TRANSCRIPT OF AUDIO From the Cumberland Gap to the Atlantic Ocean, this is Virginia Water Radio for the week of October 25, 2021.  This episode is part of a series this fall about water connections to the human body and human biology.  We start this week with three mystery sounds, all related to a very numerous group of disease-causing, or pathogenic, microbes that have enormous impacts on human health.  Have a listen for about 20 seconds, and see if know this microbial group.  And here's a hint: big hits on social media are said to spread like this group. SOUNDS – ~19 sec If you guessed viruses, you're right!  You heard a person coughing due to a viral disease; handwashing, an important method of reducing viral transmission; and a spray bottle of isopropyl alcohol, an effective disinfectant against many kinds of viruses.  With attention focused this fall both on the COVID-19 coronavirus and the annual influenza virus season, we explore in this episode some basic information about viruses and some viral connections to water.  Here are 10 key points about viruses. 1.  Viruses are one of four groups of microbes responsible for human disease, along with bacteria, fungi, and protozoa, which are single-celled animals.  As a group, viruses are the smallest of these microbes, although some are larger than some bacteria. 2.  Viruses aren't made up of cells, but instead exist as particles composed primarily of molecules of protein and nucleic acids, that is, DNA or RNA.  They require a cellular host for reproduction, called replication. 3.  Viruses are more abundant than all of the cellular-based living things on earth. 4.  All living things are infected by viruses. 5.  Viruses don't always cause disease in infected hosts, but many kinds do cause significant diseases in humans, other animals, and plants. 6.  Viral disease can result from viruses taking over or inhibiting their host's cellular biochemical processes, or by cell destruction as new virus particles exit cells after replication. 7.  Depending on their type, viruses can be spread through air, in water, from surfaces, by animal vectors, or through exchange of blood or other body substances. 8.  Water-related spread of viruses can occur through water contaminated with human waste, and through animal vectors connected to water, particularly mosquitoes. 9.  Significant human diseases from water-borne viruses include intestinal disease, particularly diarrhea; hepatitis, or liver inflammation; inflammations of the brain, spinal cord, or heart; and possibly cancer.  Viral diseases spread by mosquitoes include Yellow Fever, Dengue, West Nile, and others. And last, but not least, handwashing with clean water and soap is important for reducing the spread of viruses through objects and surfaces—collectively called fomites—with which humans come into contact. And much more. So I finally decided to get a couple of true, unbiased fish oil experts on my show. is my first guest. He is an internationally recognized expert on omega-3 fatty acids and how they can benefit patients with heart disease. He obtained his Ph.D. in Human Nutrition from the University of Minnesota and did post-doctoral fellowships in Clinical Nutrition and Lipid Metabolism with Dr. Bill Connor at the Oregon Health Sciences University. His interest in omega-3 fatty acids began with his postdoctoral work when he published his first study on the effects of salmon oil on serum lipids in humans (1980). Since that time he has been the recipient of five NIH grants for studies on the effects of omega-3 fatty acids (EPA and DHA) on human health. He has more than 300 publications relating to fatty acids, including omega-3s, in medical literature and was an author on two American Heart Association scientific statements on fatty acids: “, and “ both published in the journal Circulation. Dr. Harris is a Professor in the Department of Medicine in the Sanford School of Medicine at the University of South Dakota and the President and CEO of OmegaQuant. Dr. Harris's daughter, , PhD, RD is my second guest. Kristina received her PhD in Nutritional Sciences from Pennsylvania State University in 2013 and completed her training to become a Registered Dietitian in 2014. As a graduate student, she studied under Dr. Penny Kris-Etherton, an internationally-recognized expert in fats and nutrition. She worked as a postdoctoral fellow at the University of Colorado-Denver under Dr. John Peters in the area of worksite wellness before returning to the omega-3 field. Kristina joined the family business Analytics as a Research Associate in 2014, with a particular focus on omega-3s in maternal health, helping create the Prenatal DHA test and the Mother’s Milk DHA test. In 2020, she became Assistant Professor (non-tenure) in the Department of Internal Medicine at the University of South Dakota Sanford School of Medicine. (2020-2022). She lives in Sioux Falls, South Dakota with her husband, two kids, one old dog, and four grandparents. The NIEHS Superfund Research Program (SRP) Progress in Research webinar series highlights promising research from SRP Centers awarded grants in 2020. In this session, awardees from University of Kentucky, Oregon