The Modern American Lifestyle is Not Conducive to Wellness When comparing our bodies to people living in the 1970s, we notice a remarkable increase in obesity and a decline in overall health. Today, people experience metabolic disease and unhealthy insulin resistance at alarming rates. Since the fifties, conventional westernized food has become highly processed, hyper-palatable food full of ingredients our body doesn't know how to process. Seed oils and high fructose corn syrup are cheap and convenient for manufacturers, but they are highly damaging to our bodies, and we end up not getting the nutrients we need. Ultimately, excessive consumption of processed food leads to inflammation, oxidative stress, insulin resistance, and metabolic disease. Poor Sleep and Movement Habits Create a Vicious Cycle of Stress In the past few years, modern Americans have experienced significant lifestyle changes in movement and sleep. To stay healthy, we need to move and leave our homes, but nowadays, it is possible to have everything delivered and become wholly isolated and sedentary. People stay up all night and sleep all day. They take drugs to help them wake up and also to help them sleep. This problem became more severe during the pandemic, and many Americans gained over 25 pounds. When people fail to manage their stress healthily, they only worsen things. Habits such as eating while standing and failing to connect with nature also perpetuate problems with sleep and energy levels. Waking when the sun rises, going to bed when it's dark out, and eating in alignment with circadian biology are the best practices to improve sleep and overall quality of life. Mapping Your Hormonal Cycle to Optimize Your Health Choices Women in their peak fertile years may be inclined to cruise along without paying attention to their cycle, but careful tracking is one of the best ways to optimize health. For example, keeping up with the progesterone-dominated segments of your cycle will help you understand when your body needs a break. Progesterone is like the sleepy mellow sister of estrogen, which may affect your workouts, energy, and cravings. About five to seven days before your menstrual cycle, you may need to watch your food intake better. Instead of pushing your body to do OrangeTheory or CrossFit six to seven days a week, this is a better time to focus on weight training, yoga, and pilates. Knowing when to rest and eat healthy foods during your cycle will reduce cravings, sleep problems, and other issues. Make Protein A Top Priority to Retain Muscles and Improve Insulin Sensitivity Besides avoiding highly processed foods, Thurlow recommends focusing on protein. Both men and women build the bulk of their lean muscle mass in their twenties and thirties. By forty, we begin to lose not only muscle mass but muscle strength as well. As we age, adipose tissue begins replacing our muscles, leading to insulin sensitivity. Protein helps to stimulate muscle protein synthesis, and hitting an adequate amount of protein is critically important for maintaining metabolic health. To maximize the effects of good nutrition, go for a ten to fifteen-minute walk after every meal because this will help your muscles absorb and retain glucose, which helps with insulin sensitivity. Recognizing and Adapting to Premenopausal Symptoms To conceptualize perimenopause, think of it as a sort of reverse puberty. When women first get their period, they usually do not menstruate regularly right away. Similarly, perimenopause may cause your cycle to become more irregular and, therefore, harder to track. Perimenopause occurs about ten to fifteen years before menopause, so between ages thirty-five and thirty-nine, women will begin to notice its effects. During this time, the ovaries do not produce as much progesterone. Our adrenal glands act as an emergency backup system for progesterone production, but perimenopause demands more from them, which can cause sleep disturbances, anxiety, depression, and other symptoms. Thurlow recommends speaking with your healthcare provider about hormone therapy, which comes in many different forms to combat these symptoms. Why Cynthia Loves Fasting For Women If you do nothing else, eat less often. It can impact your metabolic health and is profoundly beneficial in many ways. Women have to fast differently; they are governed by fluctuating levels of hormones, at least until menopause. Cynthia is all about empowering you to embrace the typical way that your body is designed to thrive. Many people are looking to change the narrative about health and wellness. You don't have to spend $5,000 a year on supplements! Instead, it would help if you were the driver of change in your own life. Fasting is free, and it's one of the best tools to change your health. Hormone Differences Depending on Your Cycle If you're a lean young woman, you want to be conservative with your fasting window because you just don't have a lot of extra stored fat to lose. Cycling women are prone to insulin resistance or have insulin resistance. Eating less often really can be nicely aligned with a women's menstrual cycle. The first two weeks of the menstrual cycle can be a time where a woman pushes their workouts. Along with that, you can push the fasting window. As you make the transition into the second half of your menstrual cycle, this is the time to do yoga, connect with nature, and do less intense forms of exercise. This is not the time to do an extended fast. Once you start bleeding, you can go back to your regular schedule. During menopause, if women dial in on the lifestyle choices, they have tremendous success. The Mistakes You Are Making With Fasting The first mistake people make is not getting enough sleep. If you are not sleeping throughout the night, then do not start fasting. Instead, you need to figure out why you are not