In part, this started from when he was a competitive rower, developed an allergy to a cat, and was prescribed ginger tea as a treatment. Dr. Jeremy Howick, PhD is the Director of the Stoneygate Centre for Excellence in Empathic Healthcare and a Senior Researcher at Oxford University. He is the author of Doctor You and The Philosophy of Evidence-Based Medicine as well as a book released November 14, 2023, The Power of Placebos: How the Science of Placebos and Nocebos Can Improve Health Care. He obtained his PhD from the London School of Economics.

Abstract: Expectancies, which are higher order prognostic beliefs, can have powerful effects on experiences, behavior and brain. However, it is unknown where, how, and when, in the brain, prognostic beliefs influence appetitive interoceptive experiences and related economic behavior. This study combined a placebo intervention on hunger with computational modelling and functional magnetic resonance imaging of value-based decision-making. The results show that prognostic beliefs about hunger shape hunger experiences, how much participants value food and food-value encoding in the prefrontal cortex. Computational modelling further revealed that these placebo effects were underpinned by how much and when during the decision process taste and health information are integrated into the accumulation of evidence toward a food choice. Abstract: Expectations about the quality of a medical treatment influence how much an inert treatment helps to improve patient well-being. Similarly, expectations about the quality of products influence how identical goods and services are evaluated differently after their consumption. One driver for such "placebo effects" in medical treatments is social cognition in the form of trust, which may be influenced by the hormone oxytocin. An open question is whether trust and oxytocin play similar roles in marketing placebo effects. To answer this question, we combined oxytocin administration (24 IU) and trust questionnaires in a pre-registered double-blind randomized between-subjects study design (N food tasting task = 223; N cognitive performance task = 202). We could not find evidence that oxytocin and trust play a role in placebo effects of marketing actions. Together with other recent null findings from oxytocin administration studies, these findings question the role trust might play in different types of placebo effects. In this episode we discuss the placebo effect, and some famous examples of this phenomenon. In research, placebos are used to determine if what is being studied has a significant affect above and beyond what we believe will happen. As essentially, a placebo can be extremely effective if we believe if will have an affect. So in essence, our mind can determine how effective something is. This can be witnessed in multiple different areas, especially when it comes to supplementation, diets and training  programmes.

