In this episode, Dr. Chris Motley sat down with heart surgeon, author and podcaster Dr. Philip Ovadia to learn about the underrated relationship between insulin resistance and heart disease, and what you need to know to arm yourself at home and in the doctor's office.  If heart disease has been the number one killer for so long, isn't it possible that we're tackling it the wrong way? Dr. Ovadia's belief is that our emphasis on cholesterol has done us a disservice when it comes to heart health.  If you have a family history of heart issues, or a heart condition yourself, learn how Dr. Ovadia believes you can stay off his operating table.  In this episode you will learn: Why cholesterol and LDLs are not the real enemy of the heart and how sugar is the real culprit when it comes to heart disease.  Why 70 years of cholesterol-focused medicine hasn't reduced heart disease — and what the data actually shows about LDL's role in cardiac events The Craft Test: how Dr. Joseph Craft discovered in the 1970s that 95% of heart disease patients had insulin resistance — compared to only 50% with elevated cholesterol Why your fasting insulin level is the most important number most doctors never order — and how to get it tested for around $10 What exactly to ask your doctor about insulin resistance, and signs and symptoms for it: waist circumference, blood pressure, fasting glucose, HDL cholesterol and triglycerides. How stress plays a role in heart conditions, and how emotional support can make a difference in patients' outcomes. Why statin drugs can be problematic, in that they hide the symptom rather than attacking the root of the problem.  The single question to ask your doctor that will immediately reveal whether they understand metabolic heart disease: "Am I insulin resistant?" Why skin tags, belly fat, rounded face, low energy and high blood pressure are early warning signs of insulin resistance — years before chest pain or shortness of breath appear Dr. Ovadia's recommendations for diet and lifestyle changes to keep you off the operating table: processed food needs to go, and we must move more. If you have a desk-job, try a standing desk or simply get up and move around once an hour.  Meat is medicine.     I sat down with Jordan Bales this week for one of those conversations that starts as a catch-up and turns into a full-on masterclass — and I didn't even plan it that way. It's been way too long since Jordan and I recorded together (we live in the same town, so we keep telling ourselves we don't need a podcast to catch up — turns out we do). Jordan's got a trail camera bull he's chasing this fall — a mid-July photo puts him around 340", with room to grow into the 350 range by September — and he wanted to pick my brain on how to actually turn this thing up before archery season opens. So that's exactly what we did. We get into the real difference between hunting a "201-level" six-point and hunting a genuine target bull — because trust me, those are two completely different games. We talk about why elk don't respect your neat little three-mile circle on OnX, why a good bull might blow off his summer ground and go rut ten miles away, and why "air miles" is the only way to actually think about elk movement in big mountain country. We also get into backpack-hunt logistics, meat care in the backcountry, why purple roads and hiking trails matter less than you think (until they don't), and the mental shift it takes to go from "find one bull" to "learn four different herds at once." Three Key Takeaways Think in air miles, not trail miles. The mental trap most hunters fall into is measuring distance the way a human experiences it — on foot, along trails. Elk cover ground on a completely different scale. A bull that seems "close" to a herd on the map might actually be far enough that he's already moved on to check her and bounce. Recalibrating your sense of distance is one of the biggest jumps from intermediate to advanced elk hunting. Targeting one specific bull requires understanding every herd around him, not just his. The moment you commit to "301-level" hunting — chasing one specific animal instead of just filling a tag — the job changes from finding an elk to mapping an entire drainage system: every bachelor group, every cow herd, every likely wind-and-terrain funnel within a few miles. It's genuinely harder and less fun than general elk hunting, which is exactly why it usually takes a team. Patience beats pressure with a known target bull. The instinct to hike straight into a bull's last known location and start calling is usually the fastest way to lose him for the season. Watching from a distance, learning what the herd is doing morning and evening, and staying in "kid glove" mode until you actually have eyes on him gives you a far better shot than blowing him out of the country on day one.

Also 2 mishnayot. 1 - No meat and milk together, not even on the same table, where "meat" refers to domesticated animals, non-domesticated animals, and birds, but not to fish or grasshoppers. 2 - That policy of no meat and milk on the table together is a matter of dispute between Beit Hillel and Beit Shammai. Note that this is one of the 6 unusual times when Beit Hillel is more stringent than Beit Shammai is, as per a mishnah in Eduyot. Plus, the construction of the mishnah for the sake of mentioning the name of the source, as that is said to bring redemption (and for which the Book of Esther is, perhaps oddly, used for the prooftext).
