See all the Healthcasts at https://www.biobalancehealth.com/healthcast-blog The newest miracle drug for weight loss is changing the lives of thousands of people who have battled obesity for extended periods of time….These GLP-1 medications are also treating or preventing the diseases that go with long term obesity: Diabetes, Heart Disease, Joint replacements, Arthritis, Sleep Apnea, and Alzheimer's Disease. Researchers are finding more indications every day for patients to take these weight loss medications. But like anything else there is no perfect answer to any problem. Among the few side effects of this drug, the most frequent side effect is reflux, also called acid indigestion, or GERD (gastroesophageal reflux disease). Often my patients don't even know what their diagnosis is, they just tell me about their symptoms. The symptoms of GERD include: Asthma symptoms A bad taste in the mouth Difficulty swallowing Dry, hacking, cough Chest pain after meals These symptoms are worse after a big meal, at bedtime, after spicy food, or dose related. Most of my patients don't want to discontinue the GLP-1 inhibitors because they are finally losing weight! We manage the GLP-1 side effect of GERD by decreasing dose of the medication and slowly increase the dose back to an effective level. We also offer lifestyle and dietary treatments before we offer prescription medication. Therefore, if reflux is not constant, and is not causing any lasting damage to the patient's esophagus, we can treat it with lifestyle changes and over the counter medication, to lower the stomach acid that is refluxing into the esophagus. The lifestyle changes patients can employ on their own are described below. Lifestyle changes needed to avoid or treat Gastric Reflux caused by GLP-1 agonists. What can you do to prevent and treat this side effect: Eat smaller meals: Large meals expand your stomach and put pressure on your lower esophageal sphincter (LES). Don't go to bed less than 2 hours after eating Avoid trigger foods see below Sleep on your left side Elevate the head of your bed Avoid tight clothing: Chew your food well– chew each bite for 20 seconds. Quit smoking: Smoking weakens your LES and makes your stomach more acidic. Stop drinking alcohol Chew (non-mint) sugar-free–gum In addition to changing your active lifestyle, changing your diet is necessary as well. There are trigger foods to avoid minimizing your reflux symptoms. tomato sauce and other tomato-based products high fat foods, such as fast food and greasy foods fried foods citrus fruit juices soda-diet and regular Caffeine Garlic onions mint of any kind milk based products My patients ask me, “So what can I eat?” …I admit I did take away some of the most exciting foods, however my patients ask me what they can eat so the list of foods that help avoid and treat GERD are listed below. High-fiber foods: vegetables, fruit, and whole grain bread. Alkaline foods. Foods fall somewhere along the pH scale (turns litmus paper blue). Drink alkalinized water (PH > 8) Ginger—fresh sushi Ginger from Asian food stores. Apple cider vinegar on salads and a Tablespoon in water every morning Lemon water—just squeeze a slice of lemon in your water. Coconut water Honey. Lean Protein including meat Low-Fat and Nonfat Dairy Products. Non-Citrus Fruits like apples, pears, bananas, and melons Vegetables like broccoli, Carrots, Corn, Cucumbers, Green Beans, Green peppers, Potatoes and Sweet potatoes For my patients who take herbal and other supplements, the following is a list of the supplements that may decrease your symptoms of GERD. Chamomile Tea Licorice Marshmallow Slippery Elm Tablets Probiotics-Mega Brand Prebiotics-Mega Digestive Enzymes Aloe Vera Juice Baking Soda Magnesium glycinate What happens when you have made all the lifestyle changes you can and have lowered your GLP-1 dose or changed to a different type of GLP-1 Agonist, and you still have GERD? As a physician I prescribe medications to help my patients treat their GERD, however most of the medications have been placed over the counter so I can recommend them to my patients, and they can buy the medication without a script. The class of medication that treats GERD include Antacids, H2 Blocker, and Proton Pump Inhibitor. Antacids neutralize stomach acid, but they typically only work for short periods. They are generally made of calcium. This type of medication is best used prn for symptom relief. Side effects of antacids may include constipation and diarrhea. The second option for treatment of GERD is an H2 blocker. These drugs reduce the amount of acid the stomach releases. Eg. Pepcid, Tagamet, Gaviscon. Proton pump inhibitors (PPIs): These drugs are available by prescription from a healthcare provider, and now some doses are over the counter. PPIs help reduce the amount of acid the stomach makes. They should be used for a two-week period only for severe attacks and then you should change to an H2 blocker or antacid. WHY stop a PPI after 2 weeks? Omeprazole is an example of a PPI. Theses medication kill the good bacteria in your intestines, change your breath and can affect how you absorb your nutrients. If you must take them chronically to treat and prevent the progression of damage to the esophagus. What if I did everything and GERD is still a problem: If your condition is severe, your doctor may recommend a consultation with a GI doctor for an endoscope or other diagnostic procedure. In addition, you may have to hold your GLP-1 Agonist for a period of time while you treat your esophageal inflammation. Just as in all medical issues there are many ways to treat side effects of drugs. Your provider will prescribe the medication that she or he is most comfortable with. What next? So if you have reflux and are on a GLP-1 inhibitor, you may be advised to decrease your dose or switch to Tirzepatide medication (Mounjaro, Zepbound). There are many steps you can take before you need prescribed medication. Your doctor may even change your GLP-1 agonist prescription or refer you to a GI doctor, but before this is necessary you should try the lifestyle and dietary changes that I recommend in this Blog first. This side effect of GLP-1 agonists doesn't affect every patient and can be managed as you see above, however the lifestyle changes and dietary changes can only be done by you, so the ball is in your court!

