Have you ever wondered how biases in medical research and education can significantly impact women's health and health in the LGBTQ+ community? Do you want to learn how you as a CME/CPD professional can promote inclusive and equitable language in your work, and why this is important in CME content creation? My guest today is Caitlyn Tivy DPT, a physical therapist, medical writer, and femtech pioneer. In today's episode, episode 101, we explore concrete ways CME/CPD professionals can help propel a shift toward more inclusive and equitable education and, ultimately, patient care. Caitlyn shares where harm has been done to women and LGBTQ+ patients through exclusion and bias in everything from clinical trials to medical curricula and terminology. We explore the real-world consequences of these biases, such as the underdiagnosis of heart attacks in women and the mismanagement of endometriosis. Ferring is a leader in reproductive medicine and maternal health, as well as in specialty areas within gastroenterology and urology, including microbiome therapeutics and orthopedics, and has been developing treatments for mothers and babies for over 50 years. Dr. Garner has broad responsibilities for U.S. Clinical Development, Medical Affairs, Pharmacovigilance, Project Planning and Regulatory Affairs. Dr. Garner has nearly 30 years of experience in medicine and industry. Today she is here to talk with us about a foundational position paper that AMWA and Korn Ferry Health published at the end of September 2022, “Women in Medicine- Setting the Agenda for Change” by Dr. Eliza Lo Chin and Liz Bickley of Korn Ferry Health. The report "explores the challenges that confront women in medicine and the work that is still needed to close the gender gap in healthcare leadership". It explores areas such as burnout, mental health crises, caregiving conflicts, fertility concerns and underrepresentation of women in leadership roles. We hope you enjoy the conversation and the many areas she discusses including: -Empowering women, creating opportunity -Creating clear goals in your job role, and regularly debrief and make sure the boundaries and goals you are in line with your expectations in order to support your career but also provide support to prevent burnout. -Institutions should see wellbeing as more than a "checkbox". Consider having coaching more available for physicians without the burden of having to seek it out. Examine onboarding practices of physicians and increasing time with other colleagues in order to help them more familiar with processes, expectations, and feel more comfortable in the environment. -Re-evaluation of recruiting mechanisms in order to increase the diversity in leadership positions. -Time and talent reviews. -Thinking outside the box, opening our eyes to other possibilities. -Making childcare -- both sick and well-- more accessible to alleviate a burden that is often cited as a reason for women leaving the workplace. Dr. Chin is an assistant clinical professor of Medicine at the University of California in San Francisco. She has been actively involved in American Medical Women's Association (AMWA) for over a decade. Working with and collaborating with women physician leaders is her passion and she is honored to be AMWA's executive director and to serve the organization that has provided her with so much support. Dr. Chin is a graduate of the University of California in Berkeley, Harvard Medical School and Columbia University's Mailman School of Public Health. She completed her training in primary care at the Brigham and Women's Hospital in Boston. She was Assistant Professor of Medicine at Columbia for many years before relocating to California where she continues to teach medical students and practice medicine part-time with a focus in geriatrics. Dr. Chin currently resides in California with her husband and their three children. Dr. Theresa Rohr-Kirchgraber, a California native, is an internist and professor of medicine at the AU/UGA Medical Partnership in Athens, GA. As the president of the American Medical Women's Association, her passion is equity in medicine and healthcare, both for physicians and the community. In this episode, Theresa talks about her unique path to medicine and the resets she has made as a clinician-educator.  Who knew that baton twirling would have the outcome it did!  She shares her strategy to determine the timing for those resets noting it is ok to sometimes need a change of scenery. Her outlook on options as a physician, particularly as a generalist, is a refreshing take.  We recorded this interview in mid-June 2022 prior to the Supreme Court decision but she was already highlighting the powerful need for advocacy in all aspects but particularly equity and women's reproductive health. She appeals "Let's not go backwards." Listen in to learn more about AMWA, the Power of Women book she just read, and the power of a community of physicians.  