Medical residency in a rural area is one of the key influencing factors for providers to stay and continue practicing in a rural area. This week on Rural Health Leadership Radio we are talking to Yoana Cruz, Clerkship and Sub-Internship Coordinator in Obstetrics and Gynecology at the University of Kansas Medical Center. We talk with Yoana about the growing need for more providers in rural healthcare, and how she is bringing awareness to the need with up-and-coming medical students and working to connect those students with rural residencies. “Although I may not be the person that gives direct care for patients, I hope to encourage medical professionals at an early stage of their career to provide the best quality care for patients.” – Yoana Cruz Born and raised in southwest Kansas, Yoana Cruz grew up in a welcoming community, full of hard-working immigrant families from more than thirty countries. Despite its rich heritage and tolerant culture, this agrarian city has many health disparities due to its growing number of immigrants and political refugees. The regional healthcare delivery system is slow to react to the complex medical and social needs of these diverse groups. Yoana's own experiences with barriers to utilizing the healthcare delivery system as a child of immigrants have informed her perspective on health equity and motivated her to pursue mission-driven work in healthcare administration. Yoana has 6+ years of project planning experience in the healthcare setting while previously working at Kearny County Hospital, Cerner Corporation, and now at the University of Kansas Medical Center. She is a 1st generation academic student where she received her BBA from Fort Hays State University and recently graduated in May 2024 in her Master of Health Services Administration from the University of Kansas Medical Center. In her spare time, she holds professional development workshops for undergraduate students pursuing business or healthcare-related careers.

We hear from people who aren't happy with their Medicare supplement plan and they think they can change it during open enrollment. We've heard from people who didn't get a Medicare supplement or Medigap policy when first eligible, and they think now they can add it like if they would with a drug plan. However, Open Enrollment doesn't apply to Medigap policies. If you want to change your Medigap policy, you can apply at any time during the year, and medical underwriting may apply. If you are trying to change from Medicare Advantage to a Medicare supplement Medigap policy, there's only two times you can drop that Advantage plan and one of them is coming up. So we say shop for your Medicare supplement first, get your guaranteed acceptance in hand and then dis-enroll from the Advantage plan during Open Enrollment and you'll be good to go January 1." On Common Mistakes with Medicare Open Enrollment "I think waiting until the last minute, then they can't get through or something gets messed up. The mistake I already mentioned: assuming that you don't have to do anything if you love your plan. That happens. I did a client last year. I did his wife early this year, and she says, I'm not supposed to tell you this, Diane, my husband didn't take any drugs. He was in the cheapest plan that he could be in, but then he didn't open his Annual Notice. And come to find out, I think this was two years ago, the plan he was in was discontinued and he was enrolled automatically and that went from $10 to $69. It's a mistake - not opening your Annual Notice of Changes as far as Open Enrollment goes." On What to Do with Medicare Open Enrollment "Don't pay attention to the commercials. Look at your Annual Notice of Changes and then do your homework. So my words to people, and again,

Some things are not as easy as you may think. And discerning the right Medicare choices for you is one of those things. When preparing to enroll in Medicare, or evaluating your coverage, it pays to look past the commercials, do your homework - and to read the fine print. When I researched Medicare in 2022, I engaged Diane Omdahl and her service 65 Incorporated. She's written a new book Medicare for You: A Smart Person's Guide and joins us today to discuss some key things you need to know. Diane Omdahl joins us from Wisconsin __________________________ Bio The author of Medicare for You: A Smart Person's Guide, Diane Omdahl, RN, MS., is a nationally recognized Medicare expert and the author of Medicare and You: A Smart Person's Guide. That is the way that they will enroll in Medicare when it's time. And we have run into people trying to enroll in Medicare who did not have an account. And it's not always easy to set up. And people who have an account haven't kept it up to date, they haven't informed Social Security of their address change. And after a divorce, women with a name change, that's quite involved. And with the Social Security offices just coming back out from Covid, many of them are still not open completely. If you have to submit documents, you don't want to send originals in the mail. And so that's what we say, keep it up to date. Then when you're actually approaching 65, you can't do anything until your initial enrollment period, which is three months before your birthday, unless it's on the first of the month and it's four months. But people like to start about six months in advance. Given they're getting all of the advertising even earlier than that, we tell them that's when you should start thinking about it. And one of our best pieces of advice is that they can toss out that stuff they get prior to age 65 because as they get closer and know what they need, then they can look for the specific information that will work for them." On The Two Medicare Paths "The two Medicare paths is how Medicare is structured for most people over 65. It is probably the most important decision people will make going forward, yet people give it the short shrift. I read that on average people spend 14 hours shopping for a new car. What do they do when it comes to Medicare? They go with what their spouse has, or what works for a friend, or what's in the commercials.