State University, and Baylor College of Medicine will describe their research projects, accomplishments, and next steps. The University of Kentucky SRP Center, "Nutrition and Superfund Chemical Toxicity," explores human health challenges arising from exposure to halogenated organic substances such as polychlorinated biphenyls (PCBs), trichloroethylene, tetrachloroethene, and per- and polyfluoralkyl substances (PFAS). They conduct research on lifestyle changes such as nutrition and exercise and the relationship with pollutant exposure and disease risk. Center scientists also study remediation systems and engineering solutions for toxicant removal. The goal is to develop prevention strategies for diseases associated with chlorinated organic contaminants through a combination of enhanced remediation and healthy lifestyle components. The Oregon State University SRP Center, "PAHs: New Technologies and Emerging Health Risks," investigates polycyclic aromatic hydrocarbon (PAHs) mixtures before and after remediation. Center scientists work to track PAH movement through the environment, measure PAH exposure in individuals located near contaminated sites, predict the products of PAH transformation during remediation, determine the toxicity of complex PAH mixtures, and link PAH exposure to health outcomes. The Baylor College of Medicine SRP Center, "PAHs: Ultrasensitive Detection, Early-Life Exposures - Clinical Outcomes (Preterm Births, Chronic Lung Disease, and Neurocognitive Deficits), Prevention and Remediation," works in Harris County, Texas to explore maternal exposure to PAHs and the increased risk of preterm birth. They are investigating the molecular mechanisms behind the increased preterm birth risk after maternal exposure to PAH mixtures. Center scientists are also working to develop methods for detecting PAH-based compounds in air, water, and soil, remediation technologies to treat contaminated sediment, and strategies to prevent and reduce the health burden associated with PAH exposure. The Maxie L. Parks Community Center in West Fresno has been closed since Sept. 3 due to contaminants found in the building. Fresno Councilmember Miguel Arias announced Monday the city will secure funding to clean up the site. Before the community center was built in 2007, there was a laundromat on the site, Arias said at a press conference at City Hall. He said the city will spend half a million dollars to address two contaminants in the building associated with the dry cleaning industry. “There are a lot of unknowns that only public health experts and toxicologists can answer,” Arias said. “That's why we are going to be hosting a community meeting where residents can directly pose those questions.” Arias said the meeting will be on Zoom and is meant to create transparency with the residents of West Fresno. But Brunette Harris, who is in her 60’s and has lived in West Fresno for nearly 50 years, said she’s worried the community will not be properly represented. “I have a flip phone, I

The title of Ms. Bandong's presentation is “Risk Based Hazards Analysis: A Tool for Building A Comprehensive Analytical Testing Program,” in which she will explore the vulnerability of products/ingredients to contaminants and present a risk-based process for identifying potential hazards. Her presentation will also include a review of current regulation and requirements for pesticides, heavy metals, residual solvents and other contaminants in cannabis and cannabis-derived products. Ms. Bandong is a sought-after expert, speaker and author on the subject of product safety and risk in the cannabis industry. She is the Scientific Strategy Leader for Contaminants for Eurofins Food Integrity and Innovations and has spent over 25 years in the food and supplement industry specializing in contaminants analysis and risk assessments. Over the years, Ms. Bandong has developed contaminant testing programs that support supplier verification for food manufacturers and recently, the hemp industry. She has developed a process for a hazards-based chemical risk assessment of ingredients that has been used by companies to respond to global regulatory requirements. She is an expert on the analysis of risk assessments of contaminants in food. Product safety and quality are obviously a major concern for the cannabis industry. But an area that is often neglected in the development of standards is the definition of specifications for contaminants in general and chemical hazards in particular. The Food Safety Modernization Act of 2011 calls out microbial contamination, as well as the concern for chemical hazards and requires a risk-based hazard analysis for ingredients used in the manufacture of food. In the cannabis industry, regulations concerning microbial contamination vary from state to state, compounding the issue of insuring quality control for cannabis products. But what's worse is that regulations regarding pesticide use and other chemical contaminants are seriously lacking. Ms. Bandong's position is that adapting regulatory practices from the food industry to the cannabis industry would be highly beneficial. And here's where a risk-based analysis model could