Today, Ali and Asif discuss the career of the late Gene Hackman (2:04).  They discuss his early life, getting into acting and his friendship with Dustin Hoffman and Robert Duvall. They  also discuss Hackman's other interests including architecture and writing. Asif and Ali then discuss their favourite Gene Hackman roles including in the movies ‘Superman', ‘Hoosiers', ‘Crimson Tide', ‘Unforgiven', ‘The Royal Tenenbaums' and ‘The Birdcage'.    Then, Asif and ALi discuss the details of the untimely deaths of Gene Hackman and his wife and caregiver, Betsy Arakawa (27:54). Because Ms. Arakawa died of hantavirus infection, Ali then asks Asif about this rare infection and how it is transmitted by exposure to rodent droppings. Asif discusses how people contract it, how common it is and where in North American you can be exposed to it. He then discusses the two serious forms of the disease –  hantavirus pulmonary syndrome and hantavirus renal syndrome. He then goes over the treatment and the prevention of the disease. Und da fällt das Fazit der Wissenschaftsjournalistinnen Daniela Remus und Korinna Hennig aus aktuellem Anlass besorgt aus: Die Massenentlassungen in US-Gesundheitsbehörden und Wissenschaftseinrichtungen bedrohen die internationale Seuchenüberwachung und Forschung - auch zum Vogelgrippevirus H5N1. Allein in Europa haben die Covid-Impfungen laut WHO 1,4 Millionen Menschenleben gerettet. Trotzdem halten sich Gerüchte über die mRNA-Impfstoffe hartnäckig. In Deutschland wurde besonders viel über vermeintliche Impfschäden berichtet. “Da ist eine Diskongruenz, die erklärt werden muss durch einen Aufmerksamkeitseffekt”, sagt der Virologe Christian Drosten. In dieser Folge berichtet der Wissenschaftsjournalist Joachim Budde gemeinsam mit Daniela Remus und Korinna Hennig, was tatsächlich hinter den gemeldeten Verdachtsfällen steckt und wie Impfschäden überhaupt nachgewiesen werden. "Be fast, have no regrets”- Schnelligkeit ist wichtiger als Perfektion: So die Botschaft des von WHO-Exekutivdirektor Mike Ryan zu Beginn der Pandemie. Wie gut einzelne Maßnahmen wirken, lässt sich oft gar nicht sofort sagen. Doch mittlerweile gibt es auch Forschungsdaten dazu. Die Wissenschaftsjournalistinnen Daniela Remus und Korinna Hennig fragen in dieser Folge: Wie viel Evidenz gibt es für die Wirksamkeit von Maskentragen, Kontaktreduktion und Teststrategien? Und wie viel wäre nötig, um in einer nächsten Pandemie gezielter handeln zu können? Denn: Das wichtigste Element einer Pandemie ist der Mensch, sagt der Modellierer Dirk Brockmann: “Davon ernährt sich das Virus. Das Virus ernährt sich von unseren Kontakten.” Er rechnet vor, warum im Zweifel schon die Halbierung von Gruppen dazu führen kann, dass wir nur noch ein Viertel so viele Ansteckungsmöglichkeiten haben. It is time we look for alternative antibiotic treatments and prevention strategies for syphilis. In conversation with Prof Jeffery Klausner, Professor of Medicine, Infectious Diseases and Public Health at the Keck School of Medicine of the University of Southern California, we will discuss a new study published in the Lancet in 2024. 