This molecule is gas called nitric oxide. Nitric oxide (NO) is a molecule that plays a crucial role in various physiological processes in the human body, including inflammation. It is a gaseous signaling molecule that acts as a vasodilator, meaning it helps to relax and widen blood vessels. When the blood vessels widen it allows oxygen and nutrients to EVERY cell inside the body. Nitric oxide is produced by various cells, including endothelial cells (cells lining blood vessels), immune system (macrophages), and brain cells (neurons). Controls mitochondria and telomeres. It helps your body recover from injury and workouts. It's been called the holy grail of anti-aging medicine. Nitric oxide can also have anti-inflammatory effects. It can modulate the immune response by inhibiting the production of pro-inflammatory cytokines and chemokines. Additionally, nitric oxide can regulate the activity of immune cells, such as T cells and neutrophils, to prevent excessive inflammation and tissue damage. Citrus fruits: Fruits like oranges, lemons, and grapefruits are rich in vitamin C, which is a cofactor for the enzyme that produces nitric oxide. Including citrus fruits in your diet can support nitric oxide synthesis and promote vascular health. Pomegranate: Pomegranates are packed with antioxidants, including polyphenols, which have been shown to enhance nitric oxide production and improve endothelial function. Nuts and seeds: Almonds, walnuts, flaxseeds, and chia seeds are good sources of arginine, an amino acid that serves as a precursor to nitric oxide synthesis. These foods can help boost nitric oxide levels in the body. Dark chocolate: High-quality dark chocolate with a cocoa content of at least 70% or more contains flavonoids that promote nitric oxide production and improve blood flow. Enjoying moderate amounts of dark chocolate can have cardiovascular benefits. If you are dealing with heartburn, most of the time it's NOT a problem of too much acid, it's a problem with too little and the LES (lower esophageal spincter) is not functioning well. Getting into a parasympathetic state before eating can help, humming ( Humming also helps with NO. When you hum you produce nasal NO) and gratitude before eating helps, also adding 1 tablespoon of acv before meals. Mouth breathing - Nitric oxide is released in the nasal airways in humans when you breathe through your nostrils. When you primarily breath through your mouth, it prevents NO from being produced. Focus on nostril breathing throughout the day, and mouth tape at night. I personally use Somnifix, I will drop a link below for them. Sedentary lifestyle - Yo've heard sitting is the new smoking, it's true. If you don't use it, you lose it. Get 10k steps per day, and the best exercises for NO are sprinting, strength training, bonus points if you do it outside getting sunshine. Also, a good general protocol is 20-30 minutes of aerobic exercise 2-3 times per week. Statins disrupt lipid rafts and cell signaling and mitochondria. When you lower your cholesterol below 200, it blocks nitric oxide from binding to the cell receptors. Cholesterol is not the boogeyman , it is vital for liver and hormone health. In this podcast, we're going to talk about three ways to get rid of heartburn without using antacids. Antacids only work for a short time, requiring you to take more and more to keep your symptoms at bay. Through prolonged use, antacids will only make heartburn worse by depleting your stomach acid. Antacids are associated with the following side effects: • Bloating • Indigestion • Allergies • Increased infection in the stomach • Depression Antacids create a condition known as achlorhydria, which depletes your stomach acid, but you need your stomach acid to get rid of heartburn. Your stomach is very acidic and should have a pH between 1 and 3. Enzymes cannot be activated if you do not have the right pH. Heartburn is not a problem resulting from too much acid—it's a valve problem. Keeping the pH of the stomach between 1 and 3 keeps the valve shut. How to get rid of heartburn naturally: 1. The most important nutrient to eliminate and prevent heartburn is vitamin B1 (thiamine). Your stomach can not produce acid when you're deficient in vitamin B1. A diet high in carbs depletes B1. Low B1 deprives the autonomic nervous system, which controls the valves of the stomach. Look for a natural source of vitamin B1 to get rid of heartburn. 2. Potassium and zinc are very important for your stomach acid. Try a good electrolyte powder and trace mineral supplement to increase your intake of these important minerals. In this podcast, we're going to talk about the concept of hormesis. Hormesis describes two things that might oppose each other. In terms of health, hormesis refers to a controlled dose of stress that makes positive changes for your body. With hormesis, you can adapt to various types of stress, including exercise, radiation, cold, heat, hypoxia, plant chemicals, pesticides, insecticides, and fungicides. Sulforaphane found in broccoli sprouts and radish sprouts triggers the heat shock protein that allows the body to strengthen its detoxification enzymes. In this instance, the body is exposed to a small amount of poison that strengthens its ability to get rid of other poisons. The liver contains phase 1 and phase 2 detoxification enzymes that can take a poison and turn it into a harmless particle. Sulforaphane triggers this system. Exercise is one of the best examples of hormesis. Exercise induces microinflammation but then allows the body to get rid of inflammation. Fasting activates the body's mechanisms that counter starvation. It encourages our brain cells to grow, increases mitochondria, and regenerates the immune