Because people change, people evolve, and things around the people evolve. And I think the pace of change, if anything, has just kind of 10xed, right? My dad was like a feminist, way ahead of his time. I mean, just a man... The thoracic cavity is separated into different compartments by membranes The lungs exist in their own pleural cavities, and the mediastinum is everything in between The mediastinum extends from the sternum to the thoracic vertebrae and includes the heart, the aorta, the trachea, the esophagus, the thymus, as well as many lymph nodes and nerves. What is a pneumomediastinum? Air in the mediastinum How can pneumomediastinum be categorized? Traumatic Ex. Stab wound to the trachea Ex. Boerhaave's Syndrome of the esophagus, possibly from an endoscopic procedure. This mechanism in particular is a higher risk of infection because not only air but food can accumulate in the mediastinum Ex. Intubation with a bougie These will likely need surgical repair Nontraumatic Ex. Forceful inhalation causing microperforations in the trachea. Possibly while inhaling something like drugs Ex. Bad asthma for similar reasons Ex. Gas forming bacteria What happens if you use positive pressure ventilation on a patient with a hole in their trachea? The positive pressure will force extra air into the mediastinum The air will move between the layers of subcutaneous tissue and can track up into the neck and face regions recognized as crepitus on exam This can also cause a tension pneumomediastinum in which the air pressure in the compartment constricts the heart, impeding its ability to fill during diastole These patients can undergo bronchoscopy because that procedure does not require positive pressure and will not worsen the condition. Endoscopies do require positive pressure so endoscopies are not an option How is a tension pneumomediastinum treated? By inserting a needle into the space from below the xiphoid process to allow the air to escape, similar to a pericardiocentesis As a temporizing measure, if the hole is high enough in the trachea, the intubation can be continued by deliberately pushing the endotracheal tube into the right main bronchus, creating a seal, and only ventilating the right lung while the patient heads to surgery. This Day in Legal History: Alcohol Control Administration CreatedOn December 4, 1933, President Franklin D. Roosevelt established the Federal Alcohol Control Administration (FACA) by executive order, a pivotal moment in the transition away from Prohibition. This action came under the National Industrial Recovery Act, as part of Roosevelt's broader New Deal agenda aimed at economic recovery. FACA was designed to regulate the burgeoning alcohol industry, which was reemerging after years of prohibition, ensuring it operated within fair competition guidelines. The administration focused on creating voluntary codes for brewers, distillers, and wineries to prevent monopolistic practices and maintain equitable market conditions.FACA's creation coincided with the formal repeal of Prohibition under the 21st Amendment, which took effect later that month. However, the agency's lifespan was short-lived. By August 1935, it was replaced by the Federal Alcohol Administration (FAA) through new legislation. The FAA consolidated alcohol regulation within the Treasury Department, laying the groundwork for modern alcohol oversight. Today, the Alcohol and Tobacco Tax and Trade Bureau (TTB), a successor to the Bureau of Alcohol, Tobacco, and Firearms (ATF), continues to fulfill the regulatory role first envisioned by FACA. The establishment and swift evolution of alcohol regulation reflect the complexities of ending Prohibition and integrating alcohol back into the economy under federal oversight.A federal judge in California dismissed Hunter Biden's tax case but criticized the presidential pardon issued by President Joe Biden. Judge Mark C. Scarsi expressed skepticism about the validity of the pardon, noting that it was supported only by a press release and could be interpreted as covering actions beyond its signing date. He questioned the argument that the charges against Hunter were politically motivated, highlighting that the investigation was conducted under the supervision of the President's own Department of Justice.The judge also pointed out inconsistencies in statements by the President, who claimed his son was treated unfairly compared to others with similar tax issues. However, Hunter Biden had admitted to evading taxes while sober, contrary to claims of addiction-related leniency. Despite these critiques, Scarsi determined that the part of the pardon covering Hunter's past conduct required dismissal of the charges in his Los Angeles tax case. In a Dec. 2 post, Armstrong criticized Milbank LLP for hiring Gurbir Grewal, the former SEC enforcement director who led numerous actions against crypto companies, including Coinbase. Immediately prior to his appointment, Rahat was Special Counsel to New Jersey Governor Philip D. Murphy, overseeing all high-profile litigation impacting the Governor and the Administration. Previously, he was the Director of Community Engagement at the New Jersey Attorney General's Office, where as part of Attorney General Gurbir S. Grewal's Executive Leadership Team, he led the Attorney General's efforts to strengthen the office's relationships with community leaders, faith leaders, and the public. Rahat held several other leadership roles within the Attorney General's Office, practiced in a boutique corporate law firm, and taught law and public policy at Temple University Beasley School of Law as an Adjunct Professor.Rahat is a former member of NAPABA's Board of Governors and a former chair of NAPABA's Civil Rights Committee. Episode 313: Douglas Donald Moore, known in Meadowvale, Ontario, as a drug supplier for young teens, gained notoriety in Mississauga for killing three young men before taking his own life in his jail cell while awaiting trial on 11 charges for sexual assaults on three boys. Peel Regional Police assert that Robert Grewal, 22, of Meadowvale, and Giuseppe (Joseph) Manchisi, 20, of Milton, who were close friends, were killed in 2003 by Moore. After Moore's death, he was named the prime suspect in the murders of Grewal and Manchisi. Additionally, police believe Moore was responsible for killing Rene Charlebois, 15, also of Meadowvale. All three victims disappeared in late 2003, and their bodies were discovered in spring 2004. Charlebois' remains were found in an Orangeville landfill, while Grewal and Manchisi's remains were located in wooded areas near Montreal. In 2005, Moore's former common-law wife and an unidentified 16-year-old were convicted of accessory to murder after the fact. The teen assisted in disposing of evidence, including driving with Moore to Quebec to bury the bodies of Grewal and Manchisi. Moore believed the two men had stolen drugs and cash from him, but it was later revealed that the 16-year-old was responsible for the robbery. The police have never disclosed the motive behind the killing of Rene Charlebois. Gurbir Grewal has been the Director of the Enforcement Division at the U.S. Securities and Exchange Commission since 2001. Prior to that, he had the distinction of being the first Sikh American to be Attorney General in any of the fifty states, and only the second South Asian to hold such an office after the former Attorney General of California, Kamala Harris. Gurbir was the Chief Prosecutor of Bergen County in New Jersey, and before that the Head of the Economic Crimes Unit in the U.S. Attorney's Office in New Jersey.