近來國人對於所謂突破性感染聞之色變,但突破性感染真的有這麼恐怖嗎?讓我們來看看英國對於施打疫苗後再突破性感染患者的研究。主要想探討會突破性感染的危險因子,還有感染後的症狀為何。 Risk factors and disease profile of post-vaccination SARS-CoV-2 infection in UK users of the COVID Symptom Study app: a prospective, community-based, nested,case-control study ⁠ ⁠https://www.thelancet.com/journals/laninf/article/PIIS1473-3099(21)00460-6/fulltext⁠ 1.刺胳針感染醫學期刊9月1日刊登英國倫敦國王學院的研究指出,疫苗仍能提供保護力,防止重症、住院及後遺症,突破性感染的風險極低。 2.這項研究採用病例對照研究方法,資料來自英國超過100萬名成年人在應用程式「covid症狀研究」(COVID Symptom Study)上所記錄的疫苗施打狀況及染疫記錄。突破性感染者是在2020年12月8日到2021年7月4日間接種至少一劑疫苗,並去抓出在注射疫苗後陽性確診者。然後再以年齡,性別,醫護人員與否,來抓出有打疫苗檢查陰性的對照組,和沒打疫苗檢查陽性的對照組。 3.結果在124萬名打完一劑疫苗的成年人,發現有6030人(0.5%)出現「突破性感染」。 在97萬多名打完兩劑疫苗的成年人,發現只有2370人(0.2%)出現「突破性感染」。 4.容易發生突破性感染的風險因子: 超過60歲較衰弱的年長者在接種第一劑疫苗後: 風險是1·93倍 生活在較貧困地區的人接種第一劑疫苗後:風險是1·11倍  沒有肥胖的人接種第一劑疫苗後出現突破性感染的機率較低:風險是0·84倍 p.s.衰弱frailty是在老人醫學中很重要的一個概念,不是所有老人家都衰弱,也有很健康的老人!老人衰弱是有評估量表的,這群人可能容易失能,跌倒,住院等等。 5.受試者施打的疫苗為輝瑞BNT、莫德納以及AZ 第一劑:BNT 44萬,AZ 75萬,莫德納一萬七千人 第二劑:BNT 33萬,AZ 62萬,莫德納3417人 6.在出現突破性感染的人當中,和沒打疫苗感染者相比:所有的症狀都比較少。 無症狀感染的機率打一劑增加了63%,打二劑增加了94% 所有急性症狀打一劑減少了26%,打二劑減少了31% 住院的機率打一劑減少了69%,打二劑減少了73% 出現長期副作用(Long COVID)的機率,打兩劑減少了49% 7.有快兩倍的機會即使感染也是無症狀感染,特別是在60歲以上老人。這個對於個人來說應該算是好事,但對整體防疫來說比較麻煩,就是無症狀感染者比例變多。 8.結論指出,對於放鬆NPI的措施,特別是這篇找到容易突破感染的三個族群,即使在打過疫苗後可能都要小心。此外這三個族群可能值得考慮施打加強針。 04b解讀: 1.首先要強調突破性感染本來就是可能出現的。即使是BNT,保護力95%,也是指相對於沒打疫苗的人降低你95%的得病機率呀。而英國這篇研究已經是Delta開始流行的時間,打完兩劑疫苗發生突破性感染也不過千分之2。你不能只放大這個2,忘記那998人還是沒有染疫的呀。 2.打疫苗後即使突破性感染,所有症狀都比較輕,也有近兩倍無症狀感染的這件事,對個人來說是好事,但對整體防疫來說當然就比較麻煩了。 對高風險已接種疫苗者,比方說這次台灣的機師,應該還是要定期檢測,避免因為無症狀感染而讓Delta溜進來。未來如果Delta進社區,對打疫苗後的長照機構服務人員或是醫護人員來說定期檢測也很重要,避免傳給病人。 3.接種第一劑疫苗後到第二劑之前,在這三群人比較容易發生突破性感染,對於目前我們台灣多半人衝第一劑的政策也有啟示。在一定的覆蓋率後應該要回頭衝第二劑覆蓋率了,且可能該從老人家,肥胖者開始。 4.英國近來也要準備公布其加強針(booster)的政策了,這個針對突破性感染的分析可說是目前為止規模最大的,內容也很重要,告訴我們哪些人是比較容易發生突破性感染的族群。未來應該要針對這群人優先考慮施打加強針的政策。但這些人施打加強針是否能夠有效增加保護力,則有待更進一步的研究了。 混打疫苗可行嗎?AZ混打輝瑞/BNT AZ混打莫德納等等 https://linshibi.com/?p=39613 給長輩的AZ疫苗懶人包 https://linshibi.com/?p=39590 高端 聯亞 國產疫苗懶人包 第二期結束就緊急授權可行嗎? https://linshibi.com/?p=39547 新冠快篩懶人包 普篩 抗體快篩 抗原快篩 https://linshibi.com/?p=36564 新冠肺炎疫情下的防疫須知 常見問題解答FAQ https://linshibi.com/?p=35408 新冠疫苗常見問題懶人包 https://linshibi.com/?p=38945 林氏璧醫師的電子名片 https://lit.link/linshibi Powered by Firstory Hosting

Learning From World Of Warcraft’s Virtual Pandemic The widespread infection of roughly four million virtual characters all started with a giant snake demon. In 2005, the massively multiplayer online video game World Of Warcraft introduced a special event raid, where groups of players could team up to fight a giant snake demon named Hakkar the Soulflayer. Hakkar would cast a spell called “Corrupted Blood” on players, which would slowly whittle down their health. The effect of the spell was only supposed to last inside the raid arena—when players returned to the main world of the game, the spell would dissipate. But thanks to a software glitch, that wasn’t the case if the player had a pet companion. When the pets returned to the main world, they started infecting players and non-playable