example, in the STEP 1 trial, about 80% of the weight lost on semaglutide came from fat, and only about 20% from lean tissue (as we mentioned earlier). The SURMOUNT-1 trial with tirzepatide showed similar results—significant fat loss with relatively preserved muscle, especially when paired with resistance training. And that's important, because preserving muscle during weight loss helps protect metabolism, strength, and overall health. With good nutrition and movement, GLP-1s can lead to healthier body composition—not just a lower number on the scale. Okay, moving along to the next myth … Myth #2: GLP-1s Can Cause Blindness The truth: This myth stems from concerns about diabetic retinopathy worsening, which is tied to how quickly blood sugar drops, not to the drug itself. In the SUSTAIN-6 trial (Marso et al., NEJM, 2016), a small subset of patients with pre-existing advanced diabetic retinopathy saw transient worsening—but only in those with rapid improvements in A1c. No increased rates of blindness or new-onset retinopathy have been found in non-diabetic patients using GLP-1s for weight loss. The bottom line is that those without advanced diabetic eye disease, there's no increased risk of blindness. Patients with diabetic retinopathy should be monitored closely—but this is about glycemic management, not a direct effect of the medication. Myth #3: GLP-1s Cause Kidney or Liver Damage The truth: This is false. In fact, GLP-1 agonists may protect kidney and liver function—especially in patients with diabetes or fatty liver disease. The most recent notable study showing kidney‑protective effects of a GLP‑1 receptor agonist is the FLOW trial, which evaluated semaglutide in people with type 2 diabetes and chronic kidney disease (CKD). This double‑blind, randomized, placebo‑controlled trial included 3,533 participants followed for a median of 3.4 years and found that semaglutide reduced the risk of major kidney‑related events—including kidney failure, substantial eGFR decline, and death from renal or cardiovascular causes—by 24% compared to placebo. A 2025 meta-analysis of multiple randomized controlled trials (11 studies, 85,373 participants) concluded that GLP‑1 receptor agonists reduced the risk of composite kidney failure outcomes by 18%, kidney failure by 16%, and all‑cause death by 12%. And let's not forget the SMART trial, involving obese patients with kidney disease but without diabetes, found that semaglutide protected kidney function in this non‑diabetic, CKD‑affected population. When it comes to the liver, there's actually growing evidence they're actually helping reverse non-alcoholic fatty liver disease (NAFLD). The STEP 1 MRI substudy and SURPASS-3 MRI substudy have shown people on these medications can reduce liver fat by 30 to even 50% and in some cases, completely resolve liver inflammation — that more serious form called NASH, where fat is combined with inflammation and early scarring. The LEAN trial found that nearly 60% of people taking semaglutide had resolution of NASH, without worsening their liver scarring. That's huge. And even better, we're seeing these effects even in people who don't have diabetes. Just losing weight helps fatty liver, but these meds seem to do more than that — they actually target inflammation and fat storage in the liver itself.. The bottom line is GLP-1s are not nephrotoxic or hepatotoxic. In fact, they may be organ-protective—especially for people with underlying metabolic issues. Myth #4: These Drugs Lead to Bone Loss The claim: “You'll get osteoporosis from losing too much weight!” The truth: While extreme weight loss can affect bone density, GLP-1s themselves do not cause bone loss, and may even have neutral or protective effects on bone. A 2022 study in Bone found no significant change in BMD (bone mineral density) in adults treated with semaglutide for obesity. While the SUSTAIN and PIONEER programs found no increased risk of fractures in semaglutide-treated patients versus placebo. Truly, concerns about bone loss are more relevant in extreme calorie restriction or eating disorders—not evidence-based GLP-1 treatment with appropriate nutrition. Myth #5: Everyone Gets Gastroparesis The claim: “These medications paralyze your stomach” The truth: GLP-1s slow gastric emptying, which is part of how they work—making you feel full longer. But this is dose-dependent and typically reversible. A 2023 FDA safety review found that true gastroparesis is extremely rare and resolves when the drug is stopped. Reality check: Nausea, early satiety, and mild bloating are common but manageable side effects. True, lasting gastroparesis is not typical, especially when doses are titrated gradually. Myth #6: GLP-1s Make Your Hair Fall Out The claim: “You'll lose a ton of hair—just like with crash diets” The truth: Hair shedding is not directly caused by GLP-1 medications. Instead, it's often a temporary, non-scarring condition called telogen effluvium, which can happen with any rapid weight loss, regardless of the method. A 2023 analysis from the American Academy of Dermatology emphasized that telogen effluvium is common with surgical or medical weight loss, especially if patients lose more than 10% of their body weight within a few months. In clinical trials like STEP and SURMOUNT, hair loss was not listed as a common side effect, but patient-reported data show it occurs occasionally—likely tied to nutritional stress, not the drug itself. So why does hair loss happen? We've talked about this before, but I don't want to leave