Il 13 gennaio 2012 alle ore 21.45 la Costa Concordia si schiantò contro gli scogli de le Scole, naufragando davanti all'Isola del Giglio e provocando 32 vittime e 157 feriti. Sono trascorsi tredici anni dal disastro della Costa Concordia e dal “tragico inchino” eseguito dal comandante Francesco Schettino e i suoi ufficiali, ovvero una rischiosa manovra di avvicinamento alla costa effettuata dalle navi da crociera in segno di saluto. La Concordia partì dal porto di Civitavecchia il 13 gennaio 2012 alle 19.18 e attorno alle 20.45 raggiunse l'Isola del Giglio. Il comandante iniziò la manovra di inchino ma seguì una rotta non autorizzata: l'urto che ne seguì generò un enorme squarcio che fece naufragare la nave. Ma cosa è successo? Come è avvenuto il disastro? In questo episodio raccontiamo la ricostruzione del momento dell'impatto dal punto di vista tecnico scientifico e la rimozione della Concordia attraverso il parbuckling, una complessa operazione di recupero della nave da crociera date le sue 110 mila tonnellate di peso. On the 13th of January, 2012 the Italian cruise ship Costa Concordia struck a rocky outcropping off the coast of Italy. The ship lost power and propulsion, and came to rest on its starboard side, just meters from the island of Giglio. Panic and chaos ensued on board, with over 4000 passengers and 1000 crew. Captain Francesco Schettino altered the planned course of his ship to perform a sail-by salute, close to Giglio, without informing Costa operations. His mistress, Moldovan dancer Domnica Cemortan, was on the bridge at the time of the accident. Twenty-seven passengers and five crew were killed in the accident, and countless more were injured. Schettino and other deck officers abandoned the helpless ship well before all the passengers were off the ship, earning him the nickname, "Captain Coward." In 2012 the passenger ship Costa Concordia sank off the coast of Italy, resulting in the death of 32 people. Amongst other things, the captain was criticised for his casual attitude towards formulating the ship's voyage plan, sailing 0.5 miles too close to the coast, and ‘disregarding to properly consider the distance from the coast'. The captain was sentenced to 16 years in the prison. While the rest of the world (including leading experts in the industry) criticised the captain for navigating the safe too close to the islands. even five years after the accident when I met the captain in his hometown he maintained that this was a normal practice in the cruise industry. It was a powerful insight into the culture of the cruise industry. In this episode, we will discuss why culture is so often misunderstood within the risk and safety industry using this story. 1) what are normal practices and why do we need to understand them? 2) where are we today with our understanding of ‘normal practices'? 3) how a high-risk manoeuvre became a normal practice in the cruise industry? 4) what safety sciences won't tell us about normal practices? 5) how can we uncover ‘normal practices' within our organisations? Each of these questions will be addressed through stories and experiences that we can all relate with. This is the second in a series of episodes based on my recent book, ‘Are We Learning from Accidents?'. A previous video in this episode was focused on why people don't speak up. The Italian investigation into the deadly sinking of the 'unsinkable' Bayesian superyacht could drag on a while.   New Zealand-born captain James Cutfield is being investigated for possible manslaughter and negligent shipwreck charges.   He's being advised by two Italian lawyers, and has so far exercised his right to silence.   Italy correspondent Jo McKenna told Mike Hosking the investigation could take some time, particularly as the yacht needs to be brought to the surface.   She says before they do that they'll have to secure fuel supplies to ensure there's no leakage, which could take several weeks.   McKenna says it took 11 months for the captain of the Costa Concordia to be charged.   LISTEN ABOVE See omnystudio.com/listener for privacy information.