prove to be worthwhile. Chemical contaminants can cause serious damage to customer's health and have major effects on a business. But it's often not realistic to test every lot of an ingredient for every chemical contaminant. A risk analysis model would be able to identify and weigh risk for various ingredients resulting in a better understanding of the product, from the ground to the shelf, and allowing for a balanced allocation of resources for monitoring ingredients as well as final products. Ms. Bandong is in an ideal to affect the adoption of such recommendations. She is an active member of AOAC and is currently on the Executive Board of the AOAC Pacific Southwest section. She is also on the Technical Committee of the US Hemp Authority and a member of the HIA's Sampling and Analytical Task Force. She received her B.S. in Chemistry from the University of the Philippines and her M.S. in Chemistry from the University of Houston. The notes are about in the same order as the interview. =miles= Contamination and Testing/Monitoring The state (CA) only requires the water district to test at the source of the water, i.e. where they start pumping it into the distribution system. The rest (downstream testing) is up to the water district. Contamination can come from multiple sources, including backflow when the system becomes depressurized and air contaminated with smoke leaks into the pipes. It can also result from homes that were equipped with plastic pipes that burned. As well as the pipes we know about that burned, i.e. the 5 miles of HDPE pipe through the forest. The monitoring tests that the water district routinely runs downstream are limited to only particular VOC’s (Volatile Organic Compounds) and are targeted at only those that might be produced by adding chlorine to the system. These less stringent tests are for THM’s (trihalomethanes) which are a form of VOC. These tests are not adequate to certify the safety of water possibly contaminated from a fire (as described above). In the Paradise fire, the State refused to take ultimate responsibility for certifying that the water is safe, saying it was up to the local district to do so. (but there are ways the district can reach out for qualified help, discussed below) It is unclear whether or not SLVWD realizes this yet. Additionally, he said “there is nuance” regarding these DND/DNB orders, which I think means that experts disagree on what exactly they mean. (ndlr: good luck with that!) “You have to test fast.” The 7-day turnaround and only 40 samples are woefully inadequate. They should be getting results within a day by utilizing more labs. Apparently the number of laboratories used is a key bottleneck. VOC’s and SVOC’s Benzene can come from burning brush or trees. This VOC (and others) prefer to be airborne, so washing vegetables or taking a shower could release them. Quantities that are toxic may be odorless, so don’t count on being able to detect them by smell. Multiple contaminants can interact, which is one reason why thorough and rapid testing is essential. SVOC’s (Semi-Volatile Organic Compounds) are larger and heavier, so they tend to stay in the water. They also collect in the body in fatty tissue, so the acceptable limits for safe exposure are lower than those for VOC’s. Both VOC’s and SVOC’s can work their way into plastic pipes, so they are hard to get rid of. That’s not a problem with metal, but metal is more expensive. The Water District can reach out for help SLVWD can and should call in other agencies for “mutual aid” as the fire and PG&E crews are doing. They really need to be taking 100-300 samples per week, and use multiple laboratories for effective turnaround. The current turnaround of 7 days is way too long. The equipment required is a GCMS system (Gas Chromatograph Mass Spectrometer). UCSC probably has them, but (a) not the funding (which might be able to come from FEMA) and (b) are probably not certified by the state, though they still could be useful for preliminary results. The current 40 samples will not be representative, and the conditions of the water can change at any time. Contaminants can flow from one place to another within the system. Note that the State is responsible for providing emergency water if the local district cannot in an emergency. Home Water Treatment It’s possible to install a water treatment system for your own home. In general, this is not the best way to deal with the VOC/SVOC contamination for several reasons. There is no official certification for these systems, and the company that sells them will typically run only a single test (for $250-300) which means they could easily misestimate the amount of actual contamination. This has actually happened in Paradise. So what happens is that your insurance pays for it, then they’re done. So if it turns out the system is inadequate, you are on your own. In other words, the financial incentive is designed to create a potentially hazardous situation. It’s much better for the water district to handle everything. One measure Paradise implemented was requiring new homes to have backflow check valves (at $3000-$4000 a piece). Sometimes FEMA money can pay for these.