this important information out. Hair follicles are sensitive to internal stress. Rapid changes in caloric intake, nutrient levels (like iron, zinc, and biotin), or hormone balance can push hairs into the shedding phase. This is a delayed effect, often showing up 2–3 months after weight loss begins, and it typically resolves within 6–12 months. What helps is slower, sustained weight loss, prioritizing protein intake, supplementing iron, zinc, and biotin if deficient, and avoiding very low-calorie diets and over-restriction. Myth #6: GLP-1s Cause Dehydration It's a common myth that GLP-1 medications cause dehydration — but that's not exactly true. The medication itself doesn't directly dehydrate you. What can happen is that some people experience nausea, vomiting, or a reduced appetite early on, which can lead to drinking less water without realizing it. That's where the dehydration risk comes in. A good general rule for staying hydrated is to aim for half your body weight in ounces of water per day. So, for example, if you weigh 160 pounds, try to drink around 80 ounces daily — more if you're active or live in a hot climate. Electrolytes can also be really helpful, especially if you're feeling tired, dizzy, or crampy. I like LMNT packets — they're a clean option with no sugar and a good balance of sodium, magnesium, and potassium. The sodium in LMNT packets helps keep you hydrated by pulling water into your cells and helping your body retain the fluids it needs to function properly. Just one a day can make a big difference in how you feel. Myth #7: You Have to Stay on GLP-1s Forever or You'll Gain All the Weight Back The claim: “As soon as you stop taking it, all the weight comes back” The truth: Yes—some weight regain is likely after stopping GLP-1 medications. But that doesn't mean they're ineffective or that you're doomed to rebound completely. The same pattern happens after any type of weight loss intervention, whether it's a diet, surgery, or medication. The STEP 4 trial (Wilding et al., 2022) showed that participants who stopped semaglutide after 20 weeks regained an average of 6% of their weight loss over the next year. But it's important to note that they still weighed less than at baseline—and many continued to experience improvements in blood pressure, cholesterol, and insulin sensitivity. Similarly, in SURMOUNT-4, patients who stopped tirzepatide also regained weight, but less than they lost. So why does this weight gain happen? I feel like the answer to this is obvious, but I've found that it's not. GLP-1s change your appetite and hunger cues. Once the medication is stopped, your body's baseline hunger signals return—and often with increased intensity, due to metabolic adaptation. But this isn't unique to GLP-1s. The same thing happens after crash diets, keto, intermittent fasting, or bariatric surgery if long-term changes aren't made. The real issue isn't the drug—it's the lack of a plan after the drug. To help make results sustainable, we need to use the medication as a tool, not a crutch. We should use it to help us lose weight and understand our hunger cues, while transitioning to a whole foods, protein based diet coupled with resistance training to help preserve and build muscle. Just remember, if you're coming off a GLP-1 and want to keep the momentum going, the key is to approach it thoughtfully. Tapering slowly under medical supervision can help your body adjust and reduce the chances of weight regain. At the same time, this is a great moment to double down on the habits that helped you feel your best while on the medication. Think ongoing support—like working with a health coach, joining a support group, or even doing behavioral therapy—to help reinforce those long-term lifestyle changes. It's not just about what you stop; it's about what you keep doing that matters most. You don't necessarily have to stay on GLP-1s forever—but if you stop without a plan, some weight regain is very likely. Think of them like glasses: they help you see clearly while you build the habits to eventually navigate without them. For some, that may mean staying on a lower maintenance dose long-term—just like with blood pressure or cholesterol meds. What are my final thoughts? I want to be clear—GLP-1s aren't magic. But they are powerful tools when paired with education, support, and smart lifestyle changes. Myths like ‘you'll go blind,' ‘you'll lose all your hair,' or ‘you'll be stuck on these meds forever' aren't just misleading and downright false—they discourage people from getting real help. TEAM for Troubled Couples A New Twist! Today we are joined by a favorite guest, the brilliant Thai-An Truong. Thai-An is a Licensed Professional Counselor (LPC) and Alcohol and Drug Counselor (LADC). She is the first Certified TEAM-CBT Therapist and Trainer in Oklahoma. She has found TEAM-CBT to be life-changing professionally and personally and is passionate about training other therapists in this “awesome approach.” In her private practice, Thai-An specializes in the treatment of trauma and OCD. To learn more about her TEAM-CBT Trainings, visit www.teamcbttraining.com Thai-An has been featured on many Feeling Good Podcasts focusing on Depression and social anxiety (Live demonstration, 187) Postpartum Depression and Anxiety ( 218) How to Get Laid (Ep. 264) OCD ( 283) Grief (Ep 344) Now Thai-An adds an important dimension to the TEAM Interpersonal Model—working with trouble couples, as opposed to working with individuals with troubled relationships. She also