In January 2012, the Costa Concordia went aground and capsized off the coast of Italy. One of the most disturbing aspects of this accident was that a group of competent officers and an experienced captain on the bridge were not able to detect that the ship was heading straight into the rocks.   In this podcast, I will present the power of framing using the story of the Costa Concordia accident. I interviewed the captain before he was sentenced to prison, and I was surprised by how the captain responded to my open-ended questions during the course of our interview and how it led me to a completely different view of the accident and learning than the one presented in the public domain. I have also recently written a book on this accident – Are We Learning from Accidents?   We will focus on the following questions: 1. Why was the Costa Concordia accident framed as a problem of ‘speaking up' and the failure of ‘bridge resource management'? 2. What are the parallels between concepts such as ‘psychological safety', ‘crew resource management', ‘non-technical skills' and ‘soft skills'? 3. Why do these concepts appeal so much to the risk and safety industry? 4. What are the alternative frameworks to think about ‘speaking up'? 5. And importantly, why we should never underestimate the power of framing?   This is the first in a series of videos based on my recent book, ‘Are We Learning from Accidents?'. It was one of those scenes in the news that's hard to forget. It was the image of a Korean ferry just rolling into the sea with all the passengers disappearing with it beneath the waves. It was heartbreaking to look at all those loved ones on the dock grieving inconsolably over children who would never come home again. What was outrageous was that the captain was one of the first to abandon ship. They charged him with negligence of duty and abandoning people in need. Sadly, there are a lot of captains that can be charged with crimes like that. I'm Ron Hutchcraft and I want to have A Word With You today about "How the Captain Can Sink a Family." See, I'm talking about being the captain of your family. The ship starts to drift...the crew gets confused...the vessel is in danger of a fatal turn when Dad keeps "abandoning the ship." When the cruise ship Costa Concordia shipwrecked off the coast of Italy, again it was the captain who was charged. A maritime lawyer said at that time: "The captain is the master of the vessel. Every crew member looks to the captain for guidance and leadership. It's the captain's responsibility to know the waters and avoid coming close to any shoals and reefs." Sad to say, I've sometimes gotten too busy to know the waters that our family ship was navigating; when this captain was "below decks" at a critical point. We know there's a devastating epidemic of fatherlessness in families these days. But it isn't just dads who are physically absent. You can be emotionally absent, and that may be even more damaging; around your family but not with your family. There, but not really there. "Abandoning people in need." A daughter, missing her father's love, ends up looking for that love in all the wrong places. A son who can't get his father's approval or attention, growing angrier by the day. A wife who's left to run things alone. In many ways, that ferry disaster mirrors the family disaster caused by a "captain's" three tragic mistakes. Number one - leaving the wheel. It appears that the ferry captain left the ship in the hands of a third mate at a decisive turning point. How many of us husbands have forfeited leadership when the going got rough, when finances were turbulent, when discipline was needed, when hard choices had to be made? Criminal negligence. Our word for today from the Word of God, Proverbs 27:23-24. Here's what they say: "Be sure you know the condition of your flocks." (May I say your family.) "Give careful attention to your herds. For riches do not endure forever and a crown is not secure for all generations." Translation: Take care of it or risk losing it all. There's a second way a captain can sink his family ship - looking out for me. Apparently, that ferry captain was so concerned about himself that he left his passengers to fend for themselves. "Me first" while those he was responsible for were going down. Consumed with our personal pursuits, our work, our recreation, our sports, our hobby, our toys; effectively oblivious to the leadership that we're abandoning. And then there's a third way that any captain can sink his ship. There are rocks to avoid. There are crises requiring direction. And a husband - a father - cannot be AWOL when a steady hand at the wheel is needed. Along with a wise, reassuring guide when the water is rising. I have to confess this to you. I don't know how in the world to be a dad today without the power of Jesus Christ. If I didn't have Jesus as my Savior, being a dad would have driven me to my knees to find Him. Being a dad is when you find out you are not enough by yourself. You're not in control. You're not in charge. You need a Savior. Today if you're a dad, and maybe the ship is not going the right direction, let this be the day your family gets a new dad, as you turn that dad over to Jesus Christ. Want to know how?