Stacy D’Andre, MD is a board-certified internal medicine specialist and oncologist who sees patients at Sutter Health in Northern California. She is also a Principal Investigator for National Cancer Institute-sponsored oncology group clinical trials and studies supported by the Sutter Institute for Medical Research. She has authored numerous publications, book chapters, and abstracts on emerging treatment options for gynecologic and GI cancers. She has also been an NBT client for the last two years. In this interview, Stacy and I talk about her recent switch to an integrative medicine approach to cancer treatment. She describes some of the progressive cancer therapies she uses in her practice, including lifestyle change, cannabis, and turkey tail mushrooms. This week Stacy and Sarah are talking about seafood All the seafood and all the things people are concerned about when it comes to seafood And whether or not these concerns are legitimate Stacy and Sarah did discuss this topic on a previous episode (here), but it was time to revisit the discussion Eating seafood is a common talking point on this show since it is so nutrient-dense, pollutant/processing chemical that might be introduced to fish There are dyes added to some low-quality fish to make it look redder There are times when farmed-fish are fed feed that contain dyes to change the color If dyes are added after the fish are processed, that is going to be added to the label Things that are not going to be on the label: Mercury It irreversibly binds to selenium based proteins and enzymes in our bodies so that those proteins can't do their job It impacts 3 different systems the most: thyroid hormone productions liver detoxification protecting the brain against oxidative damage Seafood is one of our best food sources of selenium and the mercury that the fish are exposed to actually binds with selenium based proteins in their bodies Once it binds with the fish's selenium it can't bind with our selenium When we are consuming that fish, almost all of the time, we are consuming more selenium than we are getting exposed to mercury Even fairy contaminated fish and top predators will have more selenium than mercury So that selenium that we are ingesting in fish is actually still helping to protect us from mercury exposure The surveys that have been done now show that with the exception of a few top predators in fairly polluted waters there is typically much more selenium than mercury in all ocean fish And probably with 97% of lake fish, you are getting more selenium than mercury If you are eating these top predator fish, don't eat them that often Examples: mako shark, pilot whale Your body can still detoxify some of this mercury You can handle a little bit of exposure if you are eating a healthy diet and have a healthy lifestyle A little bit here and there is not a big deal Swordfish is probably fine if it comes from non-polluted waters And again is something you shouldn't eat every day Fish and shellfish are some of our best sources of zinc On average 73% of Americans never meet the RDA of zinc Zinc is supposed to be the second most abundant mineral in the human body It is phenomenally important for a whole host of functions within the human body We are getting really interesting nutrients from fish that are hard to get from other sources Fish protein is the best protein for the gut microbiome There have been studies that actually show that consuming fish protein can make up for high sugar diets The omega-3's are really important for every system in our body Our neurological system, immune system, gut bacteria The omega-3's from seafood are the long-chain that our body can use directly without having to convert them The kind from flax or chia have to be converted before our bodies can use them Fish has all of these amazing things so as we go through the less than ideal things, the cons are outweighed by the pros Alana asked about other heavy metals as well (lead and cadmium) There have been some environmental impact studies that have looked at heavy metal in farmed fish The study found that the levels in the fish are still extremely low and below the World Health Organization's guidelines In places where there are more environmental protections you are going to end up with basically levels of heavy metals that