describes a new way to use Positive Reframing to reduce patient resistance to giving up David's famous list of “Common Communication Errors,” and she adds five new errors to the list. At the start of the podcast, Thai-An described a woman who complained that her husband often “shuts down” when they are communicating about a sensitive topic, and she wondered why. Thai-An decided to invite him to join the session so his wife could find out why. This really opened things up, and the wife discovered that her husband shut down because he was feeling inadequate when she pointed out all the things that were wrong with the house, and he was taking her comments as criticism. However, the more he shut down, the more she complained, and this pushed him away even further since her criticisms intensified his feelings of inadequacy. Thai-An then used Positive Reframing to help her see why he shut down. One of Thai-An's new ideas was to use Positive Reframing to cast our list of “errors” on the “Bad Communication Checklist” in a positive light, just as we do with the negative thoughts and feelings of people who are using the Daily Mood Log. By siding with the patient's resistance and listing all the good reasons NOT to change, nearly all patients paradoxically let down their guard and powerful urges to oppose change. Instead, they open up and become receptive to the many methods for challenging distorted thoughts. Thai-An has observed the same phenomena with troubled couples. When they see the GOOD reasons to why they or their partners use dysfunctional ways of communicating, they paradoxically let down their guard and become more willing to use the Five Secrets of Effective Communication. She says: Positive reframing started to open them up to each other, and helped them see each other in a more positive light. At the same time, they discovered that they shared the same values. Voicing the good reasons to maintain the communication errors as well as the cost of change (e.g., it'll be hard work, I'll have to focus on changing myself, it'll be vulnerable) allowed each partner to melt away their resistance to change. David comment: This is an excellent example of a “double paradox.” Once again, instead of trying to “help,” which often triggers intense resistance, the therapist sides with the resistance, and this paradoxically triggers strong motivation to change! Thai-An reminded us that it's important to go through the TEAM structure before moving forward with tools to help the couple change. For testing, she asks both partners to complete the version of David's Brief Mood Survey that includes the Relationship Satisfaction Scale, and asks both to complete the Evaluation of Therapy Session at the end. She makes sure both partners rate her empathy toward them at 20/20 (perfect scores) before proceeding to the next steps. During the Assessment of Resistance, she begins to work with David's Relationship Journal to get a specific moment in time of conflict. Then when they do Steps 3 and 4, where they identify their own communication errors and their impact on their partners, she does positive reframing of the bad communication errors, which you can see here, along with five new errors that Thai-An has listed below. The Bad Communication Checklist* Instructions. Review what you wrote down in Step 2 of the Relationship Journal. How many of the following communication errors can you spot? Communication Error (ü) Communication Error (ü) 1. Truth – You insist you're "right" and the other person is "wrong." 10. Diversion – You change the subject or list past grievances. 2. Blame – You imply the problem is the other person's fault. 11. Self-Blame – You act as if you're awful and terrible. 3. Defensiveness – You argue and refuse to admit any imperfection. 12. Hopelessness – You claim you've tried everything and nothing works. 4. Martyrdom – You imply that you're an innocent victim. 13. Demandingness – You complain when people aren't as you expect. 5. Put-Down – You imply that the other person is a loser. 14. Denial – You imply that you don't feel angry, sad or upset when you do. 6. Labeling – You call the other person "a jerk," "a loser," or worse. 15. Helping – Instead of listening, you give advice or "help." 7. Sarcasm – Your tone of voice is belittling or patronizing. 16. Problem Solving – You try to solve the problem and ignore feelings. 8. Counterattack – You respond to criticism with criticism. 17. Mind-Reading – You expect others to know how you feel without telling them. 9. Scapegoating – You imply the other person is defective or has a problem. 18. Passive-Aggression – You say nothing, pout or slam doors. * Copyright ã 1991 by David D. Burns, MD. Revised 2001. Thai-An Truong's 5 Additional Communication Errors: Shut down—You shut down and ignore the other person or give them the silent treatment. Avoidance—You hide your feelings and avoid talking about hard topics, or disconnect through some form of escape. Rejection—You make threats to leave – “I'm done with you,” or “I can't deal with this anymore,” or “I want a divorce.” Control—You insist that the other person “needs” to behave or communicate differently, or “should” or “shouldn't” behave the way they do. Invalidation—You tell the other person they shouldn't feel the way they feel. Here's how Thai-An did the Positive Reframing with this couple. First she asked the wife, “Why might your partner suddenly want to “shut down” and stop communicating during a conflicted exchange?” She also asked, “What does this do for the