The Joint Readiness Training Center is pleased to present the ninety-fifth episode to air on ‘The Crucible - The JRTC Experience.' Hosted by the Task Force Senior for the TF Sustainment (BSB / CSSB), LTC Bruce Roett on behalf of the Commander of Ops Group (COG). Today's guests are two seasoned medical professionals with TF Sustainment (BSB / CSSB), CPT Victor Velez and SFC Jason Kohne. CPT Velez is the Senior Medical Operations Officer OCT and SFC Kohne is the Medical Operations NCO OCT. In this episode, we examine the critical role of medical operations during large-scale combat operations (LSCO), emphasizing the complex realities faced by medics in high-intensity environments. The conversation underscores the physical, emotional, and tactical demands placed on Army medical personnel who must operate with minimal rest, high casualty volumes, and limited evacuation windows. The discussion highlights the need for rigorous, realistic pre-deployment training—such as TCCC, paramedic certification, and stress-inducing field exercises like “Gator Top Dog”—to prepare medics for the brutal realities of sustained combat. A key theme is the importance of leader involvement, particularly from NCOs, in building emotional resilience, monitoring burnout, and creating a culture of trust where medics can ask for help and remain mission-effective throughout the fight. The episode also delves into key best practices for organizing and supporting medical operations across echelons. Notable lessons include the effective use of FLA (field litter ambulance) staging forward with infantry battalions, use of Class VIII speedballs during casualty evacuation, and the importance of a synchronized and well-communicated medical concept of support. Dr. Kerl is pleased to welcome Dr. Danny Joffe, Vice President of Medical Operations for VCA Canada. Listen in as Dr. Joffe shares his journey in veterinary medicine, from deciding to become a veterinarian in second grade to opening his own practice. He discusses the importance of internships in shaping a veterinarian's career and the value of continuing education and certification. Dr. Joffe also highlights the initiatives and programs implemented in VCA Canada, such as the feline declaw ban and the support program for veterinarians experiencing patient safety events or board actions. He also emphasizes the importance of veterinary technicians and their vital role in providing high-quality care.Dr. Joffe graduated from the Western College of Veterinary Medicine in Saskatoon, Saskatchewan (with Great Distinction) in 1985. He completed a small animal medicine and surgery internship at Cornell University in 1985-86, and then entered private practice, eventually starting his own small animal practice, Landing Animal Clinic, in Calgary Alberta in 1989, practicing there until 2020. He was a founding partner in a multi-specialist referral and emergency clinic (Calgary Animal Referral and Emergency Centre—C.A.R.E. Centre) in 2006 and was the Medical Director for this 30-doctor facility until the fall of 2020. In the fall of 2012 Danny became the National Medical Director for VCA Canada, Canada's largest group veterinary practice, and in 2018 he became VCA Canada's Vice-President of Medical Operations. He completed his board certification by the American Board of Veterinary Practitioners (Canine/Feline Specialty) in 1992, becoming the first practicing veterinary specialist in Alberta. He has re-certified this specialty via examination in 2000 and 2010. In 2020 Danny was granted Emeritus status by this specialty group. Danny is a Past President of both the Alberta Veterinary Medical Association and the Canadian Academy of Veterinary Dermatology. Covid-19 is the most significant global health event to take place in our lifetime. Since Lili settled in Michigan in 1992, she held various bedside RN positions ranging from rehabilitation, and med surg to Trauma Neuro intensive care. Her leadership positions as Manager and Director span from Nursing Departments, Care Management, Utilization Management, Home Care, Patient Care Logistics, and Throughput to third-party payer Medical Operations and Prior authorization divisions. In addition to these leadership roles, she has worked on the Tur-State IHI projects related to decreasing acute-level readmissions. Overall, Lili is extremely passionate about patient-focused healthcare services and finding opportunities to improve our healthcare system today. In her personal time, she enjoys spending time traveling with her husband Mark, and two daughters Jelena and Tiana.

Army brass have been pursuing a modernizing strategy for some time now via the Army Futures Command. It has focused on doctrine and lethality and the systems to keep the Army out front. But the Futures Command is also looking at modernizing another component where the Army has historically innovated. Namely, its medical operations. For an update on that, Federal Drive host Tom Temin spoke with the chief of medical readiness and integration, Col.Mike Mansell.