sleeping. Next, you are being too restrictive with your fasts. We need to hit our proteins, fats, and non-starchy carbohydrates. However, many people are too rigid. It would help if you believed in the value of a refeeding day. Depending on where you are in your menstrual cycle, maybe it's time to add some root vegetables or squash. Cynthia broke her fast early today because she did a heavy lift day. Lean into what your body is telling you! Lastly, many people are exercising too much. Cynthia says you need to exercise smarter, not harder. Why Cynthia Loves Ketosis Cynthia loves the ability to have sustained energy and mental clarity. When you're in ketosis, you have more control than you thought you did. After all those years of being hangry and having energy slumps, you can finally take control back. In ketosis, people feel so much better. Cynthia loves the empowerment people feel during ketosis. “Women are not “mini men” and cannot fast the same way men do. But women can benefit from intermittent fasting when we make modifications based on our hormones.” Cynthia Thurlow [@cynthia_thurlow] nurse practitioner and CEO of Everyday Wellness Project shares from her experience as a practitioner and woman going through perimenopause how mastering intermittent fasting changed her life and health. - In this episode of How Humans Heal, Cynthia shares insights from her new book, Intermittent Fasting Transformation. She says that instead of being overfed, we need to be strategically fed. Women need more options besides prescription medications and surgery. Instead of just taking another pill or hormone for a symptom. We can use food choices as powerful ways to address symptoms. By making small, sustainable changes in the way we feed ourselves, we can address inflammation, insulin resistance, cardiovascular disease, and other health issues caused by “metabolic inflexibility”. - The bulk of us are not metabolic healthy, Cynthia says. Women are given reflux medications, anti-depressants, sleeping agents, and estrogen replacement. This creates a slippery slope and women are likely to feel wrecked by the time they hit perimenopause. In the sandwich generation, women are responsible for so much. We as women are care takers, but who is taking care of us. Cynthia is wanting to influence women's lives in a sustainable way, and that is why she wrote her book. - In this episode and in the book, Cynthis talks about: -As estrogen decreases, insulin sensitivity decreases -It shows up as fatigue, anxiety, weight gain, and menstrual irregularities -It used to be that restricting food and exercising more would work for weight loss, but in perimenopause, that stops working -She is not anti-carb; she encourages women to consume the right carbs -Intermittent fasting is free and flexible -Align food intake with chronobiology – our internal clock. Don't eat within 3 hours of bedtime. Eat when it is light outside. - We are so focused on fertility years, and then women feel invisible and unrepresented in perimenopause, and yet we spend half of our lives in that state. Over 35 to 40 years old, indiscretions of snacking or having a glass of wine is more likely to be an issue, increasing inflammation and blood sugar levels. We need to be eating less often and eat the way our bodies are designed to thrive. We would not be here as a species if our bodies could not go through periods of food accessibility and scarcity, and to use different types as fuel sources. Our bodies are not designed to be in a surplus of food all the time. When you go to eat a snack, ask yourself, am I really hungry? Or am I stressed or bored and either need to take a walk or go to sleep. Use of a continuous glucose monitor (CGM) – which is currently by prescription – is one of the most impactful things you can do. It can allow you to experiment with different foods to find out how your body responds to a certain meal. Being aware of what your blood sugar is doing can be hugely influential to understand how stress, travel, and certain foods affect you. - As hard as it can be to admit to ourselves that foods we love are an issue, when we do, it can allow us to make drastic changes in our health and wellbeing. So 45 seemed to be the perfect way to get people to become fat-adapted. This book is a labor of love because Cynthia wants women to have a blueprint for success. Sadly, at least 88.2% of the population is metabolically inflexible. Now is the time to get healthier. [04:10] Health Advice That Isn't True: Breakfast Is The Most Important Meal of The Day Breakfast is NOT the most important meal of the day. Sadly, breakfast has turned into a dessert; it's one of the worst ways to start the day. Instead, fast in the morning and allow your body to burn off stored energy. If you want to eat breakfast, start with eggs! Cynthia is a big fan of eggs, bacon, and avocado. Also, you can eat leftovers from the night before. [08:15] Calories In, Calories Out – Does That Really Matter When It Comes To Health? Calories are a measure of energy. You should be more concerned about the choices of macronutrients that you consume. Stop counting calories; start counting your carbohydrates. Start being conscientious about your protein intake and being conscientious about the quality of fats you're consuming. Worrying about calories has done us a tremendous disservice. [11:05] Why It's A Bad Idea To Snack Throughout The Day Many people still think that you need to eat for meal frequency to stoke your metabolism and keep your blood sugar stable. However, we are better off not evoking an insulin response throughout the day. When we're frequently eating, we are not giving our digestive system time to break down food. Meal frequency can be hugely problematic for people with imbalanced hormones. Our bodies are designed to eat less often. To really optimize our health, we