person who is shutting down?” This is the list of positives they came up with. Shutting down . . . Keeps me safe and protects me from more criticism Protects my partner from hurtful comments I might make. Shows that I value our marriage and my partner's feelings. Shows my love for my partner, and for myself. It shows that I'm feeling hurt and want to be appreciated. Guarantees that I won't make things worse. Shows that I want to protect myself from becoming overly vulnerable and getting invalidated again. Shutting down feels less risky than sharing my feelings. Once she saw why he shut down, she realized the negative impact of her complaints, and began to provide more genuine words of appreciation to him. He said that this meant so much to him and made all the hard work worth it. Her common communication errors included “truth” and “making complaints.” He realized, again through positive reframing, that she also wanted validation, that raising children can be hard, and that she ALSO wanted appreciation for how well she was keeping up with the home and the care of their children. So, when she wasn't getting validation and appreciation from him, she was even more likely to complain to try to voice her perspective. Once he was able to stop shutting down, and instead began to make more disarming statements, use feeling empathy, and stroking, she was much less likely to complain. They also realized they had the same values of wanting healthier communication and to provide a safe and happy home for their children. Was this effective? Both went from 10/30 and 11/30 on the relationship satisfaction scale (shockingly poor scores) to 26/30 by the end of the relationship work together (extremely high scores indicating outstanding scores on my Relationship Satisfaction Scale.) Thai-An provided us with a cool Positive Reframing document for all of the communication errors. You can check it out if you CLICK HERE. I (David) pointed out that Positive Reframing can also be used in conjunction with the Relationship Journal in another way. In step one of the RJ, you write down one thing the other person said, and you circle all the many feelings they were probably having, like hurt, alone, anxious, angry, sad, unloved, and many more. In step two you write down exactly what you said next, and circle all the feelings you were having. This would be an ideal time to do Positive Reframing of your partner's negative feelings, so as to shift you perception that the other person is “bad” or “to blame” or some negative interpretations that you may be making. This reframing might be helpful in the same sense that my technique, Forced Empathy, can sometimes cause a radical shift in how you see the person you're at odds with. Mama!Overeating isn't something that I recommend because it doesn't leave you feeling good, but the extra calories consumed aren't as big of a deal as some people make them out to be! One night of bad eating won't ruin a week, a month, or a year's worth of hard work!What does hurt your health is going back to old habits and thoughts that haven't been serving you for years! Instead of falling prey to these three myths, I want to give you new truths to lean into and use so that you don't have to feel guilt, shame and regret when you overeat!Myth 1: You have to workout harder to burn off the bad food you ateRaise your hand (or tell me in the comments) if you've ever guilted yourself into going to the gym or doing double workouts because you overate the day before - or so that you could overeat?I've been there too, mama! I used to believe that I HAD to hit the gym extra hard to make up for my bad eating. But do you know what that belief did to me? It made me hate the gym! And I love a good workout! For those of you who already struggle with working out, that isn't going to make building healthy habits any easier!Truth: You have extra energy waiting to be put to good use:Instead of making working out a punishment, make it something you do to feel better! Because you ate more, you're going to have more energy to burn, which makes this an opportunity to hit a new goal!If you're a runner, see if you can hit a new mile PR. If you like to lift, see if you can up your weights a little! Whatever you want to do for exercise, try to push yourself just a little further than your comfort zone! You might even be amazed when you realize how good your workout feels the day after consuming some extra calories!Now, you might be thinking, isn't pushing myself to work harder the same as the myth, working harder to burn off the extra food? And that's a totally valid question! The difference between the myth and the truth is how you think and feel about it!With the myth, you believe that you've done a bad thing (making you a bad person) and you need to punish yourself for that. But, with the truth, you're accepting the choices you made and turning a situation that could have brought you down into an opportunity to succeed!How you think about your actions is so, so, so important!! I talk about mindset with my Macros Made Easy mamas all the time because how we think and feel about ourselves and our actions is the difference between success and failure!I would still encourage you to put that extra food to use by moving your body, but I want you to do it in a way that builds a better relationship with healthy habits, like exercise! If that means you take the family ice skating the next day to get extra movement in, awesome! If that means you add 10 minutes to your workout because you