This week Quint talks with Dr. Katherine Meese (UAB), who conducted an extensive study on what's driving burnout in health care workers and it's not at all what you'd expect! In healthcare, we often talk about the financial margin (Do we have enough money to keep the doors open and pursue our mission?) and the operational margin (Do we have enough beds, space, speed and quality?). Dr. Katherine Meese says it's time to focus on the Human Margin. In its simplest form, healthcare is humans working with humans to heal humans. Dr. Meese says that for every decision we're making, every conversation we're having at every level of the organization, we must start asking, “How will this impact our human margin?”From 2020 to 2023, Dr. Meese has conducted an extensive survey of healthcare workers from all parts of the organization (both clinical and non-clinical), asking what would improve their well-being or experience at work. She looked at more than 40 different variables. The things that contributed the most to their stress levels were really surprising and not all what she had assumed was driving burnout.Dr. Meese says we can utilize this research to create conditions that maximize the human margin. She says it's not just about the absence of burnout, but creating a culture where our people can actually flourish. When people bring their best selves to work, it solves a lot of other problems.###Katherine A. This week Quint talks with Chuck Stokes, interim department chair at the University of Alabama at Birmingham in the Department of Health Services Administration, and Dr. Paula Song, Richard M. Bracken professor and chair of the Department of Health Administration at Virginia Commonwealth University. The discussion centers on how universities are preparing students for careers in healthcare and supporting those already in their career, in light of the pace and scope of change we all must navigate.Educators are rethinking how they do things, shifting modalities, and adjusting instruction to meet the changing needs and expectations of students and practitioners as they grapple with changing circumstances. Stokes and Song see this as an opportunity to reinvent themselves. Listeners will learn about a few things the academic community is doing differently. Here's an overview:Providing simulation programs (It's not just the what but the how). There's a lot of power in putting students in real-life situations that require both hard and soft skills. Instructors put them through a simulation to see how they deal with a situation and give them feedback on it. Creating more options and providing more support for people already working in the field. Programs are allowing more flexibility for working professionals, whether they're early, mid, or late careerists. One example is the tailored certificate options in sub-specialties to supplement master's programs (including topics like health equity, financial management, and climate and sustainability).Giving students various opportunities to gain different perspectives in all sorts of ways. For instance, they're partnering with national associations and making sure their programming includes broader health communities. Students are exposed to different career stories and the unique challenges providers and patients face.Providing more assessments and coaching before students enter the workplace. For example, early testing on personality profiles gives them insight on what they need to work on. There's a big focus on self-reflection.Looking to build lifelong partnerships with students. Schools are focused on helping students continue lifelong learning and development throughout their career and helping them leverage the value of alumni communities.Forming partnerships with healthcare organizations in the community. Faculty are spending more time in local hospitals, which informs them on what's really happening on the ground in real time. This gives schools an opportunity to help improve the health status of their community by solving problems. It also helps build important relationships for students.Aiming a laser focus at engagement. This is one of the biggest issues facing our industry. Engagement impacts safety/high reliability, reduces costs, improves consumer-centered care, and boosts innovation. How well the academic community trains leaders to engage and motivate their workforce will be paramount to our success. Encouraging professionals to get involved in their local healthcare education programs (even if they aren't alums). Schools are eager for students to hear different voices, see different experiences, and be exposed to a variety of career paths. People don't need to be formally tied to a program to have an impact and be important mentors and coaches for students. Economist Matt Salois, Ph.D. joins Dr. Andy Roark to discuss the driving economic forces in the veterinary profession today. They discuss the impact of corporatization across the profession, how private equity is changing the way practices operate, and how buying practices at ridiculously high prices is supposed to pay off in profits. They also discuss rising wages in the profession (which are good!) and rising costs to pet owners (which are bad!). Will the price of veterinary services really DOUBLE in 7-10 years? What can we do to prevent that? Let's get into this episode! Today I want to highlight the work of another pharmacy and entrepreneur colleague, Dr. Stephanie Young Moss (aka “Yomo”). She caught my eye on Instagram, where she shines her light on wellness, family dynamics, pharmacy practice, and disparities in healthcare. A practical tip that she shared in the episode has stuck with me, about how learning goes both ways. For example, shadowing allows healthcare students to learn from professionals, but the mentors also get the opportunity to learn. A relationship with someone different than you helps you understand where they're coming from and how you can serve different populations. Dr. Stephanie received her Doctor of Pharmacy degree from Xavier University of Louisiana College of Pharmacy and a Masters of Health Services Administration with a concentration in Health Economics from The University of Wyoming. She has worked in various areas in pharmacy, Community, Regulatory Compliance, Managed Care, and Health Economics and Outcomes Research. Over the past 20 years Dr. Moss has advocated for decreasing disparities in health care through her volunteer work. She uses her platform to educate families across the U.S. on ways to decrease and eliminate health disparities by incorporating simple wellness and mental health techniques. Dr. Moss has shared her views on international and national podcasts and local television news stations. Dr. Stephanie is the creator of the website www.drstephanieyomo.com where you will find practical health and wellness tips, ways to reduce healthcare disparities and simple tips for health and wellness professionals to enhance their presence on social media so they can stand out in their niche, build a successful brand and advocate for important causes. She is also the owner of Integrative Pharmacy Outcomes and Consulting, which focuses on educating underserved communities on ways to reduce and prevent health disparities. For nearly 2 decades, Dr. Stephanie has served on various boards and volunteer organizations that focus on health equity, decreasing implicit bias, addressing social determinants of health and empowering communities to advocate for their health. She has served on the boards for The Minority Health Coalition of Marion County and Eskenazi Health Center where she has served as the Clinical Quality Committee Chair, Board Secretary and is currently the Board Treasurer. She resides in Indianapolis, Indiana. You can connect with Dr. Yomo on all social media (@Dr. Stephanie Yomo) and www.drstephanieyomo.com. Learn more about your ad choices.
