Meet our guest, Dr. Brent Jackson, the Vice President/Chief Medical Officer at Mercy General Hospital, a Dignity Health Facility in Sacramento, California. Dr. Brent had been a Practicing General Surgeon for 15 years before moving to administrative roles. In this episode, I sit with Dr. Jackson and discuss the healthcare industry in regards to burnout, the importance of taking time to recharge outside work, as well as dynamics of teamwork, and defining quality metrics. Top Takeaways [00:20] With a positive mindset, it's amazing what people can accomplish when they put their minds to something. [01:45] Burnout is insidious, and the deeper you get in it, the more blind you get; this was the major reason behind Dr. Brent's move from clinical practice into an administrative role. [07:00] Losing the autonomy of private personal practice was one of the factors responsible for burnout. If you reflect and see that you've become your career, you're probably on your way to burnout. You need to take time away from your career and develop outside interests. [14:30] The definition of a team is a small group of individuals with a shared goal who have mutual trust and accountability. When you bring a team together, you need to focus on developing relationships with them and encouraging them to develop relationships with one another. Also, often allow for shared decision-making, then they have ownership of it. [16:53] Best 'Aha' moment: While taking time off work, Dr. Brent had realized he always wanted to do an MBA, and finally had time to do it. [23:35] Current changes in healthcare: Error prevention is becoming critical, with new high-reliability organizational principles and tools being implemented. [28:40] The healthcare industry needs to be more geared towards defining quality for the upcoming quality leaders. We need to spend more time finding good metrics for defining good quality. [32:25] Newsletters and giving awards to recognize efforts and inspire. [36:00] Best career advice: Listen more and ask more questions. [37:57] Personal habit for leadership success: When trying to solve a problem, it is important to ask 'who are the right people to pool together and what are the right questions to ask?' Brennan and Gale found that the ecosystem has more players than ever.A good mix of organizations attended, including Amazon, AARP, Fidelity, AHA, IBM Watson, Maven, Philips, ResMed, Teladoc, Verizon, Vertex, CVS, Walmart, Zoom, Optum, Oak, Cigna, Aetna, Humana, and the Blues+.HLTH orchestrated an indoor event of 6000+ attendees while ensuring that everyone in attendance was vaccinated or had tested negative within the last 72 hours of the conference. They made COVID-19 testing accessible in the lobby. The reinvention of primary care Primary care is attracting heated discussions because many of the core themes in healthcare transformation are coming together in primary care.Amazon Care is expanding its virtual and hybrid primary care service beyond its employees, offering other employers a covered benefit in selected cities.UnitedHealth is rolling out a virtual-first primary care product, NavigateNow, to select employers with self-funded plans in nine markets, expanding to 25 markets by the end of 2022.Primary care is a strangely heated space. This year, Rushika Fernadopulle of Iora Health clashed with serial entrepreneur Adrian Aoun of Forward about tech-based primary care. Adrian equated the entrance of pharmacy retailers to "wanting an iPhone and being sold a Motorola."Our hosts found that Adrian came across as disconnected from the reality of primary care and that retailers like Walgreens, CVS, and Walmart have advantages like a massive physical presence, logistical network, longitudinal understanding of consumers, and high net promoter scores. Digital health, of course Digital health has always been the focus of HLTH, but this year was less about an incoming digital revolution and more about the ways different players collaborate and integrate solutions.Walmart Health put it well: It's not just about going digital; it's about having different mechanisms for different consumer preferences.Hybrid care was widely recognized as a critical element in appealing to consumer preferences and building trust.Much of the conversation was focused on chronic care management and wellness, with Noom, Thirty Madison, Big Health, and Everly Health all present. The goal of digital tools in this space is to become a reimbursed benefit.If healthcare delivery wants to transform healthcare delivery, organizations need to take the lead. To lead healthcare delivery, organizations need to change their business model from a pipeline to a platform where all players can connect to your health system.Plans + employers + digital health As employers take a more proactive role in workforce health, plans seem to be reinventing care delivery using digital health and the workplace to do it. As Divya Paliwal, Chief Clinical Transformation Officer of Horizon BCBS said, "How can we use these dollars better than shelling out to a hospital ED or ICU?"The drivers of the growth in digital health are plans and employers. And employers are moving away from PPO, and high deductible plans to incent healthy behaviors – the key focus of digital health tools.In a world of remote work, benefit offerings become even more important. When features of physical workspace become irrelevant, benefits are essential in recruiting and retention.Employers are certainly stepping up to improve access to health solutions through employee benefits, but this only intensifies the division between the "haves" and "have nots" by excluding small businesses and the unemployed.