feel like you're crushing it at the gym, great! Do whatever feels good for you!Myth 2: You should skip meals to make up for itPlease, please don't restrict yourself tomorrow because you enjoyed some food today!! This is the recipe for binge eating which doesn't help you lose weight.Binging like this sends your body into stress mode. It doesn't know when it's going to get enough food, so instead of burning the extra calories that you consumed the day before, it holds onto them causing you to gain weight! It's a vicious cycle that never serves you!Truth: Get back to your normal asapWhat your body actually wants from you is consistency. It wants to know what it can expect from you each day. When you eat a consistent amount every day your body can leave that fight or flight stress behind and it will do what it's meant to do!Start your day with a protein-packed breakfast. Make smart food choices during the day. Eat the same amount that you normally do! If you do these three things your body will realize that it just got a little extra energy and it will take care of it on its own!Your body was literally built to process food into energy so allow it the chance to do that! You may see the scale go up for a day or two, but as you return to your normal eating patterns, the weight will come off!Myth 3: You need to avoid carbs since you overateOh, carbs. We are so mean to carbs! We've made them out to be the villain that is solely responsible for our weight gain. But carbs are absolutely necessary for sustaining life!If you grew up in a generation where you were told carbs were terrible for you and would make you fat, then you probably are going to feel a lot of resistance to the idea that you need carbs to live a healthy life.It's important that you work on that thought process because carbs are going to be your body, and especially your brain's, first source of energy! Limiting them can make you feel more sluggish the next day!Truth: Eat a normal amount of carbsAnother truth that is probably going to blow your mind, is that overeating in carbs occasionally can actually help you lose weight!For real!! I will use this tool with my clients to help them get over a hurdle in their weight loss journey. It's called a refeed. What I typically do, is give my clients an extra 20-30 carbs for one day. When they weigh up the next day they might see the scale go up a pound or two because carbs attract water and it causes some slight inflammation.But then, over the next few days, they will hit their lowest weight yet. This works is because when you're eating in lower carbs and then increase them your metabolism revs up and uses the quick burning energy to power your life!Hardly anyone believes me when I tell them this, but they do their refeed and then 2-3 days later they freak out when the scale drops! It's the best thing.Just because you were taught something about nutrition growing up, doesn't mean it's necessarily true! And most of the myths we talked about today are proof of that!I help busy mamas learn about nutrition on a personal level so that they can lose weight and have the energy to keep up with their kiddos. The full salvation by Christ's hand changes how we look at our relationship to Him, our neighbors, and our government. Everything is wrapped around the Love of God to His people in Christ and in response, He sends His Holy Spirit so that we live a holy life of love of Him and to others. Jesus' confrontation with the Pharisees concerning the Christ reminds us to continuously go back to the Scriptures for our understanding of Him and to go back to Christ's salvation for our hope. “Lord Jesus, Your Word is Truth—You are the Truth. Call us out for sin and continuously bring us back to You for our hope. Reform our lives that we may love the LORD with all of our heart, soul and mind and love others as ourselves. O Holy Spirit, enter in. Amen”

Empath: You have care and concerns for others but don't have enough boundaries, which turns into enabling and people-pleasing. Asking Questions: You're starting to come to terms with the fact you're not going to get answers to some of your questions.Connecting the Dots: Researching your past to understand why you're feeling what you're feeling. Occasionally Create & Inspired: You enjoy it while you're doing it, but you don't recognize its full potential. Aware of Manifesting: Gaining an understanding of manifesting and its power. A Desire to Learn: You're learning to learn. Interested in Change: Acknowledges that change is required.Recognizing Triggers: You're observing your emotions and adjusting your behavior.Searching for Peace: You're ready to settle your mind.Believes there is a Purpose: Wants to find their purpose.Notices Toxic Behavior: Finds it difficult not to call out inappropriate behavior. Seeking Challenges: Pushing yourself to get out of your comfort zone.Holding Onto Relationships (98's) Hasn't grasped that you must let go of 98's to reach higher heights.A 298 that doesn't continue to grow remains a 298, which is equal to a 98. Self-Discovery is leading you to your Self-Love, but it requires discipline. You are reprograming your mind, creating new neural pathways. You are gaining strength in your peace which will keep you moving forward to becoming the awakened, the 2%. 2's are here to reinforce and guide the willing 298's to become awakened. The 2% know what to expect because they've done the work and continue to do it. It's their purpose to lead you to your inspiration, creativity, and your higher purpose.