are far below any level that we would want to worry about The other heavy metals are a moot point Fish has been demonized as a source, while it's actually much much richer in nutrients that will help us detoxify It is also much lower than other foods, and yet it gets all the blame Stacy finds it interesting how pervasive mainstream media can be when it comes to creating cultural assumptions Sarah shared information on MTHFR gene variance and MTHFR enzyme function (28:41) More on Contaminants in Fish Cesium isotopes from Fukushima There have been levels detected in fish caught off the California coast Fukushima was the second-worst nuclear disaster ever after Chernobyl, and there is a lot of fear around the aftermath from this event This is an ongoing challenge There is a small amount of cesium-134 and cesium-137 in the ocean thanks to Fukushima What is important to understand is that there are radioactive isotopes in nature all over the place We are exposed to them on a daily basis If you live somewhere with high radon levels you are exposed to more Our body is fairly resilient to these low levels of exposure The amount of cesium isotopes in the most contaminated fish's flesh is even 2,000 times lower than the threshold for health effects So as it pertains to eating fish from the Pacific ocean; no we should not be concerned For more on this, check out this blog postfrom Sarah The equation is: If you consumed 12 ounces (which is a very large portion) of contaminated bluefin tuna every day for an entire year, the cumulative dose of radiation that you would consume from all of that tuna would equate to 12% of the radiation dose from a one-way cross country flight from LA to New York Stacy appreciates that analogy - it is so helpful Sarah and Stacy had a sidebar discussion about personality types Gretchen Rubin's 4 tendencies personality categories Enneagram Sarah shared a story about her experience playing with her kids at the playground recently Concerns around BPA in canned fish packaging Sarah wrote a blog postabout BPA and its' impact as an endocrine disruptor This has been confirmed Sarah shared more on the links between BPA exposure and various medical conditions Our dominant BPA exposure is through our plastic use, not through BPA lined cans It is added as a coating inside a can to stop acidic liquids from corroding the aluminum You can reduce your exposure by: Not heating your food in the can Not letting your canned goods sit in a hot car for a long amount of time Be careful when you are removing food from the can so that you are not scraping the edge coating into your food The BPA alternatives for canned good linings have been minimally tested for safety Many of them have also been shown to be endocrine disruptors There are a lot of unanswered questions around these alternatives The benefits of fish still outweigh the potential harm of BPA exposure If you are making efforts to reduce BPA exposure from other places (plastic food storage, plastic water bottles, plastic wrap) Where you can, mix it up with some fresh and frozen But overall Sarah thinks that again the benefits of eating canned fish outweigh the risks Stacy shared her experience with canned goods and why she doesn't worry about the canned goods they consume How they balance the quality of foods they consume Don't let perfection be the enemy of good If you are unable to afford or find canned goods that are BPA free, don't lose sleep over it Antibiotic use in farmed fish While wild-caught is the best, avoiding fish because wild-caught is not monetarily accessible is probably doing more harm than consuming farmed fish Ask the worker at your fish counter where the fish comes from and they are grown In most Western countries, there are regulations on how much antibiotics can be used and how long they have to be discontinued before fish can be harvested Antibiotic residues are linked with all kinds of problems, so if there isn't a washout period then yes the antibiotic residues can cause health problems Where we see this is in developing nations where the practices are not as tightly regulated and they don't have a vet administering the antibiotics Or using the right dosage and/or are failing to follow directions Don't eat farmed fish when traveling to developing countries Stacy shared on her food
