Welcome to Episode 78 of the Movement Logic podcast! In this episode, Laurel and Sarah discuss what current science, versus outdated advice and conventional wisdom, have to say about the knees—namely whether “bone on bone” is a thing, and if deep squats, knee valgus aka “knee cave”, or high impact are inherently bad for your knees. Learn what research has to say about some of the most common fragilizing beliefs people hear about their knees, and why these scary tales are just plain wrong. In this episode you will learn that: The knee is strong and adaptable, capable of handling various loads with training. The knee has a wide range of safe positions, especially with progressive exposure. Knee pain doesn't always mean injury, and injuries can heal with proper care. The knee isn't a simple hinge; it allows rotational and lateral movement. The kneecap doesn't always need to face forward in standing. Knees can lock or hyperextend without causing harm, depending on the person. The knee can safely move past the ankle and toes during squats or lunges. Running and landing don't require the knee to track perfectly forward. Knee valgus is not inherently dangerous. Deep squats, high-intensity exercise, and running do not cause arthritis or "wear and tear"; they strengthen the knee. A good friend of mine is a world class triathlete. He won the 70-74 age group at the Ironman world championships in 2017 and has aspirations to repeat that victory in the next few years. Unfortunately, like many triathletes, he has also suffered some knee issues which are currently interrupting his progress. Consequently, we have spent the last few months chatting about options available to him and which (if any) offer the best chance of a return to running.   I have some inside knowledge here. In the last 15 years I've had 3 minor operations to remove damaged cartilage from my knees. After each op I questioned whether I could return to running and how sensible this would be for my long term knee health. To date it has resulted in me being a little more cautious with some activities, but I can still run. It seems I'm not the only one. Does running damage your knees? And is cycling any better? Runner, cyclist, GP and Inside Health regular, Dr Margaret McCartney goes to the new Motion Analysis Lab at Glasgow's Jubilee Hospital and asks orthopaedic surgeon and competitive cyclist Jason Roberts about the latest evidence. Around 30,000 people a year suffer cardiac arrest - their heart suddenly stops pumping blood around their body - and fewer than one in ten survive. Paramedics and ambulance crews will give CPR and use a defibrillator to try to restart the heart, and for the past 50 plus years, most patients will be given a shot of adrenaline too. But a landmark new study funded by the government and run by Warwick Medical School reveals that giving adrenaline barely increases survival and almost doubles the risk of severe brain damage. Dr Margaret McCartney discusses likely changes to policy with Dr Mark Porter. It's said that eyes are the windows to the soul - and certainly looking into other peoples' is the key part of human interaction. But what if one of yours isn't real? Sixty thousand people in the UK have an artificial eye and Europe's largest maxillo-facial laboratory at Queen Victoria Hospital in East Grinstead offers a bespoke service where specialists make individual eyes from live sittings. Susan lost one eye as a child and she tells Mark that her latest prosthesis is her favourite. Why? Because it's almost half the weight of eyes she's had fitted before. Dr Emma Worrall, principal prosthetist, has invented a lighter sphere. In a lightbulb moment sitting in a café stirring a sugar cube into her coffee and watching it melt, Emma tells Mark that she realised she could build the plastic sphere around sugar, drill a tiny hole, then melt the sugar out of the middle! Twenty patients at the hospital are now benefiting from lighter eyes (which means less surgery). And there's another plus. The new eyes float in the swimming pool and the sea! Producer: Fiona Hill.

Find out how ACL injuries occur, as well as how to treat and prevent them.The anterior cruciate ligament (ACL) is one of four ligaments that stabilizes the knee. It's the most commonly injured knee ligament. There are nearly 200,000 ACL repair surgeries in the United States each year. Women are seven to eight times more likely to suffer ACL injuries than men. The ACL prevents the knee from derotating in unnatural ways. Accelerating, decelerating and turning puts torsional stress on the knee. The ACL prevents that twisting. An injury is often accompanied by a pop and/or the knee giving out. It typically swells but doesn't need emergency treatment. A stabilizer and anti-inflammatory are the usual treatment. The injury can recover in several weeks. An ACL tear will not repair itself and leads to knee instability. Rest, ice, compression and elevation can help the knee feel normal again. Physical therapy can help with swelling, soreness and mobility. Nearly every athlete is offered ACL repair surgery because they want to return to athletic activity. Bracing is effective for those who opt out of ACL repair surgery and engage only in low-level physical activity. ACL Injury Prevention Neuromuscular training is the best prevention. It teaches the body how to land, jump and cut properly. This could reduce ACL injuries in women by 50% but is also effective for men. Minding fatigue and resting your body can reduce your body's required recovery time. If your child is involved in two sports, be sure she's getting the necessary time to recover from athletic endeavors. Stick to efficient workouts and get plenty of sleep.