Pearls of Wisdom: 1. Finding a supportive community can lead to opportunities of "Why Not!" 2. In this episode, I have an epiphany about shame as related to feeling emotions, courtesy of Dr. Katie Jobbins! I have never connected shame with mothering or doctoring, and I need to think about it more. See what you think! Dr. Katie Jobbins, DO, MS, FACP is an Associate Program Director in the Internal Medicine Department at UMMS-Baystate in Springfield, MA, a primary care internist at High Street Health Center Adult, and an academic Hospitalist at Baystate Medical Center. Dr. Jobbins received her undergraduate degree in Nutrition Science from Syracuse University and her Masters in Science from Case Western Reserve University in Nutrition and Metabolism. She attended Ohio University Heritage College of Osteopathic Medicine where she graduated in 2010.  Initially, Dr. Jobbins started her training in general surgery residency at the Cleveland Clinic--South Pointe but after 2 years found her true passion was in Internal Medicine. She completed 3 years of Internal Medicine at Baystate Medical Center and stayed on to be Chief Resident in 2015-2016. In her current role as Associate Program Director in the UMMS-Baystate Department of Internal Medicine, Dr. Jobbins serves as a primary care preceptor in the resident clinic, an academic hospitalist, director of the resident self-reflection, wellness, and ambulatory quality improvement curriculums, co-director of the humanities track, and director of the Chief Resident leadership curriculum for the health system.  She also leads the “Be the Change Social Justice” working group focusing on health inequities in patient care and in medical education. Dr. Jobbins is also an Assistant Professor at UMMS-Baystate. She is an active member both nationally and locally in SGIM, AMA, AMWA, and the ACP. She currently serves as the Meeting Chair for the New England Regional SGIM chapter. In the Massachusetts ACP Chapter, she is the immediate past co-chair of the Early Career Physician Council (CECP) and was an active member of the Governor's council. She currently is on the National CECP.  She recently won one of the AMA Inspiration Awards for her work in medical education and was AMWA's 2021 recipient of the Exceptional Mentor Award.

Chlorine can have a negative impact on softener resin over time, causing it to break down and lose its structural integrity. At normal free chlorine levels, you may not have ever noticed it, but at higher levels, you may have seen the impacts on softener resin. One option available is what some refer to as “chlorine-tolerant” softener resin, but what does this really mean? Is the resin made of tougher material than before, or is the resin just made of more of the same material so it can last longer in its harsh environment? Take this week to think about the impact of chlorine on softener resin and what a chlorine-tolerant resin ultimately is.    Quotes: “If you put a badly fouled Cooling Tower louver into a basin of a Cooling Tower, your water treatment products will clean it for you, you can hose it off, and it will look brand new.” - Trace Blackmore “If you have to order a new Cooling Tower louver… order 2, and rotate through which one is in use and which one is being cleaned in the basin with your water treatment products. She explains that she has been carrying out this effort in collaboration with the American Medical Women's Association (AMWA) who has recently made a concerted effort to bring this issue out to the public and the medical profession. This began with an AMWA-sponsored Physician Fertility Summit at which Dr. Guidry was amongst the many guest physician leaders and speakers shedding light on this little discussed topic. During our visit with Dr. Guidry, she bravely shared with our audience her own personal issues regarding infertility so that others may know that they are not alone. She explains that Female Infertility amongst physicians is 3-4 times higher than the general population and that much research is needed to understand both the causes of this medical condition and how to best prevent and treat it. Dr. Guidry reveals many of the obstacles, biases and misconceptions regarding infertility and family planning amongst physicians. She stressed that this is an issue that can seriously affect women physicians and their families. She advocates for early awareness of this issue amongst those she mentors and her colleagues. Together, Dr. Fethke and Dr, Guidry agree that physicians wishing to have children are more well-rounded and healthy people when they are able to do so, thus improving their professional and personal lives. Through organizations like AMWA and RESOLVE, Dr Guidry is optimistic that the issue of Physician Infertility is now out in the open. She strongly believes that physicians have a unique platform to address this issue for themselves and the public at large in order to improve awareness and access to infertility-related medical care. She looks forward to a day when physicians can study and practice in environments that are supportive and proactive for all physicians who are in need of evaluating their fertility as a significant component of their physical and psychosocial health. Change is difficult. When situations change, we enter the unknown. We may fear losing opportunities or making the wrong choice. Today's guest wants us to embrace change and approach it with an open mind. In this way, curiosity, the opposite of dread, can lead us to greater things.   