characters with the Corrupted Blood spell. If the player wasn’t powerful enough to heal themselves, they would die and erupt in a fountain of blood before turning into a skeleton. What followed was a virtual pandemic that startlingly resembled today’s COVID-19 pandemic, from the spread, human behavior, and cultural response. Blizzard, the developer of the game, wanted players to social distance. Some players listened, others flouted the rules, traveling freely and spreading the disease with them. Conspiracy theories formed about how the virus was engineered by Blizzard on purpose, and others placed blame on players with pets as the cause of the outbreak, mirroring the racist anti-Asian attacks and rhetoric surrounding COVID-19 today.  Coincidentally, two epidemiologists, Nina Fefferman and Eric Lofgren, were there to witness the World Of Warcraft outbreak unfold. They studied and used the incident to model human behavior in response to a pandemic. Their findings were published in The Lancet in 2007. Many of their observations came to pass in 2020 when COVID-19 appeared.  SciFri producer Daniel Peterschmidt sat down with Eric Molinsky, host of the podcast Imaginary Worlds, who reported this story for his show. He talks about the epidemiologists who studied the outbreak and how it prepared them for public responses to COVID-19. Why Is Daydreaming Difficult For Grownups? But for many adults, falling into a daydream is hard, especially when our minds are filled with worries about tomorrow’s obligations, finances, and a global pandemic.  Turns out those who feel this way are not alone. New research shows that adults report getting to a daydreaming state is harder than experiencing their unguided thoughts. Adults often require a prompt to think about something pleasant, and tend to ruminate on unpleasant things.  Daydreaming can be an antidote to boredom, and researcher Erin Westgate of the University of Florida says that’s important. Her previous research shows that boredom can cause sadistic behavior in people. Westgate joins guest host John Dankosky and Manoush Zomorodi, host of the TED Radio Hour and author of the book “Bored and Brilliant,” who argues leaning into boredom can unlock our most creative selves. Can We Geoengineer Our Way Out Of A Natural Disaster? Humans have always altered their landscapes—from simple agriculture used to cultivate specific crops to huge projects like damming rivers to change the flow of entire ecosystems. And many of these human interventions have unintended consequences and have led to major environmental disasters. In her book Under A White Sky: The Nature Of The Future, author Elizabeth Kolbert talks to scientists and people working on geoengineering projects and technology to mitigate and avert damage caused by humans in the natural world like climate change. The projects range from electrifying rivers to turning CO2 emissions into rocks. Kolbert discusses if we can solve these natural problems with the tools that created the problems in the first place, and at what cost?