need to follow along with that. [14:15] The Differences Between Brown Fat and White Fat White fat is the typical fat that you think about. There's not a lot of activity; there's a lot of inflammation, cytokines, etc. Brown fat is fat that has mitochondria. There's a lot more energetic capacity. When you're fasting, you can actually turn white fat into this more metabolically active mitochondrial-rich tissue. We want our fat to be as metabolically active as possible in a positive direction. [16:30] The Freedom That Fasting Can Create In Your Life When you are fat-adapted, you won't be focused on food. Plus, there is a spiritual side of fasting. Fasting is incorporated into all major religions. On so many levels, there is a lot more to fasting than people think. Fasting will give you a clean slate every day. [23:30] What You Need To Know About Oxytocin Oxytocin is really an underappreciated hormone. Oxytocin is a bonding hormone; also, it's an intimacy hormone. We want to have oxytocin throughout our day because oxytocin is an instant way to lower stress. It's a stress-relieving hormone that brings us joy! So, hug your loved ones and even your dogs to get a boost of oxytocin. [32:40] Fasting and Eating Schedules When You're In Certain Phases of The Menstrual Cycle The follicular phase starts from the first day of your bleeding cycle, all the way up until ovulation. During the follicular phase, estrogen predominates, so you have a lot of energy, and you feel really good. This is when you can push your workouts and have a lot more insulin sensitivity. You have a lot more flexibility with your nutrition plan during this time, and you can do longer fasts. After ovulation is predominantly the luteal phase where progesterone predominates. Progesterone is a wonderful hormone, much like estrogen, but they need to be balanced with one another. During this part of your cycle, you can do more restorative exercises like yoga. You should not be doing extended fasts during the five to seven days preceding your menstrual cycle. [36:30] Are You Sleeping Throughout The Night? If you are not sleeping throughout the night, do not start fasting. Remember, fasting is a stressor. If you add on stressors at the wrong time, it can be detrimental to your health. So, fix your sleep quality before starting a fasting schedule. This advice applies to both men and women. [49:40] What Postmenopausal Women Should Pay Attention To When It Comes To Their Health Fasting for women at this stage can be particularly beneficial because they don't have to worry about a menstrual cycle. Postmenopausal women and men are at a level playing field in the sense that there isn't as much hormonal fluctuation. So, women can really capitalize on fasting during this stage of life. Once you get your sleep dialed in, then you can get deliberate about a ketogenic diet. AND MUCH MORE! My passion is helping women find wellness through the healing power of nutrition and solving health problems from the inside out. But, what I'm most proud of are my titles of mom and wife! I have all boys, including two Doodles and a loving, patient and supportive husband. I worked in clinical medicine, both in the ER and cardiology, for nearly 20 years. The power of Western medicine's approach to acute and life-threatening disease and emergencies is undeniable. However, I started to question our approach to the on-going symptoms of chronic health problems. I started to think more about the root causes of the chronic health problems of my patients. I became passionate about learning how I could help my own family, patients, and clients avoid chronic health issues and diseases before they even became a problem. One of the most rewarding aspects of being a nurse practitioner and nutritionist expert are the relationships I develop with patients and clients. I started to see a powerful relationship between the foods that we eat and improved symptoms, health and wellness. When I saw my patients see big improvements in their health, I was inspired to do advanced training in nutrition and functional testing. Unexpectedly, in 2019 I had a significant health care hiccup. I was traveling back home to Washington, DC, from a romantic Hawaiian vacation with my husband, and 48hrs later I found myself in the ER with a ruptured appendix, inflamed colon and bought myself a 13 day hospital stay with further complications. Things got really serious when I couldn't leave my bed, I couldn't eat, I had tubes everywhere, drains coming from my abdomen, and was too weak to deal with anything in my normal daily activities. I was away from my home, my family and my world was very small. My two things that pulled me through this were my boys and the visualization of seeing myself on the TEDx stage, delivering my intermittent fasting talk. I used this technique to keep me motivated on recovery. Nothing prepares you for being a patient when you are used to being the health care provider. I'm happy to report that I graced the TEDx stage 27 days after returning home from my hospitalization and my life changed forever. The YouTube video of my TEDx talk, as of March 2020 has been viewed by over 5 million people. This has lead to incredible opportunities that I am so grateful for. I am uniquely positioned to support women as they struggle with and try to make sense of issues related to lack of energy, weight gain, and food cravings. I understand their frustration when they don't get the answers they want and need from traditional medicine. My patients inspire me to not only create unique one-on-one programs, but also group programs that help address, support and educate women on the profound mind-body connection and provide the tools to address these concerns. I do not believe nor do I support the limiting belief that women have to accept weight gain as a normal function of aging.