When considering a change: Examine your limiting beliefs Learn from others who have gone before you Determine your priorities Ask for help from your support group   About Dr. Marion McCrary Dr. Marion McCrary is an experienced primary care internal medicine physician and a national board-certified integrative health and wellness coach. She is passionate about physician wellness and serves as the Well-Being Champion for the NC chapter of the American College of Physicians.  Dr. McCrary is also a coach for ACP and AMWA leadership programs. Her newest role is the Associate Director for GME Coaching at her home institution and she is looking forward to guiding residents and fellows in their professional and personal development. She is also a writer and a podcaster. She is one of the co-creators and hosts of the ResetMD podcast where physicians have conversations about what has sustained their well-being.   Self-Examination is Part of Making Changes in Your Life In this episode, Dr. McCrary describes how to break away from the daily grind. These aren't decisions to be made on a whim. It takes time to examine limiting beliefs and understand how to move forward. Big changes require big introspection. Self-reflection is necessary to move forward in an informed way. When embraced with a sense of curiosity, transitions are an invaluable time to learn about yourself. You'll see just how much control over your circumstances you really have. It's also great for your confidence to see that you can take risks and turn out okay.   Change as a Solution to Burnout One of the instigators of change is burnout. We discuss common causes of burnout. When you say yes to too much or lack the support you need, exhaustion is a natural consequence. Dr. McCrary wants us to fight burnout by instituting whatever control we have. Take control over your thoughts and time. She then completed 3 years of Internal Medicine and stayed on to be Chief Residents in 2015-2016 at Baystate Medical Center.     In her current role as Associate Program Director in the MMS-Baystate Department of Internal Medicine, Dr. Jobbins serves as a clinician-educator working with residents in both the inpatient and ambulatory setting. She leads the social justice working group, the director of the resident’s self-reflection and wellness curriculums, ambulatory quality improvement curriculum, co-director of the humanities track, and director of the Chief Resident leadership curriculum for the health system.   Dr. Jobbins is also an Assistant Profession at UMMS-Baystate. She is an active member both nationally and locally in SGIM, AMA, AMWA, and the ACP. In the Massachusetts ACP Chapter, she is the co-chair of the Early Career Physician Council and supervises the resident/fellow council. She recently won one of the AMA Inspiration Awards for her work in medical education.    Prioritizing Yourself During a Pandemic When this pandemic hit, Dr. Katie Jobbins found herself falling back into old habits where she didn’t prioritize her own health. Despite previously going to therapy and private programs, it wasn’t until she joined a group program that Katie felt she had the tools and accountability to make lasting changes. The pandemic has impacted all of us in one way or another, and for so many of us, it’s been detrimental to how we take care of ourselves. Katie explains why it was essential for her to find time for herself. Katie shares some of the ways she started prioritizing herself throughout the last year. It doesn’t matter that there’s a pandemic going on - it shouldn’t change the way you care for yourself.   Creating Space for Yourself Every Day Katies says you need to make sure you’re doing something for yourself every single day - and notice when and what this is. It’s not only about doing something for yourself. The act of recognizing that you’re putting yourself first has a positive impact on your overall health, too. Katie shares some ideas for how to look for and create space in your life. Can you go for a walk during your lunch break? Take advantage of what space you already have in your schedule. Finally, Katie talks about how she started enjoying exercise and movement. It’s so different to approach movement and wellness from a place of love and joy than from a space of necessity. I needed something that could help me have a deeper connection with other people.”  [7:13] “The pandemic’s going to keep going. It’s not going away, this is our life now. For us to say that that we’re going to get to a new life at some point, great. But this is where we are now. I can’t change that. I can control what I do, I can try to control what goes on in my family. The biggest part of the transformation is that I’m okay with the chaos now.” [13:12] “You should do something for yourself every day. The idea that there is space for you if you let yourself have it. It’s okay, don’t feel guilty. Create that space for yourself.” [15:25] “I think that the biggest takeaway from the Transform process is that I’m taking care of myself and I feel less helpless. When the pandemic hit, I felt helpless, and I couldn’t figure out where that came from. It’s that I really needed to help myself. I was already helping so many other people. Chrissa is a textile design MA graduate from the world=renowned Chelsea College of Art and Design, University of the Arts London. She was born and raised in London, but her roots are very much Ghanaian. This is the country from which she draws her inspiration. Other countries that are of significance in her life include Togo and Benin. The episode focusses on understanding the duality that describes Chrissa’s life and attempts to understand how she successfully navigates her two worlds. What does it take to be an inspirational designer? What does she describe as art and what is her creative process? Amuah, who grew up in London’s Brixton neighbourhood, feels a strong connection to her African heritage. An important part of her designs is Adinkra symbology, which stems from a Ghanaian oral history tradition and is a visual way to communicate important values. These symbols are re-interpreted in her designs, from the obvious to the very subtle. My other interest is in sex and gender medicine, and my textbook on this topic will be available in late October, published by Elsevier.

Dr. Kaci Durbin is a medical writer and board-certified obstetrician. She currently works as a regulatory writer for a contract research organization (CRO). In her spare time, Kaci also provides freelance medical writing, focusing on continuing medical education (CME) materials and needs assessments. Kaci was employed at a private practice for 5 years and then transitioned into OB hospitalist work. She obtained an MBA, believing she would move into a hospital leadership position. But her experiences with management did not support her initial assumptions about the career. Kaci explored many nonclinical careers on the way to medical writing. In addition to healthcare administration, she performed chart reviews for insurance companies, dabbled in the disability arena, and served as a medical consultant. She discovered medical writing, joined the AMWA, took a writing course, and began her part-time freelance medical writing career. Initially, she was writing a variety of materials including CME enduring materials, needs assessments, manuscripts, and presentations. Then she began working as an independent contractor for a CRO. She now works there as a full-time technical medical writer focusing on regulatory documents. Carolina Vogel is a medical student whose mother is a registered nurse. After falling ill and being placed in quarantine, she received a terrible call from her mother. She’d fallen ill days before but had been so short of breath she knew it was time to go to the hospital. Freelance medical writing can be a great nonclinical career for several reasons: It can be started part-time. There is a lot of flexibility. The demand for writers continues to grow. It does not require a big cash investment to get started. But it’s probably best-suited for physicians with a certain personality. My guest, Dr. Emma Nichols, addresses all of these issues in today’s interview.  She has a doctorate in molecular biology.  And, she’s a seasoned medical communications professional specializing in continuing medical education and news writing.  Her company, Nascent Medical, provides expert medical writing services through its team of experienced MD- and PhD-level writers. As she was building her company, she needed to vet and train capable writers. That led to developing a course to train freelance writers in all aspects of the business. Dozens of physicians have launched their own writing careers as a result of the training provided by Dr. Nichols.  I’ve personally spoken with several writers who have recommended her course. So, I thought she'd be an excellent guest for the PNC podcast. If you’ve considered a career in medical writing, Emma provided some great advice during our interview.  She described the benefits of a career as a freelance medical writer. And she outlined the personality traits that best fit such a career.  She suggested that writers start out by writing part-time to see how well they like it. She mentioned the American Medical Writers Association (aka “AMWA”) as a good resource for physicians considering a writing career. It’s next big annual meeting – the 2018 Medical Writing & Communication Conference – is being held in Washington, DC from November 1st through the 3rd.  She also told us about a free webinar she’s hosting called “Freelance Medical Writing -- A Lucrative Work-From-Home Career Choice.”  The webinar is being held on Thursday, October 25th, at 2:00 p.m. Eastern. It’s a very good way to learn more about freelance writing. She’ll be going into more details about some of the topics we covered, including the potential salary expectations.