Our guests today are Mike Guadango and Alex Brooker.  Mike and Alex (“Brooker”) met when Alex interned for Mike at DeFranco’s gym a decade ago, and they now have a podcast together, “The Mike and Brooker Show”, in addition to their coaching careers. Alex is the owner and operator of Pathfinder, a private training service focused on performance psychology and physical preparation for professional athletes.  In addition to traditional schooling, Alex is now pursuing his PhD in Self-Hypnosis at the University of Bern.  Mike is currently a Coach, Writer & Owner at Freak Strength.  He has been mentored by coaching greats such as Buddy Morris and James Smith, and started his career working at DeFranco’s gym.  Mike has coached levels of athletes from many different professional sports to Olympic medalists to pre-pubescent athletes, as well as consulting for high caliber athletes and coaches worldwide. As an ever-optimistic individual, it’s important for me to have conversations with those who have a different way of looking at what actually works in the world of sports performance.  In the coaching world, it is extremely easy to have worked with an athlete who has achieved a high result, and then rationalize the factors that led to their success.  It is very easy for us all as coaches to think of our own training as highly optimal, but a question to ask is how often and effectively we truly challenge our reasoning? In looking at training closely, it is helpful to fully understand the power of belief, as well as placebo effects in not only training, but also pain science and rehabilitation.  Understanding human adaptation to training and rehab stimulus requires, not only an understanding of the body, but also of the mind. In today’s podcast, Mike and Alex “Brooker” talk about how they have evolved themselves as coaches, moving into the realms of hypnosis/mental training, acupuncture and rehabilitation.  We spend a lot of time chatting about the power of belief and the ability of the mind to supercede a “poor” training program, and how the fundamentals of adaptation style can be seen in rudimentary rehab.  Finally, Mike, Brooker and I spend some time discussing some training points such as play, competition in training, and training transfer.  This was a fun show with speakers of 3 clearly diverse viewpoints, which always makes for great discussion. Her poems have been included in more than three dozen anthologies and textbooks. She won the 2009 Dana Award for Poetry, and also The Greensboro Review literary award for poetry in 2003 and was a finalist for the Writers’ League of Texas Book Award, 2011. From 1992 to 2000, she was co-editor of the literary magazine American Letters & Commentary. Background and Objective: A recent Cochrane review on placebo interventions for all kinds of conditions found that `physical placebos' (which included sham acupuncture) were associated with larger effects over no-treatment control groups than `pharmacological placebos'. We re-analyzed the data from this review to investigate whether effects associated with sham acupuncture differed from those of other `physical placebos'. Methods: All trials included in the Cochrane review as investigating `physical placebos' were classified as investigating either (sham) acupuncture or other physical placebos. The latter group was further subclassified into groups of similar interventions. Data from the Cochrane review were re-entered into the RevMan 5 software for meta-analysis. The primary analysis was a random-effects analysis of trials reporting continuous outcomes of trials that used either sham acupuncture or other physical placebos. Results: Out of a total of 61 trials which reported a continuous outcome measure, 19 compared sham acupuncture and 42 compared other physical placebos with a no-treatment control group. The trials re-analyzed were highly heterogeneous regarding patients, interventions and outcomes measured. The pooled standardized mean difference was -0.41 (95% confidence interval -0.56, -0.24) between sham acupuncture and no treatment and -0.26 (95% CI -0.37, -0.15) between other physical placebos and no treatment (p value for subgroup differences = 0.007). Significant differences were also observed between subgroups of other physical placebos. Conclusion: Due to the heterogeneity of the trials included and the indirect comparison our results must be interpreted with caution. Still, they suggest that sham acupuncture interventions might, on average, be associated with larger effects than pharmacological and other physical placebos.

Evidence for placebo effects on physical but not on biochemical outcome parameters: a review of clinical trials

Background: Recent reviews on placebo effects in clinical trials suggest that objective changes following placebo treatments may not exist or, at least, have been considerably overestimated. However, the possibility that yet unidentified subsets of parameters are responsive to placebo treatments has not been taken into account. Therefore, the aim of the present study is to examine the effects of placebo treatments on objectively measured outcome parameters by specifically focusing on peripheral disease processes. Methods: An initial dataset was collected from a MEDLINE search for placebo-controlled, randomized clinical trials. Trials with stable disease conditions were identified, and the effects of placebo treatments on peripheral outcome parameters were estimated by the changes from baseline in the placebo groups. An explorative data analysis was conducted in order to identify parameter classes with differential responsiveness to placebo treatments. A subgroup meta-analysis of a second dataset was performed to test whether the preliminary classification would also apply to placebo effects derived from the comparison of placebo groups with untreated control groups. Results: The explorative analysis of outcome parameters and strength of placebo effects yielded a classification into responsive "physical" versus non-responsive "biochemical" parameters. In total, 50% of trials measuring physical parameters showed significant placebo effects, compared with 6% of trials measuring biochemical parameters. A subgroup meta-analysis substantiated the differential response ( physical parameters: n = 14, Hedges' pooled effect size g = 0.34, 95% CI 0.22 to 0.46; biochemical parameters: n = 15, g = 0.03, 95% CI - 0.04 to 0.10). The subanalysis of the second dataset supported the classification and revealed a significant improvement for physical parameters ( n = 20, g = 0.22, 95% CI 0.07 to 0.36) and a deterioration for biochemical parameters ( n = 6, g = - 0.17, 95% CI - 0.31 to - 0.02). Conclusion: The results suggest that placebo interventions can improve physical disease processes of peripheral organs more easily and effectively than biochemical processes. This differential response offers a good starting point for theoretical considerations on possible mediating mechanisms, and for future investigations in this field.
