Discover the intricate dance of energy expenditure as we unravel the myths with Dr. Eric Trexler, an authority in this field. Our conversation breaks down the complex interaction between physical activity and calorie burn, guiding you through the additive and constrained models of total daily energy expenditure. With Dr. Trexler's insights, we shed light on Herman Ponzer's eye-opening research with the Hadza tribe, which turns the tables on our understanding of energy usage in sedentary Western societies compared to their highly active counterparts.Timeline:00:20 – Dr. Trexler explains in simple terms the additive vs. constrained model of total daily energy expenditure (TDEE).4:38 - Hadza tribes (modern-day Tanzania); Dr Trexler joined Herman Ponzer's lab in 2023.  7:00 – Some do up to 35,000 steps per day.  How much movement is that?10:00 - Once you reach a certain amount of total physical activity, there appear to be compensatory adjustments vis a vis TDEE. 12:24 - Maximal measured EE per day over months seems to be around a functional limit of 2.5 x a person's basal metabolic rate.  It scales to body size in general. 15:00 – For pregnant women in the 2nd or 3rd trimester we may see a 2.2 range of metabolic scope17:00 – Metabolic scope values as high as 10 in the short run.28:00 – Metabolic adaptation31:14 – Additive vs. constrained models36:40 – Our energy intake is driven by the hedonic value of a given meal, not necessarily related to the energy needs of the person.39:59 – Quality of exercise is best served by choosing better foods; exercise is a behavior that affects so many other behaviors.47:37 – Are there tissue-specific changes in energy expenditure with age?  Does this explain, in part, the drop in RMR after age 60?48:00 – Liver, Brain, Heart and Kidneys are much more metabolically active than skeletal muscle – do these organs decrease in EE? About our special guest:  Eric Trexler PhD Dr. Eric Trexler is a postdoctoral researcher in the Department of Evolutionary Anthropology of Duke University, where he conducts research on cardiometabolic health and energy expenditure regulation. We discuss how exercise and diet may lead to a healthier microbiome, and the potential for functional redundancy among the thousands of types of bacteria in our gut. The Campbell Lab has recently focused on the links between gut microbiota, brown adipose tissue, skeletal muscle, and environmental toxicants like ozone. The Campbell Lab has published papers on how exercise impacts gut microbial ecology and intestinal inflammation and permeability in a sex specific manner, how gut microbiota depletion impacts exercise capacity and mitochondrial content of key proteins related to oxidative phosphorylation and biogenesis, hormone deficiency (estrogen in particular) and gut health and the bidirectional link between the gut microbiota and exercise. In a riveting conversation with researcher Dr. James L Nuzzo, we dissect the differences in skeletal muscle fiber types between men and women. James brings unique insights from his meta-analysis of over 100 studies, unraveling fascinating intricacies about muscle power and strength differences between men and women.  Then, we turn the spotlight on the less examined subject of gender differences in exercise sciences and academia. The absence of discourse on the physiological disparities between men and women, the controversy of transgender females competing against biological females, and the need for a proactive dialogue in this realm are just some of the issues we unpack in this episode.Timeline:0:20 – About Dr. James Nuzzo2:46 - Sex differences in skeletal muscle fiber types; yes, men have more fast-twitch muscle fibers!11:22 – Female exercise scientists – are they silent on the issue of women's sports and transgender athletes? 31:37 – Men's health disparities and institutional support.34:00 - Are women under-represented in scientific research? Meaning, women are not being studied? The data is surprising.45:48 – Reconsidering the value of flexibility About our guest:James L. Get ready to shatter stereotypes as we journey through the controversial domain of testosterone and steroids in sports with our high-profile guest, Rick Collins Esq., an internationally recognized legal authority on everything from sports nutrition and bodybuilding supplements to performance-enhancing substances. Tune in for an enlightening discussion that promises to challenge preconceived notions, bringing you face to face with the surprising reality that 80% of non-medical steroid users are not athletes but individuals seeking physical enhancements for purely cosmetic purposes. Timeline:0:00 – Introduction – the 1996 publication on Androgens – it's not as harmful as folks make it out to be in otherwise healthy males.4:43 – Remember 100-meter sprinter Ben Johnson? Why is testosterone maligned? What are the socio-cultural aspects of this?7:14 – Testosterone as a metaphor for masculinity; traditional masculinity is maligned.7:39 - American Psychological Association in 2019 – Traditional masculinity is toxic9:05 – Does bodybuilding contribute to the stigma of anabolic steroids?9:20 – Rick was the only lawyer that lifted weights when he was in law school. C'mon lawyers, lift some weights!12:21 - Who actually uses anabolic steroids and why? Cyclists? Pro Athletes? Not so fast…80% of users in a non-medical context are using it for cosmetic purposes. 18:00 – T helps body composition but is frowned upon; juxtapose that with cosmetic procedures that women routinely take that likely carry greater risk. A double standard?23:47 – Rick covers the legality of the possession and sale of androgens.28:28 – ACSM promoted total BS about androgens back in the 70s and 80s.32:57 – Gender dysphoria (DSM-5) and muscle dysmorphia (bigorexia nervosa). An interesting analogy, as Rick opines.41:15 – Bodybuilders and Transgender individuals – what do they have in common?46:18 – Can testosterone turn Democrats red?Rick Collins, Esq. is an internationally recognized legal authority in the field of sports nutrition, bodybuilding supplements, and performance-enhancing substances. “Glucagon-like peptide-1 (GLP-1) agonists (also known as GLP-1 receptor agonists, incretin mimetics, or GLP-1 analogs) represent a class of medications used to treat type 2 diabetes mellitus and, in some cases, obesity. Examples of drugs in this class include exenatide, lixisenatide, liraglutide, albiglutide, dulaglutide, and semaglutide. According to the American Diabetes Association, metformin remains the preferred first-line therapy for treating type 2 diabetes. However, the addition of a GLP-1 analog should be considered in patients with a contraindication or intolerance to metformin, in patients with a hemoglobin A1c greater than 1.5% over target, or in patients who do not reach their target A1c in three months, particularly in patients with atherosclerosis, heart failure, or chronic kidney disease.” (Collins and Costello, 2023).Edwin Davila DO MS CISSN is a senior resident of Internal Medicine, TIGMER San Antonio.