Dr Alan Desmond, a plant-based gastroenterologist from the UK, is fascinated with the diet that is most likely to improve the gut health of his patients, especially those with gastrointestinal problems like inflammatory bowel disease. What’s the best way to speak when helping people understand the benefits of a plant-based diet?   This week’s New Normal Project podcast guest, Dr Scott Stoll, has been thinking about this question for a long time. After representing the USA in the Olympic bobsled, Scott completed his medical specialist training in Physical and Rehabilitation Medicine, and then around 15 years ago found he did not have the answers to all of his patient’s health woes. He read widely and came to recognize that a plant-based diet, especially whole-foods plant-based nutrition, had an excellent scientific basis for improving human health. As his family, including his children, transitioned to this diet, he realized that his patients would likely benefit from this too. What has followed is a radical transformation of both his medical practice and his family. His wife and 6 children, like Scott himself, are vibrantly healthy and thrive on this way of living. Since then Dr Stoll has become a leading light of the plant-based nutrition movement, being a co-founder of the Plantrician Project, an organization aiming to educate doctors and other healthcare practitioners about the benefits of plant-based nutrition. The Plantrician Project runs the annual International Plant Based Nutrition Healthcare Conference, which is simply amazing for it’s quality of speakers and information. Dr Stoll has spent a lot of time thinking about ways to influence both patients and healthcare practitioners, especially with regards to changing behavior, and therefore has many valuable insights to share in this episode. Is heart disease from clogged arteries a food-borne disease? Can eating the right foods really reverse heart disease? Which foods have a powerful antioxidant effect in your blood vessels?   Dr Caldwell B. Esselstyn Jr is a superhero in our family. And this American surgeon who turned his career on a head to become a provider, researcher, advocate and international expert of a wholefoods, plant-based nutrition to prevent and reverse heart disease, is this week’s special guest. Dr Esselstyn grew up on a beef farm, won a Gold medal at the Olympics with the United States rowing crew as a university student and began his medical life as a surgeon. He then had an important mid-career epiphany, realizing that cutting out the cancer of an individual person was less valuable than understanding why some populations had extremely low rates of both cancer and heart disease. When he learned these healthier populations ate a predominance of plant-based wholefoods he began eating that way himself. He then took a group of patients with very severe heart disease and taught them to eat a wholefoods plant-based diet without oils as part of a formal research study. These patients had dramatic reversal of their heart disease symptoms, lived longer than they were expected to, and a powerful nutritional intervention for heart disease was launched. Caldwell then published this research, wrote the book “Prevent and Reverse Heart Disease” and has since devoted his life to directly helping hundreds of patients with heart disease, and to indirectly influencing thousands of patients (and doctors like me) to adopt the same way of eating. One of those people is Claire’s aunt, Dulcie, who after adopting the diet in “Prevent and Reverse Heart Disease” had a near overnight cessation of the frequent chest pain her blocked heart arteries was causing. This inspired Claire and I to adopt a plant-based diet which we haven’t looked back from. Many of you may have heard Dr Esselstyn on a different podcast or in a documentary. You may have read his book too. What has Claire been up to lately? And what is her New Year’s resolution?   In this episode, Claire is back on the show! Early in 2018, my wife and co-host Claire Davies announced she was taking a break from the podcast. This is only a temporary appearance and you should enjoy the opportunity of hearing what Claire has been doing over the past year and just what her ongoing passion is. Claire is an amazing human being. After working as a nurse for over 2 decades, the last decade as a working Mum, she took some time off to help get the New Normal Project off the ground. Her calm, measured and wise way of being brought a valuable voice and philosophy to this podcast, something that many of you have fed back to us. But after a year of speaking on the podcast about the plant-based lifestyle we adopted, and how much healthier and happier we both feel for living this way, Claire realised this was neither her true purpose nor the best use of her skills and experience. It shocked me when she said she no longer wanted to work on the podcast, at least for the time being, but having seen the growth and fulfilment she has enjoyed over the last year, I now see why she made that decision. Claire has begun a career in clinical pastoral care. In 2015 Andy co-founded the global lifestyle phenomenon OneYearNoBeer.com, a 28, 90 or a 365-day alcohol-free challenge to help transform people's relationship with alcohol. His first best-selling book The 28-day Alcohol-free Challenge, co-authored with his colleague Ruari Fairbairns, was released in December 2017. Andy is one of the world’s leading behavioural change & performance coaches. He works with CEOs, business professionals, athletes, entrepreneurs, actors and influencers to transform their relationship with alcohol to improve their health, their careers and their day to day lives. My father’s recent death with cancer has reinvigorated my passion to help as many people as possible to live the healthiest lives possible to give us the best chance of a longer and disease-free life. The podcast focuses on the pillars of a healthy lifestyle such as eating more plants, becoming more mindful, moving our bodies more, improving our sleep, developing healthier connections and refining our sense of purpose. This interview is with Dr Neal Barnard, a physician, clinical researcher, author, and an adjunct associate professor of medicine at the George Washington University School of Medicine and Health Sciences in Washington DC, USA. In 1985, Neal established the Physicians Committee for Responsible Medicine (PCRM) to promote preventive medicine, conduct clinical research, and advocate for higher ethical standards in research. The PCRM also provides direct medical care through its subsidiary, the Barnard Medical Center, in Washington, where patients with diabetes, obesity, and other chronic conditions can be treated, and/or participate in clinical research studies, aiming to improve the prevention and treatment of these health problems. Panel discussion at a conference on conscientious objection in medicine and the role of conscience in healthcare practitioners’ decision making, Oxford 2015. The conference aimed at analyzing from a philosophical, ethical and legal perspective the meaning and the role of “conscience” in the healthcare profession. Conscientious objection by health professionals has become one of the most pressing problems in healthcare ethics. Health professionals are often required to perform activities that conflict with their own moral or religious beliefs (for example abortion). Their refusal can make it difficult for patients to have access to services they have a right to and, more in general, can create conflicts in the doctor-patient relationship. The widening of the medical options available today or in the near future is likely to sharpen these conflicts. Experts in bioethics, philosophy, law and medicine explored possible solutions.