system. Health is often counterintuitive. For example, cutting fat won't help you lose fat, and sterilizing your environment won't protect you from getting sick. You can't develop a strong immune system without getting sick. Antibiotics destroy good bacteria in the body, which can lead to pathogen overgrowth. Antacids can lead to more heartburn because you need more acid to get rid of heartburn. When you don't expose yourself to the sun, you put yourself more at risk for certain types of cancers. Many people have been told that the sun causes cancer, but the sun gives us vitamin D. Having a deficiency in vitamin D can increase your risk for cancer. One last example is dieting. The more dieting you do in your lifetime, the more weight you will gain—especially if you're doing it incorrectly. Today, we're going to talk about the potential dangers and benefits of alkaline water. Natural water from a well or spring is alkaline. It's alkaline because it contains minerals like calcium and magnesium. Many people believe that things that are acidic are unhealthy and things that are alkaline are healthy. But that's not altogether true. We have a lot of different acids in our bodies that have many important functions. We don't need to worry about alkalizing the body. The body regulates the pH in different areas. For example, your blood should be slightly alkaline, but your stomach should be very acidic. It's possible to get alkaline water created by a machine that uses electricity to alter it instead of using minerals to alkalize it. If you drink alkaline water that's created artificially, you're going to alter the pH in the stomach. Potential side effects of alkaline water made artificially can include: • Heartburn • Acid reflux • Gas • GERD • Infection • Hair loss • Heart arrhythmias However, natural water, like alkaline spring water, has no side effects. Alkalizing the stomach could lead to a deficiency in stomach acids. The stomach should be very acidic to help you break down protein, absorb minerals and vitamin B12, and kill pathogens. As we age, we lose stomach acid. Antacids also weaken stomach acid. But, low stomach acid is the root cause of symptoms like heartburn, GERD, and acid reflux. Alkalizing the stomach instead of acidifying it could worsen these symptoms and lead to many other serious side effects. I've found that I can digest better when I drink acidic water, like carbonated water—especially if I add a teaspoon of either apple cider vinegar or lemon juice. Overall, alkaline water has certain health benefits because it contains minerals. But alkaline water is only beneficial if it's natural and not created by a machine.

Show Notes: First ask, “Are you having pain now?” If your patient answers no, consider this unstable angina until proven otherwise even if the pain went away with an antacid. Antacids relieve pain in about 15 percent of ACS. Why? Coincidence. The real lesson here is that the percentage of acute coronary syndrome that presents as unstable angina is probably 15 percent at a minimum. It's actually probably at least twice that because not everyone tries an antacid. If you are looking for unstable angina, you are less likely to be fooled by false-negative troponins. This is still true of high-sensitivity troponin, although supposedly some literature says it can rule out unstable angina if the level is below the level of detection at least three hours out from peak pain and presentation. It has to be better than normal; it has to be undetectable. Second Troponin v. Second History: I would like to be spread the following lesson to everyone in EM: “Before you do a second troponin, do a second history!” You need to really nail down the timing and duration of symptoms the best you can or you may be misled. Some patients speak their own language: “Constant” can mean “frequent.” Most of us already know that “no medical problems” can mean “no untreated medical problems” to a lot of people. It is safest to start with the assumption that all chest pain is unstable angina until proven otherwise, that the pain is episodic lasting five to 10 minutes, and that the troponin and ECG may both be useless. HEART Score Logic: The HEART score is super useful and can help protect you if you send the wrong patient home, but you should still try to avoid doing that. The HEART score's major blind spot is unstable angina because the troponin and ECG may be normal if pain lasts less than 20 to 30 minutes. Another caveat of the HEART score is grouping together all the patients with scores from 0-3. If the risk of this group is about one to two percent on average, logic dictates that patients with a score of 0-1 are actually at lower risk, but those with a score of 2-3 are likely at higher risk, perhaps three to four percent or even more. What is the actual risk associated with a HEART score of 3? We won't know until someone does the study, but risk jumps to 10 to 20 percent once you hit the 4-6 score group. If you extrapolate the data, the risk of a MACE at 30 days is probably two to three times the HEART score, so for 3 it would be nine percent, if you use logic. Back to the question, “Are you having pain now?” If your patient answers yes, your next question should be, “When did the current episode start?” Or better yet, “When was the last time you had no discomfort?” If it was less than two hours earlier, you should be doing serial ECGs every 30 minutes if you can, so you don't miss a STEMI with an initially nondiagnostic or even near-normal ECG. It can take two hours or even more for ST elevation to manifest in some cases. You want that to be picked up early, not eight hours later when your patient is on the floor and it is too late for cath to help much.