Cathy Meeks, MS, DVM, DACVIM, started her veterinary career as a veterinary technician while getting her master's degree in veterinary medicine with a focus on forensic toxicology. After veterinary school, she did an internship and then a residency in internal medicine. Her specific interests in internal medicine include immune mediated diseases and endocrinology. In 2012, Meeks took on a lead veterinary role in a private practice as a medical director while continuing to practice internal medicine. In 2015, she became a Group Medical Director for Bluepearl, working with the hospitals in the Northeast. Until 2019 when she was promoted to Regional Vice President of Medicine where she managed 15 hospitals in the Northeast. In 2021 she joined Antech as the Vice President of Medical Operations providing medical leadership throughout various areas in the company including clinical and anatomic pathology, quality assurance and internal consulting. When you talk about the millennial generation a lot of them want some kind of fulfillment out of the work that they're doing and not feel like they're just gonna be a cog in the machine. In September of 2019 we introduced you to the Eisenhower Series College Program. Members of the Eisenhower Program began the year on the road visiting colleges and universities, interacting with audiences often unfamiliar with members of the U.S. Military. Unfortunately the DOD's Travel Policy, as a result of the ongoing COVID-19 Pandemic, has curtailed the Spring schedule for the program. It is our hope at WAR ROOM to bring you a glimpse of what some of those presentations might have looked like via A BETTER PEACE: The WAR ROOM Podcast. The first three episodes discussed diversity and inclusivity in the military and social media's impact on national security and technology's role on the battlefield. In this fourth and final episode of the series A BETTER PEACE editor Ron Granieri is joined by three members of the U.S. Army War College AY20 resident course Aaron Sadusky, Eric Swenson and Melissa Wardlaw. The four of them discuss the relationship between higher education in the United States and the military. Their conversation ranges from compulsory national service, to the impact of current education standards on the military recruiting pool and the all volunteer force, to a free 13th and 14th grade. Aaron Sadusky is a Lieutenant Colonel and a Field Artillery Officer in the U.S. Army. Eric Swenson is a Colonel and an Engineer in the U.S. Army. Melissa Wardlaw is a Lieutenant Colonel and a Medical Operations officer in the U.S. Army. All three of them are graduates of the AY20 resident class at the U.S. Army War College. After a year at NASA Headquarters he left the space agency to serve as Provost for International Space University, Strasbourg, France.Upon returning to the USA, he consulted for The RAND Corporation. A founding board member of the American Telemedicine Association, Dr. Logan has served as a telemedicine consultant to a variety of professional organizations, international and domestic hospital-based health care systems and the Department of Defense. Dr. Logan returned to NASA's Johnson Space Center in 1999 and served as Chief of Medical Informatics and Health Care Systems, Chief, Dive Medicine Board, Medical Director of NASA's Neutral Buoyancy Laboratory (NBL) as well as senior aerospace medical officer in the Clinical Services Branch of the Space Medicine Division. The study that they put together truly had some eye opening information and we really had a great conversation about these important and necessary topics. Dr. Samantha Dutton is currently a Program Dean and the Director of the Social Work Program in the College of Humanities & Sciences, where she is creating the Bachelor of Science Social Work program. The program is currently in the accreditation process. Previous to this she held positions of Deputy Commander of Medical Operations at Mike O’Callaghan Military Medical Center as well as the Medical Squadron at Joint Base Lewis-McChord. She also commanded the Mental Health Clinic at Nellis Air Force Base, Nevada. Her military experience spans 27 years with increasing levels of leadership. She has been the recipient of numerous Air Force level awards and was deployed in support of Operation Iraqi Freedom and Operation Enduring Freedom where she was the lone mental health practitioner for 2500 personnel. Dr. Dutton’s passions include military transition, single parents serving in the military and ensuring veterans have access to services. She has spoken in different forums surrounding these passions as well as publishing an article on single parents in the Air Force. Dr. Dutton created the military social work curriculum for another online institution where she brought real world experience and research to the program.Dr. Dutton is married and has 4 children and a granddaughter. And yet achieving them isn’t so simple, especially at large health systems. It requires a number of underlying system improvements, including well-designed hospital flow. This has been a focus for health care for the past few decades, often prompted by bottlenecks and overcrowding in emergency departments (EDs). Things are getting better in some places, but there’s definitely a ways to go. IHI’s work on hospital flow dates back to the early 2000s. That’s when we and hospital teams started learning about the ways most hospitals scheduled their operating rooms, practically ensuring backups and delays there and in EDs because of the frequent mismatch between planning for emergency surgeries and elective ones.
